Beyond biomedical control: governance, social protection, and system gaps in tuberculosis control in Kazakhstan - a qualitative study of decision-makers.

BACKGROUND: Tuberculosis (TB) remains a major public health challenge globally and in Kazakhstan, despite progress in reducing incidence. Increasing evidence highlights the role of social and structural determinants in shaping TB risk, disease progression, and treatment outcomes. However, limited research has explored how decision-makers conceptualize these factors and identify system-level gaps in TB control. This study aimed to examine regional decision-makers' perspectives on TB burden, social protection, health system capacity, and governance in Kazakhstan. METHODS: A qualitative study wa

Diagnostic value of biochemical ratios in the differential diagnosis of tuberculous and malignant pleural effusions.

BACKGROUND AND OBJECTIVE: The differential diagnosis between tuberculous pleural effusion (TPE) and malignant pleural effusion (MPE) in patients with pleural effusion (PE) continues to pose significant clinical challenges. While conventional pleural fluid cytology has limited sensitivity, invasive diagnostic procedures are often costly or inaccessible, particularly in resource-limited settings. In this study, we investigated the diagnostic contribution of ratios derived from pleural fluid adenosine deaminase (ADA), total protein, serum lactate dehydrogenase (S-LDH), and pleural effusion lactat

From shore to sequelae: delayed hypersensitivity reaction mimicking softtissue infection following stingray envenomation: a case report.

INTRODUCTION: Stingray envenomation is a relatively common occurrence among water sports enthusiasts, including swimmers, scuba divers, and surfers, as well as among fishermen. However, there is a lack of medical literature on the pathophysiology of immediate and delayed hypersensitivity reactions and, more importantly, on the range of bacterial and mycobacterial infections that can result from these envenomation injuries. This paper aims to provide a general overview of the mechanisms, diagnostic challenges, and management of late hypersensitivity reactions and infections that can occur follo

Metabolomics profiling reveals changes in peptidoglycan and redox metabolism between ancient and modern Mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb), the cause of tuberculosis, kills over 1 million people worldwide each year. Mtb is classified into 10 distinct lineages, each harboring unique genetic changes that influence its survival, virulence, and transmissibility. Although metabolism plays a critical role in Mtb infection and persistence, the metabolomic profiles of different Mtb lineages remain poorly characterized. Here, by using metabolomic and bioinformatic approaches, we have determined the metabolome of representative strains belonging to three main lineages, namely lineage 1, lineage 2, and linea

Systems analysis reveals alternate metabolic states adopted by Mycobacterium tuberculosis across species.

Mycobacterium tuberculosis (Mtb) survives within macrophages by adapting its metabolism to host-imposed constraints, yet how host species shape bacterial metabolic state remains poorly defined. Here, we directly compared the intracellular transcriptional and metabolic states of Mtb during infection of human and mouse macrophages. Cross-species transcriptomics revealed distinct host-imposed microenvironments that drive Mtb into separable metabolic programs, with mouse macrophages inducing stress-associated pathways and human macrophages promoting fatty acid import and catabolism. Strikingly, th

Adaptations in electron transport chain complexes of clinical Rhodococcus equi revealed through comparative genomics.

Rhodococcus equi is an important veterinary and zoonotic pathogen that survives inside macrophage phagolysosomes. Nitrate reductase NarGHI is a known virulence factor for this pathogen. This enzyme complex serves as the terminal electron acceptor in anaerobic respiration in other pathogens, such as the phylogenetically related Mycobacterium tuberculosis. A recent study demonstrated that R. equi survives under anaerobic conditions. In this work, we endeavored to uncover the electron transport chain complexes of R. equi. By in silico comparison to known complexes in the Transporter Classificatio

From immunoinformatics insights to assay development: establishment of a peptide-based indirect elisa for cynomolgus tuberculosis detection.

Tuberculosis (TB) is a severe zoonotic disease that represents a considerable threat to the health of captive non-human primates (NHPs). Conventional serological diagnostic methods often suffer from insufficient sensitivity and specificity, making them difficult to meet the requirements of large-scale screening in monkey colonies. In this study, we predicted and screened Mycobacterium tuberculosis antigenic epitopes using immunoinformatic approaches, and identified a novel antigenic peptide MP71 from the MPT64 protein. Based on the MP71-KLH conjugate, we further developed an indirect ELISA ass

Comparative in vitro antimicrobial susceptibility profiles of clofazimine and pyrifazimine against clinical isolates of Mycobacterium tuberculosis in southwest China.

