Exploring the nucleus accumbens circuit and oxytocin therapy in a Sapap3 knockout mouse model of trichotillomania.

Trichotillomania (TTM), an understudied psychiatric disorder, was investigated using Sapap3 knockout (KO) mice to elucidate nucleus accumbens (NAc) circuit dysfunction and oxytocin's therapeutic potential. Under aversive conditions, KO mice exhibited TTM-like behavioral phenotypes compared to wild-type (WT) controls: elevated anxiety-like behavior (reduced total distance 33328.45 ± 6703.97 mm vs. WT 47787.22 ± 12221.33 mm; decreased standing episodes 34.20 ± 19.41 vs. WT 58.10 ± 15.55; increased immobility duration 175.05 ± 54.46 s vs. WT 90.23 ± 70.22 s, all p < 0.05), excessive grooming dura

A Case-Control Study of Trichotillomania Patients Using a National Database.

INTRODUCTION: Trichotillomania (TTM) is a body-focused repetitive disorder linked to psychiatric comorbidities. Comprehensive studies mapping its associations with a range of conditions remain limited. This study examines TTMs associations with psychiatric, neurodevelopmental, substance-related, reproductive, and social factors using a national database. METHODS: We performed a case-control study using the All of Us Research Program database. TTM cases (n = 40) were matched to controls (n = 400) based on age, sex, and ethnicity. Multivariate logistic regression was used to examine association

Hidden in plain sight: diagnostic challenges of gastric trichobezoar in an adolescent-a case report.

Gastric trichobezoars are rare and often pose a diagnostic challenge due to their nonspecific symptoms and association with underlying psychiatric disorders. In this case, a 14-year-old girl presented with a 3-month history of intermittent abdominal pain, vomiting, early satiety, and a palpable epigastric mass. Although she initially denied any history of trichophagia or trichotillomania, further questioning revealed a year-long habit of hair ingestion that began after an appendectomy. Contrast-enhanced computed tomography imaging confirmed a large gastric trichobezoar extending into the duode

Examination of the relationship between internalized stigma and coping strategies in patients with trichotillomania.

BACKGROUND: This study aimed to examine the relationship between internalized stigma and coping strategies in patients with trichotillomania (TTM). METHOD: The study included 34 patients with TTM and 34 healthy controls. The Sociodemographic Data Form, Beck Depression Inventory (BDI), and the Revised Coping Orientations to Problems Experienced (R-COPE) were administered to the TTM and control groups. In addition, the Massachusetts General Hospital Hair Pulling Scale (MGH-HPS) and the Internalized Stigma of Mental Illness Scale (ISMI) were administered to the TTM group. RESULTS: The scores of

Atypical Rapunzel Syndrome in an Adult Female: Trichobezoar Beyond the Gastric Outlet Leading to Gastrointestinal Perforation and Septic Shock.

Trichobezoars are rare accumulations of ingested hair typically seen in young females with psychiatric disorders such as trichotillomania and trichophagia. Few case reports describe separate trichobezoar masses within the GI tract. We report the case of a 57-year-old female with trichophagia history who presented with abdominal distensions and rebound tenderness consistent with peritonitis. Imaging raised suspicion for bezoars, and emergent laparotomy revealed two separate trichobezoars, one in the gastric antrum and one in the jejunum, as well as two jejunal perforations. This case highlights

Rapunzel Syndrome Resulting in Multiple Sites of Simultaneous Intussusception.

We present a case of Rapunzel syndrome in a nine-year-old girl with nonverbal autism, pica, and trichotillomania, who presented for acute worsening of abdominal pain and vomiting. Imaging revealed a bezoar extending from the stomach through most of the small intestine, resulting in multiple small bowel-small bowel intussusceptions. Open laparotomy confirmed the presence and enabled the removal of gastroenteric trichobezoars. Trichobezoars are seen most often in adolescent females with a history of trichotillomania and trichophagia. Trichobezoars can cause serious complications, including obstr

Predictors of clinical trial discontinuation in trichotillomania: a secondary analysis of previous clinical trials.

BACKGROUND: Participant discontinuation is a significant challenge in pharmacological trials for trichotillomania (hair-pulling disorder). Attrition in clinical trials reduces statistical power, introduces error, and potentially wastes financial and medical resources. Identifying predictors of discontinuation may help researchers enhance participant retention and improve study outcomes. METHODS: Data from five completed pharmacological trials for trichotillomania were aggregated, and participants were classified as either Discontinued or Completed. Differences in demographic and clinical vari

Trichotillomania and its treatment: an updated review and recommendations.

INTRODUCTION: Trichotillomania is an impulse control disorder in which individuals fail to resist urges to pull out their own hair and is associated with significant psychiatric comorbidity and functional impairment in affected children, adolescents, and adults. Onset in childhood or adolescence is typical, yet the literature on phenomenology, psychopathology, and treatment outcome involving pediatric samples remains particularly sparse. Efficacious treatments have been developed and found efficacious, most notably cognitive-behavioral interventions known collectively as habit reversal trainin

Tangled Troubles: A Rare Case of Rapunzel Syndrome in a Young Female Patient.

