Comparison of simulation training on self and using a doll for teaching generalized menstrual care to women with severe mental retardation.

This study examined the effects of two simulation methods of general case instruction in teaching generalized menstrual care; long-term maintenance was assessed up to 18 months. Within a split multiple-baseline design across subjects, three middle-school students and one adult woman with severe or profound mental retardation initially were assigned either to on-self instruction or to instruction using a doll. The two learners receiving doll training also participated in instruction on self. A modified forward chaining procedure with identical task analysis steps was used with each method. The

Assessing food preferences among persons with profound mental retardation: providing opportunities to make choices.

Increased attention has been directed recently to assisting persons with severe handicaps to express preferences concerning events in their lives. We evaluated a program for assessing choice-making skills to provide opportunities for persons with profound mental retardation to express food and drink preferences. In Experiment 1, the assessment procedure involving repeated, paired-item presentations resulted in active choice making and the identification of preferences for all 5 participants. Results also indicated that caregiver opinion was not predictive of participant food and drink preferen

Rett syndrome: a longitudinal developmental case report.

Rett syndrome is a recently described progressive neurological disorder of unknown etiology occurring only in females, causing severe to profound mental retardation and characterized by loss of purposeful hand use and stereotypic hand movements. The present study examined development in five areas: gross motor skills, fine motor skills, self-help skills, communication, and cognition. Results indicated a general stagnation in all developmental areas beginning at approximately 15 months. No skills progressed beyond the 2-year level; this, despite several years of intensive, interdisciplinary int

The syndromes described by Kanner and Rett-Hagberg: overlap in an extended family.

An extended family is reported in which Rett-Hagberg syndrome, autism with mild ataxia and autism with profound mental retardation occurred separately in three female second-cousins. The authors suggest alternative explanations for the possible association between autism and Rett-Hagberg syndrome: that they may be closely related genetic disorders, or that they may constitute relatively homogeneous phenotypes with several possible aetiologies.

Naltrexone attenuates self-injurious behavior in mentally retarded subjects.

The effect of naltrexone on the frequency of self-injurious behavior (SIB) was investigated in 6 male subjects with profound mental retardation. Following a double-blind placebo-controlled crossover design, naltrexone was administered in a dose of 50 mg once daily for 3 consecutive weeks. In 2 of 5 subjects, a significant decrease of SIB frequency could be demonstrated, and in 1, a tendency to a reduction was found. No effect on duration of restrain time was found in 3 subjects. These data suggest that disturbances of the endogenous opioid systems may be involved in the pathophysiology of SIB

Autism, profound mental retardation and atypical bipolar disorder in a 33-year-old female with a deletion of 15q12.

A case report of a 33-year-old woman with atypical bipolar disorder, autistic disorder, profound mental retardation and a chromosomal anomaly is presented. This patient is behaviourally similar to a patient reported by Akuffo et al. (1986), who apparently did not have a chromosomal investigation. The chromosomal anomaly and similarities between the two patients are discussed.

We report clinical features and course of Lowe's syndrome with regard to three cases. All of them are males and clear inherited transmission was demonstrated in patients 2 and 3 and was suggested in patient 1. Age at the moment of diagnosis oscillated between 7 and 18 years. The three cases showed weight and height percentiles under p 3. Congenital bilateral cataract and search nystagmus were found in all of them. Profound mental retardation, muscular hypotonicity and diminished or absent tendon reflexes constituted distinctive findings in the neurological area. Among renal manifestations stoo

A prevalence study of Rett syndrome in an institutionalized population.

An institution for the mentally retarded was surveyed to determine the prevalence rate of Rett syndrome (RS). Four patients with definite RS and one with probable RS were identified in a population of 350, yielding a prevalence rate of 1 in 87, about 1% of institutionalized male and female patients with mental retardation. In this population of patients with severe and profound mental retardation (N = 297), 138 females were surveyed, suggesting a prevalence rate among females of 1 in 34 in an institutional population of persons with mental retardation. Surveys of institutions for persons with

Perception of figure orientation by nonambulatory children with profound mental retardation.

An habituation-dishabituation procedure was used to study perception of changes in figure orientation by nonambulatory children with profound mental retardation. On each of 3 days, 16 participants were given familiarization trials with either a vertical or an horizontal pattern, followed by test trials with the familiarized stimulus and one of three novel stimuli--a 90 degree or 45 degree rotation of the pattern, or a rearrangement of the pattern elements into a square. Visual fixation times decreased over familiarization trials. Fixation times on test trials were longer for novel than for fam

[A clinical study of death in profound mental retardation with motor disturbance].

