The Inventory of Family Feelings (IFF), a self-administered measure of interpersonal affect, maps a family's affective structure showing patterns of conflicted relationships and alliances. Three studies demonstrate that IFF scores have high reliability, construct and concurrent validity, and potential utility in family research. Families with identified patients and couples in marital therapy score less positively toward each other than control families, most of the negative affect in such families centers on and emanates from the identified patients, IFF scores are positively correlated with

I reviewed studies examining the hypothesis that life events that occur during childhood or early adolescence may predispose a person to a depression in adulthood. Of studies that compared the incidence of childhood bereavement or other childhood loss events among depressed patients and controls, the majority found an increased incidence among the depressives. Although discrepant negative findings exist, it seems that the childhood loss of a parent by death generally increases depressive risk by a factor of about 2 or 3. In addition, early loss events also seem to be related to the severity of

The author reports a prospective study of the mental health and alcohol use of 184 men first studied during their college years. When the men were 50 years old a rater blind to all other data classified the subjects' alcohol use as little (N = 48), social (N = 110), or abuse (N = 26). The warmth of the men's childhood environment and their personality stability in college were assessed by other raters blind to data on subjects' lives after college. Vignettes identifying "oral" adult behavior (pessimism, self-doubt, passivity, and dependence) were collected for each man by a rater blind to subj

Twenty-nine male offspring of "continuous schizophrenics" (chronic, borderline, and chronic schizoaffective schizophrenics), plus controls, were given neurological and psychological examinations at age 7. Eight of the 29 were found to have high ratings on a factor score that was termed "hyperactive" (increased activity, impulsivity, distractibility, and emotional lability), and three of these boys had high ratings for neurological signs as well. These frequencies were significantly greater than the control values. Mild incoordination, such as awkwardness in performing rapidly alternating movem

Based on a one-and-half year experience in a small group setting with drug dependent adolescents the change of group structure, the functioning of the group for its members, the leader's position within the group and the therapeutic relationship are analysed. In its beginning the group develops what can be called a "symbiotic group structure". Its transformation into that of a "working group" is the main task of therapy. The underlying processes can be compared with those of the beginning individuation in early childhood. Ego development begins, as M. Mahler and other modern investigators have

In the survey of 74 Danish patients with temporal lobe epilepsy who underwent temporal lobectomy, a total of 20 patients were psychotic. Nine of these became psychotic during the follow-up period, six of them after cessation of their epileptic seizures. There were 13 schizophrenia-like psychoses, six paranoid delusional and depressive psychoses, and one childhood psychosis. Operation was on the right side in 39 and on the left side in 35 patients. When the various psychotic groups were compared with each other or with the nonpsychotic patients, the side of operation was not found to be statist

The background to social and emotional deprivation is discussed and applied to a study of kwashiorkor in East African children. A group of 107 children with kwashiorkor was compared with 111 controls. Age, sex and tribe were all found to have significances of their own: fifty of each group were therefore matched for these three factors. Ten other factors were found to be significant in the background of children with kwashiorkor, all of which could be associated with social or emotional deprivation or both (see Table 14). It is concluded that, in childhood, sustained personal care and affectio

A case-history format was utilized to compare interrater agreement on childhood and adolescent psychiatric disorders, using DSM-II and DSM-III. The average interrater agreement was 57% for DSM-II and 54% for axis I (clinical psychiatric syndrome) of DSM-III. There was high agreement in both systems on cases of psychosis, conduct disorder, hyperactivity, and mental retardation, with DSM-III appearing slightly better. There was noteworthy interrater disagreement in both systems for "anxiety" disorders, complex cases, and in the subtyping of depression. Overall, the reliability of DSM-III appears

Psychological reactions to chronic medical illness can be categorized as follows: the chronically ill patient is vulnerable, first and foremost, to eight types of psychological stress, all of which have their roots in early childhood. Specifically, chronic illness evokes a threat to the patient's self esteem; fear of strangers; separation anxiety; fear of loss of love, and of the control of developmentally achieved functions; fear of loss of, or injury to body parts; guilt and fear of retaliation; and fear of pain, which cuts across all of these stresses. Other psychological reactions to chron

Many authors have shown that adolescent breakdown is often associated with emotional deprivation in childhood. Psychotherapeutic intervention before the personality becomes too fixed and rigid should offer a means of improvement through the creation of substitute empathetic relationships, and prognosis should be improved to the extent to which this is successful. This hypothesis is tested through a follow-up study involving 100 adolescents, and the hypothesis is found proved in the diagnostic category of adolescent personality disorder, but of no apparent relevance in other groups.

