Terminology and frameworks used for the classification of voice disorders: a scoping review protocol.

OBJECTIVE: This review aims to examine the literature on the terminology and descriptions for the broad classification of voice disorders, and outline the criteria described to guide clinicians in differential classification. INTRODUCTION: The process of classifying the etiology of voice disorders is complex. A key challenge for clinicians and researchers is a lack of consensus on agreed terminology to define umbrella terms commonly used in the published literature. Consistency in the terminology for voice disorder classification, and well-defined conditions within those groups, will provide

Anxiety in Children with Selective Mutism: A Meta-analysis.

This study evaluates the current conceptualization of selective mutism (SM) as an anxiety disorder in the DSM-5 using a meta-analytic approach. In the absence of any systematic assessment of anxiety in the field of SM, we pooled prevalence data of comorbid anxiety disorders in a random-effects meta-analysis. On the basis of 22 eligible studies (N = 837), we found that 80% of the children with SM were diagnosed with an additional anxiety disorder, notably social phobia (69%). However, considerable heterogeneity was present, which remained unexplained by a priori specified moderators. The findin

Cognitive behavioural therapy for anxiety disorders in children and adolescents.

BACKGROUND: A previous Cochrane review (James 2005) showed that cognitive behavioural therapy (CBT) was effective in treating childhood anxiety disorders; however, questions remain regarding (1) the relative efficacy of CBT versus non-CBT active treatments; (2) the relative efficacy of CBT versus medication and the combination of CBT and medication versus placebo; and (3) the long-term effects of CBT. OBJECTIVES: To examine (1) whether CBT is an effective treatment for childhood and adolescent anxiety disorders in comparison with (a) wait-list controls; (b) active non-CBT treatments (i.e. psy

Selective mutism: a review of etiology, comorbidities, and treatment.

Selective mutism is a rare and multidimensional childhood disorder that typically affects children entering school age. It is characterized by the persistent failure to speak in select social settings despite possessing the ability to speak and speak comfortably in more familiar settings. Many theories attempt to explain the etiology of selective mutism.Comorbidities and treatment. Selective mutism can present a variety of comorbidities including enuresis, encopresis, obsessive-compulsive disorder, depression, premorbid speech and language abnormalities, developmental delay, and Asperger's dis

'More than 100 years of silence', elective mutism: a review of the literature.

Elective mutism is a rare disorder of communication, where the child speaks fluently in familiar situations, such as home, despite lack of speech in less familiar settings, for example school. A variety of temperamental and behaviour characteristics, co-morbid psychiatric conditions, neurodevelopmental delay and family factors have been associated with the disorder. EM children are described as excessively shy, withdrawn, 'slow to warm up', inhibited, often avoid eye contact, fear social embarrassment and experience significant separation anxiety, on separation from their attachment figures. Don't look back in anger: learning from my mistakes with Mark and his family.

The article offers a reflective account of work with Mark and his family. The work concerned a teenage boy who had presented with elective mutism; when this ameliorated he presented later with obsessive compulsive disorder. In the therapy there was a recurrent pattern of disappointment for all concerned that positive change, which at times looked so likely, did not materialize. A description of the work is given, interspersed with reflections about my feelings at the time, and current thoughts and ideas about what might have gone wrong and why. Reflections on the process of writing this piece

[Day care treatment of 2 siblings with elective mutism].

This case report deals with the day care treatment of two seven- and eight-year-old siblings with elective mutism. Their treatment entails a combination of psychopharmacological and intensive behavior therapy. The multimodal therapeutic process is presented together with continuing psychosocial steps. Behavioral intervention focuses on building verbal expressive capacity, reducing speech anxiety in social situations and generalization to non-therapeutic situations. The case report is discussed in the context of the current literature on elective mutism.

[Elective mutism--a disorder of social functioning or an emotional disorder?].

Elective mutism is a relatively uncommon disorder characterized by consistent failure to speak in specific social settings, such as in school, despite speaking fluently in a variety of other situations. Elective mutism is a very debilitating disorder for the affected child, as well as for their families, with detrimental implications for the development of social skills and learning if not offered appropriate support and treatment. The most promising treatment results have thus by far been achieved through a combination of cognitive behaviour therapy and supportive cooperation between parents,

The Quiet Child: A Literature Review of Selective Mutism.

