After a year spent working for Jean-Martin Charcot, Georges Gilles de la Tourette published an article in 1885 that would become a seminal work, reporting on nine cases of tic disease. However, only four observations meet the current criteria of the eponym, including echolalia and coprolalia, the latter term coined by Gilles de la Tourette. In 1886, Charcot asked Georges Guinon to complete the clinical picture by adding obsessional disorders temporary control of tics, and the premonitory sensation leading to their occurrence. In 1888, Charcot presided over the jury for Grégoire Breitman's thes

Tics are the most common hyperkinetic manifestations during development. The clinical phenomenology of motor tics ranges from mild twitches affecting a single facial muscle to orchestrated contractions of different muscular districts resembling purposeful behaviors. Likewise, the repertoire of vocal tics (also called phonic tics) covers the whole spectrum between isolated grunting noises and meaningful strings of words. Simple and complex tics arguably sit on a continuum of symptom severity and respond to the same treatment interventions. The diagnosis of Gilles de la Tourette syndrome (GTS) i

Sleep disturbances are highly prevalent in patients with tic disorders and Tourette syndrome, with up to 80% of patients experiencing sleep difficulties. Insomnia is notably present, and pediatric patients often experience parasomnias, including sleepwalking and night terrors. Disruptions in circadian rhythms are also reported, with a pronounced evening chronotype. Concurrently, patients may also suffer from sleep-related movement disorders, and tics are reported to often persist during sleep, further contributing to significant sleep fragmentation. The presence of comorbid attention-deficit h

Tourette syndrome (TS) is a neuropsychiatric and a neurodevelopmental disorder often presenting alongside other neurodevelopmental conditions such as attention deficit/hyperactivity disorder (ADHD), autism spectrum disorder (ASD), learning difficulties, and dyspraxia. TS shares genetic and neurobiologic features with ASD and ADHD, suggesting common underlying mechanisms. Recent diagnostic frameworks such as the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, APA, 2013) allow for multiple co-occurring diagnoses, reflecting the complex overlap among neurodevelopmenta

Sexuality and sexual health are essential for the overall well-being of all individuals. In neurology, research traditionally focused on disorders of sexual function associated with syndromes that present with specific lesions of the spinal cord and brain. In neuropsychiatric tic disorders, however, early research addressed sexuality and sexual behaviors in a wider context and beyond the strict diagnostic categories of sexual dysfunction, including gender identity and sexual orientation. Although these studies provided important insights in the behavioral structure of adults with primary tic d

The objective of this chapter is to provide a clinically useful review of epidemiologic studies of Tourette syndrome (TS). The viewpoint of this chapter is that there are significant challenges to validity in epidemiologic studies of TS and co-occurring conditions but that, nonetheless, understanding this literature has value for clinicians, educators, and healthcare policymakers. The topics covered in the chapter, following background provided in the introduction, are as follows: (1) prevalence estimates, (2) risk factors, and (3) co-occurring conditions.

Tourette syndrome (TS) is a childhood-onset neurodevelopmental disorder characterized by multiple motor tics and at least one vocal tic lasting for more than a year. It is a complex, polygenic, and multifactorial disorder. In this chapter, we briefly discuss the early genetic studies on TS, and then provide a detailed overview of current efforts to uncover common and rare variants contributing to TS susceptibility. We also summarize pathway analyses which combine information from multiple loci to identify converging cellular processes and pathways associated with TS pathogenesis. Since TS is h

In the past decades, the connection between the immune system and Tourette syndrome (TS) has been explored as a potential pathophysiologic key aspect. This idea is supported by studies describing the effect of immune cells and inflammatory mediators in neurodevelopment by acting in synaptic arrangement, pruning, and maturation of neural circuitries. Supporting evidence for that correlation is varied, including genome-wide association studies, transcriptome analysis, animal models, and clinical populational-based studies. Moreover, the influence of environmental aspects typically involved in im

Gilles de la Tourette syndrome (GTS) is increasingly conceptualized as a neurodevelopmental network disorder. In this chapter, we have synthesized two decades of neuroimaging findings spanning gray- and white-matter morphology, structural connectivity, and functional network dynamics in GTS across childhood and adulthood. Structural MRI studies using voxel-based morphometry (VBM) have reported heterogeneous volume differences-both decreases and increases-across cortico-striatal-thalamo-cortical regions, motivating the use of meta-analytic approaches to identify more consistent loci, including

Tourette syndrome (TS) and primary tic disorders are increasingly understood as neurodevelopmental conditions arising from dysfunction within the cortico-basal ganglia-thalamo-cortical circuits, which govern motor, cognitive, and affective processes. This chapter reviews current knowledge on the neurochemical underpinnings of TS, drawing on evidence from neuroimaging studies - notably positron emission tomography - genetics, animal models, and neuropathology. Particular emphasis is placed on the roles of dopamine, serotonin, glutamate and γ-aminobutyric acid (GABA). Dopaminergic hyperactivity,

