Options and Indications in Gender-Affirming Phalloplasty.

Phalloplasty, once guided only by binary ideals, has evolved into a patient-centered, individualized surgical process for transmasculine individuals seeking gender-affirming surgery. Modern approaches recognize that not all patients desire a comprehensive reconstruction; many opt for variations based on personal goals and risk profiles. Consultation involves thorough assessment of anatomy, expectations, and donor site suitability. Techniques include staged approaches to urethral lengthening, shaft-only phalloplasty alternatives and diverse donor site options. Composite phalloplasty techniques

Body Contouring in Gender-Affirming Care.

Body contouring procedures in gender-affirming care is rising in popularity. These procedures help individuals achieve a body image that is more congruent with their gender identities, where the torso and lower extremities, have been cited as a source of gender dysphoria. Despite this, literature focused on non-chest body contouring in transgender patients is lacking, where current techniques are modeled after cis-gender body contouring procedures. This article explores the role of non-chest body contouring in gender affirming care, with a review of existing literature and surgical techniques.

Mental Health and Psychosocial Support During the Perioperative Period.

Gender-affirming care (GAC) encompasses a comprehensive, patient-centered approach designed to support individuals in affirming their gender identity and expression. It includes a spectrum of medical, surgical, behavioral, social, psychological, and legal interventions tailored to the unique needs of each person. The mental and behavioral health needs of transgender and gender-diverse (TGD) individuals are highly individualized, requiring clinicians to provide care that is responsive to patients' mental and medical health needs. Before addressing perioperative psychosocial concerns, it is esse

Gender-affirming genital surgery in the Nordic countries: Narrative review of current practices and challenges.

Genital gender-affirming surgery is a crucial yet often overlooked component of gender-affirming care for transgender individuals. Genital gender-affirming surgery is a rapidly evolving field, and it is not uncommon for surgeons - sometimes without prior experience in gender-affirming care - to encounter transgender individuals in clinical practice; hence, it is essential that all healthcare providers possess a foundational understanding of gender-affirming procedures and individual's needs.Research has shown that genital gender-affirming surgery significantly improves the quality of life for

Plasma proteome adaptations during feminizing gender-affirming hormone therapy.

Sex differences manifest in various traits, as well as in the risk of cardiovascular, metabolic and immunological conditions. Despite the clear physical changes induced by gender-affirming hormone therapy (GAHT), little is known about how it affects underlying physiological and biochemical processes. Here we examined plasma proteome changes over 6 months of feminizing GAHT in 40 transgender individuals treated with estradiol plus one of two antiandrogens: cyproterone acetate or spironolactone. Testosterone levels dropped markedly in the cyproterone group, but less so in those receiving spirono

Pedicled Flaps Versus Free Radial Forearm Flap for Phalloplasty in Female to Male Gender-Confirming Surgery: A Systematic Review.

BACKGROUND: The field of gender-affirming surgery is rapidly evolving, with plastic surgery units worldwide increasingly offering these services. Phalloplasty is a critical component of female-to-male (FTM) transition, providing significant psychological and functional benefits. Although robust comparative trials are scarce, expert consensus favors the radial forearm free flap (RFF) when feasible. This systematic review directly compares outcomes of pedicle anterolateral flap (pALT) and the RFF in FTM gender-affirming surgery. METHODS: A systematic review was conducted following the PRISMA gu

Mortality among transgender persons: a Taiwan matched-population and sibling-comparison cohort study.

BACKGROUND: Transgender persons have been reported to experience excess mortality, but evidence is dominated by Western cohorts and seldom addresses familial confounding. METHODS: We conducted a nationwide, retrospective cohort study using two prespecified comparison designs: matched population controls (1:4, matched on legal sex and birth date) and within-family cisgender sibling comparisons. Transgender persons were identified by ≥2 psychiatrist-recorded gender identity disorder diagnoses during 2001-2021; cohort entry required age ≥6 years. Participants were followed until death or Decembe

Sexual function and its association with psychosocial factors after penile inversion vulvo-vaginoplasty without scrotal skin grafts: a retrospective study using the oMtFSFI.

BACKGROUND: Penile inversion vulvo-vaginoplasty is the most commonly performed genital gender-affirming surgery for transfeminine individuals. Sexual function following vulvo-vaginoplasty has been primarily assessed using instruments developed and validated for cis women. This study aimed to evaluate postoperative sexual function outcomes using the only validated tool specifically designed for trans women. Additionally, potential associations with psychosocial factors were explored. METHODS: Data from trans women who underwent penile inversion vulvo-vaginoplasty without scrotal skin grafts at

Gender-affirming surgeries and patient-reported outcomes in South Korea: A multicenter cohort survey.

