Longitudinal substance use and adult adjustment.

Relationships among long-term use patterns of tobacco, alcohol, marijuana, and other substances from late childhood to young adulthood and adjustment in young adulthood were examined. The sample consisted of 133 participants from the New York Longitudinal Study. Cluster analysis revealed four groups of individuals for each of the substance categories, as well as for use of multiple substances, over the five points of measurement. Cluster membership was related to adjustment differences in young adulthood. Results from a series of ANOVAs indicated that individuals consistently using the highest

The Rorschach and traumatic loss: can the presence of traumatic loss be detected from the Rorschach?

Loss, an expected part of everyone's life, can be a catalyst promoting significant growth. But all losses are not the same or affect everyone in like manner. Some losses are of such magnitude and intensity that individuals cannot cope, and, therefore, they keep reliving them (posttraumatic stress syndrome) as if through repeated attempts (Freud, 1923-1922/1961) they might master what had been initially so overwhelming. This study, using an inpatient hospital sample, examines the Rorschach protocols of individuals who had experienced traumatic loss in childhood or early adolescence and compares

Ecology of depression in late childhood and early adolescence: a profile of daily states and activities.

This study investigated daily states and time use patterns associated with depression. Four hundred eighty-three 5th to 9th graders reported on their experience when signalled by pagers at random times. Depressed youth reported more negative affect and social emotions, lower psychological investment, lower energy, and greater variability in affect. These differences were weaker for 5th and 6th graders, suggesting that self-reported feeling states are a poor indicator of depression prior to adolescence. No differences were found in the daily activities of depressed youths nor in the amount of t

Attention-deficit hyperactivity disorder.

The attention-deficit hyperactivity disorder is a common chronic disorder of childhood. No precise definition or approach to treatment is universally accepted; however, an extensive literature exists on which to base a rational approach to management. Symptomatic treatment with stimulant medication in selected patients is effective and safe, but not curative. Successful outcome depends on multimodality therapy, involving parents, teachers, and other professionals. Associated conditions, including learning disorders and emotional disturbance, must be identified and dealt with.

Diagnosis of panic disorder in prepubertal children.

Few reports on panic disorder in children are available, despite the retrospectively documented onset in childhood of about 20% of the cases of adult panic disorder. The authors report on six prepubertal children, aged 8 to 13 years, who met DSM-III-R criteria for adult-type panic disorder. Hyperthyroidism, cardiologic, and respiratory problems were excluded as well as abuse of caffeine or other drugs. The first panic attack occurred between 5 to 11 years of age, with an average interval of 3 years between onset of the disorder and diagnosis. Mitral valve prolapse was documented in two cases.

Maternal expressed emotion and parental affective disorder: risk for childhood depressive disorder, substance abuse, or conduct disorder.

Expressed emotion (EE) refers to a set of emotional aspects of speech for which ratings have been derived. Seven independent studies have established that higher EE ratings in the relatives of patients with schizophrenia predict higher rates of relapse in these patients and two studies have established an association of higher EE in spouses with relapse of depression in their mate. There are no previous studies of parental EE as a predictor of childhood affective disorder or other disorders not in the schizophrenia spectrum. In this study we investigated the relationship between the level of m

Physical contact experience and depression.

Out of a consideration of the relevance of interpersonal physical contact to mental health is developed the hypothesis that unsatisfactory physical contact experience predisposes to depression. This hypothesis is then systematically explored using self-ratings of depression and physical contact (and love) experience obtained on admission and at discharge from 254 unselected psychiatric in-patients. Following the demonstration of a strong association between unsatisfactory physical contact experience and depression a significant relationship is also found between depression and the experience o

Concerning knowledge of mental states.

How does a child acquire knowledge that other people are subjects of experience, and come to differentiate amongst mental states? In this paper I review some philosophical perspectives on these matters. In order to illustrate how the arguments have contemporary relevance, I criticize the 'theory of mind' approach currently favoured by many psychologists and point out certain implications for our view of early childhood autism.

The Zurich Study. IX. Panic disorder and sporadic panic: symptoms, diagnosis, prevalence, and overlap with depression.

The purpose of the present epidemiological study is to investigate and describe panic disorder and sporadic panic attacks among a cohort of young adults, aged 28 years, from the Canton of Zurich in Switzerland. Both DSM-III panic disorder and sporadic panic are characterized by frequent symptoms of somatic anxiety and tension, as well as by frequent symptoms of depressed mood and low vitality. Sporadic panic is more prevalent than panic disorder and shows a greater excess of females over males. The association with depressions (major depression and recurrent brief depression) is similarly high

Suicide attempts among adolescent drug users.

Substance use has been identified as a significant risk factor in nonfatal and fatal suicides during adolescence. A comprehensive questionnaire on patterns of alcohol and other drug use, early childhood and nuclear family psychological and behavioral history, and previous suicidal attempts was completed by 298 (88%) of 340 outpatient adolescent substance abusers in three geographic regions east of the Mississippi River. An abbreviated Beck Depression Inventory was included to ascertain current symptoms of depressed mood. Adolescents admitting to a previous suicide attempt (30%) were compared w

New uses of hypnosis in the treatment of posttraumatic stress disorder.

