Compulsive exercise in adults with trichotillomania and skin picking disorder.

BACKGROUND AND AIMS: Compulsive exercise (CE) has significant theoretical overlap with obsessive-compulsive disorder. No research has examined the relationship between CE and trichotillomania and skin picking disorder, common conditions on the obsessive-compulsive spectrum. METHODS: A total of 382 adults (mean age = 29.2, 78.0% female) with trichotillomania (n = 203) and/or skin picking disorder (n = 276) completed an online survey. Participants who met criteria for CE on the Minnesota Impulse Disorders Interview were compared to those who did not meet these criteria on demographics, a clinic

The efficacy of psychotherapeutic and pharmacological interventions for trichotillomania: A review and meta-analysis.

This study provides an updated meta-analytic review of the efficacy of pharmacological and psychotherapeutic interventions for trichotillomania (TTM) based on randomized clinical trials. Both standard and network meta-analyses were conducted. Based on extensive searches, 29 trials met the inclusion criteria for standard meta-analysis and 30 met the criteria for network meta-analysis. When considering both the magnitude of effect sizes and replication, strong support was found for the efficacy of three interventions: behavioral therapy with habit reversal, Acceptance and Commitment Therapy-enha

Trusting your gut: a hairy situation-gastric trichobezoar case report.

Trichobezoars are rare masses made from ingested hair that are commonly seen in young females. We report a case of a 7-year-old girl who presented with generalized abdominal pain and rapid weight loss. Initial evaluation with radiography, magnetic resonance imaging (MRI), and upper gastrointestinal series (UGI) suggested superior mesenteric artery (SMA) syndrome but failed to provide a definitive diagnosis. A gastric trichobezoar was ultimately identified on esophagogastroduodenoscopy (EGD). While uncommon, trichobezoars should be considered in the differential diagnosis of pediatric patients,

Case series of pediatric gastric trichobezoars: diagnostic challenges, management outcomes, and the imperative for psychiatric follow-up.

INTRODUCTION AND IMPORTANCE: Trichobezoar (TBZ) is a rare gastric mass composed of ingested hair, strongly linked to psychiatric conditions like trichotillomania (TTM) and trichophagia (TPH) that predominantly affect young females. Diagnosis, based on clinical suspicion, is best confirmed by CT or gastroscopy, though point-of-care ultrasound (POCUS) is a valuable initial tool. Management is ranging from endoscopic retrieval for small bezoars to surgical resection for large or complex cases. We present this series to highlight the diagnostic challenges, underscore the essential psychiatric dime

Neurofeedback interventions for obsessive-compulsive and related disorders: Current evidence and future directions.

Obsessive-compulsive and related disorders (OCRDs) are characterized by obsessions, compulsive or repetitive behaviors, hoarding and saving, or recurrent body-focused repetitive behaviors. Although pharmacological, cognitive-behavioral, or behavioral therapies provide relief for many for these conditions, a substantial proportion respond insufficiently or experience relapses. Neurofeedback (NF) enables individuals to self-modulate neural activity in real time and has been explored as a non-invasive neuromodulation strategy in obsessive-compulsive disorder (OCD), the only OCRD in which NF has b

Psychocutaneous Disorders in Pediatric and Adolescent Populations: A Narrative Review.

Psychocutaneous disorders (PCDs) in children and adolescents lie at the intersection of dermatology and psychiatry and include primary psychiatric conditions with cutaneous manifestations, dermatologic diseases with psychiatric sequelae, and functional or overlap syndromes. Despite substantial psychosocial burden and chronic morbidity, pediatric PCDs remain under-recognized and inconsistently managed. We conducted a narrative review of PubMed, Embase, PsycINFO, Scopus, and Web of Science (2000-2025), including epidemiologic studies, clinical trials, meta-analyses, and consensus statements addr

Paediatric acute-onset neuropsychiatric syndrome (PANS) with trichotillomania.

This report describes a child diagnosed with a presentation consistent with paediatric acute-onset neuropsychiatric syndrome (PANS), accompanied by obsessive-compulsive symptoms and trichotillomania. A school-aged girl in middle childhood developed sudden-onset washing rituals, contamination fears, hair-pulling behaviour and progressive academic decline shortly after a severe throat infection. Her symptoms included prolonged handwashing, repetitive utensil cleaning and recurrent hair pulling resulting in a patch of incomplete alopecia. Psychiatric assessment confirmed moderate obsessive-compul

The effect of the menstrual cycle on obsessive-compulsive and related disorders: A scoping review.

INTRODUCTION: Anecdotal evidence suggests that the menstrual cycle influences symptoms of Obsessive-Compulsive and Related Disorders (OCRD), although scientific literature investigating this effect is scarce. The premenstrual phase has been associated with the exacerbation of certain psychiatric conditions, with some reports showing premenstrual worsening of OCD. This scoping review aimed to systematically synthesize the evidence on the effect of the menstrual cycle on OCRD symptoms. METHODS: Systematic searches were conducted on MEDLINE, EMBASE, APA PsycINFO, Cochrane Database of Systematic

Psychometric Evidence of the Trichotillomania Dimensional Scale (TTM-D) in Adults From the General Population.

