Variables associated with frequency of rumination in a boy with profound mental retardation.

The relationships between frequency of rumination in a boy with profound mental retardation and a variety of environmental, interpersonal, and temporal variables were investigated by collecting and analyzing data during all waking hours over a 4-week period. Low levels of rumination were associated with periods of special education programming (versus nonschool hours), individual attention (versus group activities and independent play), and time spent with caretakers who like the child (versus those who like him less). The findings also revealed a mealtime effect (decreasing rumination as time

Pupillary responses in recognizing awareness in persons with profound mental retardation.

Pupil-size changes have been shown to indicate positive or negative reactions to several sensory and psychologic stimuli in normal persons. Whether this could be extended to mentally retarded patients who lack ability for verbal or motor behavior was tested by pupillary responses to several visual, tactile, and auditory stimuli. 24 institutionalized severely and profoundly retarded patients were examined by video camera in their usual environments rather than in a controlled laboratory setting. The videotapes were later projected onto a screen for pupil-size measurements. Pupils dilated with p

Trisomy 18 at age 21 years.

Here we report on a woman with non-mosaic trisomy 18, who is now 21 years old. She has the characteristic face of older trisomy 18 survivors, marked kyphoscoliosis, spastic quadriplegia with contractures, and profound mental retardation. She has pyorrhea with loss of three canine teeth. At 20.5 years, she had her first menstrual cycle with development of secondary sexual characteristics.

Abuse and neglect in psychiatrically hospitalized multihandicapped children.

Medical charts of 150 consecutive admissions of multihandicapped children to a psychiatric hospital were examined to determine the extent and characteristics of abuse and neglect. Results indicated that 39% of the sample experienced or had a history that warranted suspicion of past and/or current maltreatment. Physical abuse was the most frequent type of maltreatment, followed by neglect and sexual abuse. Maltreated multihandicapped patients admitted to the psychiatric unit were less likely to receive diagnoses of organic brain syndrome or profound mental retardation than nonmaltreated multiha

Porencephaly and hydranencephaly: a neuropathological study of four autopsy cases.

Four autopsy cases of porencephaly and hydranencephaly were evaluated clinico-pathologically. The patients showed profound mental retardation, convulsive seizures and spastic quadriplegia. The life spans were much shorter in the hydranencephalic cases. Pathologically, the cerebral cortex in the vicinity of the parenchymal defects often showed an abnormal cytoarchitecture, which appeared to be closely correlated with superficial glial proliferation. Several neurofibrillary tangles were found in the nucleus basalis of Meynert and the locus ceruleus in a 23-year-old porencephalic patient, and so-

Neurosonographic features of Aicardi's syndrome.

Aicardi's syndrome occurs in females and is associated with profound mental retardation, agenesis of the corpus callosum, chorioretinal lacunae, and infantile spasms. We report a distinctive combination of abnormalities which was observed on real-time cranial ultrasound scans of two infants with Aicardi's syndrome. These abnormalities include bilateral prominent cysts in the choroid plexus in conjunction with agenesis of the corpus callosum. Recognition of these radiologic findings may suggest the diagnosis of Aicardi's syndrome in early infancy even prior to the onset of seizures or visualiza

Growth in congenital rubella syndrome and correlation with clinical manifestations.

Growth patterns, final stature, and clinical manifestations were studied in a review of the records of 105 patients with congenital rubella syndrome followed longitudinally. Of the patients (35 male, 51 female), 86 had achieved final heights. Three patterns of growth were observed: normal growth, growth consistently below the 5th percentile, and growth within the normal range or slightly below the 5th percentile followed by early cessation of growth and final height usually below the 5th percentile. Significant cognitive deficits were not observed in patients with normal growth patterns, excep

Provision of services for mentally handicapped children in the Witwatersrand area.