Hypnosis is associated with the treatment of posttraumatic stress disorder (PTSD) for two reasons: (1) the similarity between hypnotic phenomena and the symptoms of PTSD, and (2) the utility of hypnosis as a tool in treatment. Physical trauma produces a sudden discontinuity in cognitive and emotional experience that often persists after the trauma is over. This results in symptoms such as psychogenic amnesia, intrusive reliving of the event as if it were recurring, numbing of responsiveness, and hypersensitivity to stimuli. Two studies have shown that Vietnam veterans with PTSD have higher tha

Long-term sequelae of epilepsy.

The long-term prognosis for epileptics vary from an entirely normal life to early death or complete incapacity. Studies to date have not provided clear answers on the long-term sequelae of epilepsy. This is because there are many epileptic syndromes with different outcomes. Most studies include a heterogeneous group of patients who may not be representative of all the epileptics in the community. The prognosis for permanent remission of seizure varies with the seizure type. In childhood epilepsies, overall there is at least 50% chance of permanent remission. The more seizures the patient has h

Significance of extreme temperament in infancy for clinical status in pre-school years. I: Value of extreme temperament at 4-8 months for predicting diagnosis at 4.7 years.

The relationship between extreme temperament in infancy and clinical status at 4.7 years of age was studied in temperamentally different groups of infants matched for sex and SES, and subselected from a large birth cohort representative of the general population. The effects of certain dimensions of family functioning and of other risk factors were examined. By itself, extremely difficult temperament in infancy had no strong direct association with clinical outcome at four years of age, whereas temperament assessed at four, family attitudes to discipline, and stressful events did. However, ext

Significance of extreme temperament in infancy for clinical status in pre-school years. II: Patterns of temperament change and implications for the appearance of disorders.

Few, if any, of children's behavioural or cognitive characteristics assessed in the first years of life demonstrate stability until later childhood; early characteristics have so far failed to show an association with future psychopathology. This longitudinal study, from 4-8 months to 4.7 years old, focused on stability and change of extreme temperamental traits in groups of infants subselected from a large birth cohort. Persistent extreme temperament at four and eight months old did not increase stability of temperament to four years of age, relative to other children in the whole population.

Clinical differentiation between major depression only, major depression with panic disorder and panic disorder only. Childhood, personality and personality disorder.

A consecutive sample of 298 nonpsychotic psychiatric outpatients was classified according to DSM-III and divided into 4 diagnostic groups: pure major depression, mixed major depression/panic disorder, pure panic disorder and a remaining group of other disorders. The patients' report of childhood relationship to parents and siblings, family atmosphere, their own personality characteristics as children and precipitating events were compared in the various groups. In addition, differences in personality and frequencies of personality disorders were investigated by means of various instruments. Ou

Assessment of dimensions of childhood firesetting among patients and nonpatients: the Firesetting Risk Interview.

A model of firesetting risk poses that high curiosity, involvement in fire-related activities, exposure to peer/parental models, limited knowledge and skill related to fires, and poor parental supervision, among other features, are characteristic of firesetters. To operationalize several domains of the model, the Firesetting Risk Interview (FRI) for parents was developed consisting of 15 a priori dimensions. To provide a partial test of the measure and its generality across children with and without clinical dysfunction, 343 children (ages 6-13) and parents were drawn from samples of nonpatien

New concepts in primary fibrositis syndrome.

PFS is a painful rheumatologic disorder that may be detected by the wary clinician attuned to the presence of seven or more tender points. This common disorder may be seen at any age, including childhood, and may be associated with secondary symptoms of depression and other affective disorders. It may also be associated with findings of disturbed sleep, hearing and vestibular abnormalities, and profound complaints of fatigue. The vagueness of this latter complaint means that PFS must be distinguished from the newly described CEBV syndrome. Although the etiology of PFS remains unknown, recent i

[Engel's "psychogenic pain and the patient susceptible to pain": a retrospective, controlled clinical study].

Engel's hypothesis of pain-prone patients having a distinct pattern of developmental psychosocial experiences was tested in a controlled design including four groups of 20 patients each: a) psychogenic pain, b) organic pain, c) psychogenic bodily symptoms, d) organic disease. On admission an open-ended interview, including childhood experiences was tape-recorded. Measures were taken to minimize observer-bias. Patients in group a) had, compared to the other groups, significantly increased prevalence of: "Parents, verbally and/or physically abusive of each other", "parents, abusive of the child"

Anxiety syndromes in childhood and adolescence.

Anxieties are very widespread in children and adolescents, they only rarely to lead to the need for medical care. Physiological anxieties as phenomena dependent on maturity are differentiated from anxiety syndromes typical to age according to the degree of severity or the persistence of the signs and symptoms. Typical forms of age-typical syndromes are fear of separation, childish phobias, anxiety syndromes with shyness, going into one's shell, avoidance behaviour as well as school phobias. These confront the generalized anxiety disorders and the phobias as anxiety syndromes, which occur in ch
