Normalization of network activity in an epilepsy model with a constitutively active GABBR2 variant.

The neurotransmitter GABA activates G protein-coupled GABAB receptors (GBRs) that mediate neuronal inhibition in the brain. These receptors function as obligate heterodimers, consisting of the GB1 and GB2 subunits, with GB1 binding GABA and GB2 interacting with the G protein. The monoallelic variants p.A567T, p.S695I and p.I705N in the GABBR2 gene, which encodes the GB2 subunit, have been associated with epileptic encephalopathy and Rett-like disorders. The clinical phenotypes overlap with those seen in individuals with monoallelic loss-of-function variants in GABBR1, the gene encoding the GB1

Running rich: how excess fatty acid oxidation drains the cardiac engine.

Fatty acid oxidation (FAO) provides the healthy heart with 60%-90% of its ATP, with the remainder coming from metabolism of glucose. Metabolic flexibility is key to heart function, ensuring an uninterrupted source of fuel. In heart failure, a shift from FAO to glucose-dependent metabolism occurs as disease progresses, supporting the widely held notion that fat is the optimal substrate in the heart. In this issue of the JCI, Kim et al. challenge this assumption. In studies of acetyl-CoA carboxylase-deficient (ACC-deficient) mice, they found that unregulated use of fat as a substrate led to card

MECP2 Rare Variants in Boys With Central Precocious Puberty.

CONTEXT: Central precocious puberty (CPP) has strong genetic and epigenetic influences. MECP2, an X-linked gene, encodes a DNA methylation reader with a role in gene transcription regulation. MECP2 loss-of-function mutations are linked to neurodevelopmental disorders, particularly with Rett syndrome, a severe disorder that may be associated with early puberty. MECP2 variants were identified in girls with CPP with or without neurodevelopmental disorders. OBJECTIVE: This work aimed to investigate potential MECP2 variants in boys with idiopathic CPP at 3 academic hospitals. METHODS: MECP2 DNA s

Loss of the Mecp2 gene in parvalbumin interneurons leads to an inhibitory deficit in the amygdala and affects its functional connectivity.

BACKGROUND: The MECP2 gene is located on the X chromosome and encodes a methyl-CpG-binding protein 2 involved in transcriptional regulation. Loss-of-function mutations in the MECP2 gene lead to Rett syndrome, a severe neurodevelopmental disorder. The clinical picture of Rett syndrome includes, among other symptoms, social deficits, learning impairment, and heightened anxiety. The amygdala is a brain region responsible for emotional learning and is involved in the regulation of social behaviour as well as fear and anxiety. Parvalbumin interneurons tightly control the excitability, oscillation a

Longitudinal Rett syndrome behaviour questionnaire scores and their associations with genotype and trajectories of mobility, weight and seizure frequency status.

INTRODUCTION: The Rett Syndrome Behaviour Questionnaire (RSBQ) describes behavioural and emotional features. This study investigated total RSBQ score trajectories and their clustering, and for trajectory groups, relationships with genotype and mobility, weight-for-age z scores, and seizure frequency. METHODS: Females in the Australian Rett Syndrome Database who were genetically confirmed with RSBQ data collected between 2000 and 2019 were included. The age trend was modelled with mixed-effects linear regression. Unique trajectory classes for total RSBQ scores and time-varying predictors (mobil

Whole exome sequencing facilitates early neurodevelopmental diagnosis in an outpatient clinic.

The American College of Medical Genetics and Genomics (ACMG) and other professional organisations recommend whole exome sequencing (WES) as a first-tier genetic test for paediatric patients with congenital anomalies, developmental delay and/or intellectual disability, which has contributed to rapidly increasing rates of genetic testing and diagnosis in this population. We present a case of Rett syndrome diagnosed in early childhood following an atypical presentation of the condition with no regression using WES. This diagnosis was facilitated by a multidisciplinary outpatient neurodevelopmenta

Longitudinal analysis in Mecp2-het female mice reveals atypical nociceptive behaviours.

Rett Syndrome (RTT), a neurodevelopmental disorder predominantly affecting females, is characterised by evolving symptoms impacting motor and sensory domains. Herein, we present a study of longitudinal analyses, from 2- to 6-month of age, of Mecp2 heterozygous (Mecp2-het) female mice to comprehensively explore pain perception in RTT. Interestingly, we found a significant variability in the timing and progression of symptom onset among Mecp2-het females, with individuals classified as either early- or late-symptomatic based on the emergence of hallmark neurological features such as clasping and

Synaptic and intrinsic membrane defects disrupt early neural network dynamics in Down syndrome.

Down syndrome, caused by trisomy 21, affects around six million people worldwide and features learning, memory and language deficits. However, the mechanisms underlying trisomy 21 neurophenotypes involving human cortical circuitry are unknown. By characterising developing neural network dynamics and single cell excitability profiles, from synaptic and voltage-dependent ion channel behaviour using an isogenic induced pluripotent stem cell-derived neuronal model, we show that trisomy 21 impairs the activity and development of cortical circuitry. This is caused by deficient glutamatergic synaptic

Unexpectedly competent immune response to SARS-CoV-2 vaccination in Rett syndrome.

Rett syndrome (RTT) is a rare neurodevelopmental disorder of genetic origin characterized by chronic low-grade inflammation, immune imbalance, and frequently associated with compromised respiratory function. During the COVID-19 pandemic, individuals with RTT were classified as high-risk and invited to follow stringent vaccination protocols that included multiple booster doses. However, the immune response elicited by vaccination in this population has never been systematically investigated. Here, we provide the first characterization of spike-specific antibody and memory B cell responses in 23

Post-marketing safety concerns with trofinetide: a disproportionality analysis of the first therapeutic agent for Rett syndrome based on the FDA adverse event reporting system (FAERS).

