[Preoperative psychoprophylaxis in childhood. Results of a hospital program].

Results of a surgical psychoprophylaxis program, theoretically and technically framed within psychoanalytic theory is presented. It also comprises a description of the method used, as well as criteria by which authors have determined whether or not a child is ready for surgery. Results obtained with 134 children and a description of those who showed post-surgical disturbances are presented. Analysis is carried out of the percentage of disorders according to age group, showing that highest risk is among children up to five years of age, coinciding with the finding put forth by other authors. Blood-injury phobia: a review.

Natural human uneasiness about blood, injury, or deformity sometimes becomes a specific phobia, which can lead to serious disability if vital medical procedures are refused. Blood-injury phobia usually starts in childhood and is often familial. Unlike other phobic cues, which cause persistent tachycardia, blood-injury phobic cues evoke an initial rise in heart rate followed by vasovagal bradycardia and, frequently, syncope. Although blood-injury phobia may have an evolutionary, genetic, and physiological basis, it can be treated effectively by exposure. The tendency to faint early in exposure

Successive generations of child maltreatment. The children.

Families were identified, in north-east Wiltshire (UK), (population 200,000), in which there was a pattern of two or more generations of child maltreatment and where there were also two or more children maltreated in the current generation (born between 1960 and 1980). These children had high rates of mental handicap, backwardness, and antisocial behaviour, and there were also characteristic patterns of emotional disturbance. The post-neonatal death rate for the 294 index children was very high, but higher still for their brothers and sisters. Some of the deaths, and at least 11 cases of viole

Children's Depression Scale: validation with child psychiatric inpatients.

The present investigation examined the psychometric characteristics and correlates of the Children's Depression Scale (CDS) with psychiatric impatient children (n = 185; ages 7-12) and their parents (n = 154). The results revealed: high internal consistency of the CDS, significant correlations of CDS scores with other measures of depression and related constructs (hopelessness, self-esteem) and significant differences in CDS depression and positive experience scores for children independently diagnosed as meeting Research Diagnostic Criteria for depression versus other disorders. Parent CDS sc

Positive and negative symptoms in psychotic and other psychiatrically disturbed children.

The positive and negative symptom distinction has been found to be valid in research on schizophrenia in adulthood. The present study examined age-related changes in the occurrence of positive and negative symptoms in psychiatrically disturbed children who were receiving psychiatric treatment for the first time. The effect of age on the manifestation of positive and negative symptoms was significant; positive symptoms increased linearly with age, while negative symptoms were most frequent in early childhood and late adolescence. This finding held for the entire sample of children, as well as t

Brain opioids and autism: an updated analysis of possible linkages.

Considerable clinical evidence suggests that autistic children lack the normal ability or desire to engage others socially, as indicated by their poor social skills and inappropriate use of language for communicative purposes. Specifically, these children seem to lack normal amounts of social-emotional interest in other people, leading perhaps to a decreased initiative to communicate. This paper summarizes experimental evidence supporting a neurological theory, which posits that autism, at least partially, represents in the brain, such as brain opioids. These substances modulate social-emotion

Screening for psychiatric morbidity. The pattern of psychological illness and premorbid characteristics in four chronic pain populations.

Three hundred and seventy-eight patients from 4 chronic pain populations have been examined by self-assessment questionnaire methods to estimate the amount of psychiatric morbidity present. Using the General Health Questionnaire-28 scaled version (GHQ-28) the findings for probable psychiatric illness by clinic were: for anaesthetists' pain clinics serving a mixed urban and rural population--37%; for an oral medicine facial pain clinic--30%; for a rural hospital pain clinic--37%; and for a psychiatrists' pain assessment and treatment service--51%. These findings demonstrate the effects of selec

The psychological facet of psychosomatic disturbances.

The first part of this paper deals with the psychological background to the most common difficulties in the relation between doctors and patients, particularly when the two parties disagree about the diagnostic and therapeutic approach to a psychosomatic affection. The following causes for discord are discussed: Alexithymia--a mental deficiency that obstructs the insight into the connection between emotional and physical reactions to all types of life experiences; Anal object relationships--a disturbance of the psychosexual development in early childhood, involving in particular the risk of a

Nonpsychotic patients referred to a consultation service.

In order to examine characteristics of nondangerous and nonpsychotic patients referred to a consultation service, the authors administered self-rating scales and questionnaires to fifty referred medical patients and nonreferred controls. Recalled abuse in childhood appeared a stronger predictor for referral than some other commonly researched ones such as parental death, divorce or separation in childhood or recent losses. Anxiety, hostility and depression were significantly higher in referred patients, whereas hypochondriacal attitudes did not differ between the two groups. The findings appea

Sleep problems in early childhood: continuities, predictive factors, and behavioral correlates.

A longitudinal study, based on interviews with 308 middle-class, preponderantly white mothers, provided an opportunity to evaluate the continuity, predictive factors, and behavioral correlates of sleep problems in young children. When their children were 8 months old, 10% of the mothers reported that their babies woke three or more times per night, 8% reported that the babies took an hour or more to settle after waking, 5% complained that their own sleep was severely disrupted by the child, and 18% reported at least one of these problems. At 3 years of age, 29% of the children had difficulty g

Psychosocial ramifications of childhood malignancy for the child and family.

