Congenital Syphilis: Strategies for Prevention.

Once nearly eradicated, rates of syphilis among pregnant people have increased over 200% and rates of congenital syphilis have increased over 750% since the 2010s. Syphilis disproportionately affects historically marginalized populations, including racial and ethnic minorities, and those with barriers to prenatal care. Syphilis is caused by the spirochete Treponema pallidum and is spread through sexual contact or through vertical transmission in utero. Syphilis during pregnancy can lead to negative outcomes for the pregnant person, adverse pregnancy outcomes, and congenital syphilis for the ch

Epidemiological and clinical characteristics of gestational and congenital syphilis in Hungary: A cross-sectional, single-center case analysis.

BackgroundIn recent years, gestational and congenital syphilis have re-emerged as significant public health concerns in Hungary. Although national surveillance data are available for syphilis, they do not include gestational cases, and recent studies describing the epidemiological and clinical characteristics of gestational and congenital syphilis remain limited. The aim of the study was to analyze 10 years of data to evaluate the clinical characteristics and outcomes of gestational and congenital syphilis and to identify gaps in prenatal screening and care.MethodsWe conducted a cross-sectiona

Trends of syphilis before, during, and after the COVID-19 pandemic in central India: A retrospective study.

BACKGROUND: Syphilis is a sexually transmitted disease that varies in presentation according to the stage of the disease. Susceptibility to penicillin contributed to the near elimination of the disease during the 1990s to 2000 period. Recently, there have been reports of the re-emergence of syphilis from all parts of the world. AIM: To study the clinical presentation and trends of syphilis. MATERIALS AND METHODS: A retrospective study was conducted from January 2013 to December 2024. All clinically and/or serologically diagnosed cases of syphilis were included in the study. Spatial Distribution and Epidemiological Characterization of Congenital Syphilis in Monterrey, Mexico, 2016-2024.

Congenital syphilis is a preventable infectious disease that is rising in low- and middle-income countries. The CDC classifies congenital syphilis into four clinical scenarios to guide diagnosis and treatment: Scenario 1, proven or highly probable congenital syphilis; Scenario 2, possible congenital syphilis; Scenario 3, less likely congenital syphilis; and Scenario 4, unlikely congenital syphilis. Geolocation is crucial for analyzing the distribution of infectious diseases by pinpointing high-prevalence areas and enabling targeted interventions. This retrospective observational study analyzed

Advances in Syphilis Diagnostics to Address the 21st-Century Epidemic.

BACKGROUND: For 20 years, the United States has experienced a progressive rise in syphilis cases, increasing to >209 000 cases in 2023. The current case rate of 62.5 per 100 000 persons represents a 30-fold increase since 2000-2001, when a concerted national effort nearly eliminated the disease. This review highlights epidemiologic trends, current gold-standard diagnostic methods, emerging diagnostic tests, and key challenges for laboratorians. CONTENT: The current outbreak in the United States has been marked by dramatic increases in cases among women and men who have sex with women, resulti

A case report of congenital pemphigus syphiliticus in a preterm neonate: Insights into diagnosis, treatment, and long-term follow-up in a resource-limited country (Cambodia).

Congenital syphilis remains a preventable cause of neonatal morbidity, often resulting from undiagnosed or untreated maternal infection. We report a case of a late-preterm infant born to a mother with late latent syphilis underdiagnosed during pregnancy. Vertical transmission occurred transplacentally, given that the neonate presented with respiratory distress and pathognomonic signs such as pemphigus on both feet at birth. Laboratory evaluation confirmed syphilis infection, with high titer of RPR (Rapid Plasma Reagin) and TPHA (Treponema Pallidum Hemagglutination Assay) and markedly elevated

Syphilis in pregnancy: A practical guide for prenatal care providers.

