Fifty-five preschool children with Down's syndrome were assessed using the Griffiths Scale of Mental Development. They were all the children with Down's syndrome living in Cape Town who spoke English or Afrikaans as their home language. There was a significant decrease in developmental quotient with increasing age and the hearing and speech subscore was lower than the other subscores. No significant associations were found between maternal age, sex, social class or race of the child and development. There was a negative association between developmental quotient and the number of siblings. The

Language deficits and developmental delays may be helpful clues in the diagnosis of child abuse and neglect. Speech-language assessment of 27 children burned through abuse or neglect identified 22 (81%) with language delays. Other significant trends included a decreased attention span, inappropriate affect, irritability, and withdrawn behavior. We recommend speech-language assessment for all burned children suspected to be victims of abuse or neglect as part of the abuse team investigation and to identify children who may benefit from intervention.

A 2.5-year-old girl presented with severely delayed speech development and mild motor retardation. Urinary organic acid analysis showed the presence of 4-hydroxybutyric acid and other metabolites consistent with the diagnosis 4-hydroxybutyric aciduria. Succinic semialdehyde dehydrogenase activity was absent in lymphocyte lysates. The clinical symptoms in this case were unusually mild compared to previously reported patients. No correlation was found between the mild symptoms and the levels of metabolite excretion or the residual succinic semialdehyde dehydrogenase activity in this patient comp

Hypertrichosis is an unusual but well-recognized genetic condition. Hypertrichosis may be generalized or limited to specific body areas, in which case it is usually not associated with other anomalies. Five previous cases of hypertrichosis cubiti have been reported, with short stature in 2 sibs being the only other associated abnormalities. We report on a child with hairy elbows, developmental delay, facial asymmetry, and delayed speech with normal parents. Our patient may represent severe expression of the hairy elbow syndrome or constitute a previously unrecognized syndrome.

Developmental expressive language disorder is a frequently occurring condition in children, characterized by severe delay in the development of expressive language compared with receptive language and cognitive skills. Opinions differ regarding whether expressive language delay is a disorder worthy of active intervention or an indication of normal variation in the onset of expressive language. The purpose of this research was to follow for 5 months 26 2-year-old children in whom expressive language disorder had been carefully diagnosed to discover the rate of improvement and its predictors. The association of behavior problems with preschool language disorders has been documented extensively. However, researchers have typically failed to differentiate subgroups of language-impaired children, to use observational data in documenting the behavior disorders, or to study children at the youngest ages. Using a multimodal assessment, this study examined parent-child interaction and behavior problems in a clearly defined subgroup of language-impaired children, those with developmental expressive language disorder (ELD). These children exhibit a delay in expressive language compared with

Autism is a severe form of childhood psychopathology first described by Leo Kanner in 1943. While over the years there has been substantial controversy about many features of the syndrome, there is today some consensus as to the behavioral characteristics associated with the diagnosis. These include onset of the disorder in the early preschool years, severe and pervasive deficits in social behavior and attachments, deficits in speech and language, insistence for the preservation of sameness, unusual responsiveness to the sensory environment, self-stimulation, self-injurious behavior, isolated

After a discussion of the changes of the MCD concept over the years, the authors describe their own investigation of a total sample of 3280 children, attending outpatient and inpatient facilities of three counties with a total population of 574,000 inhabitants. The aim of the study was to investigate the frequency, sex distribution, and social class dependency of the MCD-diagnosis and its relationship to clinical-psychiatric syndromes. The following results were obtained: There is a strong correlation of MCD-diagnosis with age, sex, and social class. MCD was most frequently diagnosed in the ag

Fragile X syndrome is a newly recognized X-linked disorder which has been associated with a high prevalence of psychiatric disturbance, particularly attention deficit disorder and autism. The present study involved the neuropsychiatric evaluation of 14 males with the disorder who were between the ages of 3 to 27 years. Pervasive hyperactivity, impulsivity, and attentional deficits were found among all of the subjects, while a significant degree of anxiety was manifested by more than half. Although the majority of subjects exhibited poor eye contact, atypical speech and language functioning, an

The communicative behavior of autistic and developmental language delay (DLD) children matched for nonverbal mental age and mean length of utterance (MLU) was compared to that of normally-developing (ND) 2-yr-olds. Autistic children were less able than other children to respond correctly to language or gestures used to direct their attention, used attention directing (pointing, showing) less frequently, and produced more echolalic speech when making requests. DLD and ND children differed only in number of pronouns used while requesting. Attention-directing gestures were associated with recepti

