Overview of Feminizing Genital Gender-Affirming Surgery.

There are many reasons why transgender and gender diverse (TGD) patients may opt for genital gender affirming surgery. As the TGD population continues to grow, there is ongoing demand for competent providers who can not only perform these surgeries, but also provide long-term care for patients after surgery. As such, this article hopes to provide gynecologists and other healthcare providers with an overview of feminizing genital surgeries and a framework for delivery of comprehensive ongoing care, screening, and support.

Perioperative Considerations for Gender-Affirming Hysterectomy.

Hysterectomy is medically necessary for transgender and gender diverse patients experiencing dysphoria. During the surgical evaluation, patients should be asked about their desires for childbearing and counseled about gamete preservation. There should also be coordination with a multidisciplinary team of surgeons if patients desire other gender-affirming procedures at the time of hysterectomy. Many perioperative considerations should be given, including the continuation of testosterone perioperatively. Surgery should be performed minimally invasively if possible. Complications specific to gend

Feminizing and masculinizing gender-affirming hormone therapy affects fibrin clot characteristics in opposite directions.

BACKGROUND: The mechanisms behind the increased occurrence of thrombosis associated with gender-affirming hormone treatment (GAHT) are not clarified. We hypothesized that GAHT alters fibrin clot characteristics (formation, lysis, and structure) in a prothrombotic direction. OBJECTIVES: To investigate changes in ex vivo fibrin clot characteristics after 12 months of feminizing or masculinizing GAHT. METHODS: We included 251 transgender women and 320 transgender men aged >17 years in a prospective cohort study. Clot formation (velocity [V max], maximum absorbance [MA], and overall hemostasis p

Long-term satisfaction after gender-affirming laparoscopic sigmoid vaginoplasty in transgender women: a comparison with penile inversion vaginoplasty.

BACKGROUND: Gender-affirming laparoscopic sigmoid vaginoplasty (SV) is an established alternative to penile-inversion vaginoplasty (PIV) when there is insufficient penoscrotal tissue available. However, long-term satisfaction with the sigmoid vagina remains unclear. AIM: To assess the long-term patient-reported outcomes (PROs) after SV in transgender women, and compare these outcomes with a reference group of transgender women who underwent PIV. METHODS: A single-center, cross-sectional survey study was conducted, including transgender women who underwent primary SV at least 5 years prior an

Nutritional Monitoring During Gender-Affirming Hormone Therapy: Body Composition and Metabolic Implications.

Background/Objectives: Gender-affirming hormone therapy (GAHT) is associated with clinically relevant changes in body composition, energy metabolism, and functional capacity in transgender and gender-diverse individuals. The nutritional implications of these adaptations remain insufficiently characterized, and current assessment models, largely derived from cisgender populations, may not fully capture hormone-related body composition and metabolic changes. This narrative review aims to synthesize the metabolic and body composition effects of GAHT, evaluate methodological limitations in assessi

[Diagnosis or encounter : On the medical perception of gender, transgender and nonbinarity].

This article reflects on the significance of medical perception in encounters with gender, trans identity and nonbinarity. Starting from the role of urology in the medical care of gender-diverse people, it shows that gender-affirming and gender-modifying treatments cannot be understood solely as somatic-medical or organ-focused interventions. They are part of complex developmental processes in which bodily well-being, gendered self-experience, social participation, sexuality, quality of life and other dimensions are closely intertwined. Gender is understood as a multidimensional self-experienc

Vascular Health and Gender-Affirming Hormone Therapy: The Immune System in Motion.

Given that fluctuations in sex hormone levels greatly influence cardiovascular homeostasis, this review addresses how gender-affirming hormone therapy impacts vascular function and the immune system, and how these effects may contribute to altered cardiovascular performance in transgender individuals. Gender-affirming hormone therapy alters leukocyte activation and cytokine secretion, that may impact vascular responsiveness, with distinct effects in feminizing and masculinizing regimens. Moreover, gender-affirming hormone therapy-induced changes in the gut microbiome and body composition, toge

Cross-sectional comparison of cardiometabolic markers in transgender men receiving gender-affirming hormone therapy.

OBJECTIVE: Cardiovascular diseases (CVD) are the most common cause of death in the world. A number of scoring systems have been proposed to predict CVD risk. In this study, we aimed to assess the cardiovascular risk scores of transgender men receiving gender-affirming hormone therapy (GAHT). METHODS: A total of 73 participants were included in this cross-sectional study: 43 transgender men, 15 cisgender men, and 15 cisgender women. For each participant, sociodemographic data were collected, glucose, lipid profiles and total testosterone levels were measured and the atherogenic index of plasma

The Experiences of Transgender and Gender Diverse Individuals after Intonation Training for a More Feminine-Sounding Voice: A Qualitative Study.

BACKGROUND: Transgender and gender diverse (TGD) individuals presumed male at birth often pursue voice and communication training (VCT) to develop a more feminine-sounding voice, as hormone therapy does not affect the voice. Previous research supports the integration of intonation training into gender-affirming VCT for TGD individuals seeking a more feminine-sounding voice. However, TGD individuals' experiences with intonation training for a more feminine-sounding voice remain unexplored. AIM: The aim of the present study was to explore the experiences of TGD individuals who underwent intonat

Approach to the patient: cases of hormonal management in older transgender and gender diverse adults.

As the general population continues to shift to an older demographic, more transgender and gender diverse (TGD) adults will seek out endocrinologists and other healthcare providers to help them navigate both initiation and continuation of gender-affirming hormone therapy concurrent with the potential development of age-associated diseases. The purpose of this Approach to the Patient article is a case-based approach to management considerations in older TGD adults in the settings of neurocognitive disorder, increased cardiovascular disease risk, prostate cancer, bone health, and menopause. Menopausal Considerations in Transgender and Gender-Diverse Individuals.

Assessing and treating menopause in transgender and gender diverse (TGD) individuals is of increasing interest to clinicians providing gender-affirming care. There currently exists a small body of literature that describes cardiovascular, bone health, neurocognitive, and other outcomes in the TGD population who experience loss of endogenous sex hormone production, whether through gender-affirming hormone therapy, surgical gonadectomy, or natural aging. However, the literature is currently insufficient to provide clear evidence-based guidance for the medical management of menopause in TGD indiv

Total intravenous anaesthesia (TIVA) has many advocates and offers potential benefits, yet the direct costs of this technique are commonly greater than those of inhalation-based alternatives. Although many advantages are claimed for TIVA, in reality all modern anaesthetics are effective and have good safety and tolerability profiles, rendering these differences of less importance now than was perhaps once the case. The majority of direct comparisons between inhaled and intravenous anaesthetics have failed to demonstrate significant differences in recovery times, yet they have consistently show