The changes that occur in a child's life following the diagnosis of cancer extend to other members of the family and, through time, far beyond the biologic cure of the disease. The responses of the family to this stress and the ways in which they cope with it will depend partially on their own resources and partially on the severity of the disease and its treatment. This paper describes the factors that influence the family's adaptation to illness and presents a case example that illustrates criteria useful in assessing the need for psychologic consultation in childhood cancer patients and the

Loss of parent in childhood and adult psychiatric disorder: the role of lack of adequate parental care.

The inconclusiveness of the literature on the role of loss of parent in influencing psychiatric disorder in adulthood is well known. A number of reasons involving sampling, location and other methodological features, are given to account for these contradictory findings. A study specially designed to cope with these features is then described and basic results are reported. These indicate that, in a sample of women aged 18-65, loss of mother before the age of 17, either by death or by separation of one year or more, was associated with clinical depression in the year of interview. Loss of fath

[Psychosomatic aspects of parent-child relations in atopic eczema in childhood. I. Psychodiagnostic test procedures in parents and children in comparison with somatic findings].

A total of 23 children with atopic eczema and 13 control children suffering from non-atopic dermatological disorders were studied. For children between 8 and 14 years the "Hamburg neuroticism and extraversion scale for children and adolescents" (HANES-KJ) was used. In this test no statistically significant differences were observed between children with atopic eczema and children with other dermatological disorders. The mothers and fathers of atopic children were examined for personality profiles using the "Freiburger personality inventory" (FPI). In the FPI, mothers of children with atopic ec

Neurologic presentations of panic disorder in childhood and adolescence.

Four children with panic disorder, two boys and two girls between the ages of nine and 16 years, are discussed. They presented with complaints suggesting neurological disorder: 'dizziness', headache, episodic anxiety and 'blackout spells'. Neurological examinations and investigations were normal. Depression and/or anxiety were prominent in all cases. Drug treatment and psychotherapy were of some benefit. Four other children with attention deficit disorders are presented, whose mothers have panic disorder. This association points towards common biochemical influences and suggests that tricyclic

Agoraphobia with panic attacks. Development, diagnostic stability, and course of illness.

A structured psychiatric interview was used to examine the symptom history of 55 patients meeting DSM-III criteria for agoraphobia with panic attacks and five patients meeting DSM-III criteria for panic disorder. Anticipatory anxiety and generalized anxiety occurred in over 80% of the patients, and these anxiety states together with panic attacks and phobic avoidances had courses that were chronic and unremitting. Major depression occurred in 70% of the patients and had an episodic course that differentiated it from the anxiety states. Other frequently reported disorders were childhood separat

Pregnancy as a reaction to early childhood sibling loss.

Surrogate mothers, illegitimately pregnant teenagers, married women seeking abortions, women convicted of manslaughter, and sister survivors of early childhood sibling loss may share a common conflict. These women may be in a state of incomplete mourning, unconsciously wishing to master a trauma by turning a passive experience into a life event they can control. Pregnancy can function as that life event. Replacement daughters and/or sister survivors can present as our patients with a search for or an avoidance of pregnancy as a crucial though unconscious issue. Literature written about this se

Childhood sibling loss. A family tragedy.

The loss of a significant object, the loss of a home (security, personal possessions, familiar space that has emotional meaning), the dislocation from one's home or land as occurs in wars or disasters, gives rise to stress-strain responses that may have short-term or long-term effects, eg, post-traumatic stress disorder. The hidden or neglected victims of such occurrences frequently are children--be they siblings or direct descendants. In childhood sibling loss, the effects of the loss are mediated through different members of the family. The acute stressors can give rise to later adversity un

Risk factors for depression in adolescence.

Public health concern regarding depression has recently increased as a result of the rise in the rate of adolescent suicide, with a probable concomitant rise in the rate of depression in this age group. The rise appears to be both a period effect, in that increased rates are now observed across age categories, and a cohort effect, in that being born after 1960 also contributes to the increase. The clinical phenomena and epidemiology of depression in adolescence are reviewed. Diagnostic criteria for depressive mood and depressive syndrome are similar to those in adults. However, the predictive

[Coping with stress as a paradigm of the psychopathology in childhood and adolescence].

Stress coping can be valid for a paradigm of psychopathology of children and adolescents. This concept is supported by an extensive literature which shows the way of coping from a "footnote to stress theory" (Lazarus) to its central concept. Neither the "objective" stress (stimulus side) nor the specificity of stress reaction (adaptation syndrome) are the important factors, but rather the mediation between stress coping and the individual, the transaction in the sense of an exchange process. Above all, permanent personality attributes have been examined here, most intensively extraversion/intr

Development of children with a chronic illness.

About 5 to 10 per cent of all children have sometime during childhood a moderately to severely handicapping long-term illness or disability. These children are at risk, not only medically, but for complex social, educational, and emotional difficulties. The potential interferences with normal development imposed by a chronic illness are varied but pervasive, and depend on the specific characteristics of the illness, the child, and the child's family. There are known psychological effects of illness on a child, which can best be understood within a developmental framework such as that outlined
