Linking socioeconomic context to functional brain network abnormalities and clinical severity in children with Tourette syndrome.

BACKGROUND: Tourette syndrome (TS) shows substantial clinical heterogeneity, but the factors contributing to this variability remain unclear. Socioeconomic status (SES) may influence brain functional connectivity (FC) and symptom expression, yet its role in pediatric TS is still not well understood. OBJECTIVE: To clarify the role of SES in modulating brain-behavior relationships in TS. METHODS: Resting-state fMRI data were acquired from 67 children and adolescents with TS and 49 matched controls. FC was computed within and between seven predefined brain networks. We identified group differen

Subacute convexity subdural hematoma in a child with malignant Tourette syndrome: a case report.

Tourette syndrome is usually benign, but in malignant forms violent motor tics can cause serious injury. We describe a 14-year-old boy with malignant Tourette syndrome and comorbid attention-deficit/hyperactivity and obsessive-compulsive disorders who developed progressive, treatment-refractory trunk and head tics despite multiple pharmacological regimens. Headache and vomiting were initially attributed to recent psychopharmacological changes, and neurological examination and laboratory tests were unremarkable. A brain MRI ordered to investigate these symptoms unexpectedly revealed a subacute,

From cradle to Tic: early developmental profiles of tourette syndrome and its comorbidities in the MoBa cohort.

Tourette syndrome (TS) is heterogeneous and frequently co-occurs with attention-deficit/hyperactivity disorder (ADHD) and other recorded neurodevelopmental diagnoses. We examined whether early parent-report questionnaire profiles in the Norwegian Mother, Father and Child Cohort Study (MoBa) differed across later TS diagnostic subgroups, focusing on developmental phenotyping rather than individual-level prediction. The analytic sample included 92,875 children with linked registry follow-up and at least one retained questionnaire predictor at 6, 18, or 36 months. For this project, registry acces

Brain volumetric changes in pediatric patients with Tourette syndrome.

Tourette syndrome (TS), a common pediatric neurodevelopmental disorder characterized by motor and vocal tics, is often accompanied by high rates of comorbidities, including attention-deficit/hyperactivity disorder and obsessive-compulsive disorder. The TS pathophysiology remains incompletely understood, with current theories focusing on the basal ganglia and cortico-striatal-thalamo-cortical (CSTC) circuit dysfunction. However, discrepancies in the brain volumetric changes were reported by studies involving children with TS. Therefore, the present study aimed to analyze the brain volumetric ch

An Online Mindfulness-Based Intervention for Tic Disorders: Effect on Comorbid Symptoms.

BACKGROUND: Mindfulness-based approaches show promise for reducing tic severity, yet little is known about their effect on comorbid symptoms. OBJECTIVES: The objective was to evaluate secondary outcomes from a pilot randomized controlled trial comparing mindfulness-based intervention for tics (MBIT) with psychoeducation, relaxation, and supportive therapy (PRST). METHODS: Twenty-eight adults with Tourette's syndrome or persistent tic disorder were randomized to MBIT or PRST and completed measures of obsessive-compulsive (OC) symptoms, attention-deficit/hyperactivity disorder (ADHD) symptoms,

MindTS-Net: A multimodal mindfulness-based intervention framework for children with Tourette syndrome.

Tourette syndrome affects children's daily functioning and creates a need for supervised support that responds to tic behaviors, emotional states, and interaction contexts. We developed MindTS-MMD, a Chinese multimodal dataset capturing emotion, tic-related behaviors, interaction context, and Valence-Arousal ratings, and MindTS-Net, a supervised digital adjunct linking multimodal emotion perception, mindfulness strategy planning, child-appropriate guidance generation, and dialogue-state tracking. Evaluation included dataset benchmarking, a 14-day primary clinical study with 60 children, and an

The potential role of aberrant microglial synaptic pruning in the neurodevelopmental pathogenesis of tourette syndrome.

Tourette syndrome is a neurodevelopmental disorder traditionally attributed to dopamine system hyperactivity within the cortico-striato-thalamo-cortical circuitry. However, classical neurotransmitter hypotheses fail to fully explain the spatiotemporal and developmental specificities of the disorder. Consequently, research focus has shifted toward the neuroimmune microenvironment, specifically the role of microglia. This review aims to comprehensively explore the potential mechanisms of microglia-mediated aberrant synaptic pruning in the pathogenesis of tourette syndrome and to evaluate emergin

[Beyond words: The complexity of differential diagnosis and treatment of severe ritualized behaviors in an autistic adolescent without functional language and with an intellectual disability: A case report].

Autistic youth diagnosed with an associated intellectual disability (ID) represent nearly one third of the autistic population. However, the majority of studies on mental health and autism focus on autistic individuals without ID. Therefore, the mental health needs of autistic youth with an ID are still poorly understood. This case report focuses on the psychiatry follow-up of Nabil (fictitious name), an autistic adolescent with ID who has been treated for nearly 10 years for various symptoms, including ritualized behaviors that are part of a complex and rare diagnostic profile. In terms of ne

Microbial metabolites at the nexus of gut-brain communication and neurodevelopmental disorders.

