In search of an adult attachment stress provocation to measure effect on the oxytocin system: a pilot validation study.

BACKGROUND: Oxytocin is a promising biomarker for psychiatric conditions arising from early relational trauma, childhood maltreatment, and attachment dysregulation, including posttraumatic stress and dissociative disorders. OBJECTIVE: This exploratory pilot study examined plasma oxytocin as a biomarker for alterations in the attachment system. DESIGN: We used a single group, repeated-measures design with 15 women. The protocol used a film clip previously validated as a provocation to the hypothalamic-pituitary-adrenal axis. RESULTS: The repeated-measures ANOVA showed differences in oxytocin

Treating complex trauma: critical interventions with adults who experienced ongoing trauma in childhood.

Complex trauma (CT) most often results from exposure to severe stressors that begin in childhood or adolescence, occur repeatedly, and take place within the caregiver system. Typically, CT involves repeated incidence of maltreatment over an extended period of time (i.e., months or years) that includes emotional abuse, physical abuse, sexual abuse, neglect, and witnessing family violence. These individuals experience lifelong difficulties related to self-regulation, relationships, psychological symptoms, alterations in attention and consciousness, self-injury, identity, and cognitive distortion

Clinical significance of a proposed developmental trauma disorder diagnosis: results of an international survey of clinicians.

OBJECTIVE: Maltreatment, family violence, and disruption in primary caregiver attachment in childhood may constitute a developmental form of trauma that places children at risk for multiple psychiatric and medical diagnoses that often are refractory to well-established evidence-based mental health treatments. No integrative diagnosis exists to guide assessment and treatment for these children and adolescents. This study therefore assessed clinicians' ratings of the clinical utility of a proposed developmental trauma disorder diagnostic framework. METHOD: An Internet survey was conducted with

A fragile bond: adoptive parents' experiences of caring for children with a diagnosis of reactive attachment disorder.

AIMS AND OBJECTIVES: To understand how adoptive parents caring for children with a diagnosis of reactive attachment disorder (RAD) make sense of their life-worlds by establishing the meanings that underlie and structure their experiences of their everyday lives. BACKGROUND: Reactive attachment disorder is a serious psychosocial disorder of childhood that causes short- and long-term relationship, health and social consequences for children. It is more likely to be observed in children being cared for by foster carers, kinship carers or adoptive parents. Exploration of adoption from parents' pe

Association Between Insecure Attachment and ADHD: Environmental Mediating Factors.

OBJECTIVE: Psychological theories have postulated an association between insecure attachment and ADHD. The objective of this study is to investigate possible association between insecure attachment and ADHD in children and adults. METHOD: Review of literature was performed using the PsycINFO, Medline, and EMBASE databases. RESULTS: Twenty-nine studies were included in the review. Overall, the studies showed that parental attachment problems and environmental mediating factors were significantly associated with childhood ADHD. Adults with ADHD had a much higher incidence of insecure attachmen

Early childhood attachment and its impact on later life resilience: a comparison of resilient and non-resilient female siblings.

Prior research has repeatedly identified early childhood attachment as a primary protective factor. In this study the authors sought to assess if there were significant differences in the attachment levels of resilient females in comparison to their non-resilient female siblings. The authors examine, using standardized attachment scales, the levels of attachment through the use of an intra-family research design. The study consists of a comparative analysis of female inmates from five Midwestern state correctional departments to their resilient female siblings' attachments, in correlation with

Risk factors for vulnerable youth in urban townships in South Africa: The potential contribution of reactive attachment disorder.

Reactive attachment disorder (RAD) is a psychiatric disorder developing in early or middle childhood as a consequence of significant failures in the caregiving environment. RAD results in children failing to relate socially, either by exhibiting markedly inhibited behaviour or by indiscriminate social behaviour and is associated with significant socio-behavioural problems in the longer term. This study examined RAD in South Africa, a setting with high environmental risks. We recruited a sub-sample of 40 10-year-old children from a cohort enrolled during pregnancy for whom early attachment stat

[The link between ambivalent-preoccupied mode of attachment and narcissistic vulnerability in adolescence].

Attachment research has deepened our understanding of the essential continuities and discontinuities of psychological development from childhood to adolescence and to adulthood. This paper considers the relevance of ambivalent-preoccupied attachment, one of the two types of insecure attachment, for understanding emotional vulnerability during adolescence. How current social and economic conditions have exacerbated the effects of ambivalent-preoccupied attachment in the attainment of the developmental tasks of adolescence is considered. By describing three clinical vignettes clinical recommenda

[Do childhood psychological trauma result in neurobiological changes in the adult brain?].

Since 1995 it is known that aversive and traumatic experiences in childhood, adolescence or adulthood correlate with measurable changes in the brain. Particularly, hippocampus-atrophies as well as reduction in amygdalae volumes have been investigated and documented. Furthermore, experiences of extreme form of early neglect have been associated with general reduction of brain volumes. Recent research documents neural correlates of dissociation.

The significance of insecure and disorganized attachment for children's internalizing symptoms: a meta-analytic study.

