Neural peer pressure: intercellular dynamics and emergent phenotypes in the mosaic Rett syndrome brain.

During female embryogenesis, each cell randomly inactivates either the maternal or paternal X chromosome, creating a natural mix, or mosaic, of cells with a different active X chromosome. In X-linked neurological disorders such as Rett syndrome, this results in a brain composed of two cell populations: one expressing the mutant allele and one expressing the wildtype allele. Rather than behaving as independent entities, these populations influence one another through the exchange of extracellular vesicles, free-floating biomolecules, and direct cell-cell contacts. The bidirectional communicatio

Early postnatal DNA methylation dynamics define neuronal subtypes and are disrupted by MECP2 loss.

DNA methylation is the foundational layer of epigenetic regulation underlying mammalian tissue specification. During synaptogenesis, neurons accumulate high levels of a unique type of methylation in which a cytosine precedes a C, A, or T (mCH) instead of the canonical guanine (mCG). Disruption of the mCH reader methyl-CpG binding protein 2 (MECP2) causes the devastating neurodevelopmental disorder Rett syndrome, yet the role of mCH in neurodevelopment remains unclear. In this study, we generated the first large-scale single-cell methylation atlas of the early postnatal mouse brain and resolved

Inhibition of protein tyrosine phosphatase PTP1B function ameliorates pathophysiological deficits in Rett Syndrome.

UNLABELLED: Rett syndrome (RTT) is a severe neurodevelopmental disorder in which current therapeutic strategies remain largely focused on providing symptomatic relief without addressing underlying disease mechanisms. In contrast, we have identified the protein tyrosine phosphatase PTP1B as a mechanism-based therapeutic target and evaluated a class of selective, allosteric small-molecule inhibitors in female murine models of RTT. We show that one of these compounds localizes to brain regions central to motor coordination and cardio-respiratory control, which are core domains of RTT pathology. P

Expanding the Phenotypic Spectrum of Raynaud-Claes Syndrome: A Rett-like Presentation with Two New Cases.

Pathogenic variants in the CLCN4 gene are associated with a rare X-linked neurodevelopmental disorder, Raynaud-Claes syndrome, characterized by intellectual disability, epilepsy, language impairment, motor deficits, stereotypies, and structural brain abnormalities. Although heterozygous females are often considered to be only mildly affected, severe phenotypes have also been reported, and the clinical presentation shows considerable heterogeneity. The present study aims to summarize the current knowledge on CLCN4-related neurodevelopmental disorders through a review of the available literature

Astrocytic Regulation of Aberrant Perineuronal Net Formation in Mecp2-Null Neocortex.

Rett syndrome (RTT), caused by mutations in MECP2, is a complex neurological disorder characterized by myriad physiological disruptions, including early closure of the critical period of developmental plasticity and precocious formation of perineuronal nets (PNNs). PNNs are lattice-like substructures of extracellular matrix (ECM) that enwrap specific subpopulations of neurons. PNNs are essential in the modulation of neuronal plasticity and brain maturation, and their enzymatic disruption can partially restore plasticity in adults and improve memory. Although precocious PNN formation is well-es

Real-World Demographics, Clinical Characteristics, and Predictors of Persistence Among Males with Rett Syndrome Treated with Trofinetide.

INTRODUCTION: Rett syndrome (RTT) is a rare neurodevelopmental disease that primarily affects females but also occurs in males. Trofinetide (TROF) is approved in the USA and Canada for individuals with RTT regardless of sex aged ≥ 2 years; efficacy and safety of TROF was primarily studied among females, thus real-world data for males remain limited. This study evaluated real-world characteristics, treatment persistence, and predictors of non-persistence among males with RTT. METHODS: This retrospective cohort study using IQVIA database linked to TROF specialty pharmacy claims from 01/01/2021

Minimum data requirements and automated preprocessing for reliable EEG biomarkers in Rett syndrome.

BACKGROUND: Electroencephalography (EEG) is a promising biomarker for Rett syndrome (RTT), but excessive artifact and variable tolerance for longer recording sessions pose challenges for reliable biomarker development. Establishing an automated preprocessing pipeline that matches human review and defining the minimum data needed for stable quantitative EEG (qEEG) features can support more patient-friendly protocols and provide consistent multisite analysis results. METHODS: A mean of 10 min of resting-state EEG from 117 participants (1-18 year old; 236 sessions) in the multisite R61 RTT study

Single-cell multiome profiling reveals neuronal bias of modulatory role of MeCP2 phosphorylation.

MeCP2 is subject to many post-translational modifications, such as phosphorylation, in response to diverse stimuli. Several MeCP2 phosphorylation sites have been identified and studied individually. However, the combinatorial effect of phosphorylation at multiple sites has not been directly examined. Building on previous studies characterizing single mutations of MeCP2 phosphorylation sites, we created a mouse model carrying two mutations, serine 80 to alanine and serine 421 to glutamic acid (S80A;S421E, or A;E), in which the MeCP2 protein phosphorylation is fixed in a state observed in active

Altered vagus nerve stimulation evoked locus coeruleus activity in a rodent model of Rett syndrome.

