The Experiences of Mothers Feeding Their Infants Later Diagnosed With Autism Spectrum Disorder: Bottle Feeding and Complementary Feeding Challenges.

INTRODUCTION: Children diagnosed with Autism Spectrum Disorder (ASD) more commonly experience feeding difficulty than their neurotypical (NT) peers, although less is known about how the feeding behaviors present during infancy. METHODS: Twenty-four mothers participated in interviews exploring their experiences feeding their autistic and NT children during the first year of life and the context behind feeding practices that may also contribute to feeding difficulty in autistic children during infancy. RESULTS: Thematic analysis identified four main themes: (1) bottle feeding and cup behaviors

Refining the Neonatal Phenotypic Spectrum of Distal Deletion 14q Syndrome: Early Genomic Diagnosis in Infancy.

Distal deletion 14q syndrome is a rare chromosomal disorder characterized by variable features, including growth restriction, craniofacial dysmorphism, developmental delay, and congenital anomalies. Diagnosis is often delayed because conventional G-banding may appear normal. Neonatal recognition is rarely reported, and early phenotypic features remain insufficiently defined. We report the case of a male infant born at 37 + 6 weeks of gestation with intrauterine growth restriction, micrognathia, feeding difficulties, hypotonia, and cardiopulmonary instability. Prenatal echocardiography suggeste

Maternal reported long-term outcomes of early feeding challenges from hospital discharge to early childhood.

BACKGROUND: Feeding difficulties are common in early infancy and may persist beyond the neonatal period, affecting growth, development, and the parent-infant relationship. While existing research has largely focused on short-term breastfeeding outcomes, there is limited understanding of how early feeding challenges evolve across infancy and early childhood. AIM: To examine associations between maternal-reported feeding challenges experienced during the birth hospitalisation and the presence of ongoing feeding difficulties in infancy and early childhood, and to describe the frequency, nature,

Case report: Tirzepatide-responsive refractory diabetes mellitus in an adult female with prader-willi syndrome.

RATIONALE: Prader-Willi syndrome (PWS) is a complex genetic disorder that affects multiple systems and results from loss of expression of paternally inherited genes in the 15q11.2-q13 region. It is characterized by severe hypotonia and feeding difficulties during infancy, followed by hyperphagia in later childhood and adolescence, which, without strict control of excessive eating behaviors, ultimately leads to severe obesity associated with type 2 diabetes mellitus. PATIENT CONCERNS: We present the case of a woman who was diagnosed with PWS and diabetes mellitus in adulthood. She had a histor

Deep Phenotyping in ReNU Syndrome Identifies a Recognizable Age-Dependent Clinical Trajectory.

Pathogenic variants in the noncoding gene RNU4-2 cause ReNU syndrome, a common neurodevelopmental disorder. Although the core phenotype is well described, longitudinal symptom progression and family perspectives remain insufficiently characterized. We identified 11 individuals with RNU4-2 pathogenic variants through reanalysis of clinical whole genome sequencing (WGS) data from a previously undiagnosed cohort. Using deep phenotyping, including retrospective medical record review and parental insights, we delineated a recognizable age-dependent clinical trajectory. Infancy was characterized by

Cognitive development in children with Prader-Willi syndrome receiving early growth hormone therapy.

BACKGROUND: Prader‒Willi syndrome (PWS) is a genetic disorder characterized by hypotonia, feeding difficulties in infancy, excessive eating in childhood, intellectual disability and short stature. Growth hormone therapy (GHT) improves body composition and decreases fat percentage in patients with PWS. However, the effect of GHT on cognitive function is not clear. OBJECTIVES: The aim of this study was to evaluate the beneficial effects of GHT on cognition in younger patients with PWS. METHODS: A retrospective analysis of patients with PWS who received GHT in four hospitals was conducted in Ta

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Obesity Prevalence in DDX3X-Related Neurodevelopmental Disorder.

The purpose of this study is to analyze the prevalence of obesity in those affected by DDX3X-related neurodevelopmental disorder (DDX3X-NDD). Initial descriptions suggested that individuals with DDX3X-NDD suffered from poor weight gain or failure to thrive in early childhood, likely in the setting of feeding difficulties and secondary to associated hypotonia. However, through a patient's parent, we came to learn that obesity was a common problem for older patients. To investigate this, we developed a survey to be distributed to caregivers of patients with DDX3X-NDD. The flyer contained a QR co

The Role of the Arcuate Nucleus in Regulating Hunger and Satiety in Prader-Willi Syndrome.

Prader-Willi syndrome (PWS) is a rare genetic disorder. The main characteristics are muscular hypotonia, failure to thrive and feeding problems in infancy, which switch to hyperphagia in early childhood and continue into adulthood. Due to hyperphagia, the risk of developing morbid obesity is high without treatment. PWS is considered a hypothalamic disease, and within the hypothalamus the arcuate nucleus (AC) is of central importance for controlling metabolism, hunger, and satiety. The AC has been studied in several animal models as well as in humans, including PWS. The function of AC is regula

Perinatal risk and protective factors associated with diagnosis of mental health disorders in children by age 9 years.

