Clinical experience using trofinetide in Rett syndrome and related MECP2 diagnosis at the children's hospital of Philadelphia post approval.

INTRODUCTION: Rett syndrome (RTT) is a rare neurodevelopmental disorder primarily caused by pathogenic variants in the MECP2 gene. Trofinetide (Daybue) became the first FDA-approved medication for RTT in March 2023. This study evaluates the real-world effectiveness and side effect profile of Trofinetide in 55 individuals (50 females, 5 males) with RTT or MECP2-related neurodevelopmental disorders over a 12-month period. METHODS: Data was collected through clinic assessments and caregiver reports. RESULTS: Results demonstrated that 75.9% of individuals experienced some improvement in RTT symp

Sleep-disordered breathing in children with neurodisabilities.

The prominence of sleep-disordered breathing (SDB) in children with neurodisabilities is increasingly becoming apparent. Patients can manifest not only obstructive sleep apnoea but also central sleep apnoea, nocturnal hypoxaemia and hypoventilation or often a combination thereof. The impact of SDB on quality-of-life measures has been increasingly demonstrated. This in turn is powering a paradigm shift, where investigation of SDB and subsequent treatment with surgical procedures or respiratory interventions, such as noninvasive ventilation, are increasingly considered in this cohort of patients

Perceived quality and needs in speech-language therapy for Spanish children with Rett and Angelman Syndromes.

INTRODUCTION: Early Childhood Intervention (ECI) services in Spain support children aged 0-6 and their families, particularly those with developmental disorders. Recent ECI practice emphasizes family-centered, relational, and participatory approaches. This study explored families' perceptions of speech-language therapy for children with Rett syndrome (RTT) and Angelman syndrome (AS), aiming to evaluate perceived service quality, identify unmet needs, and assess the impact on children's communicative development. METHODS: A cross-sectional, descriptive study was conducted using a purpose-desig

Impaired NF-κB/Nrf2 Crosstalk in Rett Syndrome.

Rett syndrome (RTT), caused primarily by mutations in the X-linked MECP2 gene, is a neurodevelopmental disorder marked by systemic alterations, including mitochondrial dysfunction, chronic oxidative stress, and persistent subclinical inflammation. This OxInflammatory state suggests that disruption of redox-inflammatory regulatory pathways may contribute to disease pathophysiology. In this context, NF-κB and Nrf2 represent two important signaling regulators that operate in a coordinated crosstalk to balance inflammatory activation and antioxidant defense. In this study, we explored the function

A high-throughput screening platform to facilitate treatment development in Rett syndrome.

Rett syndrome (RTT) is a rare X-linked progressive neurodevelopmental disorder affecting predominantly females with no cure and a prevalence of ~1 in 10,000 female birth worldwide. Before mutations in the methyl-CpG binding protein 2 (MECP2) gene were identified to cause classic RTT, there were suggestions that RTT is a mitochondrial disease. Being an essential organelle for all eukaryotic cells, the mitochondria produce energy, buffer calcium, and regulate the generation of reactive oxygen species. Indeed, accumulated reports documented mitochondrial abnormalities in RTT patient biopsies, and

The role of the gut mycobiota in neurodevelopmental disorders: a multikingdom disruption of the gut-brain axis.

Research on the gut-brain axis in neurodevelopmental disorders (NDDs) has historically focused on bacteria, overlooking fungal contributions. This review synthesizes emerging evidence to propose that intestinal fungi may actively contribute to NDD pathophysiology through multikingdom interactions, though causality requires validation. We delineate a consistent gut mycobiota dysbiosis signature across autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), and Rett syndrome (RTT) characterized by reduced diversity and Candida expansion. Potential mechanisms include comp

Female cortical cellular mosaicism underlies shared MeCP2 and PCB impacted gene pathways.

Rett syndrome (RTT) is an X-linked, dominant neurodevelopmental disorder caused by mutations in MECP2, encoding the epigenetic regulator methyl CpG binding protein. Variability in severity and timing of progression in RTT, influenced by factors including mutation type, genetic background, and X chromosome inactivation patterns, suggests potential interaction with environmental neurotoxicants such as lipophilic polychlorinated biphenyls (PCBs). To understand shared mechanisms, we exposed WT and Mecp2e1 -/+ female mice to a human-relevant PCB mixture and dose, then performed single-nucleus 5' RN

Comprehensive High-Depth Proteomic Analysis of Plasma Extracellular Vesicle-Containing Preparations in CDKL5 Deficiency Disorder.

Background/Objectives: CDKL5 deficiency disorder (CDD) is a rare X-linked developmental and epileptic encephalopathy characterized by early onset refractory epilepsy and severe neurodevelopmental impairment with autistic features. Despite advances in genetic diagnosis, objective biomarkers reflecting disease mechanisms remain limited. Extracellular vesicles (EVs) circulating in the blood may contain disease-related proteins derived from the central nervous system. This study aimed to characterize the plasma EV proteome in CDD in a hypothesis-generating exploratory framework and identify the ca

Increased Osteoclast Activity Contributes to Bone Resorption and Osteopenia in a Rett Syndrome Mouse Model.