Childhood mental retardation is defined as significantly subaverage general intellectual functioning existing together with difficulty in adaptive behaviour, manifesting during the period from birth to 18 years. We addressed the issues of diagnosis, placement and management of the mentally handicapped child in the Witwatersrand area and surveyed facilities available for them. According to our calculations, facilities were adequate for 69.7% white, 26% Indian, 9.3% coloured and only 7.6% black children. Urgent attention must be given to providing more care for these children, especially those w

The reliability of the Motor Development Checklist.

A study was conducted to determine the interrater and test-retest reliability of the Motor Development Checklist (Doudlah, 1976). Thirty-six subjects with diagnosed multiple disabilities and severe and profound mental retardation were evaluated by two experienced occupational therapists. Reliability was established by means of the intraclass correlation approach, and results indicated that coefficients ranged from .93 to .99. The study results suggest that the Motor Development Checklist, when administered by experienced raters, can provide consistent information regarding the spontaneous moto

Prenatal exposure to ethanol decreases physiological plasticity in the hippocampus of the adult rat.

Prenatal exposure to ethanol has been associated with birth defects ranging in severity from physical dysmorphias and profound mental retardation to more subtle compromises of cognitive and behavioral function. Recent evidence has shown the hippocampus to be damaged both morphologically and neurochemically after such exposure in experimental animals. The functional implications of these changes have just recently begun to be addressed. We now report that long-term potentiation and potassium-induced excitability are decreased in hippocampal slices from adult animals exposed to ethanol in-utero.

Special oral concerns in people with severe and profound mental retardation.

Dental patients with severe and profound mental impairments may engage in behaviors that adversely affect their oral health. Based on experience in a residential care facility, this report sensitizes dental practitioners, who are unfamiliar with the problems of these patients, to consequences of these behaviors.

Childhood Onset Pervasive Developmental Disorder.

Two male children meeting criteria for Childhood Onset Pervasive Developmental Disorder (COPDD) are described. The current DSM-III category of COPDD may have value in separating these children from others with PDD. The authors suggest that these two children, and other children described in the literature as having dementia infantalis and/or disintegrative psychosis, have a distintegrative disorder resulting in muteness, profound mental retardation and severe autistic symptomatology. The term "pervasive disintegrative disorder" may be appropriate for such children and specific diagnostic crite

External ventricular drainage for initial treatment of neonatal posthemorrhagic hydrocephalus: surgical and neurodevelopmental outcome.

Thirty-seven infants were treated for posthemorrhagic hydrocephalus (PHH) with external ventricular drainage (EVD) in 1977-1985. The mean age at EVD placement was 13 days; the mean duration of drainage was 21 days, and an average of 1.8 drains was used per patient. Complications were apnea (10%), hemorrhage (8%) and ventriculitis (6%). Ten infants died from pulmonary disease unrelated to the surgery. Thirty-two percent of the survivors did not require a permanent shunt. Neurodevelopmental outcome was evaluated at a mean age of 38 months. Eighteen of the twenty-seven survivors (67%) had normal

Adrenocortical function and depressive illness in mentally retarded patients.

The authors administered a 1-mg dexamethasone suppression test (DST) to 85 institutionalized adults with mild to profound mental retardation after screening to exclude false-positive nonsuppression. Thirty-one (36%) of these subjects had baseline hypercortisolemia, which was significantly correlated with age, symptoms, and "modified" DSM-III criteria for major depressive disorder. Twenty (24%) of the 85 subjects were nonsuppressors (5 micrograms/dl) after testing; nonsuppression was significantly related to age, female sex, level of retardation, symptoms, and "modified" DSM-III criteria for ma

Handedness distribution in a residential population with severe or profound mental retardation.

Handedness within a sample of mentally retarded subjects was assessed using a procedure that includes a large number of items appropriate for lower functioning subjects, with multiple presentations within and between sessions one week apart. Results revealed a dramatic shift from the normal right-handed bias primarily due to the presence of a large mixed-handedness subtype, which proved to be ambiguously handed in that these subjects showed inconsistent hand preference within items. Reasons why this subtype has not been reported previously were discussed, and a model was postulated to explain