BACKGROUND: Early identification of children who are at risk of developing mental health disorders is essential for implementing prevention and intervention approaches. The perinatal period is a unique time to identify these risks, as well as protective factors, due to the high frequency of contact with healthcare professionals. METHODS: The current study linked longitudinal cohort data from the All Our Families study (Canada) with administrative health records up to age 9 years (n = 2,814), to identify risk and protective factors from the perinatal period (pregnancy through to first year of

[The avoidant/restrictive food intake disorder: a new feeding and eating disorder in the diagnostic systems].

The Avoidant-restrictive Food Intake Disorder (ARFID) was included in the fifth edition of the DSM in 2013 and in the BNO-11 diagnostic manual in 2019. Prior to this, the problem was often unrecognised or diagnosed as an eating disorder not otherwise specified or feeding and eating disorders of infancy or early childhood. The new diagnosis allows for better targeting of patients who have not been diagnosed earlier, as well as for the diagnosis of patients over the age of six. The authors, in their review, describe the diagnostic features and symptom profile of ARFID, and they discuss the role

[XVI Jesús Culebras Lecture. Development of eating habits in children: sense and sensitivity].

The first years of a child's life are periods of rapid growth and significant neurocognitive development. Adequate energy and macro- and micronutrient intake are essential for proper growth and development. The development of feeding skills is a very sensitive stage that requires, on the one hand, exposure to new foods and different textures and, on the other, an appropriate response from parents to their children's demands and cues. This development of the foundations of eating habits occurs in utero and progresses throughout infancy and early childhood (3 to 5 years). A woman's diet during p

Barth syndrome: Natural history in infants and young children.

Barth syndrome (BTHS) is a rare X-linked mitochondrial disorder caused by pathogenic variants in TAFAZZIN. It is characterized by cardiomyopathy, neutropenia, growth delay, skeletal myopathy, and developmental concerns. Advances in genetic testing have enabled earlier diagnoses, creating opportunities to better define the natural history of disease in infancy and early childhood. We conducted a longitudinal, observational study of 21 male patients (ages 0-48 months) with genetically confirmed BTHS evaluated at the Barth Syndrome Interdisciplinary Clinic at Kennedy Krieger Institute. Data colle

From Congenital Torticollis to Leigh Syndrome: A Case Report of Diagnostic Evolution in an Infant.

Leigh syndrome is a rare, progressive mitochondrial disorder of childhood. Early diagnosis is often challenging due to nonspecific clinical manifestations. We report a 1-month-old male infant initially referred for suspected congenital muscular torticollis who ultimately received a diagnosis of Leigh syndrome. Despite unremarkable perinatal history, he subsequently developed persistent feeding difficulties, recurrent vomiting, failure to thrive, and global developmental delay. Early neurological assessment revealed poor repertoire patterns on General Movement Assessment. The Neonatal Oral-Moto

Neuropeptides and the Autonomic Nervous System in Prader-Willi Syndrome.

Prader-Willi syndrome (PWS) is a rare, multisymptomatic genetic disorder caused by the absence or dysfunction of specific genes on chromosome 15. The genetic abnormality is anticipated to cause a dysfunction of the hypothalamus, which is also central in the regulation of the autonomic nervous system (ANS). Typical symptoms of PWS indicating a hypothalamic dysfunction include muscular hypotonia, poor growth, short stature, and feeding difficulties in infancy, which in early childhood are replaced by hyperphagia, leading to a high risk of obesity. Other characteristics, such as sleep difficultie

Early childhood-onset rumination syndrome is clinically distinct from adolescent-onset rumination syndrome.

OBJECTIVES: Rumination syndrome (RS) beginning in early childhood or infancy is understudied and challenging to treat. Our objective is to compare the characteristics and outcomes of early-onset (EO) and adolescent-onset (AO) patients with RS. METHODS: We conducted an ambidirectional cohort study of children diagnosed with RS at our institution. Patients were included in two groups: EO (RS symptom onset ≤5 years and diagnosis ≤12 years) and AO (onset >12 years). Patient characteristics, severity, and outcomes were compared between the groups. RESULTS: We included 49 EO and 52 AO RS patients.

A novel termination site in a case of Stüve-Wiedemann syndrome: case report and review of literature.

Stüve-Wiedemann syndrome (SWS) is a rare autosomal recessive disorder that is characterized by bowing of long bones, dysautonomia, temperature dysregulation, swallowing and feeding difficulties, and frequent respiratory infections. Respiratory distress and hyperthermic events are the leading causes of early neonatal death, and most patients are not expected to survive past infancy. Here, we report on the survival of a 5-year-old male with SWS, discussing his case presentation, providing a brief clinical course, and discussing the outcome. This case adds to the literature surrounding rare insta

Dysregulation of miR-146a in human milk of mothers having children with autism.

Autism spectrum disorder (ASD) is a set of neurobehavioral manifestations that impose poor social interaction and stereotyped repetitive patterns. Several mircoRNA (miRNA) dysregulations underpin ASD pathophysiology via impairing the neurogenic niches. For instance, miR-146a and miR-106 differential expressions are linked to deregulation of ASD-related genes and the severity of clinical symptoms, respectively. Breastfeeding provides newborns with many bioactive compounds that support their neurodevelopment including miRNA. Our pilot study evaluated the expression pattern of miR-106a and miR-14