The aim of the present study was to analyse the psychometric properties of the Trichotillomania Dimensional Scale (TTM-D) in Peruvian adults. A total of 507 adults (66.7% women) participated, completing a sociodemographic questionnaire, the Frequency of Suicidal Ideation Inventory (FSII), the Patient Health Questionnaire-2 (PHQ-2), and the Generalized Anxiety Disorder-2 (GAD-2). Structural validity was examined using confirmatory factor analysis within a covariance-based structural equation modelling framework, and item functioning was analysed using a two-parameter item response theory (IRT)

Endogenous opioid peptides in trichotillomania: An exploratory analysis of focused and automatic hair-pulling subtypes.

OBJECTIVE: Trichotillomania (TTM) is a body-focused repetitive behavior disorder characterized by recurrent hair pulling and functional impairment. Emerging evidence suggests that reward- and stress-related neurobiological mechanisms may contribute to its pathophysiology. This study investigated peripheral endogenous opioid peptide levels in TTM and examined differences across hair-pulling subtypes. METHODS: This cross-sectional, case-control study included 45 adults with DSM-5-diagnosed TTM and 45 age- and sex-matched healthy controls. TTM subtypes (focused vs. automatic) were determined thr

Body-Focused Repetitive Behavior Disorders: From Competing Paradigms Toward Iterative Integration.

Body-focused repetitive behaviors (BFRBs), such as hair-pulling and skin-picking, have been described by physicians from Hippocrates onward. The descriptions have not been theory-neutral; clinicians and researchers have applied specific conceptual frameworks to understand such behaviors and to intervene accordingly. These include what might be termed "turns" to psychoanalysis, cognitive behaviorism, psychopharmacology, nosology, neuroscience, consumer advocacy, and global mental health. It is timely to consider lessons learned from this range of conceptual frameworks, each of which has both st

Shame among parents of children with trichotillomania: influencing factors and management strategies.

BACKGROUND: Trichotillomania is a common psychiatric disorder in children, and the social stigma associated with the disorder and the challenges of caregiving often induce shame among parents. This study aims to investigate shame and its influencing factors among parents of children with trichotillomania, to provide a scientific basis for clinical family management. METHODS: A crosssectional survey was conducted among parents of children with trichotillomania treated in our hospital. The general information questionnaire and the Family Version of the Child Chronic Disorder Shame Scale were us

N-acetylcysteine: reinventing classic pharmacology through skin.

N-acetylcysteine (NAC), traditionally known for its mucolytic properties and as an antidote for acetaminophen poisoning, has emerged as a multifunctional molecule with growing interest in dermatology. Its pleiotropic mechanisms -including modulation of redox balance, disulfide bond disruption, replenishment of intracellular glutathione, and regulation of inflammatory, microbial, and epigenetic pathways- have opened avenues for its therapeutic repurposing in cutaneous medicine. In congenital ichthyoses, topical NAC formulations have shown promising results, improving scaling and barrier functio

Transumbilical single-incision laparoscopic-assisted removal of gastric trichobezoars in children: a technical refinement and feasibility study.

BACKGROUND: Gastric trichobezoars are rare in children and adolescents and are often associated with psychiatric disorders such as trichotillomania and trichophagia. While surgical removal remains the definitive treatment for large bezoars, optimal management must consider not only technical success but also cosmetic outcomes, psychosocial impact, and feasibility in low-resource settings. The aim of the study was to evaluate the safety, feasiblity, and cosmetic of a transumbilical single-incision laparoscopic-assisted technique for gastric trichobezoar removal in children. METHODS: We conduct

Obsessive-Compulsive Disorder (OCD) and Autism Spectrum Disorder (ASD): Clinical Fingerprints of the Comorbidity.

BACKGROUND: Obsessive-compulsive disorder (OCD) frequently co-occurs with autism spectrum disorder (ASD), but the prevalence and clinical correlates of this comorbidity remain incompletely understood. METHODS: We examined a clinical sample of 603 patients with a primary diagnosis of OCD, of whom 149 (24.7%) presented with comorbid ASD. Sociodemographic variables, clinical characteristics, comorbidities, and obsessive-compulsive symptom dimensions were compared between patients with and without ASD. RESULTS: Patients with OCD + ASD reported an earlier onset of both obsessive-compulsive sympto

Trichotillomania: The interplay between emotional states, urges and hair pulling episodes.

BACKGROUND: Trichotillomania (TTM) is a body-focused repetitive behavior disorder characterized by recurrent hair pulling and psychosocial impairment. Although theoretical models emphasize emotion regulation as a key mechanism in TTM, the precise relationship between emotional states, urge intensity, and hair pulling behavior remains unclear. OBJECTIVE: This study examined the momentary interplay between affective states, urge intensity, and hair pulling episodes in daily life using Ecological Momentary Assessment (EMA). METHODS: Sixty-one Individuals diagnosed with TTM reported seven times

Increased Risk of Trichotillomania in Neurofibromatosis Type 1: A Retrospective Cohort Study.

INTRODUCTION: Neurofibromatosis type 1 (NF1) is an autosomal dominant neurocutaneous disorder associated with substantial psychiatric comorbidity. Although repetitive and compulsive behaviors are reported in NF1, the risk of body-focused repetitive behaviors is understudied. METHODS: We performed a retrospective cohort study using the TriNetX research network (2006-2026). On January 14, 2026, individuals with a first recorded diagnosis of NF1 were compared with matched controls. Patients with prior trichotillomania or skin-picking disorder were excluded. Cohorts were balanced using 1:1 propen
