Priorities to eliminate congenital syphilis in Australia: findings from a national roundtable.

Congenital syphilis remains a significant public health threat in Australia, warranting national efforts to reduce syphilis incidence overall, focusing on women of reproductive age. To identify priority actions to eliminate congenital syphilis, ASHM convened a national multidisciplinary roundtable with attendance from organisations, colleges, and bodies across the healthcare sector with a focus on Aboriginal and Torres Strait Islander peoples' health, sexual and reproductive health, antenatal health, primary care, pathology, and pharmacy, as well as research institutes and community organisati

Gaps in Syphilis Screening Among Pregnant Women in North Carolina.

BACKGROUND: Congenital syphilis rates have risen nationally and in North Carolina (NC). We assessed syphilis screening among pregnant women in NC. METHODS: A retrospective chart review was conducted among patients with a pregnancy episode-created at first prenatal care encounter or labor/delivery-at an NC health system from January 1, 2019 to December 31, 2023. We estimated the proportion with syphilis screening at timepoints required by NC law (before 28 weeks of gestation, 28-31 weeks of gestation, and within 1 week of delivery) using syphilis test dates and delivery dates. We estimated pre

Congenital syphilis in an infant at a Nigerian Hospital.

A 2-month-old male infant was diagnosed as having congenital syphilis at the Outpatient children's Clinic at LAUTECH, Teaching Hospital, Ogbomoso. He was delivered at a primary health center with an abnormally shaped nose and oval flesh-like swelling adjacent to the lateral wall of the left nostril. Over the first 2 months of life, he developed a slowly progressive swelling around the wrist. Despite several courses of antibiotics prescribed by several doctors at both tertiary and private hospitals, for an unknown illness, he did not get better. The mother of the infant referred her infant clos

Maternal and congenital syphilis in Fiji 2019-2022: a secondary analysis of clinical trial data.

Maternal syphilis and congenital syphilis (CS) have been increasing globally, but there are limited data from Fiji. We performed secondary analysis of clinical trial data of intrapartum azithromycin given to pregnant women to determine the prevalence of maternal syphilis and estimate the incidence of probable CS. Healthy, pregnant women at a tertiary hospital were routinely tested for syphilis, and their infants, were enrolled between July 2019 and January 2022. World Health Organisation case definitions for maternal and CS were used. Of the 2112 women enrolled, 2073 (98.2%) were tested for sy

Trends in Congenital Syphilis Cases by Maternal Country of Birth, Spain, 2016-2024.

The number of congenital syphilis cases in Spain remains low; 40 cases were confirmed during 2016-2024. However, a slight increase has been observed, particularly in children born to migrant mothers. Young maternal age, migrant status, and social disadvantages are warning signs that underscore the need to strengthen prenatal screening.

Estimating the Value of Novel Tests for Active Syphilis in Zimbabwe: How Much Overtreatment Can Be Avoided?

BACKGROUND: Syphilis diagnosis in resource-limited settings relies on syndromic management and treponemal-based antibody screening tests that cannot distinguish active infection from past-treated infection, both of which can lead to overtreatment. Point-of-care (POC) tests that detect active infection could reduce unnecessary treatment. METHODS: We developed an agent-based model of co-transmitting syphilis and human immunodeficiency virus, calibrated to Zimbabwe using population-based survey data (ZIMPHIA) and UNAIDS estimates. We modeled five scenarios involving two hypothetical new POC diag

The Potential for Combined Treponemal/Nontreponemal Rapid Point-of-Care Test and Treponema pallidum Polymerase Chain Reaction in the Diagnosis of Gestational and Congenital Syphilis in a Low-Resource, High-Prevalence Setting: Pilot Data From Malawi.

BACKGROUND: Congenital syphilis (CS) remains a major cause of stillbirth and neonatal morbidity, with an estimated 700,000 cases and 390,000 adverse birth outcomes annually. We evaluated the diagnostic utility of combined treponemal (TT) and nontreponemal (NTT) rapid point-of-care test and Treponema pallidum polymerase chain reaction (PCR) for detecting active maternal syphilis, CS, and treatment response in a high-prevalence, low-resource setting. METHODS: Secondary analyses were conducted from a prospective case-control study at Queen Elizabeth Central Hospital, Malawi. Women were recruited

Evaluation of Partner Notification Strategies to Improve Syphilis Management in Pregnancy in Blantyre, Malawi: A Mixed-Methods Study.