Rett syndrome is a severe neurodevelopmental disorder caused predominantly by loss-of-function mutations in the X-linked gene MECP2. In addition to a vast array of neurological and physiological impairments, patients also frequently develop severe osteopenia with increased fracture risk; however, the mechanisms underlying these skeletal defects are not completely understood. Previous work in Mecp2-null mouse models has suggested that osteopenia is mainly due to impaired osteoblast function and reduced bone formation. Here, we examined bone mass, microarchitecture, and remodeling parameters in

New insights into Rett syndrome pathogenesis: defining the role of MEPC2 in DNA damage.

Rett Syndrome (RTT), a severe neurodevelopmental disorder, is caused by mutations in the X-linked MECP2 gene, which encodes a key chromatin-modifying protein. Originally described as a transcriptional repressor due to its ability to bind methylated DNA, RTT pathology was first to the misregulation of target genes. However, we present a synthesis of recent research that could re-frame the etiology of the disease. This model proposes that MECP2 deficiency triggers a cellular stress response that is one of the direct and primary causes of RTT pathology. The central hypothesis emerging from this b

IQSEC2-related developmental and epileptic encephalopathy with a Rett-like phenotype: two cases with novel variants and a review of the literature.

Rett syndrome, related to methyl-CpG binding protein 2 (MECP2), primarily affects females, though recent studies have identified additional causative genes, such as IQ motif and SEC7 domain-containing protein 2 (IQSEC2), which mainly affect males and are characterized by prominent developmental encephalopathy. Variations in these genes produce clinical manifestations similar to Rett syndrome but do not fully align with it, posing diagnostic challenges. Currently, they are clinically categorized as Rett-like syndromes. The genotype-phenotype relationships in IQSEC2-associated Rett-like syndrome

Utilization of Non-pharmacological Interventions in Rett Syndrome: A Systematic Review of the Literature on Supportive Care Management.

INTRODUCTION: Rett syndrome (RTT) is a rare neurodevelopmental disorder characterized by early childhood loss of mobility, reduced verbal communication, and behavioral impairment. Although non-pharmacological supportive interventions are widely used, there is a need to better understand their role in RTT care globally. METHODS: Following PRISMA guidelines and a PICOST framework, a systematic review of white (PubMed, Embase, Cochrane) and gray literature was conducted. Non‑pharmacological interventions examined in the United States (US) and outside the US (OUS) settings included ancillary serv

Atypical Atypical MECP2-Related Rett Syndrome Presenting with Movement Disorders- Predominating Phenotype.

BACKGROUND: Rett syndrome (RTT) is an X-linked neurodevelopmental disorder characterized by a typical natural history, including early stagnation, rapid regression, a pseudostationary phase, and late motor deterioration. Seizures, autistic features, breathing abnormalities, stereotypies, and various movement disorders are often present. Currently, the diagnosis of atypical RTT requires a period of regression and fulfillment of at least two main criteria, and at least five of 11 supportive criteria. CASES: We describe five patients (aged 20-61 years), unexpectedly diagnosed with RTT, exhibitin

MeCP2 is Necessary in Cerebellar Purkinje Cells for Precise Network Dynamics During Associative Motor Learning.

Loss-of-function variants of MECP2 cause Rett syndrome; however, their impact on cerebellar computations for learning remains poorly understood. Here, we show that Mecp2 deletion specifically in Purkinje cells does not broadly disrupt cerebellar-dependent behaviors but selectively compromises those that require precise timing, coordination, and associative updating in the cerebellar cortex. Mecp2 loss altered the emergence of learning-related Purkinje cell activity in vivo and disrupted their intrinsic and synaptic properties that support adaptive cerebellar output. These findings identify MeC

DNA methylation, nucleic acid structure, and rett mutations tune MeCP2 binding affinity and cooperativity.

Methyl-CpG-binding protein 2 (MeCP2) is a chromatin-associated factor whose dysfunction causes Rett syndrome. Although MeCP2 preferentially binds methylated DNA, its affinity for methylated substrates is only ∼ threefold higher than for unmethylated DNA, raising the question of how MeCP2 selectively recognizes its targets. Here, we quantify binding of full-length WT MeCP2 and Rett-associated variants (R106W, T158M, R270X, and R306C) to nucleic acid substrates that vary in length, secondary structure, methylation pattern, CpG symmetry, and strand composition. WT and mutant MeCP2 preferentially

Single-nucleus profiling reveals a core disease signature and cell type-specific vulnerabilities in early Rett syndrome.

Rett syndrome (RTT) is an X-linked neurological disorder caused by MECP2 mutations, creating distinct cellular environments in females (mosaic) versus males (nonmosaic). Despite female patients representing most cases, how mosaicism contributes molecularly to RTT pathogenesis, particularly in presymptomatic stages, remains poorly understood. To address this question, we profiled hippocampal transcriptomes of young female and male RTT mice using bulk and single-nucleus RNA sequencing. We identified a core disease signature of consistently dysregulated genes only in MeCP2- cells across RTT model