OBJECTIVE: Rett syndrome (RTT) is a severe, rare and chronic disease that necessitates long-term treatment. In March 2023, trofinetide was approved by the US Food and Drug Administration as the first treatment for RTT. Because of the constraints of clinical trials, certain delayed and infrequent adverse events (AEs) may go unreported, especially in orphan disease. Further research is required to investigate the potential AE signals of trofinetide in real-world scenarios, identify rare and severe AEs associated with this treatment, and promote the safe use of trofinetide among RTT patients. Profiling metabotropic glutamate receptor 7 expression in Rett syndrome: consequences for pharmacotherapy.

Rett syndrome (RTT) is caused by mutations in the X-linked methyl-CpG binding protein 2 (MeCP2) transcription factor. RTT patients undergo a developmental regression between 6-18 months of age, resulting in the presentation of symptoms including repetitive behaviors, seizures, autistic-like features, and apneas. We have reported that levels of metabotropic glutamate receptor 7 (mGlu7) are significantly decreased in brain samples from RTT patients carrying truncation mutations in the MECP2 gene. Additionally, we have identified decreases in Mecp2+/- mice and demonstrated that administration of

Disease-modifying therapies for Rett syndrome: a review for neurologists.

Rett syndrome (RTT) is a severe X-linked neurodevelopmental disorder affecting approximately 1 in 10,000-15,000 females, most often caused by loss-of-function mutations in MECP2. Until the recent approval of trofinetide, management relied exclusively on symptomatic treatment and multidisciplinary supportive care. The therapeutic landscape is now undergoing a rapid shift, driven by multiple gene therapy approaches designed to restore functional MeCP2 expression and achieve true disease modification. As these therapies progress toward potential regulatory approval, neurologists will play central

KCC2 Activation Reverses Neurophysiological and Behavioral Deficits in Female Rett Mice.

Rett syndrome is an X-linked neurodevelopmental disorder resulting from mutations in the MeCP2 gene, leading to intellectual disability, impaired motor coordination, decreased sociability, and seizures. Central to the underlying pathophysiology are deficits in synaptic inhibition, which are mediated by hyperpolarizing GABA A R currents. These events develop postnatally and are dependent upon increased neuronal Cl - extrusion mediated by SLC12A5 (KCC2). Therefore, we tested whether its activation modifies the disease phenotypes evident in female MeCP2 +/- mice, using OV350, a direct activator o

Unconstrained Plasticity Disrupts Memory Consolidation in a Mouse Model of Rett Syndrome.

Memory impairment is a hallmark cognitive deficit in Rett syndrome (RTT). Yet, long-term memory deficits in RTT animal models remain poorly understood, largely due to the technical challenges inherent in tracking neural activity over extended periods. Here, we used longitudinal two-photon calcium imaging to follow the same population of hippocampal CA1 neurons as RTT mice and their littermate controls formed cognitive maps of their environment during a spatial learning task. Neural representations in RTT mice were marked by excessive place cell (PC) activity, with individual PCs exhibiting pro

IGF1 peptide targets Rett Syndrome astrocytes to degrade IGF binding protein, rescue synaptogenesis and restore mitochondrial function.

UNLABELLED: Rett syndrome (RTT), a severe neurodevelopmental disorder caused by mutations in MECP2, leads to profound synaptic and circuit deficits in the brain. While neurons have historically been the focus of RTT pathology, emerging evidence implicates astrocytes in non-cell autonomous mechanisms that impair synaptic structure, function and development. Here, we uncover a central role for astrocyte-secreted IGFBP2 in mediating these deficits and demonstrate that treatment with an IGF1-derived peptide restores synapse formation by promoting IGFBP2 degradation. Using an indirect astrocyte-neu

Transcranial Photobiomodulation in Rett Syndrome: A Mechanistic Review and Therapeutic Hypothesis.

OBJECTIVE: To propose a mechanistic framework for the use of transcranial photobiomodulation (tPBM) as an adjunctive treatment in Rett syndrome (RTT). BACKGROUND DATA: RTT is a severe X-linked neurodevelopmental disorder caused mainly by MECP2 variants, with limited disease-modifying therapies. tPBM delivers red-to-near-infrared light to the brain and shows promising effects in several neurocognitive and neuropsychiatric conditions. METHODS: We reviewed key cellular mechanisms of RTT, namely mitochondrial dysfunction, oxidative stress, neuroinflammation, and impaired synaptic plasticity, and

Dendrimer-Conjugated Glutamine Antagonist, D-TTM020, Ameliorates Brain Immune Dysregulation and Improves Neurobehavioral Deficits in the Mecp2-Deficient Mouse Model.

Rett Syndrome (RTT) is a neurodevelopmental disorder characterized by mutations in the MeCP2 gene, predominantly affecting females. Recent work with MeCP2-deficient mouse models showed a significant role in glutamatergic transmission, specifically microglia-produced glutamate and glutaminase upregulation, in RTT pathology. The glutamine antagonist 6-diazo-5-oxo-L-norleucine (DON) is a potent glutaminase inhibitor; however, its use is limited due to systemic toxicities arising from its non-specific inhibition of glutamine-utilizing reactions. In this work, we determined whether dendrimer conjug
