Developmental delay in attaining adult levels of motor excitability in children and adolescents with Tourette syndrome: a mega-analysis study.

Tourette syndrome (TS) is a neurodevelopmental disorder of childhood onset characterised by vocal and motor tics and is associated with cortical-striatal-thalamic-cortical circuit [CSTC] dysfunction. TS often follows a developmental time course in which tics become increasingly more controlled during adolescence. However, many individuals continue to have debilitating tics into adulthood. This indicates that there may be important differences between adults with TS for whom the clinical phenotype is more stable, and children and adolescents with the disorder who may be undergoing developmental

The Chinese herbal formula pediatric Anshen Bunao Granules for the treatment of tic disorder: study protocol for a randomized, double-blind, active-controlled trial.

BACKGROUND: Tic disorder (TD) affects 2.68% of children in China. It is classified as a movement disorder, with dysfunction in cortical-striatal-thalamic-cortical circuits implicated in its pathophysiology. Western medical treatments for TD may be associated with adverse reactions and suboptimal adherence in some patients. Pediatric Anshen Bunao Granules (PABG) is an in-house traditional Chinese medicine (TCM) preparation with over 20 years of clinical use for TD, approved by the Guangdong Provincial Drug Administration for use in medical institutions across Guangdong Province. Observational s

From urge to action: an integrative neuropsychological model of tic generation in Tourette syndrome.

BACKGROUND: Tourette syndrome (TS) has traditionally been conceptualized as a movement disorder characterized by involuntary motor and vocal tics. However, growing evidence suggests that tic generation involves complex interactions between neural, physiological, and psychological processes, particularly the role of premonitory urges (PU). OBJECTIVE: This paper aims to develop an integrative model of tic generation by synthesizing recent evidence across neurobiological, neurophysiological, and psychological domains. METHODS: A narrative review of PubMed-indexed literature (2020-2026) was cond

Efficacy and Safety of Deutetrabenazine in Tourette Syndrome: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Tourette syndrome (TS) is a neurodevelopmental disorder characterized by motor and vocal tics that can cause significant functional impairment. Deutetrabenazine, a vesicular monoamine transporter 2 (VMAT2) inhibitor, has been evaluated in randomized controlled trials (RCTs) for tic reduction; however, individual studies have reported mixed results across clinical outcomes. We conducted a systematic review and meta-analysis to assess the efficacy and safety of deutetrabenazine compared with placebo in patients with Tourette syndrome. METHODS: A comprehensive literature search of Pu

Neuro-behavioral Impact of Tourette-Related Striatal Disinhibition in Rats.

Tourette syndrome (TS) has been linked to reduced GABAergic inhibition, the so-called neural disinhibition, in the dorsal striatum. Dorsal striatal neural disinhibition in animal models, caused by local microinfusion of GABA-A receptor antagonists, produces striatal local field potential (LFP) spike-wave discharges and tic-like movements resembling motor tics. Here, we characterized further the neuro-behavioral impact of striatal disinhibition, by unilateral picrotoxin infusion into the anterior dorsal striatum, in adult male Lister hooded rats. In vivo electrophysiology under anesthesia revea

Tourette syndrome: brain neurophysiology, circuit dysfunction, and neuromodulation across invasive and noninvasive approaches.

PURPOSE OF REVIEW: We review recent electrophysiological advances that have refined our understanding of tic generation in Tourette syndrome by characterizing dysfunction across cortico-striato-pallido-thalamo-cortical (CSPTC) circuits and their time-resolved dynamics. We integrate evidence across spatial scales - from single-unit activity to large-scale network coupling - and examine how these findings provide a mechanistic framework for interpreting both invasive and noninvasive neuromodulation approaches. Finally, we discuss how electrophysiological signatures of tics and premonitory urges

Clinical significance of serum brain-derived neurotrophic factor and serotonin levels in children with tic disorders.

OBJECTIVE: Tic disorders (TD) are diagnosed mainly through clinical observation, and accessible peripheral biomarkers that reflect neurobiological alterations remain limited. Brain-derived neurotrophic factor (BDNF) and serotonin (5-HT) are biologically relevant to synaptic plasticity and neurochemical regulation. This study aimed to investigate serum BDNF and 5-HT levels in children with TD and to explore their associations with disease severity and clinical subtypes. METHODS: A total of 125 children with TD and 100 age- and sex-matched healthy controls were enrolled. Serum BDNF and 5-HT con

Tics severity in Tourette syndrome associated with higher glutamatergic activity in the anterior cingulate cortex.

Tourette syndrome is a neuropsychiatric disorder whose physiopathology is still poorly understood. It involves alterations in the dopaminergic system within the cortico-basal ganglia-thalamo-cortical loops although several hypotheses from the literature implicate the glutamatergic system. Our objective was to study the activity of NMDA receptors (NMDARs) in vivo in patients with Tourette syndrome compared to healthy controls. PET imaging was used in patients with Tourette syndrome (n = 12) and healthy controls (n = 12) with a new radioligand, the [18F]-FNM, which binds to activated NMDARs. Cli

Effects of Tic Suppression and Urge Monitoring on Premonitory Urges and Tic Frequency in Tourette Syndrome.

OBJECTIVES: Behavioral interventions for Tourette syndrome (TS) involve tic suppression and attention to premonitory urges. However, some patients believe that focusing on tic symptoms or urges may worsen symptoms, and that suppressing tics may exacerbate urges. Using a single experimental dataset with real-time urge monitoring, this study addressed two analytical aims. Aim 1 examined whether tic suppression worsens the intensity, frequency, and persistence of premonitory urges. Aim 2 examined whether sustained attention to urges increases tic frequency or worsens subjective urges. METHODS: T

Tourette Syndrome and Rage Attacks: A Longitudinal and Cross-Sectional Study.

INTRODUCTION: Rage attacks (RA) are prevalent in patients with Tourette syndrome (TS) and affect their quality of life severely. Due to TS's relationship with attention deficit hyperactivity disorder (ADHD) and obsessive-compulsive disorder (OCD), a genetic reason or an increased level of irritability from early in life could be hypothesized. This longitudinal study aimed to examine the correlation between tic severity and comorbidities and RA at baseline and 6 years later. We hypothesize that a higher symptom load of TS and comorbidities results in more RA. METHODS: We examined 314 patients
