Community-Acquired Klebsiella pneumoniae Meningitis With Liver Abscess and Neurological Sequelae in an Immunocompetent Adult: A Case Report.

Klebsiella pneumoniae meningitis (KPM) is a rare but severe form of community-acquired bacterial meningitis that is often associated with high mortality and neurological sequelae. We report a 46-year-old previously healthy male who presented with acute altered mental status and high-grade fever. Cerebrospinal fluid analysis was consistent with bacterial meningitis, and cultures from the cerebrospinal fluid and blood showed Klebsiella pneumoniae (K. pneumoniae) growth. The clinical course of the patient was complicated with seizures and acute right-sided hemiplegia. Persistent fever (despite ap

Case report: Meningitis of a 66-year-old dog owner caused by Capnocytophaga canimorsus.

Capnocytophaga canimorsus is a fastidious Gram-negative rod transmitted from dog saliva and typically associated with immunocompromised hosts. We report meningitis in a 66-year-old immunocompetent dog owner with prior cerebral amyloid angiopathy who presented with acute global aphasia, fever and inflammatory cerebrospinal fluid (CSF) consistent with bacterial meningitis, but without meningism or history of any dog bite. Fusiform Gram-negative rods in CSF prompted extended culture in thioglycolate broth, enabling growth and subsequent species identification by MALDI-TOF MS as C. canimorsus with

The hidden burden of viral meningitis in ethiopia: Etiology and clinical outcomes.

BACKGROUND: Viral meningitis remains a significant global public health problem, particularly in resource-limited settings where the diagnosis of viral etiology is mainly based on clinical assessment. This study aimed to identify the common viral etiologies of meningitis, describe their associated clinical features, and assess patient outcomes among individuals suspected of meningitis without evidence of bacterial meningitis. METHODS: We conducted a multicenter study involving 430 patients clinically suspected of meningitis. Clinical data and cerebrospinal fluid samples (CSF) were collected f

Identification and whole-genome sequencing of blood-brain barrier-penetrating Riemerella anatipestifer isolated from chickens and ducks.

Riemerella anatipestifer (RA) is a major cause of bacterial meningitis, which poses a serious threat to the poultry industry. To identify RA strains with a strong ability to cross the blood-brain barrier (BBB) and analyze their genomic characteristics, a co-infection model was established using multiple RA strains with different serotypes in this study. We found that during co-infection with multiple RA strains, certain RA strains rapidly penetrated the BBB and were recovered from brain tissue as early as 0.25 h post-infection. The pioneer strains were selected to further evaluate their BBB-pe

Viral Infection-Induced Epilepsy and the Gut Microbiome Crosstalk.

Viral infections of the central nervous system (CNS) pose a significant risk for seizures, and in some cases, long-term epileptogenesis. Seizure development following viral infection involves diverse CNS mechanisms, with emerging evidence that peripheral factors, including gut microbiome composition and immune-inflammatory responses, also contribute. The gut microbiome is a multifaceted ecosystem with important roles in CNS health, and advances in sequencing have enabled investigation of host and microbial genomic changes associated with seizure development. While both animal and human studies

Brucella melitensis infection complicated by secondary hemophagocytic lymphohistiocytosis, meningitis, and multiorgan damage: a case report and literature review.

BACKGROUND: Brucellosis is one of the most widespread zoonotic diseases globally. Of the twelve known Brucella species that affect mammals, Brucella melitensis is the most significant cause of human infection. Others include B. abortus, B. suis, and B. canis, with transmission occurring via unpasteurized dairy, direct animal contact, or occupational exposure (veterinarians, farmers, and laboratory workers). Although hemophagocytic lymphohistiocytosis (HLH) and neurological complications are known sequelae of brucellosis, their simultaneous occurrence with multiorgan failure is rarely reported.

Respiratory Viral Positivity Is Associated with Decreased Risk of Serious Bacterial Infections in Febrile Infants.

INTRODUCTION: Febrile infants ≤ 60 days of age are at increased risk for serious bacterial infections (SBIs), often necessitating extensive diagnostic evaluation and hospitalization. Although respiratory viral infections are common in this population and may influence the likelihood of concurrent bacterial infection, the relationship between respiratory viral panel (RVP) results and SBI risk remains an important consideration in clinical decision-making. The objective of this study was to evaluate the association between RVP results and the occurrence of SBIs among febrile infants ≤ 60 days of

Duration of single-agent perioperative antibiotic prophylaxis and postoperative meningitis after endoscopic endonasal skull base surgery: a systematic review and meta-analysis of 7,981 patients.

Postoperative meningitis is a rare but serious complication in endoscopic-endonasal skull-base surgery. Although prophylactic antibiotics are routinely administered, the optimal regimen and duration remain unclear. This systematic review and meta-analysis evaluated the incidence of meningitis and intraoperative cerebrospinal fluid (CSF) leakage associated with single-agent perioperative antibiotic prophylaxis. Following PRISMA 2020 guidelines, PubMed, EMBASE, and Web of Science were searched through February 2025. Eligible studies reported postoperative meningitis or intraoperative cerebrospin

Correlation Between Gut Microbiota Shifts, Lipopolysaccharide Levels, and Neuroinflammatory Responses Following Ischemic Stroke.

