Cytochrome P450-mediated vitamin D and cholesterol metabolism in the pathophysiology of neurodevelopmental disorders.

Cytochrome P450 (CYP450) enzymes play critical roles in the pathophysiology of neurodevelopmental disorders (NDDs), including autism spectrum disorder (ASD), Rett syndrome (RTT), and attention-deficit hyperactivity disorder (ADHD). Although traditionally associated with hepatic and intestinal xenobiotic metabolism, several CYP450 isoforms are expressed in the central nervous system, where they regulate essential pathways involving vitamin D and cholesterol-two molecules fundamental to neural development, synaptogenesis, and membrane homeostasis. The cholesterol-CYP27A1-27-hydroxycholesterol (2

Sex differences in internalising and externalising symptom patterns, pathway and persistence in adolescents receiving psychiatric care: a 2-year follow-up of the MILESTONE European cohort.

BACKGROUND: Sex differences in adolescent mental disorders are well documented, but less is known about how these differences evolve during the transition to adulthood, particularly during the shift from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS), a critical period marked by vulnerability to discontinuity of care. OBJECTIVE: To examine sex-specific differences in psychiatric symptom profiles and clinical trajectories during the CAMHS-AMHS transition using data from the European MILESTONE project, a 2-year longitudinal study. METHODS: A cohort o

Single-dose administration of therapeutic divalent siRNA targeting MECP2 prevents lethality in an MECP2 duplication mouse model.

MECP2 duplication syndrome (MDS) is a rare X-linked neurodevelopmental disorder caused by duplications of the dosage-sensitive methyl-CpG-binding protein 2 (MECP2) gene. Developing therapies for MDS is challenging due to the variability in MECP2 expression among patients and the risk of inducing Rett syndrome through excessive pharmacological intervention. Reducing dosage to optimize silencing often compromises durability and necessitates increased dosing frequency. We present here a series of fully chemically modified small interfering RNAs (siRNAs) designed for isoform-selective and total Me

Early gut microbial and metabolic dysregulation with subclinical cardiac alterations in a nonhuman primate model of Rett syndrome.

Longitudinal multi-omics profiling of a nonhuman primate Rett syndrome (RTT) model reveals early systemic alterations. RTT monkeys exhibited postnatal growth retardation, intestinal structural abnormalities, and low-grade systemic inflammation. Gut microbiome analysis showed delayed microbial maturation and age-discordant dysbiosis, including altered Firmicutes/Bacteroidetes ratios and persistent community restructuring. Fecal metabolomics revealed reduced short-chain fatty acids (SCFAs), disrupted microbe-metabolite networks, and broad alterations in lipid, amino acid, and energy metabolism.

Mapping Trofinetide Polypharmacology in Rett Syndrome: A Multi-Stage Computational Analysis.

Rett syndrome (RTT) is a severe neurodevelopmental disorder caused by mutations in the MECP2 gene. Although trofinetide is the first FDA-approved drug for RTT, its pharmacological mechanism remains unclear. We present a structure-based in silico workflow that integrates target prediction, molecular docking, and 100 ns molecular dynamics (MD) simulations to prioritize potential RTT-relevant targets. Candidate receptors were ranked using a comparative pleiotropic score (PS), as well as an interaction similarity index (SSI) relative to endogenous substrates and reference modulators. Five targets

Astrocytes in Genetic Epilepsies: Supporting Actor or Key Player?

Epilepsy is one of the most common neurological disorders worldwide, affecting around 1% of the population. The epilepsies represent a diverse group of conditions, ranging from acquired forms resulting from neurological insults to common multifactorial epilepsies and rare, often monogenic epilepsies caused by highly penetrant genetic variants. The genetic epilepsies demonstrate frequent comorbidity with a range of neurodevelopmental and psychiatric disorders, and epileptic seizures are also a common feature of neurodevelopmental disorders such as Fragile X syndrome and Rett syndrome. Astrocyte

Profiles of sleep disturbances in Angelman, Prader-Willi, and Rett syndromes: analysis of standardized questionnaires.

