Genetic surveillance of first- and second-line drug-resistant isolates of Mycobacterium tuberculosis in Peru.

Tuberculosis (TB) remains a critical public health crisis in Peru, which bears the highest multidrug-resistant TB (MDR-TB) burden in the Americas. The objective of this study was to conduct a nationwide genetic surveillance of first- and second-line drug-resistant Mycobacterium tuberculosis isolates in Peru to characterize their structural and geographic distribution using a multilocus genotype approach. We analyzed 38,339 valid results from first- and second-line line probe assays (LPA 1L, GenoType MTBDRplus v2; and LPA 2L, GenoType MTBDRsl v2) routinely collected between 2019 and 2022 by the

Humans homozygous for rare or common hypomorphic IL23R variants are prone to tuberculosis.

Homozygosity for rare loss-of-function IL23R variants abolishes IL-23-dependent IFN-γ production by lymphocytes, including NK and innate-like T cells, thereby underlying clinical disease due to weakly virulent mycobacterial species. We report selective enrichment in homozygosity for four hypomorphic IL23R variants in our cohort of patients with tuberculosis. Three of these IL23R alleles are rare (G300V, G149R, and L372F), with a minor allele frequency (MAF) under 1%, but the fourth (R381Q) is surprisingly common, with an MAF as high as 10.2% in certain populations. The other 15 missense allele

Silencing Mcl-1: A novel approach to modulate macrophage polarization in tuberculosis.

BACKGROUND: This study investigated the effect of targeted silencing of myeloid cell leukemia-1 (Mcl-1) on the polarization of Mycobacterium tuberculosis-infected macrophages, both in vitro and in vivo. We also examined the associated organ lesions to clarify the potential of Mcl-1 silencing in preventing and treating tuberculosis. METHODS: Using the CIBERSORT algorithm, we analyzed the immune cell infiltration patterns in 100 cases of pulmonary tuberculosis and 79 cases of latent infection from the integrated cohorts (GSE34608 and GSE83456). Furthermore, the expression levels, clinical decis

Synergism in drug combination of new tuberculosis antibiotics for drug-resistant Mycobacterium tuberculosis in vitro.

The treatment of drug-resistant tuberculosis (DR-TB) faces significant challenges. Novel antituberculosis drugs such as bedaquiline (BDQ), pretomanid (Pa824), and delamanid (DLM) offer a promising treatment for DR-TB. However, the synergistic effects of these drugs have been insufficiently explored. In this research, we evaluated the synergistic effects of two dual-drug combinations (BDQ-Pa824 and BDQ-DLM) and four triple-drug combinations (BDQ-DLM-clofazimine [CFZ], BDQ-DLM-linezolid [LZD], BDQ-Pa824-CFZ, and BDQ-Pa824-LZD) against 30 Mycobacterium tuberculosis (MTB) strains isolated from TB

Machine learning-based analysis of drug resistance mutations in Mycobacterium tuberculosis.

Tuberculosis is a deadly airborne disease caused by Mycobacterium tuberculosis. Drug-resistant tuberculosis presents significant challenges for treatment and control of the disease. Resistant strains of Mycobacterium tuberculosis arise from specific mutations in the bacterium. Identification and characterization of resistance-associated mutations are crucial for effective treatment strategies because the first- and second-line drugs for the disease target distinct genes in the bacterium and serve different purposes. Our study developed a machine learning prediction model to analyze mutations a

Systemic mycobacteriosis caused by Mycobacterium avium subspecies hominissuis in a sheep.

Mycobacterium avium subsp. hominissuis (MAH) causes pulmonary and disseminated infections in immunocompromised humans. In pigs, MAH commonly causes subclinical infections that lead to granulomatous lesions in the mesenteric lymph nodes, liver, spleen, and jejunum. MAH infections have been reported less commonly in other animal species and in birds. Here, we report a case of systemic mycobacteriosis caused by MAH in a sheep. The affected animal had a history of hypoproteinemia, weight loss, and fever. Gross pathology findings included serous fluid accumulation in the peritoneal and pleural cavi

Identification and serological characterization of MtbAg1073, a conserved Mycobacterium tuberculosis complex antigen recognized in active tuberculosis.

BACKGROUND: Tuberculosis (TB) remains a major global health challenge, with delayed diagnosis perpetuating transmission in resource-limited settings. Although the sensitivity of molecular diagnostic methods is high, their dependence on the laboratory infrastructure limits accessibility in many regions. Serological approaches that detect host antibody responses represent a technically simple diagnostic strategy; however, the performance of currently available TB serodiagnostic tests is inconsistent, largely due to suboptimal antigen selection and heterogeneity in host immune responses. Therefor

Evaluation of clinical usefulness of a commercial multiplex PCR for diagnosing mycobacterial infections: a multicenter study.

UNLABELLED: Rapid and accurate differentiation between Mycobacterium tuberculosis complex (MTBC) and clinically relevant nontuberculous mycobacteria (NTM) is essential for appropriate patient management but remains challenging with conventional diagnostic methods. We evaluated the clinical performance of a commercial multiplex real-time PCR assay for the direct detection of MTBC, Mycobacterium avium complex (MAC), and Mycobacterium abscessus complex (MABSC) in respiratory specimens. In this prospective multicenter study, 845 respiratory samples from patients with suspected mycobacterial infect

Dual inhibition of respiratory complexes in Mycobacterium tuberculosis results in bacterial killing.

There is an urgent need for novel drug targets and combination therapies for Mycobacterium tuberculosis. The identification of inhibitors that target the mycobacterial respiratory chain, particularly when used in combination, has emerged as an exciting new avenue for addressing antibiotic resistance. However, the lack of inhibitors for respiratory complexes, possible functional redundancy between homologous enzymes, and a poor knowledge of which complexes, when simultaneously inhibited, have lethal phenotypes, have slowed progress in this space. To address these limitations, we have utilized C

Concentration-dependent dual activity of a phage-encoded lysin modulates infection dynamics in mycobacteriophage D29.

Lytic enzymes encoded by bacteriophages are conventionally understood to serve a single function in the phage life cycle: to enable progeny release through host cell lysis. Here, we identify an unexpected dual functionality in LysB, a lytic cassette protein of mycobacteriophage D29, which exerts opposing effects on phage propagation depending on the stage of infection. In our model, during the early phase of phage expansion, LysB's activity is limited to infected cells, hydrolyzing their outer membrane to facilitate efficient virion release. However, as the infection progresses, LysB accumulat
