Behavioral addictions: where to draw the lines?

Behavioral addictions can present in a variety of subtle and deceptive patterns. Because of the intense shame, guilt, and embarrassment felt by patients, it may fall to providers to utilize screening tools and deeper interviewing techniques to uncover the extent of these behaviors. Identifying when the line is crossed from recreation/habit to psychopathology relies on understanding current diagnostic criteria and consideration of cultural, ethnic, and local community standards. Individuals are also likely to cross back and forth between this line of pathology and habit, further clouding provid

[The role of the striatum in addiction].

Addiction is a notorious treatment-resistant psychiatric disorder characterized by the impairment of self-monitoring, loss of interest in other targets of pleasure, and uncorrectable impulsive/compulsive drug-seeking behaviors. The striatum, particularly the ventral striatum (= the nucleus accumbens) is deeply involved in the acquisition and expression of addiction. Although only few pharmacotherapeutic approaches against addiction are available, the currently used animal models of addiction are sophisticated enough to mimic most of the representative phenotypes observed in human addicts. In a

[Toxoplasma gondii: a potential role in the genesis of psychiatric disorders].

INTRODUCTION: Toxoplasma gondii is the most common protozoan parasite in developed nations. Up to 43% of the French population may be infected, depending on eating habits and exposure to cats, and almost one third of the world human's population may be infected. Two types of infection have been described: a congenital form and an acquired form. Although the medical profession treats these latent cases as asymptomatic and clinically unimportant, results of animal studies and recent studies of personality profiles, behavior, and psychomotor performance have led to reconsider this assumption. Skin picking disorder is associated with other body-focused repetitive behaviors: findings from an internet study.

BACKGROUND: There is a lack of consensus on how to conceptualize skin picking disorder (SPD). It has been proposed that SPD is related to other problematic body-focused repetitive behaviors (BFRBs), such as hair pulling disorder (HPD) and problematic nail biting and cheek biting. METHODS: We compared rates of BFRBs in online samples of SPD sufferers and college students. We also examined family history of HPD and skin picking problems and correlates of BFRB comorbidity in the SPD sample. RESULTS: Prevalence of BFRBs was significantly higher in the SPD sample compared with the college student

Addiction: the urgent need for a paradigm shift.

Severe addictions to drug use and to countless other habits are causing enormous harm around the globe. Massive expenditures and dedicated efforts of police, doctors, addiction therapists, and self-help groups have failed to bring the problem under control, although many individual addicts have been helped. What can society do when our best efforts continue to fail and a menacing problem continues to grow? This paper proposes that a major paradigm shift is required. The currently dominant paradigm assumes that addiction is either an individual disease or an individual moral breach. But this in

Dopamine and the Neural "Now": Essay and Review of Addiction: A Disorder of Choice.

Rather than view addiction as a disease, Heyman sees it as a choice-one that works like other choices, whereby immediate rewards outshine long-term gains. He rejects neuroscientific explanations of addictive behavior, because he believes they cast it as involuntary or disease-like. I argue that the disease-versus-choice debate creates a false dichotomy: Neuroscience does not have to frame addiction as a disease. Rather, it can help explain how addicts make impulsive choices in the moment and distort appraisal and decision-making habits in the long run. Specifically, the salience of drug-relate

Tourette syndrome: evolving concepts.

Tourette syndrome is a common childhood-onset neurobehavioral disorder characterized by multiple motor and phonic tics affecting boys more frequently than girls. Premonitory sensory urges prior to tic execution are common, and this phenomenon helps to distinguish tics from other hyperkinetic movement disorders. Tourette syndrome is commonly associated with attention deficit hyperactivity disorder, obsessive-compulsive disorder, learning difficulties, and impulse control disorder. The pathophysiology of this complex disorder is not well understood. Involvement of basal ganglia-related circuits

Towards mouse models of perseveration: a heritable component in extinction of operant behavior in fourteen standard and recombinant inbred mouse lines.

Extinction of instrumental responses is an essential skill for adaptive behavior such as foraging. So far, only few studies have focused on extinction following appetitive conditioning in mice. We studied extinction of appetitive operant lever-press behavior in six standard inbred mouse strains (A/J, C3H/HeJ, C57BL/6J, DBA/2J, BALB/cByJ and NOD/Ltj) and eight recombinant inbred mouse lines. From the response rates at the end of operant and extinction training we computed an extinction index, with higher values indicating better capability to omit behavioral responding in absence of reward. The assessment of tobacco dependence in young users of smokeless tobacco.

INTRODUCTION: As all published measures of dependence for users of smokeless tobacco (dippers) have poor reliability, in the present work the Hooked on Nicotine Checklist (HONC) and the Autonomy Over Smoking Scale (AUTOS) were evaluated for use with this population. Dippers and smokers were also compared in relation to dependence, the pleasure derived from using tobacco and the latency to the onset of withdrawal. METHODS: In 2010, an anonymous self-completed paper survey was administered to 1541 students of mixed race and ethnicity in grades 9-12 (mean age 15.9 years) in a Florida high school

Gaming machine addiction: the role of avoidance, accessibility and social support.

