Tuberculosis Development in Chronic Obstructive Pulmonary Disease: A Nationwide Population-Based Study.

BACKGROUND: Pulmonary tuberculosis is a major comorbidity of chronic obstructive pulmonary disease (COPD). However, the risk factors for tuberculosis development in patients with COPD have not been thoroughly studied. Thus, this study investigated the development of tuberculosis according to comorbidities and inhaler prescriptions for COPD. METHODS: A retrospective cohort study was conducted using data from the Korean Health Insurance Review and Assessment Service database from January 1, 2015 to December 31, 2020. This cohort included 139,589 COPD patients (≥ 40 years) with a new prescriptio

Beyond metrics: relational dynamics and impacts of TB community funding.

Global funding for tuberculosis (TB) falls far short of estimated needs, often prioritizing commodities and service delivery. TB communities, including people affected by TB, remain underfunded and underrepresented in programme planning. Despite this TB-affected communities have, over the past decade, driven significant progress in becoming recognized actors in national and global TB responses. These gains have been largely supported by the Stop TB Partnership Challenge Facility for Civil Society (CFCS), a funding mechanism which supports people affected by TB and community organizations and t

Advances in mycobacteriophage research: from lytic mechanisms to one health applications.

Mycobacteriophages are viruses that specifically infect mycobacterial hosts. Owing to their ability to penetrate the unique and complex cell wall barrier of mycobacteria, mycobacteriophages have recently emerged as promising tools for the diagnosis and treatment of drug-resistant mycobacterial infections. This review systematically summarizes the classification, genomic features, lytic and lysogenic mechanisms of mycobacteriophages, as well as the intricate defense and counter - defense strategies between phages and their hosts. Furthermore, from a One Health perspective, we explore the divers

Comparative Phenotypic Screening Identifies Protein Synthesis Inhibitors as Compounds That Enhance Early Acidification of Mycobacterium tuberculosis in Macrophages.

Treatments for tuberculosis disease (TB) are failing on a global scale, necessitating new drug discovery efforts to identify effective therapies. Mycobacterium tuberculosis (Mtb), the obligate human pathogen and causative agent of TB, resides within alveolar macrophages and combats acidic stress by inhibiting phagosome maturation. Understanding the role of acidification in Mtb pathogenesis can aid the development of improved TB therapies. Aligning with the hypothesis that acidification affects Mtb intracellular survival, we developed a phenotypic assay to identify compounds that enhance acidif

Syndemics, violence and injury: exploring historical relationships between infectious disease epidemics and violent crime in South Africa.

This paper explores historical and contemporary intersections between mass-mortality epidemics and violent crime in South Africa, focusing on four major epidemics - Spanish Flu, tuberculosis, HIV, and Covid-19. The study integrates epidemiological data and contextual historical information such as crime statistics, archival records, and secondary scholarship to explore whether epidemic-driven mortality crises are associated with subsequent changes in violence and injury profiles. With the possible exception of gendered violence, the study finds little evidence that earlier epidemics directly c

Neutrophilic LGALS2 drives tuberculosis susceptibility by regulating Th2 polarization via the IFN-γ-HLA-II-CD4 axis.

Th1/Th2 immune imbalance constitutes a critical mechanism underlying Mycobacterium tuberculosis (Mtb)-mediated immune evasion. Nevertheless, the specific regulatory mechanism of neutrophils, the pivotal upstream innate immune cells, remains poorly elucidated in this imbalance. This study integrates multi-omics technologies, combines an in vitro primary cell co-culture model and clinical samples for verification, and systematically elucidates the core mechanism by which neutrophils regulate Th1/Th2 immune imbalance in tuberculosis (TB). Our data demonstrate that LGALS2 is highly expressed in ne

Exploring tuberculosis physicians' preferences for AI explainability in China: a protocol for a discrete choice experiment.