BACKGROUND: The microbiota-gut-brain axis (MGBA) is increasingly recognized as a contributor to ischemic stroke pathophysiology, yet the relationship between gut microbial changes and neuroinflammation remains incompletely understood. This study aimed to investigate whether shifts in gut microbiota are associated with concurrent changes in lipopolysaccharide (LPS) levels and neuroinflammatory responses following ischemic stroke. METHODS: Using a photothrombotic mouse model, we measured LPS in the gut, serum, and brain via ELISA and immunohistochemistry. Gentamycin was administered to deplete

Metagenomic analysis of developmental-category-associated fungal communities in the gut contents of Achatina fulica, an intermediate host of Angiostrongylus cantonensis.

BACKGROUND: The invasive giant African snail (Achatina fulica) poses serious agricultural and ecological threats and serves as an important intermediate host of the zoonotic parasite Angiostrongylus cantonensis, which causes human eosinophilic meningitis. While its gut bacterial microbiome has been extensively characterized, the gut mycobiome remains largely uncharacterized. In particular, variations in gut fungal communities across developmental stages remain unknown, limiting our understanding of the A. fulica gut holobiont. METHODS: In November 2025, giant African snails were collected fro

A Chiral Virus-Like Nanoplatform That Broadly Mediates Antibiotic Delivery to the Brain for Bacterial Meningitis Therapy.

Blood-brain barrier (BBB) preserves brain homeostasis yet restricts therapeutics; in bacterial meningitis, antibiotics' limited penetration impairs delivery to the parenchyma and cerebrospinal fluid, constraining efficacy and motivating strategies to overcome the barrier. To address this, we developed a chiral virus-mimicking mesoporous silica nanostructure (CVSN) functionalized with L-alanine for antibiotic delivery. It offers several distinct advantages: (1) nanoscale dimensions facilitating BBB transport; (2) virus-like morphology enhancing vascular accumulation and penetration, providing a

Behavioral, Transcriptomic, and Neuroinflammatory Effects of Sleep Deprivation: Insights From Separate Acute and Chronic Experimental Paradigms.

BACKGROUND: Depression is a global health burden associated with sleep disruption and inflammation. While acute sleep deprivation (ASD) can alleviate depressive symptoms, chronic sleep deprivation (CSD) is a risk factor for mood disorders. However, the contrasting mechanisms underlying these opposing outcomes remain poorly understood. METHODS: We integrated behavioral profiling, multi-region transcriptomics, and multiplex cytokine analysis to separately characterize the effects of ASD in a pain-depression comorbidity model and CSD in normal mice. Adult male C57BL/6J mice (n = 6 per group) wer

[Astrocytes as a key structural and functional component in the development of mixed dementia].

Mixed dementia, characterized by a combination of Alzheimer's disease and cerebrovascular disease (CVD), is prevalent among the elderly population. Accumulated evidence suggests that CVD plays an active role in the neurodegenerative process, enhancing the amyloid and tau pathways and contributing to the progressive disintegration of the brain's functional and structural networks. This review presents data indicating that astrocytes serve as a critical integrative component, linking the pathologies associated with beta-amyloid and tau protein, dysfunction of the neurovascular unit, blood-brain

Bedside Transcranial Doppler as a Dynamic Warning Tool for Rapidly Evolving Intracranial Hypertension in Pneumococcal Meningitis.

In severe bacterial meningitis, an initially non-critical or technically unreliable lumbar puncture opening pressure should not reassure when hydrocephalus and coma persist. Bedside transcranial Doppler cannot measure intracranial pressure, but bilateral near-absent diastolic flow may provide an urgent warning signal prompting repeat assessment and cerebrospinal fluid diversion.

From Cognitive Impairment to Uremic Encephalopathy in Chronic Kidney Disease: A Proposed Pathophysiological Continuum.

Chronic kidney disease (CKD) is associated with neurological abnormalities ranging from subtle cognitive deficits to acute or subacute uremic encephalopathy. Although clinically distinct, these syndromes may arise from overlapping biological processes, including retained uremic solutes, cerebrovascular and blood-brain barrier dysfunction, systemic and neuroinflammation, oxidative stress, mitochondrial impairment, and altered neurotransmission. This narrative review synthesizes clinical, experimental, and cellular evidence supporting the contribution of these interconnected pathways to CKD-asso

Tuberculous Meningitis Alters the Proteomic Landscape of Brain-Derived Extracellular Vesicles.

Tuberculous meningitis (TB meningitis), the deadliest form of Mycobacterium tuberculosis infection, causes devastating mortality and severe neurological disability despite standard therapy. Brain injury and microglial activation are major determinants of outcome, yet the molecular mechanisms linking infection, inflammation, and neuronal injury remain poorly understood. Extracellular vesicles (EVs), critical mediators of cell-to-cell communication, have been investigated in pulmonary TB, but their role in TB meningitis remains unexplored. We used a young New Zealand White rabbit model of TB men
