Animal models of Gilles de la Tourette Syndrome and other tic disorders.

Tic disorders (TD) present considerable challenges in research due to their intricate and multifaceted nature. Their primary symptoms, tics, often co-occur with other comorbid conditions that overlap with other psychiatric and movement disorders. Tic development and expression are associated with alterations in different neurotransmitter systems, genetic factors, and environmental influences. However, the current understanding regarding the underlying causes and mechanism remains limited, with the available treatments lacking specificity and efficacy. To address these complexities, numerous an

Understanding tics and neurodevelopmental disorders through lived experience: The first-person experience and the parent perspective.

For many individuals with Tourette, navigating everyday judgments from strangers and peers alike can be as much of a challenge as the tics. Tourette-awareness campaigns and the rise of social media support communities have done significant work to improve the public's understanding and acceptance of tic disorders, but individuals living with it still experience unfortunate misunderstandings about their tics -- sometimes in sensitive environments, like airports -- which makes for a nervous base-pressure that "Touretters" can feel in their day-to-day and which can inflame their ticcing. Social d

Acute tic exacerbations: Tic status, malignant Tourette syndrome, tic attacks, tic fits, and other misfits.

Acute tic exacerbations in Tourette syndrome (TS) and other tic disorders are influenced by biologic, psychologic, and social factors. Different terms have been used to describe acute tic exacerbations, including tic status, malignant TS, tic attacks, and tic fits. This review traces the evolution of our understanding of acute tic exacerbations and examines the emerging clinical constructs, highlighting the overlapping features and diagnostic ambiguities of such episodes. Common features include repetitive movements lasting from minutes to hours that are rarely suppressible, cause significant

Oligogenic risk score for Gilles de la Tourette syndrome reveals a genetic continuum of tic disorders.

Gilles de la Tourette syndrome (GTS) and other tic disorders (TDs) have a substantial genetic component with their heritability estimated at between 60 and 80%. Here we propose an oligogenic risk score of TDs using whole-genome sequencing (WGS) data from a group of Polish GTS patients, their families, and control samples (n = 278). In this study, we first reviewed the literature to obtain a preliminary list of 84 GTS/TD candidate genes. From this list, 10 final risk score genes were selected based on single-gene burden tests (SKAT p < 0.05) between unrelated GTS cases (n = 37) and synthetic co

Surgical management of tic-induced myelopathy and the importance of perioperative tic control: illustrative cases.

BACKGROUND: Degenerative cervical myelopathy (DCM) is the most common cause of adult spinal cord dysfunction. Tourette syndrome (TS) represents a rare cause of DCM that is incompletely characterized in the literature. Patients with TS might develop repetitive involuntary cervical movements that can accelerate spondylotic changes. Therefore, patients with myelopathy secondary to TS or other tic disorders might exhibit DCM symptoms at younger ages. The present study examines the surgical management of DCM secondary to TS in the literature and describes the outcomes of 3 patients who were treated

Social context is a cue for tic reduction in clinical settings.

Assessment and diagnosis of Tourette Syndrome and other tic disorders relies on clinical observation and self-reported history. However, tics are highly susceptible to contextual influences, including clinical interactions. We used video-based observation to quantify the contextual impact of clinician presence on tics and evaluate the potential for these methods to improve tic detection. Youth ages 12-21 (N = 39) participated in a clinical trial with video-recorded pre- and post-treatment assessments. Established methods for precision video-based behavioral coding were used to quantify tic fre

Description, Implementation, and Efficacy of the Comprehensive Behavioral Intervention for Tics as First-Line Treatment for Tourette and Other Tic Disorders.

Objective: To provide an evidence-based review of the Comprehensive Behavioral Intervention for Tic (CBIT) disorders. Results: For close to a century, behavioral interventions for managing tics associated with Tourette and other tic disorders (TDs) were incorrectly considered ineffective and dangerous by the professional community, due, in large part, to unfounded fears that efforts to suppress tics would lead to a host of negative psychological, and even physical, outcomes (e.g., symptom substitution, tic rebound). Spurred by a growing body of research to the contrary, the Comprehensive Behav

Emerging therapeutic approaches for Tourette syndrome and other tic disorders - a systematic review of current clinical trials.

Tourette syndrome and other tic disorders are prevalent neurodevelopmental disorders typically treated with behavioral techniques or pharmacological interventions, primarily antipsychotics. However, many patients do not achieve sufficient response to conventional treatments, underscoring the need for further research in this area. To provide a comprehensive overview of ongoing research activities, we systematically searched the clinical registries of the World Health Organization (WHO) and of the United States National Institutes of Health (NIH) for currently planned or ongoing registered clin

Tourette syndrome and other tic disorders of childhood.

Tics are repetitive, patterned, and nonrhythmic movements or vocalizations/audible sounds that are misplaced in context. Phenomenology and characteristics of tics (e.g., premonitory urge, suppressibility) differentiate them from compulsions, stereotypies, functional tic-like behaviors, and other types of hyperkinetic movement disorders. With a prevalence of approximately 1% in school-aged boys, Tourette syndrome (TS) is considered a common childhood-onset neurodevelopmental disorder, defined by the combination of at least two motor tics and at least one phonic tic lasting more than 1 year. Role of histidine decarboxylase gene in the pathogenesis of Tourette syndrome.