In a videotaped free-play session with a parent, autistic children were compared with mental-age matched Developmental Language Delay (DLD) children and with normally developing (ND) 2-year-olds in the use of communicative acts by parent and child. Groups were matched for language level. Autistic children had more incidents of no responses, produced less affirming, turn-taking vocalization, and gesture, and were less likely to initiate communication than other children. Parent groups differed only in a greater amount of initiating and use of imperatives by parents of autistic children. Few rel

Developmental dyslexics, selected on the basis of very slow naming rates on the Rapid Automatic Naming Tasks (RAN), were compared to normal readers on oral language, picture categorization, and reading tasks. Findings indicated that the dyslexics' word retrieval deficits were one symptom of a more generalized, however subtle, oral language deficit which involved both receptive and expressive oral language functioning. The dyslexics' word retrieval problem also seemed chiefly related to language processing and not to deficits in semantic memory as there were no significant differences between d

Ten subjects exhibited acquired stuttering that had persisted for 10 to 15 years following penetrating missile wounds sustained during the Vietnam War. None had a history of developmental or chronic adult stuttering. In comparison with other head-injured subjects and normal control subjects, the subjects with acquired stuttering had significant deficits in skilled rapid hand movements and oral and speech movements, suggesting a motor control disorder. The identified brain lesions of the acquired stuttering group were on the right in 5 subjects, on the left in 4, and bilateral in 1. The interna

This paper reports on the correlates of psychiatric illness in children with speech and language disorders. Of 600 children selected from a community speech clinic, 50% were found to have diagnosable psychiatric disorders according to DSM-III criteria. These "psychiatrically ill" children were compared to the "psychiatrically well" children in the sample on a variety of developmental, socioeconomic, medical, and psychosocial factors. Although some differences were found between the well and ill children in other factors, the majority of the differences, and the most highly significant differen

Delay in language development may be associated with an underlying anatomical, neurosensory, or psychological disorder such as: deafness, cerebral palsy, cleft palate, autism, or mental retardation. A condition called specific developmental language delay may occur in children devoid of any other identifiable disorder or developmental delay. Language delay associated with early onset, severe-to-profound hearing impairment has been well documented. Controversial studies have also appeared in the communicative disorders' literature suggesting that fluctuating conductive hearing loss in early chi

The records of all cases of congenital ocular motor apraxia diagnosed by the Ophthalmology Department at the Children's Hospital, Boston, over the past 25 years were reviewed to ascertain other problems with motor organization. There were eight boys and two girls, ranging in age from eight months to 2 1/2 years at diagnosis. Eight of the 10 had evidence of oral motor planning problems which significantly affected their speech output. Five had evidence of awkwardness or clumsiness, suggesting difficulties with gross motor organization as well. This concurrence of motor planning problems in at l

The diagnosis of muscular dystrophy is usually made on the basis of gait difficulties or a history of late achievement of normal motor milestones. Other well-described delays, notably in the area of expressive language skills, can provide additional clues to this diagnosis. We present three patients referred to The Developmental Evaluation Clinic because of problems with expressive language observed in school, who ultimately were found to have muscular dystrophy. They are presented to underscore the association of expressive language delay in patients with muscular dystrophy, and to stress the

This study concerns the results obtained in respect of early morbidity and late development of 115 and 57 children, respectively, born between 1978 and 1983, who had been delivered by primary low cervical Caesarean section shortly before term. Early morbidity of the 115 children was analysed taking into consideration the risk factors, such as premature rupture, gestation diabetes, EPH gestosis, condition following Caesarean section, abnormal amnioscopic and antepartal cardiotocographic findings, as well as the methods of anaesthesia employed. In the study on late development 57 children betwee

The evidence in this study appears to indicate that adopting a goal-planning approach can provide demonstrable and clear improvements in the developmental functioning of multiply handicapped blind children. However, while the improvements in functioning for self-help and sensorimotor abilities are quite significant, those for communication and language skills are much less so. Three possible hypotheses have been advanced as to why this should be so. The hypothesis that the setting might be an institutionalizing one was rejected on the grounds that some of the evidence did not fit with that fro