The microbiota-gut-brain axis (MGBA) has emerged as a critical regulator of neurodevelopment, with microbial metabolites serving as key signaling molecules that bridge the intestinal ecosystem and the central nervous system. This review gathers current evidence that connects disruptions in microbial metabolites to the pathogenesis of neurodevelopmental disorders (NDDs), including autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD). Our comprehensive overview discusses major neuroactive metabolite classes-short-chain fatty acids (SCFAs), tryptophan derivatives, bi

Tic-Related Functional Impairment Drives the Relationship Between Tics and Depression in Youth With Tourette Syndrome.

Although many youth with Tourette syndrome (TS) have at least 1 comorbid psychiatric condition (eg, obsessive-compulsive disorder), the extent to which TS is associated with depression remains unclear. This cross-sectional study examined relationships among tic severity, tic-related functional impairment, and depressive symptoms in 277 youth with TS. Tic-related impairment, but not tic severity, was associated with both self- and parent-reported depressive symptoms. An age × sex interaction emerged, such that among younger participants, females reported higher depressive symptoms than males; h

Mutism in a patient with Gilles de la Tourette syndrome.

A 5-year-old boy with a prior history of stuttering presented to the neuropaediatrics clinic with prominent motor and vocal tics, including intrusive syllables, was diagnosed with Gilles de la Tourette syndrome (GTS). One month later, near-complete mutism developed acutely, which improved progressively after treatment with risperidone. During the follow-up, motor and vocal tics fluctuated concurrently, but without clear-cut mutism. GTS encompasses a range of vocal tics, including vocal-blocking phenomena, which made us postulate that this case may represent an extreme expression of vocal block

Cervical dystonia as the presenting symptom of novel calcium voltage-gated channel subunit alpha1A mutation.

A woman in her mid-50s presented with an uncomfortable pulling sensation in the right neck that worsened with movement. This was diagnosed as cervical dystonia and treated symptomatically for several years through trials of botulinum toxin. As the years progressed, her symptoms worsened and evolved with the patient eventually experiencing debilitating ataxia requiring use of a wheelchair for mobility, severe dysphagia requiring the use of a feeding tube for nutrition and dysarthria requiring the need of a family member for translation. Through genetic testing, the patient was diagnosed with sp

What can clinical disorders involving the basal ganglia tell us about the pathophysiology of obsessive and compulsive symptoms?

The neurobiology and pathological mechanisms underlying obsessive-compulsive symptoms have been explored in the literature for more than fifty years without reaching a definitive conclusion. What is clear is that some brain areas, including the basal ganglia and connected structures, have been implicated through several different approaches, including use of animal models and examination of human subjects with different cerebral lesions and pathologies. The purpose of this narrative review is to explore how examination of disorders affecting the basal ganglia can give insights into the pathoph

The Impact of Provider Goals, Perceptions, and Beliefs on the Delivery of Comprehensive Behavioral Intervention for Tics.

Comprehensive Behavioral Intervention for Tics (CBIT) is the first-line treatment for Tourette Syndrome and other persistent tic disorders. Efforts to increase access to CBIT have involved training a wide range of providers across practice settings and expanding delivery methods (e.g., teleCBIT, group-based CBIT). Literature regarding how providers implement CBIT across settings is lacking. The current study analyzed surveys from 101 CBIT providers across the United States and Canada to better understand CBIT practice patterns and beliefs. Results indicated that while respondents across discip

Adjunctive psychobiotic Lactiplantibacillus plantarum PS128 ameliorates gastrointestinal comorbidities and alters circulating miRNAs and gut microbiota in children with Tourette syndrome: a randomized controlled trial.

Tourette syndrome (TS) is a neuropsychiatric disorder characterized by involuntary motor and vocal tics. Emerging evidence suggests that microRNA (miRNA) regulation and the gut-brain axis (GBA) may contribute to TS pathophysiology. Lactiplantibacillus plantarum PS128 (PS128) is a well-studied psychobiotic strain known to influence brain function through neurotransmitter modulation. This double-blind, randomized, placebo-controlled trial investigated the efficacy of a 12-week PS128 intervention on clinical symptoms-including tic severity, attention, and gastrointestinal discomfort-alongside cir

"It's Like a Chain Reaction of Discomfort": A Phenomenological Qualitative Study on the Intersection of Autism and Gender Incongruence in Adolescents.

PURPOSE: An overrepresentation of autism spectrum diagnoses (ASD) and autistic traits have been reported among people with gender incongruence. Several quantitative and qualitative studies have highlighted the intersection of autism and gender incongruence (GI). However, we have little knowledge on lived experiences of adolescents experiencing ASD and GI. The study aimed to shed light on lived experience of gender incongruence from the perspective of autistic adolescents. The analysis was guided by the following research question: How do adolescents with ASD and GI experience their bodies and

The role of behavioral inflexibility and impulsivity in explosive outbursts among youth with Tourette syndrome.

Several children living with Tourette syndrome (TS) also present with recurrent episodes of sudden and intense anger, which are known as explosive outbursts. Nevertheless, individual characteristics influencing the onset of such outbursts remain poorly understood. In adults, impulsivity has been found to be a risk factor for explosive outbursts, but there are no such data among children. Additionally, behavioral inflexibility, which is conceptualized as a pattern of rigid, inflexible behavior in situations where it would be beneficial to adapt one's behavior to external demands, constitutes a
