Policies towards elimination of congenital syphilis in Latin America and the Caribbean: a regional review.

Syphilis remains an important public health challenge in the Americas, with a rising incidence among adults, including pregnant women, leading to an increased number of cases of congenital syphilis. The syphilis resurgence stems from health system gaps, behavioral changes and social determinants. A regional survey and policy review in Latin America and the Caribbean showed that most countries have national policies for syphilis screening and have made progress in the implementation of rapid tests, test-and-treat strategies, partner services, and timely treatment. However, missed opportunities

Screening of Rubella Virus, Cytomegalovirus, Hepatitis B Virus, Hepatitis C Virus, HIV, Syphilis, and Toxoplasma gondii Antibodies in Pregnant Women.

TORCH pathogens are often asymptomatic in healthy adults but can cause foetal death when transmitted during pregnancy; therefore, accurate regional data are essential for screening. This study aimed to determine first-trimester TORCH seropositivity and to inform the development of hospital-based and national screening algorithms. This study analysed test results from 7481 pregnant women aged 15-49 years who participated between January 2020 and December 2024. TORCH serological results obtained using the MAGLUMI X3/X6 system (Snibe, Shenzhen, China) were analysed with Statistical Package for th

Serologic response to VDRL in infants with congenital syphilis: ceftriaxone vs. penicillin.

OBJECTIVE: Penicillin remains the only safe and effective drug recommended for treating syphilis in pregnant women and congenital syphilis (CS). Nevertheless, alternative therapies are needed when penicillin is contraindicated, such as in allergic reactions, or unavailable due to supply limitations. This study aimed to compare the serological response to the Venereal Disease Research Laboratory (VDRL) test in infants reported with CS and treated with either ceftriaxone or penicillin. METHOD: A non-competitive cohort study was conducted in three public maternity hospitals in Fortaleza, Ceará,

Prenatal characteristics and factors contributing to congenital syphilis: A descriptive analysis of cases reported to the Canadian Paediatric Surveillance Program June 2021 through May 2023.

OBJECTIVES: The incidence of congenital syphilis (CS) in Canada increased from 2.1 to 14.5 reported confirmed cases/100,000 live births from 2017 to 2023 (from 8 to 53 cases). We aimed to document prenatal characteristics and contributing factors to CS among mothers or birthing parents (M/BP) of infants with CS in Canada. METHODS: Participants of the Canadian Paediatric Surveillance Program, which includes both paediatricians and paediatric subspecialists, were invited to report on CS cases meeting the study case definition between June 2021 and May 2023. A detailed questionnaire was complete

The neglected role of partner treatment in congenital syphilis control in Brazil: nationwide evidence from 2007 to 2023.

BACKGROUND: Congenital syphilis (CS) remains a significant public health issue in Brazil despite universal access to testing and treatment. Untreated partners can enable maternal reinfection, yet its national coverage and determinants are unclear. This study aimed to quantify the coverage of adequate partner treatment among CS cases and to identify sociodemographic and care-related factors associated with it. METHODS: Repeated cross-sectional study of all CS notifications in Brazil's Notifiable Diseases Information System (SINAN), 2007-2023. Records with missing information on partner treatme

Preventing Congenital Syphilis Through Prenatal Care: Qualitative Findings from a Pilot Clinical Program.

BACKGROUND: Congenital syphilis (CS) is highly preventable through prenatal screening and treatment. However, populations disproportionately impacted by prenatal syphilis often face barriers to access, in part due to housing instability and substance use. This study assessed Pregnancy Connections, a pilot program designed to prevent CS by offering comprehensive, patient-centered prenatal care, with added support and comprehensive case management services, for patients experiencing unstable housing and/or substance use. METHODS: This qualitative study involved interviews with 3 groups: program

Estimating State Trends in Congenital Syphilis Cases Averted in the United States, 2010-2023.

We estimated potential congenital syphilis cases averted using the national disease surveillance data and AtlasPlus data. Nationally, over 60% of potential congenital syphilis cases were averted during 2010-2023, with state variations. Despite an increase in the number of averted cases, the percentage of potential cases averted has declined over time.