Gender-affirming surgeries (GASs) are becoming common in South Korea; however, patient experiences remain underexplored. This study investigated experiences of transgender and gender-diverse (TGD) individuals who underwent GAS in South Korea. This multicenter study was conducted at 8 institutions between January and October 2024. TGD individuals who underwent at least one GAS were surveyed regarding demographics, type of surgery, and patient-reported outcome measures including satisfaction and self-perceived effects of GAS using a Likert scale. Among the 844 respondents, 382 (45.3%) underwent

Triptorelin: The Need for Legal Protection Aimed at the Full Political and Social Integration of Gender Variant youth. A Comparison of Different Global realities.

BACKGROUND: Triptorelin, a GnRH analogue, is used as a puberty blocker for transgender and gender-diverse adolescents experiencing gender dysphoria (GD). This treatment allows these individuals to delay puberty, offering time to explore their gender identity. Despite its recognized benefits in alleviating psychological distress, there are concerns regarding its side effects and the adequacy of psychological support during treatment. METHODS: The article examines the use of triptorelin in treating GD, focusing on its approval, efficacy, and safety. It also explores international guidelines, th

Neuroimaging insights into transgender and gender nonconfirming youth: a scoping review.

BACKGROUND: Gender incongruence in adolescents is a relevant condition in which the gender identity of a person does not correspond with the birth-assigned sex. Individuals with gender dysphoria can experience a distressing burden. The etiology seems to be predominantly biological, but only a few neurobiological correlates have been identified and are not yet embedded in a broader context. The aim of this review is to present an overview about the current state of the literature regarding MRI brain findings in adolescents < 22 years of age with GD/GI also including non-binary identifying adole

The necessity of including transgender adolescents in developing affirming gender identity research measures.

Developing best practices for accurately ascertaining gender identities has become increasingly important, especially as more adolescents identify with transgender and nonbinary identities. The research community faces the challenge of creating inclusive and representative measures that effectively capture these identities. In collaboration with the Adolescent Medicine Trials Network for HIV Interventions (ATN), we present our youth-informed multi-step process for gender, gender identity and sex-assigned-at-birth ascertainment. We also highlight the critical need to involve adolescents directl

Gender-Affirming Genital Masculinization: Part 3 of the Plastic Surgeon's Perspective of Gender-Affirming Surgery.

Over the last few decades, great strides have been made in surgical techniques and access to care for transgender and nonbinary individuals. An important step for many transgender patients is the ability to undergo masculinizing bottom surgery. This allows improved alignment of physical appearance with gender identity and improvement in symptoms of gender incongruence. The main options involved in transmasculine bottom surgery are metoidioplasty and total phalloplasty. The surgical decision is adapted to the patient and their preferences along with the clinical opinion of the provider. While b

Free Jejunal Graft Vaginoplasty in Gender Affirmation Surgery: A Novel Technique.

BACKGROUNDS: Many techniques have been described for reconstruction of the vaginal canal in gender affirmation surgery. However, none of the existing methods is known to be ideal. To overcome disadvantages of previous methods, we developed a method of vaginoplasty using a free jejunal graft. In this study, we describe the new method in detail and report postoperative results of our case series. METHODS: Between May 2022 and July 2023, 10 transfeminine patients underwent two-stage gender-affirming vaginoplasty at Korea University Anam Hospital. The first stage involved orchiectomy, urethroplas

[Problems and Prospects of Molecular Psychogenetics in the Study of Transsexsualism].

The etiology of transsexualism (TS) remains unknown today because the disease is multifactorial and is caused by a set of factors, including those affecting sexual differentiation of brain tissue during fetal development. Sexual differentiation of the brain has been shown to occur at a much later developmental stage than sexual differentiation of the genitals, and the two processes are regulated independently of each other. Various sexual characteristics, such as gender identity (self-identification of oneself as male or female) and sexual orientation (heterosexuality, homosexuality, or bisexu

Female Autoandrophilia.

A growing body of research suggests that males can have internalized sexualities such as autogynephilia (sexual attraction to the thought or image of oneself as female) which lead to the development of trans identity. Here I present evidence that females can have analogous internalized sexualities such as autoandrophilia (sexual attraction to the thought or image of oneself as male) which similarly lead to the development of trans identity. The case for female autoandrophilia presented here uses both direct and indirect lines of evidence. Female autoandrophilia is directly evidenced by cross-g
