Imaging equity: Why vascular neurologists need routine ultrasound training.

BACKGROUND: Reducing congenital syphilis has been the focus of Brazilian health programs for decades, yet the cases continue to increase. Although health interventions have targeted HIV screening and treatment, syphilis management continues to be challenging. Syphilis during pregnancy may enhance the HIV maternal seroconversion risk. The potential factors fueling the syphilis epidemic were evaluated in south Brazil, an area of high HIV or syphilis endemicity. OBJECTIVE: We hypothesized that ineffective treatment because of a lack of partner treatment, late presentation to care, and reinfectio

Rising congenital syphilis amid declining perinatal HIV: a 7-year study from a tertiary medical center in Oregon.

We analyzed laboratory testing and positivity trends for syphilis and human immunodeficiency virus (HIV) to evaluate the effectiveness of public health policies and clinical practices. Laboratory data from unique patients were reviewed for syphilis (2018-2024) and HIV (2021-2024), stratified by age, gender, prenatal or newborn status, and county of residence in Oregon. Syphilis testing volumes reflected shifts in healthcare utilization before, during, and after the COVID-19 pandemic. HIV testing increased steadily from 6,627 patients in 2021 to more than 18,600 in 2024. Despite an average of ~

Pharmacokinetics of a three-dose regimen of intramuscular benzathine penicillin G in pregnant women with syphilis.

BACKGROUND: Syphilis remains a global public health concern, particularly in pregnancy due to the risk of congenital syphilis. The WHO recommends benzathine penicillin G (BPG) as the standard treatment in pregnant women, whilst the optimal dosing strategy has not been established, particularly in relation to the impact of gestational age. This study sought to characterize the pharmacokinetics (PK) of a three-dose regimen of intramuscular BPG, given at weekly intervals, in pregnant women with syphilis. METHODS: A prospective PK study was undertaken in 30 pregnant Ethiopian women (second (n = 1

Uptake of prenatal syphilis screening and its determinants in Ontario, Canada: a population-based retrospective cohort study.

BACKGROUND: Timely prenatal syphilis screening is critical for preventing congenital syphilis infection. We aimed to examine prenatal syphilis screening patterns in Canada and to identify factors associated with missed or late screening. METHODS: For this population-based retrospective cohort study, we used linked provincial administrative health data from Ontario, Canada. We included pregnancies (> 20 wk gestation) resulting in live births or stillbirths from January 2018 to December 2023. Missed prenatal syphilis screening (at any point during pregnancy or at delivery) or late screening (ei

Congenital Syphilis Elimination in Latin America: A Systematic Analysis of Policy and Progress.

BACKGROUND: Syphilis in pregnancy and congenital syphilis (CS) remain major public health challenges in Latin America despite long-standing elimination initiatives. CS incidence continues to rise, although diagnosis and treatment are available, reflecting persistent programmatic gaps. A systematic assessment of strengths, weaknesses, opportunities, and threats (SWOT) can help refine strategies and accelerate progress toward the elimination of mother-to-child transmission (EMTCT) of syphilis. METHODS: We performed a qualitative SWOT analysis of public health policies addressing syphilis in pre

A Case of Adult-onset Presumed Congenital Syphilitic Keratitis: A Multidisciplinary Management Approach.

BACKGROUND: Interstitial keratitis is a classic manifestation of late congenital syphilis, typically presenting in childhood or adolescence. Adult-onset presentation is rare and poses a diagnostic and therapeutic challenge, particularly in patients without a known history of congenital infection. CASE REPORT: We report a 28-year-old man with an 11-year history of progressive bilateral ocular redness, tearing, and visual decline who was found to have bilateral interstitial keratitis with positive treponemal and negative nontreponemal serologies, consistent with late congenital syphilis. Despit

Elimination of congenital syphilis: a nationwide register-based study, Denmark, 2010 to 2024.

BACKGROUNDCongenital syphilis is a potentially serious, but preventable infection. Several European countries have recently reported increasing trends.AIMThis study assessed national incidence and temporal trends of congenital syphilis and prevalence of pregnancies with active syphilis in Denmark between 2010 and 2024.METHODSUsing nationwide individual-level data linked through civil registration numbers across Danish health and population registers, we identified all maternal and congenital syphilis cases among live-born children born in Denmark from 2010 to 2024. We calculated annual inciden

[Syphilis and human treponemes: a long evolutionary history revealed by paleogenomics].

Recent discoveries in paleogenomics have revolutionized our understanding of syphilis and other human treponematoses. Far from being a pathogen that suddenly appeared in Europe in the late Middle Ages, we now know that Treponema pallidum has been circulated among human populations for millennia. Ancient genomes recovered from pre-Columbian contexts in the Americas show that major treponemal lineages had already diversified well before the modern era, often in the absence of recognizable skeletal lesions. Genomic analyses further indicate that treponemal diversity is not the result of extensive

Implementation and performance of a national pilot programme for eliminating mother-to-child transmission of syphilis: a retrospective cross-sectional study in China.

BACKGROUND: Syphilis remains a significant global health burden, with congenital syphilis (CS) posing a major threat to child health. China has implemented the elimination of mother-to-child transmission (EMTCT) programme for syphilis through integrated strategies. However, few studies have analysed granular data along the entire cascade of care to identify specific bottlenecks. OBJECTIVE: We aimed to characterise the implementation status of integrated interventions in Baoan District, Shenzhen, and to identify risk factors associated with suboptimal indicators. METHODS: This retrospective c

Clinical characteristics and factors contributing to transmission of congenital syphilis in England, 2015-24: a national surveillance study.