This meta-analytic review examines the association between attachment and internalizing symptomatology during childhood, and compares the strength of this association with that for externalizing symptomatology. Based on 42 independent samples (N = 4,614), the association between insecurity and internalizing symptoms was small, yet significant (d = 0.15, CI 0.06~0.25) and not moderated by assessment age of internalizing problems. Avoidance, but not resistance (d = 0.03, CI -0.11~0.17) or disorganization (d = 0.08, CI -0.06~0.22), was significantly associated with internalizing symptoms (d = 0.1

Neurodevelopmental problems in maltreated children referred with indiscriminate friendliness.

We aimed to explore the extent of neurodevelopmental difficulties in severely maltreated adopted children. We recruited 34 adopted children, referred with symptoms of indiscriminate friendliness and a history of severe maltreatment in their early childhood and 32 typically developing comparison children without such a history, living in biological families. All 66 children, aged 5-12 years, underwent a detailed neuropsychiatric assessment. The overwhelming majority of the adopted/indiscriminately friendly group had a range of psychiatric diagnoses, including Attention Deficit Hyperactivity Dis

Childhood emotional abuse and attachment processes in the dyadic adjustment of dating couples.

In an effort to improve understanding of the mechanisms that link early maltreatment to later outcomes, this study investigated the mediation effects of adult attachment processes on the association between childhood emotional abuse and later romantic relationships among heterosexual couples. College students and their dating partners (N = 310; 155 couples) completed the Childhood Trauma Questionnaire, Experiences in Close Relationship Scale, and Dyadic Adjustment Scale. Using the Actor–Partner Interdependence Model (Kenny, Kashy, & Cook, 2006), multilevel modeling results indicated that memor

The effect of extremely preterm birth on attachment organization in late adolescence.

BACKGROUND: Prior studies have examined the impact of preterm birth on the quality of the attachment relationship to the mother in infancy, but few have examined extremely preterm born infants and almost no data have been reported on prematurity and its impact on the attachment organization attained after childhood. METHODS: Thirty-nine adolescents born extremely preterm and 39 full-term born control participants were assessed with the Adult Attachment Interview. RESULTS: The prematurely born showed lower scores regarding measures of attachment security and, in particular, a higher proportio

The experience-dependent maturation of prefronto-limbic circuits and the origin of developmental psychopathology: implications for the pathogenesis and therapy of behavioural disorders.

The maturation of prefronto-limbic neuronal pathways that mediate essential affective and social regulatory functions is experience dependent. Immediately after birth the infant's affective experiences, especially those embedded in the relationship with the primary caregiver, trigger the reorganization and adaptive fine-tuning of synaptic circuits. Research in humans and in animal models supports the hypothesis that socio-emotional deprivation and emotional trauma during early childhood may leave 'scars' in prefronto-limbic function, brain regions that are essential for emotional behaviour, le

Understanding maltreating mothers: a synthesis of relational trauma, attachment disorganization, structural dissociation of the personality, and experiential avoidance.

Treatment options are limited for families in which the child has severe and intractable disturbances of emotion and behavior, in which there is suspected or confirmed maltreatment by the mother, and in which the mother has her own history of childhood neglect and abuse. This paper proposes a model for understanding maltreatment in mother-child dyads, drawing upon the developmental psychopathology, behavior, and trauma literatures. At the core of this model is the hypothesis that a mother's maltreating behavior arises from unconscious attempts to experientially avoid the reemergence of an atta

Personalities of adults with traumatic childhood separations.

OBJECTIVES: This study examined personality characteristics and identified personality subtypes of adults with childhood histories of traumatic separations from a parent. Previous work from attachment theory and developmental psychopathology suggests that distinct developmental trajectories might lead to different styles of personality adaptation after an attachment disruption. design: Randomly selected psychologists and psychiatrists provided data on 203 adults with histories of traumatic separations using a personality pathology instrument designed for use by clinically experienced observers

[Unresolved attachment and remembered childhood trauma in patients undergoing psychosomatic inpatient treatment].

OBJECTIVE: Clinical populations are characterised by a high level of childhood trauma and unresolved attachment status ("U"). Unresolved attachment status indicates the inability to integrate attachment-related dangers. Little is known about the interaction of traumatic childhood experiences and an unresolved attachment status. METHOD: We administered the Adult Attachment Projective Picture System (AAP) to 45 newly admitted psychosomatic inpatients. Childhood trauma (CTQ) and distress were assessed by self-report questionnaires, and cognitive-emotional development was evaluated with the Level

[A case of Asperger's disorder with catatonia originally suspected of being catatonic schizophrenia].

We report the case of an adolescent male who presented with mutism, immobility, catalepsy, and mannerisms. The patient was admitted to our hospital with suspected catatonic schizophrenia; however, he was subsequently diagnosed with catatonia due to Asperger's disorder. The patient was a 16-year-old male. More than six months before presentation, his grandfather displayed bizarre and violent behavior. Subsequently, he began to experience catatonia, which eventually led to hospitalization. Treatment with diazepam improved his condition and, as no causal disorders other than Asperger's disorder w
