MECP2-Associated Rett Syndrome Without Developmental Regression-A Case Series.

Clinical phenotype expansion of neurodevelopmental disorders is increasingly important. With accessibility and diagnostic ability of genetic testing expanding, new clinical criteria will continue to be elucidated. Here we describe a case series of 4 female pediatric patients seen for global developmental delay in non-genetic, specialty clinics who received unexpected genetic diagnoses of MECP2-associated Rett syndrome (RS). These results highlight broadening clinical presentation for RS. None of the patients in this cohort met clinical RS diagnostic criteria at the time of genetic testing, as

Trajectory of skill acquisition, loss, and regain in females with classic Rett syndrome.

BACKGROUND: People with Rett syndrome (RTT) have abnormal psychomotor skill development manifested by delay or failure to acquire skills, loss of skills, and potential regain of lost skills. This study aimed to provide detailed characterization of the frequency, timing, and trajectory of gain, loss, and regain of developmental skills in RTT. METHODS: The occurrence and time of gain, loss, and regain of 51 developmental skills was collected during in-person visits in the RTT Natural History Study from 1228 females with Classic RTT and a pathogenic loss-of-function variant in MECP2. The percenta

Dysregulated Cholesterol Clearance via CYP46A1 Contributes to Cerebellar Sterol Imbalance in Mecp2-Null Mice.

Rett syndrome (RTT) is a neurodevelopmental disorder characterized by motor deficits, partly attributed to cerebellar dysfunction. RTT is primarily caused by mutations in the gene encoding the methyl-CpG-binding protein 2 (MECP2), which has been implicated in cholesterol homeostasis by mechanisms that remain poorly understood. Given that brain cholesterol is primarily synthesized de novo and that disrupted cholesterol homeostasis is linked to various neurological disorders, we aimed to investigate cholesterol regulation in the cerebellum of Mecp2-null mice, a well-established RTT model. We mea

mPFC pyramidal neuron synchrony during social competition to form social rankings is disrupted in male Mecp2 knockout mice.

Altered social behaviors are prevalent in neurodevelopmental disorders like monogenic Rett syndrome, which is caused by pathogenic variants in the gene encoding the methylated DNA binding transcriptional regulator MeCP2. Monosynaptic projections from the ventral hippocampus to the medial prefrontal cortex (mPFC) modulate social memory, and are altered in male Mecp2 knockout (KO) mice. The standard tube test was used to define the social hierarchy between age- and genotype-matched triads over six consecutive days of round-robin competitions, and revealed that male Mecp2 KO mice form social rank

Design, synthesis and evaluation of trofinetide prodrug based on diketopiperazine strategy.

Trofinetide is the only drug approved worldwide for the treatment of Rett syndrome, a rare pediatric disease. However, its extremely short half-life, large oral dose, high incidence of gastrointestinal adverse reactions, and consequently high treatment costs pose formidable clinical and economic challenges. To address these issues, we employed a diketopiperazine strategy to design a novel class of trofinetide prodrugs, aiming to alter the administration route and optimize its pharmacokinetic properties. Through the determination of solubility and in vitro conversion half-life, and based on the

Parenting behaviours and autistic children's internalizing and externalizing symptoms: a protocol of a systematic review and meta-analysis.

INTRODUCTION: Autistic children can experience elevated levels of emotional and behavioural problems, which adversely impact their psychosocial functioning. Positive parenting behaviours can significantly shape children's emotional and behavioural developmental trajectories. However, for autistic children, these parenting behaviours may vary depending on their social, sensory and intellectual profiles. Literature on the association between parenting behaviours and autistic children's internalising and externalising symptoms has been mixed. Thus, the main objective of this study will be to cond

The emerging role of gene therapy in autism spectrum disorder.

Autism spectrum disorder (ASD) is a neurodevelopmental condition characterized by heterogeneity in its genetic, epigenetic, and environmental etiologies, posing significant challenges for therapeutic development. While currently lacking curative treatments, substantial progress in understanding the genetic underpinnings of ASD, particularly monogenic forms, offers promising avenues for intervention. This review aims to elucidate recent advancements in gene therapy approaches for monogenic ASDs and discuss their potential implications for the broader autism spectrum, highlighting the crucial ro

A phase 3, randomized clinical trial of soticlestat as adjunctive therapy for Lennox-Gastaut syndrome.

OBJECTIVE: There remains a need for new treatments for Lennox-Gastaut syndrome (LGS), a developmental and epileptic encephalopathy with a heterogenous patient population that often requires polytherapy. The phase 3, randomized SKYWAY study (NCT04938427) investigated the efficacy and safety of the cholesterol 24-hydroxylase inhibitor soticlestat (TAK-935) in participants with LGS. METHODS: This global, double-blind, placebo-controlled trial enrolled participants aged 2-55 years with LGS (adjudicated by the Epilepsy Study Consortium). Participants were randomized 1:1 to stable background medica

Spatially resolved mapping of histones reveals selective neuronal response in Rett syndrome.