Congenital Syphilis: The Need for a Simpler and More Precise Diagnosis.

BACKGROUND: Congenital syphilis is resurging in the United States, with incidence at its highest level in decades. Despite significant advances in infectious disease diagnostics, clinicians still lack a sensitive method to exclude congenital syphilis in exposed neonates, especially those who are asymptomatic. Current diagnostic strategies rely heavily on maternal treatment history, infant physical examination, and comparison of maternal and infant nontreponemal titers-components that are difficult to interpret, have limited sensitivity, and may not meaningfully alter management. As a result, m

Implementation and Innovation in Syphilis Diagnostics: Insights from Global Health Systems, Molecular Testing, and Self-Testing Strategies.

BACKGROUND: Syphilis remains a persistent global public health problem. Given that low-cost diagnostics and highly effective therapies have been available for decades, the current epidemic is driven by structural weaknesses in health systems, stigma, absence of political will and failures to integrate new technologies into routine care. METHODS: At the Shanghai 2025 Syphilis Meeting, experts highlighted the evolving landscape of syphilis diagnostics. The presentations offered an overview of our changing understanding of seronegative primary disease, reinfection patterns, and asymptomatic muco

Dissemination Approaches for Prenatal Syphilis Screening Guidelines.

Professional organizations' syphilis screening guidelines are updated to align with evidence and need. Guideline awareness, current guideline dissemination sources, and future preferred sources were investigated among prenatal providers (n = 225). Some providers lacked awareness of screening guidelines, and primary sources of guideline information were practice bulletins, professional organizations, and continuing education courses.

The Population-Level Impact of Doxycycline Postexposure Prophylaxis on Syphilis in King County, WA: An Interrupted Time Series Analysis.

BACKGROUND: An initial evaluation found that doxycycline postexposure prophylaxis (doxy-PEP) has a population-level effect on syphilis among cisgender men and transgender women who have sex with men. Whether doxy-PEP's impact is generalizable across the United States and will affect syphilis incidence among cisgender women and pregnant people-populations who seldom receive doxy-PEP-and congenital syphilis (CS) is unknown. METHODS: We used public health surveillance data from January 2017 to June 2025 to conduct an interrupted time series analysis of monthly syphilis diagnoses (all stages) ove

Treatment of Syphilis in Pregnancy and Congenital Syphilis: Current Evidence, Challenges, and Future Directions.

Syphilis remains a global public health concern, with maternal infection posing a substantial risk for congenital syphilis, a preventable condition associated with severe morbidity and mortality. Penicillin, particularly benzathine penicillin G, remains the cornerstone of treatment and the only therapy with proven efficacy in preventing vertical transmission during pregnancy. However, recurrent global shortages, limited manufacturing capacity, mislabeling of penicillin allergy, and the absence of validated alternative regimens for pregnant women and neonates threaten progress toward eliminatio

Adoption of universal prenatal syphilis rescreening recommendations across Ontario, Canada: a difference-in-differences analysis.

INTRODUCTION: Congenital syphilis rates increased sharply in Ontario, Canada, between 2018 and 2023, underscoring gaps in prenatal prevention. In response, public health units (PHUs) expanded recommendations for routine repeat syphilis screening (rescreening) during pregnancy. We examined the adoption and implementation of these rescreening recommendations and assessed their impact on prenatal syphilis screening rates. METHODS: We conducted an environmental scan of all Ontario PHUs to identify changes to local recommendations and implementation strategies for prenatal syphilis screening betwe

"Ocular syphilis with reactive arthritis mimicking rheumatoid arthritis" - A diagnostic dilemma, case report, and review of literature.

Syphilis is a great mimicker of diseases and has a broad presentation ranging from painless chancre, constitutional symptoms, adenopathy, rash, synovitis, neurological, and ocular findings. Syphilis-related bone and joint involvement is usually found in congenital form, but it can also be seen in adults with acquired syphilis as a rare sequela of infectious syphilis. We report a case of a patient diagnosed as rheumatoid arthritis, who presented to the ophthalmology outpatient department with an acute onset of black spots in his visual field, whose clinical examination was a diagnostic dilemma,