Tourette syndrome (TS) is caused by complex genetic and environmental factors and is characterized by tics. Histidine decarboxylase (HDC) mutation is a rare genetic cause with high penetrance in patients with TS. HDC-knockout (KO) mice have similar behavioral and neurochemical abnormalities as patients with TS. Therefore, HDC-KO mice are considered a valuable TS pathophysiological model as it reveals the underlying pathological mechanisms that cannot be obtained from patients with TS, thus advancing the development of treatment strategies for TS and other tic disorders. This review summarizes

Dystonic motor and phonic tics in Tourette syndrome.

BACKGROUND: Dystonic tics differ from clonic tics by their slower and more sustained nature. Dystonic tics are often present in patients with Tourette syndrome (TS) and other tic-disorders. However, their phenomenology and impact on overall impairment have not been extensively studied. MATERIALS AND METHODS: We assessed clinical history and tic duration in video-recordings from patients with TS evaluated at our movement disorders clinic. Dystonic tics were defined as those lasting ≥ 1000 ms (ms). RESULTS: Of the total of 201 patients with TS, there were 156 with video-recordings suitable for

Sleep Disorders and Sleep Problems in Patients With Tourette Syndrome and Other Tic Disorders: Current Perspectives.

Sleep disorders seem to be a frequent complaint of patients diagnosed with Tourette syndrome (TS) or chronic or persistent tic disorders (CTD or PTD). In this review, we expanded a previously used search using 4 well-known databases up to February 15, 2022, looking for the coexistence of global and/or specific sleep disorders and polysomnographic studies performed on patients with TS/CTD/PTD. The references of interest in the topic were selected by hand. Sleep disorders in general, insomnia, different arousal disorders, the persistence of tics during sleep, excessive daytime sleepiness, and pe

Infra-low frequency neurofeedback in application to Tourette syndrome and other tic disorders: A clinical case series.

We describe our clinical experience in treating patients with Tourette syndrome and other tic disorders using infra-low frequency neurofeedback (ILF NF), often in conjunction with cognitive behavior therapy. Following a narrative description of our approach, we present outcome data for 100 successive cases. Many of the children and adolescents that we have treated since 2005 did not derive sufficient benefit from standard treatment for Tourette syndrome and other tic disorders. In our clinical experience, based on extensive before- and after- testing and symptom tracking, this patient group de

Aripiprazole for the treatment of Tourette syndrome.

Introduction: Tourette syndrome (TS) is a neurodevelopmental disorder characterized by multiple tics which often persisting in forms of different severity throughout adult life. The prevalence of neuropsychiatric co-morbidity in patients with TS is high. Treatment of TS can involve pharmacological, behavioral, or, in rare cases, surgical therapies. Over the last two decades there has been growing interest in the use of aripiprazole, a novel partial dopamine receptor agonist, as a promising anti-tic agent.Areas covered: The authors reviewed the available literature evaluating the role of aripip

Yale Global Tic Severity Scale (YGTSS): Psychometric Quality of the Gold Standard for Tic Assessment Based on the Large-Scale EMTICS Study.

The Yale Global Tic Severity Scale (YGTSS) is a clinician-rated instrument considered as the gold standard for assessing tics in patients with Tourette's Syndrome and other tic disorders. Previous psychometric investigations of the YGTSS exhibit different limitations such as small sample sizes and insufficient methods. To overcome these shortcomings, we used a subsample of the large-scale "European Multicentre Tics in Children Study" (EMTICS) including 706 children and adolescents with a chronic tic disorder and investigated convergent, discriminant and factorial validity, as well as internal

Efficacy and Safety of Medication for Attention-Deficit Hyperactivity Disorder in Children and Adolescents with Common Comorbidities: A Systematic Review.

INTRODUCTION: Comorbid psychiatric conditions in children and adolescents with attention-deficit hyperactivity disorder (ADHD) occur frequently, complicate management, and are associated with substantial burden on patients and caregivers. Very few systematic reviews have assessed the efficacy and safety of medications for ADHD in children and adolescents with comorbidities. Of those that were conducted, most focused on a particular comorbidity or medication. In this systematic literature review, we summarize the efficacy and safety of treatments for children and adolescents with ADHD and comor

Generating Acute and Chronic Experimental Models of Motor Tic Expression in Rats.

Motor tics are sudden, rapid, recurrent movements that are the key symptoms of Tourette syndrome and other tic disorders. The pathophysiology of tic generation is associated with abnormal inhibition of the basal ganglia, particularly its primary input structure, the striatum. In animal models of both rodents and non-human primates, local application of GABAA antagonists, such as bicuculline and picrotoxin, into the motor parts of the striatum induces local disinhibition resulting in the expression of motor tics. Here, we present acute and chronic models of motor tics in rats. In the acute mode