Rapunzel syndrome is a rare and fascinating disease diagnosed when large trichobezoars are found in the patient's GI tract, and most of them suffer from obsessive-compulsive disorder (OCD) and trichotillomania. This report describes the case of a 27-year-old female patient who had abdominal pain, a progressively increasing abdominal mass, early satiety, and profound weight loss during the past six months. Physical examination, radiology, and endoscopy showed a large inhomogeneous mass filling the lumen from the stomach to the jejunum. Exploratory laparotomy, gastrotomy, and multiple enterotomi

From trichophagia to trichobezoar: Rapunzel syndrome in a child - A case report and literature review.

BACKGROUND: Gastric trichobezoars are suggestive of Rapunzel Syndrome, a rare condition typically affecting young women with underlying psychosis. Patients show trichotillomania and trichophagia, which, over time, produce a stomach trichobezoar. This mass may develop a characteristic tail-like protrusion in the small intestine. RELEVANCE: Rapunzel Syndrome should be considered in the differential diagnosis, given its rarity and variations in symptoms among individuals. Early identification helps to avoid misdiagnosis, aggravation of the illness, and inadequate therapy plans. CASE PRESENTATIO

Treatment Strategies for Pediatric Trichotillomania: State-of-the-Art Review on Progress and Persistent Challenges.

Trichotillomania (TTM) is a condition characterized by recurrent hair pulling, often resulting in hair loss and functional impairment. The disorder remains underreported in the pediatric population, despite high rates of psychiatric comorbidities. Moreover, diagnosing and treating TTM among children remains challenging due to a lack of standardized guidelines. Hence, this systematic review aims to evaluate the current treatment approaches for pediatric TTM. A comprehensive search was conducted of five databases, using the search concepts of trichotillomania, pediatric populations, and treatmen

What do therapists with prior experience treating trichotillomania do differently?

Greater therapist experience is not necessarily associated with superior treatment outcomes, but some data suggest that experience with a specific disorder or treatment protocol is helpful. This study conducted a secondary analysis of one such finding, in which therapists with previous experience treating trichotillomania (TTM) achieved better outcomes than did TTM-novice therapists in a randomized controlled trial of the Comprehensive Behavioral (ComB) model. Sixty session recordings from this trial were randomly selected for coding on rationally-selected dimensions of therapist competence, o

Exploring the lived experiences of individuals with trichotillomania: a descriptive phenomenological study.

BACKGROUND: Trichotillomania (TTM) is a common psychiatric disorder characterized by a recurrent, irresistible urge to pull out one's hair. This disorder has widespread adverse physical, psychological, social, and emotional effects on the lives of those affected, yet these experiences are under-documented. This study aimed to explore the lived experiences of individuals with TTM in Iran. METHODS: In this phenomenological study, 12 individuals with TTM (with a mean age of 26.1 ± 6.3 and consisting of seven women and five men) were selected using purposive sampling. Data were collected through

Introduction to the Special Issue: Integration of Technologies into Cognitive Behavioral Therapies for Fear-Based Conditions.

Cognitive behavioral therapies are the first-line psychotherapeutics for fear-based conditions, including anxiety, obsessive-compulsive (OCD), and trauma-related disorders. Despite documented efficaciousness and effectiveness, various factors limit the availability of these evidence-based treatments. Technologies may be used to circumvent treatment barriers, improving access to care and optimizing treatments to address symptoms among those with fear-based conditions. Original research articles in this Special Issue include a study on provider use of digital mental health and extended reality t

Can Oxidative Stress Biomarkers Differentiate Trichotillomania from Obsessive-Compulsive Disorder and Healthy Controls?

This study investigated oxidative stress biomarkers in differentiating trichotillomania (TTM), obsessive-compulsive disorder (OCD), and healthy controls (HC), focusing on thiol-disulfide homeostasis, total oxidant status (TOS), total antioxidant status (TAS), and ischemia-modified albumin (IMA). A total of 68 adolescent females aged 10-18 were recruited and divided into three groups: TTM (n = 24), OCD (n = 21), and HC (n = 23). Oxidative stress biomarkers were assessed through blood analyses, including measurements of thiol-disulfide homeostasis, TOS, TAS, and IMA. Blood analyses revealed that

A genomic study of trichotillomania and excoriation disorder in families.

Trichotillomania and excoriation disorder are obsessive-compulsive related disorders that are often subclassified together as body-focused repetitive behavior (BFRB) disorders. While previous research suggests shared genetic factors, the genetic architecture of these BFRBs remains incompletely understood. Probands with trichotillomania and/or excoriation disorder and both of their biological parents were recruited for an ongoing genetic study of parent-offspring trios with BFRBs. Genome-wide array data were generated in 110 families (334 individuals total) to investigate the role of both commo