UNLABELLED: Clofazimine (CFZ) is a key drug used to treat drug-resistant tuberculosis (DR-TB), while pyrifazimine (TBI-166) is an improved riminophenazine derivative with better pharmacokinetics. However, there is a lack of data on its susceptibility and resistance in regions with a high disease burden, such as southwestern China. We compared the in vitro antimicrobial activities of CFZ and TBI-166 against 249 DR-TB clinical isolates (99 multidrug-resistant TB [MDR-TB] and 150 pre-extensively drug-resistant TB [pre-XDR-TB] isolates) from southwestern China. TBI-166 exhibited a concentration-de

Ribosome hibernation in zinc-starved Mycobacterium abscessus confers amikacin tolerance.

UNLABELLED: Intrinsic and acquired antibiotic resistance in Mycobacterium abscessus present unique challenges in treatment of its infections, which are rapidly emerging as a significant public health threat. The majority of clinically relevant antibiotics used against M. abscessus infections target the ribosome, which undergoes remodeling and hibernation in Mycobacterium smegmatis and Mycobacterium tuberculosis in response to zinc-limiting conditions. Ribosome remodeling involves replacement of multiple zinc-binding C+ ribosomal proteins with a CXXC motif by their respective C- paralogs lackin

Phenotypic discordance in rifampicin resistance detection among Mycobacterium tuberculosis isolates from China: insights from whole-genome sequencing and a structured literature review.

Phenotypic drug susceptibility testing (pDST) for rifampicin (RIF) resistance in Mycobacterium tuberculosis (MTB) can yield discordant results between solid and liquid culture methods, complicating accurate detection of drug-resistant tuberculosis (DR-TB). We evaluated RIF pDST concordance in 265 clinical isolates (July 2016 through June 2023) tested on Lowenstein-Jensen (L-J) medium and the Mycobacteria Growth Indicator Tube (MGIT) 960 system. Discordant isolates underwent repeat testing and whole-genome sequencing (WGS). Genotypic data were interpreted with validated pipelines. A structured

Epidemiology of bovine tuberculosis in Tanzania: Systematic review and meta-analysis.

INTRODUCTION: Bovine tuberculosis (bTB) is an important animal disease in the world, with both economic and zoonotic implications. The disease is still endemic in developing world, such as Tanzania due to lack of rigorous bTB control measures. Despite its significant economic and public health importance in Tanzania, its epidemiological information is still inconsistent. METHODS: To address this, we carried out a systematic review and meta-analysis of published articles on bTB epidemiology in Tanzania from 1993 to 2024. Eleven epidemiological themes were examined: prevalence, distribution, in

NMR-based serum metabolomic signatures distinguish active tuberculosis from latent tuberculosis infection.

BACKGROUND: Tuberculosis (TB) continues to be a major cause of global morbidity and mortality, particularly in low- and middle-income countries. A persistent challenge in TB control is the inability of current immunodiagnostic tools to effectively differentiate active TB from latent tuberculosis infection (LTBI). This diagnostic limitation hampers timely case detection and appropriate treatment, thereby sustaining community transmission. OBJECTIVES: This study aimed to identify and validate serum metabolomic biomarkers capable of distinguishing active TB from LTBI using high-resolution 1H Nuc

Rapid Detection of Resistance Mutations in Multidrug-Resistant Tuberculosis With GenoType MTBDRsl Assay.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) is becoming a major threat to the global control of tuberculosis (TB). The situation worsens when fluoroquinolone (FQ) and second-line injectable (SLID) resistant strains are involved. This study assesses the utility of MTBDRsl V2.0 line probe assay (LPA) in identifying resistance patterns to FQ and SLID in MDR-TB isolates. METHODS: A total of 74 MDR-TB isolates were tested for genetic mutations that confer resistance to FQs and against SLIDs by applying the GenoType MTBDRsl V2.0 LPA. This assay targets resistance-associated mutations in t