Premonitory sensations, or urges, have become a central focus in the study of Gilles de la Tourette syndrome (TS). These bodily sensations, which precede tics, play a crucial role in the clinical manifestations of the syndrome. Tics are often performed to alleviate the discomfort caused by premonitory urges and provide temporary relief. Despite their importance, the causes and mechanisms of premonitory urges remain poorly understood. The severity of tics and an individual's ability to control them are closely related to the intensity and presence of these urges. Current research is actively in

Tics, such as those seen in Tourette syndrome (TS), are brief, sudden, and involuntary movements or vocalizations. Although tics are involuntary, most individuals with TS can suppress them on command for short periods of time. This capacity, which is called voluntary tic suppression, is a hallmark feature of TS and differentiates it from most other movement disorders. Several studies have investigated inhibitory control of voluntary actions in TS, given its potential associations with both tic expression and voluntary tic suppression. However, studies on response inhibition in TS have produced

Treating children and adults with Gilles de la Tourette syndrome (GTS) remains a clinical challenge and typically requires a multidisciplinary approach, owing to the complexity of the disorder and the frequent presence of comorbidities such as obsessive-compulsive disorder and attention-deficit/hyperactivity disorder. This chapter focuses on the pharmacologic management of tics in the context of GTS. It reviews key considerations in the selection of therapeutic agents from the broad range of pharmacologic options available in clinical practice. The strongest evidence supports the use of dopami

Gilles de la Tourette syndrome (GTS) is a childhood-onset neurodevelopmental disorder characterized by motor and vocal tics and commonly associated with comorbid psychiatric and behavioral symptoms. Medication or behavioral therapies are effective for treating GTS in most cases; however, a proportion of individuals experience treatment-refractory tics. Deep brain stimulation (DBS) is an invasive surgical therapy reserved for select individuals with severe, treatment-refractory GTS. DBS has been targeted to several brain regions involved in the basal ganglia-thalamo-cortical networks implicated

This chapter explores evidence for the effectiveness of noninvasive brain stimulation (NIBS) for tic disorders and explores what the future may hold for these approaches. NIBS can be achieved through several means, including electrical stimulation of peripheral nerves and transcranial cortical stimulation using electrical or magnetic stimulation. Each of these techniques will be discussed with a focus on current evidence of effectiveness in treating tics, benefits, limitations, and current unknowns.

While Tourette syndrome (TS) is defined by tics, comorbidities are very much part of the clinical presentation - 90% of patients having a least one associated condition, most prominently obsessive-compulsive disorder (OCD) and attention-deficit/hyperactivity disorder (ADHD), but also learning disabilities, anxiety, depression, rage outbursts, autism spectrum disorder, sensory hypersensitivities, and sleep difficulties. Moreover, the associated symptoms often are more problematic than tics, or interact with the tics such that the different symptoms exacerbate each other, adding complexity to th

Support associations specifically for tic disorders, such as Tourette syndrome (TS), have the potential to profoundly impact individuals, families, and general awareness. This chapter explores the multifaceted roles of support associations, as well as their evolution - from small groups formed by individuals with a vested interest to large organizations managing multimillion-dollar research funding. Support associations face many challenges, making collaboration within and between countries essential. People with lived experience, clinicians, and researchers each bring valuable expertise to ef

Functional tics and tic-like behaviors belong to the wide spectrum of functional movement disorders. Until the early 2010s, functional tic disorders were rather uncommon in the differential diagnosis of tics, and only few cases describing their features had been published. However, over the past 10 years there has been a steady increase in the frequency of these cases that peaked throughout the COVID-19 pandemic. On the one hand, the rise in functional tic cases created new challenges of diagnosis and treatment. At the same time, it also pushed the field forward to delineate helpful clinical c

Movement disorder definitions that include the clinical characteristics "sudden, rapid, repetitive, brief, and involuntary" are not limited to motor tic disorders. Hence, it is essential that caregivers be aware of other relatively common conditions that could be confused with the diagnosis of tics. Included in this differential are entities such as repetitive and restricted behaviors (RRBs), habits, body-focused repetitive behaviors (BFRBs), and motor stereotypies. The importance of separation of these individual conditions is further emphasized by the recognition that they can be coexisting

This chapter provides a review of obsessive-compulsive disorder (OCD), as it manifests in primary tic disorders and Gilles de la Tourette syndrome (GTS). We examine the clinical features of OCD in GTS, focusing on how these symptoms diverge from typical OCD presentations, often blending with or mimicking tics. Key pathophysiologic mechanisms are explored, with an emphasis on the neurobiologic substrates that underlie this unique OCD subtype and distinctions from tics. Finally, the chapter discusses therapeutic approaches specifically adapted for this population, highlighting both established a