Rett syndrome (RTT), a severe neurological disorder caused by loss-of-function mutations in the X-linked Mecp2 gene, results in profound life-long neurological dysfunction. Rett syndrome patients live an apparently normal initial life until 12-18 months of age, after which a progressive accumulation of a wide range of phenotypic manifestations sets in. While MeCP2 is known to regulate chromatin, its impact on global histone composition and dynamics remains poorly understood. Here, we combine mass spectrometry imaging (MSI) and laser capture microdissection (LCM) coupled to LC-MS/MS to systemat

Utility of the Rett syndrome behavior questionnaire (RSBQ) for Japanese patients with Rett syndrome: A pilot study.

BACKGROUND: Rett syndrome (RTT) is a neurodevelopmental disorder caused by MECP2 gene mutations. It mainly affects females and is characterized by age-dependent neurological and behavioral abnormalities. The Rett Syndrome Behavior Questionnaire (RSBQ) is a caregiver-completed tool accepted by the US Food and Drug Administration as a primary outcome measure in RTT clinical trials. However, its reliability remains uncertain, and few Japanese studies have used it. METHODS: In this study, 18 female RTT patients aged 4-44 years who regularly visited our hospital were assessed using the 45-item RSB

Pilot study: the impact of simulation-based pediatric basic life support training on the performance of caregivers of children with Rett syndrome.

BACKGROUND: Rett syndrome is a rare neurodevelopmental disorder associated with an increased risk of severe respiratory dysfunction and cardiorespiratory instability. In some individuals, particularly those with autonomic dysfunction or QT interval prolongation, life-threatening cardiac events may occur. Given the frequency of life-threatening emergencies in affected children, it is essential that primary caregivers are trained in pediatric basic life support (PBLS). This pilot study aimed to evaluate the effectiveness of simulation-based PBLS training for caregivers of children with Rett synd

Early differential impact of MeCP2 mutations on functional networks in Rett syndrome patient-derived human cortical organoids.

Human cerebral organoids derived from induced pluripotent stem cells can recapture early developmental processes and reveal changes involving neurodevelopmental disorders. Mutations in the X-linked methyl-CpG binding protein 2 (MECP2) gene are associated with Rett syndrome, and disease severity varies depending on the location and type of mutation. Here, we focused on neuronal activity in Rett syndrome patient-derived organoids, analyzing two types of MECP2 mutations-a missense mutation (R306C) and a truncating mutation (V247X)-using calcium imaging with three-photon microscopy. Compared to is

Decoding MeCP2 in pain: A systematic review of mechanisms, dosage, and clinical implications.

OBJECTIVE: Methyl-CpG-binding protein 2 (MeCP2) is an epigenetic regulator essential for synaptic development, plasticity, and neuronal maturation. Altered MeCP2 dosage, such as its decrease in Rett syndrome (RTT) or increase in MECP2 duplication syndrome (MDS), has been associated with altered pain sensitivity and progression. This systematic review integrates preclinical and clinical studies to synthesise evidence on MeCP2's role in pain and identify mechanistic and translational limitations. METHODS: We followed PRISMA guidelines to systematically search three major databases (PubMed, Web

Mecp2 deficiency induces dysphagia in a preclinical model of Rett syndrome.

Rett syndrome is an x-linked genetic neurological disorder primarily caused by mutations in the methyl-CpG-binding protein 2 (MECP2) gene. This progressive neurodevelopmental condition hinders patients' ability to breathe and eat normally. It remains unclear how Mecp2 deficiency leads to the high prevalence of dysphagia and aspiration pneumonia observed in individuals with Rett syndrome. This study aims to determine the effects of Mecp2 deficiency on swallow-related neuromuscular mechanisms that contribute to dysphagia in Rett syndrome. Swallow-related submental complex duration and amplitude

Increasing MeCP2 protein in Pitt-Hopkins syndrome model (Tcf4+/-) mice does not affect abnormal myelination but induces the generation of astrocytes.

Pitt-Hopkins syndrome (PTHS) is a rare neurodevelopmental disorder that results from loss-of-function (LOF) mutations in the Transcription Factor 4 (TCF4) gene. PTHS closely resembles Rett syndrome (RTT), another neurodevelopmental disorder caused by mutations in the gene encoding Methyl CpG Binding Protein 2 (MECP2). We have recently shown that increasing MeCP2 levels, either genetically or via a viral vector approach, normalizes reciprocal behavioral phenotypes observed in Tcf4+/- and MECP2-overexpressing animals; in the current manuscript, we show that behavioral rescue also extends to a co

Leptin antagonism improves Rett syndrome phenotype in symptomatic Mecp2-deficient mice.

Rett syndrome (RTT) is a severe X-linked neurodevelopmental disorder caused by mutations in MECP2. Elevated circulating levels of the adipocyte hormone leptin are consistently observed in patients and in mouse models, yet their contribution to disease progression has remained unclear. Here, we show that reducing leptin signaling-either pharmacologically or genetically-significantly alleviates RTT-like phenotypes in Mecp2-deficient mice. In males, these interventions preserved general health, prevented weight loss, and improved breathing and locomotor functions. At the neuronal level, they rest