Syphilis during pregnancy remains a persistent global public health challenge. Untreated or inadequately treated syphilis infection during pregnancy contributes significantly to preventable perinatal morbidity and mortality. In the last five years, a resurgence of syphilis among pregnant women in several regions has led to a concerning rise in congenital syphilis cases. Vertical transmission can occur at any point during pregnancy or delivery, with the highest risk observed during primary and secondary stages of infection compared to latent phases. To align with the World Health Organization (

Syphilis: A Review.

IMPORTANCE: Syphilis is an infectious disease caused by Treponema pallidum, a gram-negative, spirochete bacterium. Worldwide, an estimated 8 million adults aged 18 to 49 years acquired syphilis in 2022. From 2019 to 2023, US syphilis cases increased by 61% overall, with diagnoses among females increasing by 112% and congenital syphilis cases increasing by 106%. OBSERVATIONS: Syphilis is transmitted via contact with infectious lesions during vaginal, anal, or oral sex or via the placenta during pregnancy. Individuals at increased risk for syphilis include people with HIV, those engaging in con

Oral linezolid compared with benzathine penicillin G for treatment of early syphilis in adults (Trep-AB Study) in Spain: a prospective, open-label, non-inferiority, randomised controlled trial.

BACKGROUND: Management of syphilis, a sexually transmitted infection (STI) with increasing incidence, is challenged by drug shortages, scarcity of randomised trial data, an absence of non-penicillin alternatives for pregnant women with penicillin allergy (other than desensitisation), extended parenteral administration for neurosyphilis and congenital syphilis, and macrolide resistance. Linezolid was shown to be active against Treponema pallidum, the causative agent of syphilis, in vitro and in the rabbit model. We aimed to assess the efficacy of linezolid for treating early syphilis in adults

Lack of Serological Response by Delivery to Syphilis Treatment Does Not Impact Pregnancy Outcomes.

Objectives: Maternal syphilis can lead to serious adverse pregnancy outcomes, including neonatal death. A 4-fold decline in blood non-treponemal titer at six months after the treatment of syphilis compared to the baseline is considered as an adequate serological response. However, the duration of normal human gestation does not allow the ascertainment of an adequate serological response. Aim: The aim of this study was to assess correlations between the lack of a 4-fold decrease in non-treponemal titer by delivery after syphilis treatment and fetal and newborns' condition and serological outcom

[Translated article] AEDV Expert Consensus for the Management of Syphilis.

Syphilis -the "great simulator" for classical venereologists-is re-emerging in Western countries despite adequate treatment; several contributing factors have been identified, including changes in sexual behaviour, which won't be the topic of this article though. In 2021, a total of 6613 new cases of syphilis were reported in Spain, representing an incidence of 13.9×100 000 inhabitants (90.5%, men). Rates have increased progressively since 2000. The clinical presentation of syphilis is heterogeneous. Although chancroid, syphilitic roseola and syphilitic nails are typical lesions, other forms o

Congenital Syphilis in the US: A Narrative Review.

Congenital syphilis (CS) continues to pose a significant global challenge. There has been a marked increase in reported cases in the United States, with 102.5 cases per 100,000 live births in 2022 compared with 11.6 cases per 100,000 live births in 2014. Congenital syphilis can lead to a range of severe complications, including premature birth, intrauterine growth restriction, miscarriage, perinatal death, stillbirth, and postnatal complications that may persist into later life. Maternal/parental factors such as age, race/ethnicity, occupation, income level, access to health care services, and

A Case of Latent Syphilis in Early Pregnancy: An Incidental Finding at Seven Weeks Gestation.

Treponema pallidum, the bacteria that causes syphilis, is typically acquired through sexual contact but can also be transmitted transplacentally (through the placenta), causing congenital infection. Syphilis in pregnancy is a major contributing factor to perinatal morbidity and mortality. Untreated neonates may develop complications affecting the central nervous system, bones, joints, teeth, eyes, and skin. This report highlights the incidental discovery of syphilis in an asymptomatic 22-year-old primigravida (pregnant for the first time) at seven weeks gestation through a positive Treponema p