BACKGROUND: Congenital syphilis has re-emerged in England despite near-universal antenatal screening coverage, indicating that important opportunities for prevention continue to be missed. We aimed to characterise the clinical presentation, diagnostic findings, and factors contributing to transmission among congenital syphilis cases in England to inform strategies for earlier diagnosis and prevention. METHODS: We reviewed probable and confirmed congenital syphilis cases in England (January 2015 to June 2024) reported through the Integrated Screening Outcomes Surveillance Service, including ma

A Public Health Scourge: The Recent Surge of an Ancient Disease.

The recent resurgence of congenital syphilis is striking, with an increase in cases 10-fold over the past 12 years, including a 30% rise within the past 3 years alone. The consequences of untreated maternal syphilis can be severe, contributing to significant fetal and neonatal illness and death in cases of congenital syphilis. We present a case involving a male fetus delivered stillborn at 33 weeks' gestation to a 30-year-old woman with limited prenatal care. The mother had been diagnosed with late latent syphilis 8 days before delivery and had a history of polysubstance use. Ultrasound at 32

Maternal Syphilis of Unknown Duration: Birth Outcomes in an Immediate Test-and-Treat Cohort often Receiving Fewer than Three Doses of Benzathine Penicillin G.

In a South African antenatal cohort, an immediate test-and-treat model for syphilis of unknown duration showed that adverse birth outcomes-including stillbirth and preterm birth-were similar between women without syphilis and those treated with penicillin, regardless of completion of the three-dose regimen. Further studies are needed to evaluate optimal treatment strategies in pregnancy.

Syphilis Screening Among Pregnant Individuals in Carceral Settings: Near Misses and a Call to Action.

Congenital syphilis is a major global public health problem with cases rapidly increasing in the United States. This article presents a case of an incarcerated pregnant individual who tested negative on routine screening at prison entry, and for whom a third trimester syphilis test likely prevented mother-to-child syphilis transmission. Third trimester syphilis screening in carceral settings is a critical tool in the prevention of congenital syphilis. Yet there are no consensus guidelines recommending universal repeat screening in the third trimester. Many cases of congenital syphilis are miss

Assessment, diagnosis and management of syphilis: considerations for nurses.

Syphilis is a preventable and curable sexually transmitted infection that remains a significant public health challenge despite the availability of effective antibiotic treatment. Following a decline in the late 20th century, syphilis has re-emerged globally, with rising rates across diverse populations and a particularly concerning increase in congenital infection. The varied and often subtle presentations of syphilis, alongside its asymptomatic stages, can contribute to delayed diagnosis and ongoing transmission. Since many cases are now identified outside specialist sexual health services,

Neglected Syphilis in Pregnancy Presenting as Condyloma Lata with Subsequent Congenital Syphilis: A Case Report.

PURPOSE: The likelihood of developing concurrent infections, such as bacterial vaginosis (BV) and sexually transmitted syphilis, tends to increase. Delayed diagnosis of maternal syphilis may allow progression to secondary syphilis and substantially increase the risk of vertical transmission. We report a neglected case of syphilis during pregnancy presenting as condyloma lata with concurrent bacterial vaginosis that subsequently resulted in congenital syphilis. CASE ILLUSTRATION: A 17-year-old married woman, gravida 1 para 0 abortus 0 (G1P0A0), at 34-35 weeks of gestation, presented with progr

The Global Re-emergence of Syphilis and Antimicrobial Resistance Among Men Who Have Sex With Men (MSM), People Living With Human Immunodeficiency Virus (PLWH), and Newborns: A Narrative Review.

Syphilis incidence continues to rise substantially among men who have sex with men (MSM), people living with HIV (PLWH), newborns, and women of reproductive age, despite preventive measures and available treatments. A major contributing factor is the increasing antimicrobial resistance, which complicates current therapeutic strategies for syphilis. This study aims to support the development of evidence-based, targeted interventions designed to address the specific needs of high-risk populations and to minimize the risk of antimicrobial resistance, while contributing to the overall reduction of

Eponymous Osseous Signs in Congenital Syphilis: A Narrative Historical Review.

The early origin of syphilis remains debated; bone involvement was recognized in early clinical accounts and described with increasing precision as pediatric pathology and radiography developed in the late nineteenth and early twentieth centuries. Many such observations entered medical usage through eponymous labels, yet these have not been examined as a coherent group. We reviewed the origins and early clinical descriptions of eponymous osseous signs of congenital syphilis between 1871 and 1928 and to outline their present-day relevance. A documentary review was conducted using PubMed/MEDLINE

Case Report: Congenital syphilis presenting with respiratory symptoms in an infant.

BACKGROUND: Congenital syphilis is preventable but has diverse clinical manifestations. Although rhinitis or respiratory symptoms may be among the initial complaints, symptomatic congenital syphilis may already present with overt multisystem disease involving the skin, liver, hematologic system, lungs, skeleton, kidneys, and central nervous system. We report a male infant aged 1 month and 18 days who presented with nasal congestion and rhinorrhea.