Empirical studies on the psychopathology of adoptees are summarized and discussed with regard to the possibility of a relationship between adoption and psychic disturbances. The results leave the question open as to whether adoptees are more prone to psychic disturbances than other individuals. Those under treatment do not seem to be more severely disturbed than others. Whereas however the adoptees display more disturbances in their social behavior rather than neurotic disturbances, the relationsship is reversed in the population of the non-dopted. The causes seem to lie in the conditions in e

This review surveys the therapeutic efficacy of tricyclic antidepressants and monoamine oxidase inhibitors in schizophrenic patients. In general, the use of these drugs alone was found not to be warranted in schizophrenia, except perhaps in the so-called pseudoneurotic subgroup. In most cases, combinations of antidepressants and phenothiazines were not more beneficial than phenothiazines alone. In particular, the conditions of agitated patients and patients with histories of social deviance dating back to childhood were often made worse by the addition of an antidepressant. However, when the p

This paper forms part of an M.D. Thesis presented at the University of Cambridge. One hundred adolescents and their parents were interviewed to ascertain details of their early history. Fifty were obtained from adolescent patients in psychiatric units in Newcastle-upon-Tyne, and were compared with fifty matched individuals who had no psychiatric history, obtained from other departments in the hospital. A comparison of these two groups showed a significantly higher incidence of separation experiences in the childhood of the patient group than in the control group. This was also true of other tr

In primary prevention, we meet many methodological difficulties: the promotion of mental health requires the transmission of specialized knowledge: this is in fact very difficult to realise. Validation of such programs is impossible. Moreover, primary prevention remains generally unspecific, often helpless--as in the case of psychosis. Problems of prediction are so important that we are not even able to predict--and then to prevent--the development of depression, one of the most common forms of psychiatric disturbance. Early diagnosis (secondary prevention) is also still inadequate: in most ca

Depressive states can appear in different phases of life. They differ in their phenomenology according to type, dynamics and power of the etiologic factors, as well as to the moment of their appearance in a child's life. Therefore, depressive syndromes during infancy and childhood are very diverse. Depression in childhood is psychopathologically more complex than it is in adulthood, because it occurs in a developing organism. A differential typologic order according to nosologic aspects is difficult to establish. A division into psychogen-reactive, neurotic, organic, structural and endogenous-

Although the FDA recommends imipramine hydrochloride (IMI) only for temporary relief of symptoms of enuresis nocturna (EN), the drug has been applied to a number of other pediatric situations, including the Hyperkinetic Syndrome (HS), childhood depression, somnambulism and pavor nocturnus, school phobia, petit mal epilepsy, allergies, autism, encorpresis and head-banging. We have reviewed the literature, with particular attention to the pharmacokinetics of IMI in children, and its putative mechanisms of action. The drug probably works through a number of different actions, and the futher delin

The first results covering the assessment period phase of a systematic study of 50 borderline psychotic and 50 psychotic adolescents are described. These 100 adolescents comprised one-third of the total number of admissions to an adolescent department during the years from 1968 to 1975. There were 58 boys and 42 girls; 53 of the 100 cases had been psychiatrically ill in childhood with evident symptoms. In 22 cases, there was positive information supporting the fact that the patients had been healthy in childhood. The rest (25 cases) were classified under "unclarified picture", showing non spec

Probands who had been hospitalized for gastroenteritis between the ages of 2 and 5 years were studied at the ages of 8 to 12 years. More conduct problems and a tendency to excessive dependence were found (1) in probands who were severely ill by objective criteria than were others and (2) in probands whose parents were specifically worried that their child might die as a result of the illness. General worry in the parents had different consequences. Other data suggest that pediatricians, because they are no longer worried about the illness after the child is discharged, may be failing to take a

The authors developed reliable clinical rating scales to compare 35 children with school phobia and their families with a matched sample of children with other neuroses and their families. Twice as many school phobic children as children with other neuroses showed excessive separation anxiety, dependency, and depression. Although a mutually hostile-dependent interaction was found in most of the families of children with school phobia, the development of school phobia appeared to be dependent on defects in character development in the children as well. The authors discuss the etiological signif

The classical notion of neurosis as the prototypical form of nonpsychotic personality disturbance must be rejected in favor of the categories of (a) personality pattern disturbance and disorder of life style, and most especially, (b) character disorder. Category (a) would include schizoid, paranoid, and cyclothymic personality pattern disturbances and compulsive and hysteroid personality as disorders of life style. Each of these would occupy one end of a 'psychotoid' continuum, at the other end of which would be a familiar type of psychosis such as paranoid schizophrenia or pseudoneurotic schi