INTRODUCTION: Artificial intelligence (AI) is increasingly used in tuberculosis (TB) diagnosis, but its clinical adoption depends not only on accuracy but also on physicians' trust and understanding of AI outputs. Explainable AI (XAI) has been proposed to address this challenge, but limited evidence exists on which explanation features are most valued by physicians in TB diagnostic settings. Discrete choice experiments (DCEs) offer a structured method to elicit physicians' preferences and quantify trade-offs among explainability attributes. This protocol describes a DCE designed to examine TB

Integrated surveillance of polio-negative acute flaccid paralysis cases for childhood tuberculosis: a novel GeneXpert-based screening approach in Rivers State, Nigeria.

BACKGROUND: Nigeria was certified free of indigenous wild poliovirus transmission in 2020; however, acute flaccid paralysis (AFP) surveillance remains operational nationwide to detect poliovirus re-emergence and monitor eradication gains [1]. Simultaneously, tuberculosis (TB) remains a major public health challenge, particularly among children, in whom diagnosis is often delayed or missed because of nonspecific clinical presentations and difficulties in obtaining bacteriological confirmation [2, 3]. Innovative strategies that leverage existing surveillance systems may help bridge the persisten

Challenges and countermeasures of the "Xinjiang Model" in tuberculosis prevention and control: a qualitative study based on health specialists' perspectives.

BACKGROUND: Tuberculosis (TB) remains a significant public health threat, particularly in Xinjiang Uygur Autonomous Region of China, showing a high incidence. The "Xinjiang Model" is a TB service delivery system integrating universal chest X-ray screening, hospitalization for infectious patients, community-based follow-up for non-infectious cases, and nutritional support. Since 2018, Xinjiang has implemented the "Xinjiang model" for TB prevention and control to improve early screening and cure rates, but it continues to face challenges in long-term implementation. OBJECTIVE: This qualitative

Peritoneal Tuberculosis Complicated by Abdominal Abscesses: Diagnostic Challenges and Clinical Insights From Two Cases.

Peritoneal tuberculosis (PT) is a rare form of extrapulmonary Mycobacterium tuberculosis infection that may closely mimic Crohn's disease (CD) or intra-abdominal malignancy, leading to diagnostic delay, particularly in endemic areas. We report two illustrative cases complicated by localized abscess formation. The first case involved a 40-year-old woman presenting with chronic right lumbar pain. Computed tomography (CT) revealed a large right psoas abscess associated with circumferential thickening of the cecal base and terminal ileum, along with necrotic mesenteric and iliac lymphadenopathy. Benefits of three consecutive sputum inductions using nebulized racemic salbutamol versus hypertonic saline in smear-negative or sputum-scarce pulmonary tuberculosis patients: a prospective randomized, double-blind study.

UNLABELLED: Although, sputum induction (SI) with hypertonic saline increases the diagnostic yield for suspected pulmonary tuberculosis (PTB) in patients with smear-negative microscopy, but, it carries adverse effects (AEs), particularly bronchospasm. This study assessed and compared the diagnostic yields and safety profiles of three consecutive SI procedures using nebulized salbutamol vs conventional 3% sodium chloride (NaCl) in patients with suspected PTB and initially negative sputum smears. A randomized, double-blind study was conducted at Songklanagarind Hospital, Thailand, between 2016 an

Medullary Thyroid Carcinoma Masquerading as Tuberculosis Reactivation: A Case of Diagnostic Concealment in an Occult Thyroid Nodule.

Medullary thyroid carcinoma (MTC) with tumour-associated granulomas presents a significant diagnostic challenge in tuberculosis (TB)-endemic regions, particularly when thyroid nodules are non-palpable. We report a 75-year-old woman with treated pulmonary TB who presented with dyspnoea and pleural effusion, leading to initial clinical suspicion of TB reactivation. An incidental non-palpable thyroid nodule discovered on ultrasound was revealed to be MTC with epithelioid granulomas on fine-needle aspiration cytology. The absence of a palpable neck mass diverted clinical attention towards pulmonar
