Elective mutism--associated with developmental disorder/delay. Two case studies.

Elective mutism (EM) is not regarded as a separate diagnostic category in the ICD-9, but is included under the heading "313: Disturbance of emotions specific to childhood and adolescence." In the ICD-10 EM is acknowledged as a separate diagnosis defined as "a marked, emotionally determined lack of speech in certain situations in a child with a normal or near normal speech/language ability." The diagnosis excludes pervasive developmental disorder and specific developmental disorders of speech and language. Two patients referred for EM to a child and adolescent psychiatry outpatient clinic, show

[Elective mutism in children: literature review].

This paper presents contemporary opinions about selective mutism in children, including epidemiology, etiology, clinical features and therapy. This is the first extensive review on this topic in Polish literature. The essential feature of selective mutism is persistent failure to speak in social situations, where speaking is expected (e.g., in school), despite speaking in other situations (e.g., at home). The authors present the diagnostic criteria according do DSM-IV and suggested by other authors. Clinical characteristics of this disorder were also presented, including personality traits and

Elective mutism: an analysis of 100 cases.

OBJECTIVE: An extended series of 100 children with elective mutism (EM) was clinically analyzed. METHOD: The total sample included two subgroups of clinically referred children at different locations and a subgroup of nonreferred children with EM. The study was based on comprehensive item sheets and, in the nonreferred sample only, the Child Behavior Checklist. RESULTS: EM is a rare disorder in the referred child psychiatric samples. It typically starts at preschool age, is more common in girls, and is seen in all social strata. A background of migration and early developmental risk factors

Fluoxetine treatment of children with selective mutism: an open trial.

OBJECTIVE: A pilot study was designed to evaluate the safety and efficacy of fluoxetine treatment for children with selective mutism (elective mutism in DSM-III-R). METHOD: Twenty-one children (mean age 8.2 years, range 5 through 14) participated in a 9-week open trial of fluoxetine in graduated doses (mean end dose 28.1 mg, range 10 to 60 mg) with systematic baseline and outcome evaluations and weekly clinical assessment. RESULTS: All 21 children met DSM-III-R and DSM-IV criteria for anxiety disorders. After fluoxetine treatment, 76% were improved, with diminished anxiety and increased spee

Silent eulogy. Elective mutism in a six-year-old Hispanic girl.

A case of elective mutism in a six-year-old Hispanic girl treated in a public mental health setting is presented. The central feature of the case was the facilitation of a delayed grieving process in a little girl who had witnessed her father's death. The presentation stresses the critical need to integrate the unfolding clinical material with the realities of the stresses of parental distress and resistance. An attempt is made to understand this "resistance" in the context of cultural and social realities, while attending to the possible underlying psychodynamic meaning.

Transient mutism following posterior fossa surgery in children.

We report 3 patients, ages 5, 9 and 13 years, with mutism following posterior fossa surgery (PFS). All presented with headache of 10-180 days duration, excellent premorbid learning performance and paucity of neurologic signs. Radiographic studies demonstrated large posterior fossa tumor occupying the fourth ventricle, and hydrocephalus, leading to gross total resection of the tumor. Within 24-48 hr, all patients were mute. None had problems with swallowing and coughing. All were able to nod "yes or no," follow commands, point to body parts, and make their wants known by gestures. Jaw, gag, sno

[Anxiety disorders in the fourth edition of the classification of mental disorders prepared by the American Psychiatric Association: diagnostic and statistical manual of mental disorders (DMS-IV -- options book].

In 1991 the American Psychiatric Association proposed a draft version of the IV edition of Diagnostic and Statistical Manual of Mental Disorders--the DSM IV Options Book. Authors of this version wanted to increase clarity of the criteria sets and to provide compatibility with the Tenth Edition of the International Classification of Diseases (ICD-10). The purpose of this Options Book is to propose some changes in wording, diagnostic divisions and to discuss various options concerning the placement of sections and disorders within the classification. Silent at school--elective mutism and abuse.

A retrospective case-control study of electively mute children from one city is reported. Eight of 18 children selectively mute in school had suffered definite or probable abuse compared with only one control with a speech or language problem, and no classroom controls. The implications for management are discussed.

Treatment of elective mutism with fluoxetine: a double-blind, placebo-controlled study.

OBJECTIVE: To evaluate the efficacy of treatment with fluoxetine in reducing symptoms associated with elective mutism. METHOD: Sixteen subjects with elective mutism were treated with placebo (single-blind) for 2 weeks. Fifteen placebo nonresponders were then randomly assigned to double-blind treatment with fluoxetine at a dose of 0.6 mg/kg/day (N = 6) or continued placebo (N = 9) for an additional 12 weeks. Outcome ratings were completed by the treating psychiatrist, parents, and teachers. RESULTS: Significant improvements over time on ratings of elective mutism, anxiety, and social anxiety,

[An unusual coincidence: elective mutism and sleep-related bioelectric seizure status syndrome].

A case study of a 9-year-old girl is presented which suffers from elective mutism and a rare, special form of cerebral seizures (ESES). After a brief review of the literature concerning these two rare diseases diagnosis, psychotherapy and anticonvulsive treatment as well as a follow-up are described. It is argued that in this case there are two independent, very rare diseases, and problems are discussed which arise from the necessity to integrate two heterogeneous therapeutic concepts.

[Inpatient treatment of a child with elective mutism--a case report].

The paper presents the case of a 10-year-old, electively mute boy, who had stopped talking with all persons outside his immediate family about four years ago. Therapeutic interventions had so far brought no change in his speech behavior. Within a short time (2 months) of residential treatment in a child care unit, his mutistic disorder and social behavior could be normalized and the child even managed to speak in this former school class where nobody had ever heard his voice. The multitude of possible reasons for the mutistic behavior and reinforcing environmental facts are reviewed, the sympt

Symbol use in a case of elective mutism.

A case of elective mutism in a 4-yr.-old girl is discussed with reference to symbol use in the phenomenology and treatment of this disorder. Findings are related to current controversies regarding the classification of this rare disorder, which may be associated with more severe psychotic-spectrum disorders of childhood.

Elective mutism and mental retardation.

The association of elective mutism and mental retardation is noted. Two cases of elective mutism associated with mental retardation are presented. Both individuals had long-standing patterns of inhibition, shyness, and anxiety manifested by general refusal to talk outside the home, particularly at school. The importance of assessment of intellectual functioning and linguistic skills for the establishment of intervention priorities and clinical management is discussed. The possibility of the presence of mental retardation in individuals with elective mutism should be considered.

A study of the effectiveness of a specific treatment for elective mutism.

Elective mutism represents a relatively rare clinical syndrome in which children with normal verbal capabilities totally refuse to speak in select settings for prolonged periods of time. It is often described as particularly treatment resistant. This study describes 20 strictly diagnosed elective mutes treated, using the Hawthorn Center approach. As found in other studies, most of the elective mutes in this sample (90%) were described as controlling, negative, or oppositional, and a high rate (50%) of maternal-child overenmeshment was found. Treatment outcome using the Hawthorn Center approach