Selective mutism is an unusual but intriguing condition with a reputation for being particularly intractable to intervention. Originally described as 'elective mutism', selective mutism is a heterogeneous disorder associated with a number of individual and family factors including developmental disorders/delay; particular temperamental and personality characteristics; and other co-morbid symptoms and psychiatric disorders. Considered by some in the past as an oppositional disorder, it is the proposal that SM might be a childhood manifestation of an anxiety disorder such as social phobia that h

Mutism: elective or selective, and acquired.

When a child does not speak, this may be because there is no wish to do so (elective or selective mutism), or the result of lesions in the brain, particularly in the posterior fossa. The characteristics of the former children are described, especially their shyness; and it is emphasized that mild forms are quite common and a definitive diagnosis should only be made if the condition is significantly affecting the child and family. In the case of mutism due to organic causes, the commonest of these is trauma to the cerebellum. Operations on the cerebellum to remove tumours can be followed by mut

A follow-up study of 45 patients with elective mutism.

Forty five patients (23 boys and 22 girls) with elective mutism (8.7 +/- 3.6 years old), who were referred to a university department and a child guidance clinic within a 15-year-period, were followed up on average 12 years later. For 41 of them, sufficient information could be obtained at follow-up, and 31 patients could be investigated personally. At follow-up, an interview and a standardized psychopathological examination were carried out as well as two standardized biographic inventories. The main results were: 1) a high load of individual and family psychopathology was characteristic of t

Elective mutism: effect of dental treatment with N2/O2-inhalation sedation: review and report of case.

Elective mutism in children is characterized by a marked, emotionally determined selectivity in speaking, such that the child demonstrates his or her language competence in some situations but fails to speak in other situations. An eight-year-old boy with elective mutism had to undergo restorative dental treatment. It was chosen to use relative analgesia to perform this treatment. The sedation with oxygen and nitrous oxide resulted in complete symptom remission as long as the sedation lasted.

Evaluation and management of the child with speech delay.

A delay in speech development may be a symptom of many disorders, including mental retardation, hearing loss, an expressive language disorder, psychosocial deprivation, autism, elective mutism, receptive aphasia and cerebral palsy. Speech delay may be secondary to maturation delay or bilingualism. Being familiar with the factors to look for when taking the history and performing the physical examination allows physicians to make a prompt diagnosis. Timely detection and early intervention may mitigate the emotional, social and cognitive deficits of this disability and improve the outcome.

[Postoperative pain relief for a patient with elective mutism by patient controlled analgesia].

We previously reported that intrathecal (i.t.) administration of morphine reduced postoperative pain in pediatric patients after spinal instrumentation for scoliosis (Cotrel-Dubousset method), and the i.t. administration of morphine before incision produced better pain relief than that given after the surgical procedure. In this study, we evaluated postoperative pain relief in a patient with elective mutism who had been given i.t. morphine 0.15 mg before surgery. The patient was scheduled to undergo patient controlled analgesia (PCA) intravenously with morphine after surgery. The grade of post

Systematic assessment of 50 children with selective mutism.

OBJECTIVE: To clarify the diagnostic significance of selective mutism (elective mutism in DSM-III-R). METHOD: Fifty children with selective mutism were evaluated systematically by means of semistructured clinical interviews and rating scales to obtain detailed diagnostic information. RESULTS: All 50 children met DSM-III-R criteria for social phobia or avoidant disorder and 24 (48%) had additional anxiety disorders. Clinical measures of anxiety and behavioral symptoms supported the presence of anxiety disorders as a characteristic of selectivity mute children. Only one case each of opposition

The family history of children with elective mutism: a research report.

The family history was studied in children with elective mutism. The samples comprised a series of N = 38 children with elective mutism and a control group of N = 31 children with a similar behavioural phenotype, i.e., the combination of an emotional disorder and a developmental disorder of articulation or expressive language. Interviews were performed with the respective mothers. There was a clear excess of the personality trait of taciturnity in first-, second-, and third-degree relatives. Although mutism was reported almost exclusively in the group of relatives of children that manifested e