INTRODUCTION: Individuals with neurodevelopmental disorders (NDDs) are at increased risk of having sleep difficulties. A variety of sleep problems have been reported in Angelman syndrome (AS), Prader-Willi syndrome (PWS), and Rett syndrome (RTT). The present study intended to expand an earlier characterization of sleep difficulties in a large AS, PWS, and RTT sample by analyzing subscales of standardized sleep questionnaires. METHODS: Scores from children (2-18 years) with AS (n = 74), PWS (n = 90) or RTT (n = 241) on components of the Children's Sleep Habits Questionnaire (CSHQ), the Sleep-R

Expert Consensus on Real-World Use of Trofinetide for Rett Syndrome Using a Modified Delphi Method.

OBJECTIVE: Trofinetide is the first approved treatment for Rett syndrome (RTT) in the United States and Canada. Trofinetide improved the core symptoms of RTT in clinical trials, and real-world evidence supports the findings of clinical trials. As RTT experts in the United States have now gained nearly 3 years of real-world experience with trofinetide, a modified Delphi process was conducted in the United States to establish consensus recommendations for the practical use of trofinetide in the treatment of RTT. METHODS: A multidisciplinary steering group of five trofinetide-experienced RTT exp

From Gene to Hope: Rett Syndrome and the Rise of Molecular Therapies.

Rett syndrome is a rare X-linked neurodevelopmental disorder caused by mutations in MECP2, a gene critical for neuronal function, chromatin organization, and synaptic plasticity. After a period of apparently normal early development, individuals with Rett syndrome experience rapid regression followed by lifelong neurological impairment. Notably, preclinical studies have shown that restoration of MeCP2 expression can reverse established symptoms in adult mice, positioning Rett syndrome as a promising target for molecular therapies. However, MECP2 is extremely dosage sensitive and both insuffici

CDKL5 deficiency results in atypical subregion-specific expression of perineuronal nets during mouse visual critical period.

Perineuronal nets (PNNs) in the primary visual cortex (V1) are specialized extracellular matrix structures that form predominantly on parvalbumin+ GABAergic neurons, marking the closure of visual critical period plasticity. More recently, PNNs are used to characterize deficits in critical period plasticity in mouse models for neurodevelopmental disorders such as Rett syndrome, Fragile X syndrome, and CDKL5 deficiency disorder. Within the mouse V1, studies typically focus on the expression and function of PNNs within the binocular zone, though PNNs are expressed in other subregions of the V1. T

Isolation and functional characterization of novel neuropeptides regulating hippocampal neurogenesis.

Neuropeptides (NPs) are small peptides, produced and secreted by neurons, serving as neuromodulators and chemical messengers that influence neuronal growth, proliferation, and differentiation. Despite their diverse and important roles, the discovery and functional characterization of many NPs remain incomplete. This study aimed to extract and identify novel NPs from rat brain tissue. For this purpose, NPs were purified using gel filtration chromatography and sequenced by mass spectrometry (MS). After characterization, one candidate peptide (NP1) was selected for functional evaluation in primar

Characteristics of adult individuals with Rett syndrome treated or untreated with trofinetide in the United States.

BACKGROUND: Rett syndrome (RTT) is a rare, progressive MECP2-related neurodevelopmental disorder with substantial lifelong morbidity that persists into adulthood. Although survival has improved, adults often experience evolving multisystem complications and fragmented transition care. Trofinetide (TROF) is approved for ages ≥2 years old, yet data on adults with RTT in the real-world setting remains limited; this study evaluated demographics and characteristics of adults >20 years of age who are treated vs. untreated with TROF. METHODS: A retrospective analysis of individuals with RTT diagnosi

Deep Brain Stimulation and Repetitive Training Restore Neuronal Activity and Improve Neurological Function in Rett Syndrome Models.

Rett syndrome (RTT) is a postnatal neurological disorder caused by loss-of-function mutations in the gene that encodes methyl-CpG binding protein 2. RTT is characterized by initially normal development, followed by developmental regression at 6 to 18 months of age. Individuals with RTT subsequently develop motor deficits, impaired learning and memory, and breathing abnormalities. In this review, we summarize recent findings on how neuronal circuitry is impaired in RTT and how deep brain stimulation and presymptomatic, task-specific training significantly improve learning and memory in RTT mice
