Syphilis Screening in Pregnancy During Emergency Department Gonorrhea and Chlamydia Testing.

INTRODUCTION: Congenital syphilis is rising in the United States, especially among infants born to women with limited prenatal care. Emergency departments (ED) offer opportunities for early detection, particularly among patients evaluated for urogenital sexually transmitted infection (STI) concerns. In October 2019, our health system implemented an ED STI order set that promoted HIV co-testing and included a preselected option for syphilis testing. In this study, we evaluated change in syphilis testing during the post-implementation period among ED encounters with both pregnancy and gonorrhea/

Congenital syphilis as a failure of the maternal-fetal prevention cascade: Clinical implications for pregnant migrant and structurally vulnerable women in high-income settings - an ESCMID study group for infections in Travellers and migrants (ESGITM) perspective.

BACKGROUND: Congenital syphilis remains a preventable but persistent cause of fetal loss, neonatal morbidity, and early-life complications. In high-income settings, its occurrence rarely reflects the absence of diagnostic tools or effective therapy; but more often exposes missed opportunities across antenatal care, sexually transmitted infection prevention, maternal treatment, partner management, and neonatal follow-up. This review investigates congenital syphilis from the viewpoint of the possible failures of the maternal-fetal prevention pathway, with a specific focus on pregnant migrant and

Temporal trends in congenital syphilis incidence per 1000 live births in the Americas.

Congenital syphilis (CS) is a significant infectious condition associated with a high social and economic burden. In the Americas, the epidemiological situation remains concerning, with varying patterns across countries. Our study evaluated temporal trends in CS incidence per 1000 live births in countries across the Americas between 2009 and 2021. An ecological study was conducted using data from the Pan American Health Organization and analyzed via the Joinpoint Regression Program and Project R. Sixteen countries were included, showing an average annual increase of 6.42% in CS incidence. Cana

Associations Between Maternal Mental Health and Congenital Syphilis Birth Outcomes: Analysis of Georgia Surveillance Data, 2018-2023.

BACKGROUND: Congenital syphilis (CS) is a preventable yet persistent public health problem in the United States. Maternal mental health conditions (MMHC) during pregnancy may contribute to missed prevention opportunities, but their association with CS outcomes is unclear. METHODS: Georgia sexually transmitted infections (STI) surveillance data (2018-2023) were analyzed to assess prebirth MMHC mentions among syphilis-affected pregnancies, their association with CS, and variation by maternal household composition as a proxy for immediate social support. MMHC mentions were identified using rule-

Syphilis Screening Among Commercially-Insured Pregnant Women in the United States.

BACKGROUND: Congenital syphilis (CS) incidence increased by 81.7% in the United States (US) between 2020 and 2024. Syphilis screening is recommended at the first prenatal visit and at delivery, but poor compliance may lead to inadequate screening and preventable CS cases. METHODS: Using a US-wide collection of longitudinal healthcare claims data, we identified a cohort of commercially-insured pregnant women aged 15-44 years. Women were included if they had a pregnancy resulting in a live birth, stillbirth, or spontaneous abortion during 2017-2021. We described patterns of syphilis screening,

Growth patterns of children with congenital Zika syndrome in Brazil: a matched-cohort study.

BACKGROUND: Children born with congenital Zika syndrome typically present with structural abnormalities and neurological impairments that can disrupt postnatal growth. We aimed to evaluate longitudinal anthropometric trajectories of children with congenital Zika syndrome compared with children without congenital Zika syndrome in Brazil. METHODS: In this matched-cohort study, we used Brazilian administrative data on singleton livebirths from Jan 1, 2015, to Dec 31, 2018, with follow-up routine primary care health data from birth to age 5 years. We excluded children with missing or inconsistent

Congenital Syphilis: Strategies for Prevention.

