The identification and measurement of hearing loss in infants and young children is necessary to implement appropriate medical treatment, to diagnose other conditions that may have hearing loss as an associated sign, and to implement habilitative measures that should prevent the delay of speech and language acquisition, psychosocial developmental problems and educational handicaps. Testing hearing in the 0 to 5-year-old age group is difficult but not impossible. Technological advances of the past 15 years have made reliable hearing test results rather common even with very young infants. The p

Because of the relative frequency of speech/language delay, all infants and preschool children should undergo routine language screening as part of health care maintenance. Diagnostic evaluation of the child with speech or language delay should answer the following questions: What is the child's descriptive diagnosis (eg, hearing impaired, mentally retarded, DLD, etc.)? What is the child's etiologic diagnosis (eg, congenital viral infection, single gene disorder, birth asphyxia, etc.)? What is the appropriate intervention strategy (amplification, orally based speech therapy, total communicatio

The prospective study presented here describes the results of follow-up examinations of the developmental stage of 116 children born between 1977 and 1980 in whom abdominal version of the foetus from breech presentation to vertex presentation was performed near term. The children were examined according to the Denver Developmental Screening Test at the age of 2 to 5 3/4 years in various fields (social contacts, minor motor functions and adaptation, speech and major motor functions). Children with abnormal findings were subjected to special examinations. Those with disturbed development were re

Phonological error patterns are analyzed in a group of 10 children presenting symptoms consistent with Developmental Verbal Dyspraxia. Results indicate a dominance of "sequentially constrained" errors primarily involving sound and syllable omissions and timing errors. Rank-order correlations among the phonological errors and between phonological errors and developmental indices suggest that these children have a specific expressive language problem dominated by phonological errors of sequential reduction. Interpretation of the data points to a central motor planning deficit. Comparisons are ma

A new instrument for the detection of the postauricular myogenic (PAM) response was used to test the hearing of 106 infants weighing less than or equal to 1,500 g when they were aged 1 to 21 months. Eighty-eight infants showed a positive response at 60 dB hearing level (HL) (normal). The other 18 did not respond; four were found to have sensory neural hearing loss and another six had conductive loss due to secretory otitis media. Of the 106 children, 90 aged 2 years or more (mean 27 months) were living in the United Kingdom, and their language development was assessed. It was normal in 67/75 c

Detection of congenital or infantile hearing loss is often delayed in otherwise normal children, but even greater delays occur in the multihandicapped. Neonatal hearing screening and high risk register techniques have not had universal acceptance. A collaborative project between medical school otology and audiology personnel and a school for multihandicapped children under the age of 3 years in the Department of Special Education in another university has significantly reduced detection time and provided speech, language and other remedial therapy while otologic diagnosis and educational funct

This paper is a comprehensive review of known examples of genetically influenced learning disabilities (LDs) and speech and language disorders (SLDs). The review is divided between 2 broad classes of studies: (a) those which begin with an LD or SLD phenotype that appears to be familial and attempt to learn more about the specifics of genetic transmission, if any; and (b) those which begin with a group of individuals, all of whom share a given documented genetic risk factor, to see if it leads to a specific LD or SLD. Included in the first category are familial dyslexia, stuttering, and other s

Children with recurrent physical illnesses such as infections of the ears and chest are more likely than average to have developmental and behavioural disorders as well. Specialist skills are needed for the assessment and management of these disorders; the belief that "the child will grow out of it" is rarely correct. Any reorganisation of the child health services should take account of the need for children with developmental and behavioural disorders to be treated within the community by doctors and other health workers with the appropriate skills.

The present paper reviews, outlines, and explores the literature concerned with the speech-language disorder of childhood autism. The problems of communication of the autistic child are compared and contrasted with those of children with a variety of other language disorders (e.g. receptive developmental dysphasia). Based upon the review of normal preverbal communicatory and symbolic development, it is concluded that childhood autism involves a pervasive language disorder encompassing communication in general, as well as certain symbolic-representational abilities necessary for language. We present a view of language that crosses modal considerations (e.g., speech vs. augmentative systems) and places language within an interaction framework. We emphasize the need to consider normal social, cognitive, and linguistic development in selecting program guidelines for developmentally delayed persons. We address the child's linguistic code not as a set of phonetic, syntactic, and semantic features that can be trained in isolation, but as a means by which he can exercise the various pragmatic uses of communication. In effect, our interest has thus expanded from the child alone to the

The relationship between language comprehension and production was examined with two severely language-deficient retarded subjects who were taught the plural form /z/. Comprehension and production were trained concurrently with different words in each mode until correct plural use occurred in the untrained mode for each word. Subsequent training of reversed plural use in comprehension for one subject and production for the other failed to result in reversed plural use in the untrained modes for those words. This mode-independence contrasted with interdependence displayed by normal children in

This paper describes the existence of severe developmental speech dyspraxia in a member of a family that is typical of other reports of X-linked mental retardation without physical abnormality (Renpenning syndrome). Other family members have evidence of motor or speech dyspraxia. Other reports of X-linked mental retardation have mentioned "verbal disability", which suggests that developmental dyspraxia may be quite common in these families. Developmental dyspraxia of speech is amenable to specific types of intervention: after two years of such intervention our patient's adaptive skills reflect

The aim of the study was to determine the effect of rehabilitation of feeding function on eye/hand coordination and speech development in cerebral palsied children. The analysis included 28 children with tetraplegic form of cerebral palsy. Age of these children was from 12 to 48 months (in one case 6 months, the other case 6 years). In the first examination, standard deviations in independent feeding function (according to "feeding test") were established as well as the level of eye/hand coordination and speech development (according to Denver Developmental Screening Test). All cerebral palsie

The expertise of speech pathologists and occupational therapists was combined to compare the relative effectiveness of an operant approach, a modified sensory integrative approach, and a combination of both methods in eliciting vocalization on other developmental skills on a sample of 27 profoundly retarded, minimally vocal, institutionalized adults. The combined results indicated that the therapy programs promoted significant gains in frequency of eye contact, frequency of vocalization, and quality of postural adaptation. There were no differences in the effects of the respective therapies. T

A developmental examination was carried out on 168 children at intervals over a 3-year period. The results suggest that developmental screening of this type is effective in picking out the "poor performers" who might well benefit from preschool education and extra health visitor supervision. On the other hand, the test given to children under the age of one year failed to pick out 2 children who were found to be moderately handicapped at the time of the 3-year examination. The study under-lined some of the problems of universal screening which require further research, and the need to assess t

This paper surveys acoustic studies of speech development and discusses the data with respect to the anatomical and neuromuscular maturation of the speech mechanism. The acoustic data on various aspects of speech production indicate that the accuracy of motor control improves with age until adult-like performance is achieved at about 11 or 12 years, somewhat after the age at which speech sound acquisition usually is judged to be complete. Other topics of discussion are (1) problems in the spectrographic analysis of children's speech, (2) formant scale factors that relate children's and adults'
