In primary prevention, we meet many methodological difficulties: the promotion of mental health requires the transmission of specialized knowledge: this is in fact very difficult to realise. Validation of such programs is impossible. Moreover, primary prevention remains generally unspecific, often helpless--as in the case of psychosis. Problems of prediction are so important that we are not even able to predict--and then to prevent--the development of depression, one of the most common forms of psychiatric disturbance. Early diagnosis (secondary prevention) is also still inadequate: in most ca

Depressive states can appear in different phases of life. They differ in their phenomenology according to type, dynamics and power of the etiologic factors, as well as to the moment of their appearance in a child's life. Therefore, depressive syndromes during infancy and childhood are very diverse. Depression in childhood is psychopathologically more complex than it is in adulthood, because it occurs in a developing organism. A differential typologic order according to nosologic aspects is difficult to establish. A division into psychogen-reactive, neurotic, organic, structural and endogenous-

Although the FDA recommends imipramine hydrochloride (IMI) only for temporary relief of symptoms of enuresis nocturna (EN), the drug has been applied to a number of other pediatric situations, including the Hyperkinetic Syndrome (HS), childhood depression, somnambulism and pavor nocturnus, school phobia, petit mal epilepsy, allergies, autism, encorpresis and head-banging. We have reviewed the literature, with particular attention to the pharmacokinetics of IMI in children, and its putative mechanisms of action. The drug probably works through a number of different actions, and the futher delin

The first results covering the assessment period phase of a systematic study of 50 borderline psychotic and 50 psychotic adolescents are described. These 100 adolescents comprised one-third of the total number of admissions to an adolescent department during the years from 1968 to 1975. There were 58 boys and 42 girls; 53 of the 100 cases had been psychiatrically ill in childhood with evident symptoms. In 22 cases, there was positive information supporting the fact that the patients had been healthy in childhood. The rest (25 cases) were classified under "unclarified picture", showing non spec

Probands who had been hospitalized for gastroenteritis between the ages of 2 and 5 years were studied at the ages of 8 to 12 years. More conduct problems and a tendency to excessive dependence were found (1) in probands who were severely ill by objective criteria than were others and (2) in probands whose parents were specifically worried that their child might die as a result of the illness. General worry in the parents had different consequences. Other data suggest that pediatricians, because they are no longer worried about the illness after the child is discharged, may be failing to take a

The authors developed reliable clinical rating scales to compare 35 children with school phobia and their families with a matched sample of children with other neuroses and their families. Twice as many school phobic children as children with other neuroses showed excessive separation anxiety, dependency, and depression. Although a mutually hostile-dependent interaction was found in most of the families of children with school phobia, the development of school phobia appeared to be dependent on defects in character development in the children as well. The authors discuss the etiological signif

The classical notion of neurosis as the prototypical form of nonpsychotic personality disturbance must be rejected in favor of the categories of (a) personality pattern disturbance and disorder of life style, and most especially, (b) character disorder. Category (a) would include schizoid, paranoid, and cyclothymic personality pattern disturbances and compulsive and hysteroid personality as disorders of life style. Each of these would occupy one end of a 'psychotoid' continuum, at the other end of which would be a familiar type of psychosis such as paranoid schizophrenia or pseudoneurotic schi

The development constructs of Mahler and her coworkers are seen as clinically based theory which enrich practice. Freud discovered the importance of the infantile past and psychosexual development in particular, he related these findings to the psychodynamics of neurotic structures, and devised a successful therapeutic model based on his findings. Mahler has uncovered and detailed other aspects of infantile mental life, related genetically not to neurotic byt also to other mental structures, thereby broadening the range of applicability of Freud's therapeutic model. Clinical vignettes are offe

With recent developments in community psychiatric services, concern with prevention has become an urgent social, as well as medical challenge. Comprehensive investigation into causation must therefore be given systematic emphasis. This paper is an effort toward clarification of etiology, specifically of the depressive disorder, in terms of early childhood experiences. The work is based on social and psychiatric history data collected by experienced psychiatric social workers on two groups of subjects from the National Institute of Mental Health (NIMH) Collaborative Depression Studies under the

The patients here concerned had a faulty psychical development. During the caracter-building period of early childhood they were unable, to assert themselves sufficiently enough to be accepted, or to feel that they are loved. Therefore they tried by means of overcompensatory efforts in the sense of extreme adjustment to others, to make up for this lack. Increasingly confined, as a preponderant attitude they finally developed a forced "assistant" pose (W. Schwidder). Under these circumstances the personality of the patients could not develop sufficiently in the way, that they were able to make

During the 1st year of the Abortion Act, 1967, sixty-five patients were seen for a psychiatric opinion. Of these 50% were single, 36% married and 14% separated or divorced. Referral was by the general practitioner in 60%, gynaecologist 36% and ‘other’ 4%. Mean length of psychiatric history was 18·9 months. Half had had unhappy childhoods and family relationships were frequently disturbed. Four (6%) were University students. Mean length of pregnancy was 11·0 weeks and for 87% of the single patients it was their first; for 36% of the married group it was their third. In fourteen (21%) the pregna

In our material, i.e., in the drawing made by "slightly cerebrally damaged school children with behavioral anomalies", we found in both the tree and figure tests disturbances in the utilization of space, disproportions of parts (relative to their age), and disturbances in the harmony of lines (coarse, schematic, and discontinuous lines). Studies of the drawings made by children are, of course, not suitable for establishing a special diagnosis, but may provide, for children of school age, along with the anamnestic data, neurological, electroencephalographic, and other investigative results, ind

Physicians should understand the psychic as well as the chemical effects of ataractic drugs, especially since they are prescribed chiefly for illnesses of emotional origin. The patient may feel that he is "being put off with a pill;" on the other hand, both patient and physician may be encouraged (and thus able to work better together) because the prescription of a drug represents "doing something" about the disorder. This ability to "do something" may tempt the physician to resort too readily to relieve normal and even healthful tensions of living. If the drug has pronounced physical effects