Among 848 cases of profound mental retardation with motor disturbance admitted to Metropolitan Medical Center of Severely Handicapped in the last 20 years, 98 died. The 94 cases whose cause of death was determined were clinically investigated. There was no difference in sex, and 72% of the patients died before the age of 15 years. Half of the patients died of pneumonia; sudden death occurred in 9, and ileus in 8. These three were thought to be the most important and characteristic causes of death in severely handicapped patients. In recent years, deaths due to pneumonia have decreased and thos

The treatment of elopement behavior in a retardate using a graduated levels program.

The present study investigated whether a levels program would effectively reduce the amount of elopement (running-away) behavior demonstrated by an adult with profound mental retardation. The subject resided in an Alternative Intermediate Services (AIS) group home. During baseline data collection, he demonstrated an average of 131 elopements per month. After implementation of the levels program, elopements decreased to 0 within 7 months. At the end of the study, the subject's elopements continued to be maintained at a very low rate.

Health care financing for severe developmental disabilities.

The 1985-86 data from 308 children and young adults under age 25 with autism and from 326 with severe or profound mental retardation can be compared to national data from the 1980 MNCUES and the 1987 NMES because the methods are similar. These data provide detailed answers to the questions, what health care services are used? what are the expenses? Who pays them? Until now, the absence of comprehensive national data had hindered the development of new approaches to financing the care of children with serious, lifelong conditions. These data permit policymakers to take into account the needs an

Children with self-injurious behavior.

Self-injurious behavior is a serious problem that is not uncommon among individuals with mental retardation. Medical and developmental characteristics of 97 children, adolescents, and young adults (age range 11 months to 21 years, 11 months) assessed and treated for self-injurious behavior in a specialized, interdisciplinary inpatient unit between 1980 and 1988 were reviewed. This population differed from those reported in previous studies in that it was of school age and predominantly community based. Severe or profound mental retardation was present in 82.5% of our patients. The causative di

Anophthalmia in a retarded girl with partial trisomy 4p and 22 following a maternal translocation, rcp(4;22)(p15.2;q11.2).

An 18-year-old girl displayed left anophthalmia and right severe microphthalmia, mild dysmorphic features of facies and distal limbs, a right preauricular pit, and moderate mental retardation. She was trisomic for the distal part of the short arm of chromosome 4 and the proximal segment of chromosome 22, due to unbalanced 3:1 segregation of a maternal 4;22-translocation. Anophthalmia is a rare finding in chromosome aberrations in general, and particularly in patients not featuring other severe malformations and severe to profound mental retardation.

Building response classes: a comparison of two procedures for teaching generalized pouring to learners with severe disabilities.

Five individuals with profound mental retardation received instruction on liquid pouring. The effects of two training strategies on performance with nontrained pitchers and receptacles were compared within a split multiple baseline design across subjects. One training approach followed general case instruction guidelines in which both difficult and easy teaching examples were selected that sampled the range of relevant stimulus and response variation. The second training approach approximated the early steps in an easy-to-hard sequencing strategy and involved using only a set of easy teaching

Sulfite oxidase deficiency: clinical, neuroradiologic, and biochemical features in two new patients.

Sulfite oxidase deficiency is characterized by severe neurologic dysfunction, dislocation of the lenses, and the accumulation and excretion of inorganic sulfite, thiosulfate, and S-sulfocysteine. We present the clinical, radiologic, and biochemical findings in two patients with this condition. In both, neurologic problems started soon after birth and progressed rapidly to profound mental retardation, microcephaly, blindness, and spastic quadriparesis. Seizures were a persistent problem throughout the course of their illness. The neurologic abnormalities were associated with progressive destruc

Misleading cues in the diagnosis of mental retardation and infantile autism in the preschool child.

The diagnosis of behavioral and cognitive disorders in preschool children is difficult. Infantile austism is more likely to be diagnosed than is mental retardation. Yet most children with infantile autism are also mentally retarded, and many of those with severe and profound mental retardation show autistic behavior. Factors misleading professionals into overlooking mental retardation when assessing preschool children were discussed, and essential components in the management of preschool children with behavioral, developmental, or cognitive deviations were examined.