Once nearly eradicated, rates of syphilis among pregnant people have increased over 200% and rates of congenital syphilis have increased over 750% since the 2010s. Syphilis disproportionately affects historically marginalized populations, including racial and ethnic minorities, and those with barriers to prenatal care. Syphilis is caused by the spirochete Treponema pallidum and is spread through sexual contact or through vertical transmission in utero. Syphilis during pregnancy can lead to negative outcomes for the pregnant person, adverse pregnancy outcomes, and congenital syphilis for the ch

Maternal Health Outcomes Associated With Syphilis Infection During Pregnancy: Evidence From a Large US All-Payer Hospital-Centered Administrative Database.

BACKGROUND: Syphilis during pregnancy is associated with preventable maternal and infant harms. As syphilis rates rise in the United States, recent, nationally representative data on maternal outcomes remain limited. METHODS: A retrospective cohort study was conducted using a large all-payer administrative database to assess the prevalence of maternal syphilis, variation in screening, testing, treatment, and associations with maternal outcomes and healthcare utilization. The study included inpatient delivery hospitalizations at 20 or more weeks of gestation discharged 1 October 2019, to 31 De

An Adenoviral-Vectored Tp0326 Vaccine Elicits Robust Functional Antibodies to Prevent Treponema pallidum Dissemination in a Rabbit Model.

Vaccination can prevent congenital infections by inducing antibodies that block transplacental pathogen transmission. For syphilis, which is caused by Treponema pallidum (T. pallidum), no vaccine is yet available to halt its vertical spread to fetuses. Here, we designed an adenovirus-vectored vaccine expressing the T. pallidum outer membrane protein TP0326 and evaluated its immunogenicity and protective efficacy in animal models. The vaccine elicited robust humoral and Th2‑biased cellular immune responses in both mice and rabbits, with resulting antibodies exhibiting potent anti‑adhesion, opso

[Ultrasound diagnosis of fetal infections: current possibilities and limitations].

INTRODUCTION: Fetal infections, particularly those caused by TORCH agents (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes), may significantly affect fetal growth and development, resulting in a substantial risk of morbidity and mortality. Since infections are mostly preventable causes of congenital anomalies, early diagnosis relies on serological and molecular testing and fetal ultrasound. OBJECTIVE: The aim of this study was to assess the prevalence of fetal infections and their detectable ultrasound features. METHOD: This retrospective s

[TORCH infections during pregnancy: clinical significance, diagnosis, and treatment].

TORCH infections (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes) constitute the classic group of congenital infections that may result in severe fetal and neonatal consequences. Transplacentally transmitted infections have been a critical component of prenatal care for decades. Although screening for this group of infections has been performed in Hungary since 1971, diagnostic approaches and screening strategies have undergone significant transformation. Current international recommendations, as well as national professional guidelines, no

Epigenetic Suppression of RASAL1 by HDAC3 and Cofactor YY1 Promotes Fibroblast-Myofibroblast Transition and Renal Fibrosis.

AIM: Astronauts return to Earth with reduced red cell masses and hypovolaemia. Not surprisingly, when they stand, their heart rates may speed inordinately, their blood pressures may fall, and some may experience frank syncope. We studied autonomic function in six male astronauts (average +/- SEM age: 40 +/- 2 years) before, during, and after the 16-day Neurolab space shuttle mission. METHOD: We recorded electrocardiograms, finger photoplethysmographic arterial pressures, respiration, peroneal nerve muscle sympathetic activity, plasma noradrenaline and noradrenaline kinetics, and cardiac outpu

Functional network analysis in hepatolithiasis: identifying novel therapeutic targets through whole-exome sequencing.

BACKGROUND: Hepatolithiasis (HL) is a prevalent condition in hepatobiliary surgery, often complicated by hepatatrophia. This study aimed to identify gene mutations in HL specimens with hepatatrophia and construct a mutation landscape using whole-exome sequencing (WES). METHODS: Genetic variants and copy number variations (CNVs) were compared between HL (n = 10) and normal control (NC, n = 10) specimens, encompassing 20 participants in total, using WES data. Hub genes within HL specimens were identified via the GeneCards database. The mutation frequencies and interaction patterns of these hub