BACKGROUND: Rett syndrome is a neurodevelopmental disorder characterized by seemingly typical development followed by a profound regression, including the loss of language and motor skills and the onset of seizures. Following this regression, intensive intervention is typically ineffective, resulting in the need for novel therapeutics. One potential intervention combines vagus nerve stimulation (VNS) with rehabilitative training, evoking synaptic plasticity and cortical reorganization in an animal model of Rett syndrome. VNS functions by inducing rapid, phasic activation of the locus coeruleus

Cellular, electrophysiological and behavioral improvements in a mouse model of Rett syndrome following gene therapy combined with focused ultrasound-mediated blood-brain barrier opening.

Rett syndrome (RTT) is a severe X-linked neurodevelopmental disorder due to pathogenic variants in the methyl CpG binding protein 2 gene (MECP2). The discovery that deficits resulting from Mecp2 loss are reversible in mice has increased interest in gene therapy as a potential cure for RTT. We have previously evaluated the efficacy of a self-complementary AAV9 vector expressing a codon-optimized version of Mecp2 (scAAV9-MCO) delivered via a systemic approach in early symptomatic Mecp2-knock-out male (KO) mice. In the present study, focused ultrasound (FUS) was used to transiently disrupt the bl

Elucidating the Role of SET as a Key Contributor to Neurodevelopmental Disability Within the 9q34.11 Deletion Syndrome Interval.

The 9q34.11 chromosomal region contains multiple neurodevelopmental genes involved in synaptic transmission, axonal structure and neuronal maturation. Pathogenic microdeletions, duplications and single nucleotide variants in numerous genes were previously linked with neurodevelopmental disorders (NDDs). Amongst them, SET has recently been implicated in a rare NDD with speech delay and facial dysmorphism. This study reports a female with a heterozygous de novo deletion impacting SET but not other NDD-associated genes at 9q34.11. The proband was initially diagnosed with atypical Rett syndrome wi

Recent advances in epigenetic therapeutics for Rett syndrome: from mechanisms to clinical trials.

Rett syndrome (RTT) stands at the forefront of the genetic therapy revolution. This severe X-linked neurodevelopmental disorder, primarily caused by mutations in the MECP2 gene, was historically considered a static condition but is now recognized as a potentially reversible neurodevelopmental disorder. This review synthesizes recent breakthroughs in our understanding of MeCP2's role in chromatin architecture, including its involvement in liquid-liquid phase separation (LLPS). We critically examine the transition from conventional symptom management to precision epigenetic therapeutics. Key adv

Heat shock factor 1 signaling: A novel pathway implicated in Rett syndrome pathophysiology.

Rett syndrome (RTT) is a neurodevelopmental disorder that is associated with loss-of-function mutations in the methyl CpG binding protein 2 (MECP2) gene. MECP2 regulates transcription both locally and globally, making it challenging to distinguish between genes that are pathogenic and those that constitute transcriptional noise. A rare subpopulation of patients lack MECP2 mutations despite presenting with sufficient symptoms to warrant a clinical diagnosis of RTT. These patients are classified as having atypical and MECP2 mutation-negative forms of the disorder. We hypothesized that identifyin

The role of astrocytes in autism spectrum disorder and associated syndromes: Evidence, animal models, mechanisms, and therapeutic potential.

Autism spectrum disorder (ASD) is a neurodevelopmental condition clinically defined by persistent social deficits and restricted, repetitive behavior. Several other neurodevelopmental disorders exhibit these core clinical features of ASD and are therefore classified as "syndromic ASD". Although neurons have been the primary research focus on ASD and associated syndromes, accumulating evidence highlights astrocytes as critical contributors to disease mechanisms. Astrocytes are essential for regulating synapse development, neurotransmitter balance, and neuroinflammation in the brain. In this rev

Programmable CRISPRtune dissects the transcriptional repressive activity of MeCP2.

The ability to control the expression of human genes is a major goal in synthetic biology, enables dissection of gene function, and can be harnessed for therapeutic applications. Advances in genome editing and transcriptional engineering often result in complete gene inactivation or full transcriptional repression. However, programmable tools to dial transcription at intermediate levels remain challenging. Here, we present CRISPRtune - a synthetic fusion of MeCP2 to catalytically dead dCas9 that tunes down transcription of endogenous genes in human cells by harnessing the mild repressor activi

Variability in sensory processing and evoked potentials in Rett syndrome.

BACKGROUND: Rett syndrome (RTT), a rare neurodevelopmental disorder caused primarily by pathogenic variants in the MECP2 gene, is characterized by severe cognitive, motor, and autonomic impairments. Atypical sensory processing-including co-occurring hypo- and hyper-responsivity-is a core yet poorly understood feature. While evoked potentials (EPs) show delayed and attenuated sensory responses in RTT, the underlying mechanisms of these impairments remain unclear. Inter-trial phase coherence (ITPC), which quantifies trial-by-trial neural response consistency, offers a promising functional biomar
