Behavioral inhibition in children: a possible precursor to panic disorder or social phobia.

A biological-environmental interaction currently provides the best explanation of an anxiety disorder's evolution. In this sense, anxiety disorders are like other medical disorders for which a person may have a predisposition. Our knowledge of the evolution of anxiety disorders would be enhanced by the ability to identify those persons predisposed to anxiety and to identify such "proneness" before an anxiety disorder emerges in adulthood. We discuss the developmental aspects of panic disorder and social phobia, in particular findings suggesting that behavioral inhibition in children may be a p

Acute respiratory illness in Adelaide children. II: The relationship of maternal stress, social supports and family functioning.

In a community-based study we investigated the relationships between maternal stress, maternal social supports, family functioning and proneness to acute respiratory illness (AR1) in childhood. 'Prone' and 'not prone' children were identified from the responses to a mail questionnaire sent to the addresses of a randomly selected group of Adelaide children who had been born in 1983. 'Prone' children (n = 255) were defined by a respiratory score (based on frequency and severity of reported symptoms in the preceding 12 months) in the top quintile of the distribution, while 'not prone' children (n

A follow-up study of childhood psychosis.

A cohort of 227 children having attended the same day treatment facility during their childhood was followed-up in adult life. A questionnaire was designed to collect data on various aspects of social and personal adjustment. The length of the follow-up period is 17 years. Information was obtained on 99 subjects whose mean age was 27-years. Of these, 55 had been admitted to the facility for childhood psychosis, and the remaining 44 subjects served as a comparison group. The results show that, in adulthood, the psychotic children fare significantly worse than the controls for employment, autono

Symptom development in childhood onset schizophrenia.

Symptom development from birth to 12 years of age was examined in 18 children who met DSM-III criteria for schizophrenia with onset before 10 years of age. Using a follow-back design, symptom development was rated at each of four age levels using a DSM-III Symptom Rating Scale and the Achenbach Child Behavior Checklist. Results revealed a gradual developmental unfolding of a broad spectrum of symptoms affecting social, cognitive, sensory and motor functioning and beginning many years before the appearance of schizophrenic symptoms--usually in early infancy. Prior to 6 years of age, severe lang

[Ethological methods exemplified by early childhood autism].

When humans are primarily impaired or overtaxed in their more elaborate abilities of coping with the social environment they seem to fall back on more primitive coping behaviours known from other mammals in severe conflict situations. This seems true for some behaviours in early childhood autism. Ethology has specialized in analyzing such non-verbal behaviour. Ethological motivation analysis, the study of context dependence and of the development of such behaviours in the individual patient should be exploited for their interpretation. Mere description of behaviour is not likely to contribute

Childhood parental death and adult psychopathology.

Childhood parental death has frequently been linked with adult mental disorders--mostly depression. The authors found no association with psychiatric diagnosis among 72 inpatients who had experienced the death of a parent when they were children, compared with 460 other patients in the Chestnut Lodge Follow-Up Study. The patients with a childhood parental death did, however, have significantly greater family pathology and impaired social and heterosexual functioning. These results refute the view that childhood parental death is singularly casual of adult psychopathology but support its role i

Neuropsychology of autism.

Childhood autism is a syndrome characterized by a triad of abnormalities in social interaction, verbal and nonverbal communication, and imaginative activity. It is clinically heterogeneous, however, and probably represents a family of syndromes, each of which is the result of a different but related type of brain dysfunction. So far, there is no direct evidence for involvement of particular brain systems, although analogies to other disorders in humans and experimental animals do allow some plausible hypotheses.

Social and familial factors in the development of early childhood asthma.

The role of social and familial factors in the development of childhood asthma by age 6 years was studied in a birth cohort of New Zealand children. Rates of asthma varied markedly with the child's sex; boys had twice the rate of asthma as girls. In addition, the factors associated with asthma varied with the child's sex. For boys, wheeze during infancy, early eczema, and parental asthma were all significant risk factors; for girls, the only risk factor was early eczema. Proportional hazards modeling of the data failed to show any significant associations between the development of asthma and

Epidemiology of Hodgkin's disease.