Commonality in etiology and clinical expression plus high comorbidity between pathological gambling and substance use disorders suggest common underlying motives. It is important to understand common motivators and differentiating factors. An overarching framework of addiction was used to examine predictors of problem gambling in current electronic gaming machine (EGM) gamblers. Path analysis was used to examine the relationships between antecedent factors (stressors, coping habits, social support), gambling motivations (avoidance, accessibility, social) and gambling behavior. Three hundred an

Cheek-biting disorder: another stereotypic movement disorder?

Recurrent cheek biting, a form of self-injurious behavior is a rare entity which presents mostly to dentists and dermatologists. We report a case of recurrent severe cheek biting in an adult male leading to mucosal ulceration. The stereotypic pattern of cheek biting and associated behavior bears striking resemblance to other impulse control disorders.

Clarifying exercise addiction: differential diagnosis, co-occurring disorders, and phases of addiction.

This paper sets out to clarify the unique features of exercise addiction. It begins by examining how this addiction can be distinguished from compulsions and impulse control disorders both of which, like an addiction, involve excessive behavior that creates adverse effects. Assessment of exercise addiction also requires that clinicians be attuned to other forms of excessive behavior, especially eating disorders that can co-occur with exercise. Finally in an effort to clarify exercise addiction, this paper uses the four phases of addiction to examine the attributes of exercise that define it as

Neurobiological mechanisms involved in nicotine dependence and reward: participation of the endogenous opioid system.

Nicotine is the primary component of tobacco that maintains the smoking habit and develops addiction. The adaptive changes of nicotinic acetylcholine receptors produced by repeated exposure to nicotine play a crucial role in the establishment of dependence. However, other neurochemical systems also participate in the addictive effects of nicotine including glutamate, cannabinoids, GABA and opioids. This review will cover the involvement of these neurotransmitters in nicotine addictive properties, with a special emphasis on the endogenous opioid system. Thus, endogenous enkephalins and beta-end

Neurobiology of food addiction.

PURPOSE OF REVIEW: To review recent work on disorders related to food use, including food addiction, and to highlight the similarities and differences between food and drugs of abuse. RECENT FINDINGS: Recent work on food use disorders has demonstrated that the same neurobiological pathways that are implicated in drug abuse also modulate food consumption, and that the body's regulation of food intake involves a complex set of peripheral and central signaling networks. Moreover, new research indicates that rats can become addicted to certain foods, that men and women may respond differently to

Addiction research centres and the nurturing of creativity: the National Institute on Alcohol Abuse and Alcoholism.

The aim of this paper is to present a concise account of the history, mission, structure and some recent achievements of the US National Institute on Alcohol Abuse and Alcoholism (NIAAA). Created by the US Congress 40 years ago, the NIAAA has evolved from an entity charged mainly with building a national system of alcoholism treatment services to one with responsibility for developing, nurturing and supporting the biomedical and behavioral science foundation necessary to reduce the significant domestic and global public health impact of alcohol use disorders. The NIAAA is unique in that it fun

Pathologic skin picking.

BACKGROUND: Pathologic skin picking (PSP) is characterized by the repetitive and compulsive picking of skin which results in tissue damage. OBJECTIVES: This article sought to examine the evidence supporting the phenomenological, and biological links between PSP and substance use disorders. METHODS: A review of the literature examining clinical presentation, comorbid psychiatric conditions, and treatment studies was used to examine the relationship of PSP and substance use disorders. RESULTS: Prevalence rates of PSP range from 1.4-5.4% in the general population, with a much higher prepondera

Suicidal behavior and alcohol abuse.

Suicide is an escalating public health problem, and alcohol use has consistently been implicated in the precipitation of suicidal behavior. Alcohol abuse may lead to suicidality through disinhibition, impulsiveness and impaired judgment, but it may also be used as a means to ease the distress associated with committing an act of suicide. We reviewed evidence of the relationship between alcohol use and suicide through a search of MedLine and PsychInfo electronic databases. Multiple genetically-related intermediate phenotypes might influence the relationship between alcohol and suicide. Psychiat

Trichotillomania (hair pulling disorder), skin picking disorder, and stereotypic movement disorder: toward DSM-V.

In DSM-IV-TR, trichotillomania (TTM) is classified as an impulse control disorder (not classified elsewhere), skin picking lacks its own diagnostic category (but might be diagnosed as an impulse control disorder not otherwise specified), and stereotypic movement disorder is classified as a disorder usually first diagnosed in infancy, childhood, or adolescence. ICD-10 classifies TTM as a habit and impulse disorder, and includes stereotyped movement disorders in a section on other behavioral and emotional disorders with onset usually occurring in childhood and adolescence.