BACKGROUND: Congenital syphilis remains a major global health challenge, driven partly by maternal reinfection during pregnancy. Partner notification (PN) is critical to prevent reinfection and reduce mother-to-child transmission, yet evidence on its uptake and effectiveness is limited. We aimed to assess PN coverage, outcomes, and barriers among pregnant women with syphilis in Blantyre, Malawi. METHODS: We conducted a mixed-methods study at 4 primary health centers (Mpemba, Zingwangwa, South Lunzu, and Bangwe). A cross-sectional survey at delivery enrolled women with documented syphilis in t

Placenta Non Grata: A Rare Presentation of a Reemerging Disease.

BACKGROUND: Maternal syphilis rates in the United States have risen dramatically, from 87.2 per 100,000 births in 2016 to 357.9 per 100,000 births in 2024, according to Centers for Disease Control and Prevention (CDC) reports. Congenital syphilis rates have also been on the upswing, increasing from 60.0 to 109.6 per 100,000 births between 2020 and 2024, with cases on the rise for the twelfth consecutive year. This trend poses a pressing public health concern. Diagnosing syphilis, particularly in pregnancy, can be challenging due to its variable and often nonspecific presentation. Delayed recog

Detection of Congenital Syphilis via Digital PCR and Next-Generation Sequencing, Colombia.

We describe congenital syphilis in a newborn whose mother had secondary syphilis diagnosed during pregnancy and received a single penicillin dose. Molecular methods detected an extremely low Treponema pallidum load in the mother but higher load in the neonate. Our findings support using molecular methods for detecting vertical T. pallidum transmission.

Syphilis clustering among young pregnant women (18-35 years) in Kampala and Wakiso districts, Uganda.

INTRODUCTION: In Uganda, spatial distribution of syphilis varies by age, gender, and region. Identifying clusters (subsets of administrative subdivisions) with high syphilis prevalence could boost efforts to eliminate mother-to-child transmission of syphilis. We examined spatial variations and clustering of syphilis prevalence among pregnant young women aged 18-35 years in Central Uganda. METHODS: We analysed secondary data from a randomised trial that evaluated the effectiveness of three antenatal syphilis partner notification approaches (NCT02262390). This study analysed clustering of syphi

Challenges diagnosing syphilis in antenatal pacific islanders: A case of secondary syphilis.

Cases of syphilis are increasing worldwide. Missed diagnosis of syphilis can have serious consequences, particularly in pregnancy. In areas where yaws, a non-sexually transmitted infection due to Treponema pallidum sub-species pertenue (T. pertenue) is endemic, the interpretation of syphilis serology is complex. In Vanuatu, where infection with yaws is prevalent, patients may have positive treponemal serology because of the latter in childhood. We present a case of secondary syphilis with an atypical rash in a post-partum woman in Vanuatu.

Stepwise breach of maternal-fetal fortress: an adhesion-degradation-immune evasion cascade underpinning Treponema pallidum placental crossing.

Congenital syphilis, a severe maternal-fetal disease caused by vertical transmission of Treponema pallidum (T. pallidum) via the placenta, affects over one million pregnancies annually and remains a leading cause of preventable adverse fetal outcomes. While the clinical consequences are well recognized, the molecular mechanisms underlying placental penetration are incompletely understood. This review synthesizes current evidence into a unified conceptual framework known as the adhesion-degradation-immune evasion cascade. Established findings, supported by direct placental studies, indicate tha

Assessing progress towards the elimination of mother-to-child transmission of syphilis in Peru: A national retrospective analysis.

To eliminate and control the mother-to-child transmission (MTCT) of syphilis, the World Health Organization (WHO) recommends meeting the following goals: (1) reducing congenital syphilis incidence to ≤ 0.5 cases per 1000 live births, (2) 90% reduction in maternal syphilis incidence between 2018 and 2030, and (3) 95% coverage of syphilis screening during pregnancy. We evaluated the progress in the Peruvian context at the national and departmental levels. A retrospective ecological analysis was conducted using national secondary data on reported cases of congenital and maternal syphilis from 201