Hodgkin's disease displays an intriguing variation in incidence with age. For adult disease, there is striking bimodality in incidence with peaks in young adulthood and in older adulthood. On epidemiologic grounds, there appear to be three different diseases: childhood (0-14 years), young adult (15-34 years), and older adult Hodgkin's disease (55+ years). There are marked geographic variations in Hodgkin's disease incidence, but the variations are different for the three age groups. There are also interesting associations between Hodgkin's disease and markers of social class. For the young adu

The social function of young adults who had arthritis in childhood.

One hundred twenty-one young adults 18 years of age or older who had been seen at this hospital with juvenile arthritis since 1955 were surveyed by a series of questionnaires to determine current social function and financial status. Chart reviews and patient questionnaires were used to determine modes of onset and the social circumstances of the family at the onset of the arthritis. The patients as a whole were functioning well: 45 were working full time, 25 were in school full time, 23 were in school and working, and 14 were married women at home. Females had a higher prevalence of persistin

Hormones and psychosexual differentiation: implications for the management of intersexuality, homosexuality and transsexuality.

During fetal development of subprimate mammals, sexual differentiation of the genitals and of specific sex-dimorphic brain systems depends on androgens; corresponding sex differences are displayed in prepubertal behaviours as well as in behaviours that depend on activation by pubertal hormones. In human beings, fetal hormones play the same role in genital differentiation. Hormone-dependent structural brain changes are also very likely but have not yet been demonstrated. The corresponding effects of fetal hormones on childhood behaviour have been found both in subhuman primates and in man, whil

Cognitive and emotional functioning in hypopituitary short-statured children.

Eleven children with documented growth hormone deficiency were studied to assess their cognitive and emotional functioning and their academic achievement before and after 1 year of human growth hormone replacement therapy. Standardized personality and intelligence measures were used, and records of school achievement, as well as developmental and family history from parents, were obtained. Although some subjects in this group were coping with problems other than their short stature (e.g., other medical and psychosocial problems), their intelligence and academic achievement were found to be pos

Child competence and psychiatric risk. III. Comparisons based on diagnosis of hospitalized parent.

The school competence of children whose parents were previously hospitalized for psychiatric disorder was studied. These children, at risk themselves for behavioral disorder, were compared to classmates as well as to each other based upon the diagnosis of the hospitalized parent. Four diagnostic groups were employed: narrowly defined schizophrenic, broadly defined schizophrenic, affective psychotic, and hospitalized nonpsychotic patients. Significant differences between the risk sample and classmates are reported along with differences among the diagnostic groups. Sons of nonpsychotic hospital

A retrospective analysis of the TATs of children at risk who subsequently broke down.

A retrospective evaluation of stories told to three Thematic Apperception Test (TAT) cards by children at risk isolated six characteristics that were associated with functioning six to 10 years later. The characteristics included lack of individual initiative, denial of mother-child relationships, denial of negative outcomes, and lack of autonomy. The TATs of children at risk (due to one parent's hospitalization for a psychotic illness) who subsequently suffered major decompensation themselves (requiring hospitalization or intensive outpatient therapy) could be discriminated from those of chil

Severe impairments of social interaction and associated abnormalities in children: epidemiology and classification.

The prevalence, in children aged under 15, of severe impairments of social interaction, language abnormalities, and repetitive stereotyped behaviors was investigated in an area of London. A "socially impaired" group (more than half of whom were severely retarded) and a comparison group of "sociable severely mentally retarded" children were identified. Mutism or echolalia, and repetitive stereotyped behaviors were found in almost all the socially impaired children, but to a less marked extent in a minority of the sociable severely retarded. Certain organic conditions were found more often in th
