Rare Fatal Event of Multidrug-resistant Shigella Septicemia and Cryptococcal Meningitis in an Immunocompromised Host.

Bacteriemia due to Shigella spp. is a rare event. However, an individual with impaired immunity remains at risk of such invasion where the infective dose is as low as 10 viable bacilli. This case report describes one such fatal event where a 40-year-old immunocompromised male presented to a tertiary-level healthcare facility with a history of diarrhea for 10 days and seizures. Prompt microbiological workup established multidrug-resistant Shigella flexneri septicemia and cryptococcal meningitis.

Methicillin-resistant Staphylococcus aureus meningitis following revision microdiscectomy: a case report.

Healthcare-associated bacterial meningitis is a rare but life-threatening complication of spinal surgery, most commonly associated with incidental durotomy. A 59-year-old man developed fever, altered mental status, and generalized seizures 1week after uncomplicated revision L4-L5 microdiscectomy. Magnetic resonance imaging demonstrated diffuse intracranial and spinal leptomeningeal enhancement with a postoperative lumbar fluid collection. Cerebrospinal fluid (CSF) analysis was consistent with bacterial meningitis. Emergent irrigation and debridement revealed copious purulent fluid without evid

Early-onset neonatal sepsis and neonatal antibiotic exposure in Hungary a nationwide population-based study.

UNLABELLED: Neonatal sepsis remains a challenge in perinatal medicine. We aimed to estimate incidence of culture positive early-onset sepsis (CP-EOS), causative pathogens and overall antibiotic exposure in newborn infants in Hungary. Retrospective national registry analysis (2020-2023). Data were mainly collected from the Hungarian National Perinatal Registry covering all level II-III neonatal intensive care units (NICUs) in Hungary. The number of livebirths in each gestational age group was provided by Hungarian Central Statistical Office. Overall CP-EOS incidence was 0.41/1,000 livebirths (0

Tuberculous Longitudinally Extensive Transverse Myelitis: A Case Report and Literature Review.

Longitudinally extensive transverse myelitis (LETM) is a rare but severe manifestation of central nervous system (CNS) tuberculosis (TB). Because of its nonspecific clinical presentation and broad differential diagnosis, early recognition remains challenging. We report the case of a 27-year-old man with concurrent pulmonary and meningeal TB who developed rapidly progressive quadriparesis. Magnetic resonance imaging demonstrated longitudinally extensive T2-hyperintense lesions extending from C2 to T12, accompanied by diffuse meningeal enhancement, consistent with TB LETM. The patient was treate

Fatal bacterial CNS infections: a systematic review and a forensic case report.

Central nervous system (CNS) infections may cause sudden unexpected death when diagnosis is delayed or missed. We report a fatal case of Streptococcus pyogenes meningoencephalitis and systematically review postmortem investigations of fatal bacterial CNS infections. A 57-year-old woman was found dead at home after untreated otalgia, hearing loss, purulent otorrhea, low-grade fever, asthenia, and vomiting. Autopsy showed diffuse cerebral edema, subdural empyema involving the posterior fossa, and cerebellar tonsillar herniation. Histology demonstrated acute suppurative meningoencephalitis with v

Diagnostic Accuracy of Urinary Reagent Strip Applied to Cerebrospinal Fluid Analysis for the Detection of Bacterial Meningitis in Children.

Bacterial meningitis is a medical emergency in children and requires early diagnosis and prompt treatment to reduce mortality and neurological complications. In resource-limited settings, urinary reagent strips applied to Cerebrospinal fluid (CSF) may offer a rapid bedside alternative for preliminary diagnosis. This study aimed to assess the diagnostic accuracy of urinary reagent strip testing in CSF analysis for the detection of bacterial meningitis in children, compared with standard laboratory methods. This cross-sectional descriptive study was conducted in the Department of Paediatrics, My

A Multisystem Presentation of West Nile Neuroinvasive Disease: Lessons in Diagnostic Complexity.

West Nile virus (WNV) is an uncommon but serious case of encephalitis and meningitis, often presenting with nonspecific symptoms that can mimic a wide range of infectious, metabolic, and endocrine disorders. We present the case of a 61-year-old male with multiple comorbidities who arrived with 5 days of fever, chills, profuse diarrhea, black stools, and worsening confusion. Initial evaluation in the emergency department revealed hyponatremia, transaminitis, and concern for tick-borne illness, prompting empiric doxycycline. However, the patient denied any tick bites. Given his known thyroid nod

Machine learning models for predicting cerebrospinal fluid bacterial culture outcomes in post-neurosurgical meningitis-development and temporal three-year validation.

Post-neurosurgical bacterial meningitis (PNBM) is a serious complication associated with substantial morbidity and mortality. Bacterial culture presently serves as the gold standard for diagnosis, however, clinical reliance on third-tier reports to address empirical antibiotic concerns, with a turnaround time of 3-5 days, can lead to imprecise antibiotic administration in patients. Therefore, we develop and validate a branch of machine learning method to predict the result of cerebrospinal fluid (CSF) culture, which can shorten the time for precise diagnosis to within 2 h. A cohort study was c

IL-12Rβ2 is Expressed in the Synthetic SMC and Detected in the Blood of Patients With Acute Myocardial Infarction.

BACKGROUND: Bacterial meningitis is a serious infection leading to increased morbidity and mortality every year due to delayed diagnosis and treatment. Previous literatures had shown that cerebrospinal fluid (CSF) procalcitonin outweighs serum procalcitonin to diagnose bacterial infections of the central nervous system. Current meta-analysis aims to find the diagnostic accuracy of serum and CSF C-reactive protein (CRP) to diagnose bacterial meningitis. MATERIAL AND METHODS: PubMed, Google Scholar, Cochrane Library and Google databases were searched from 1st January 1980 to 30th June 2022. Obs

India's Draft National Health Research Policy 2026: A Visionary Framework for Transforming the National Health Research Ecosystem.

Chemoport devices are commonly used for the administration of chemotherapeutic medication in cancer patients and can also be used for total parenteral nutrition and long term intravenous therapy. They are usually inserted through the internal jugular vein or the subclavian vein. The common complications of the procedure include pneumothorax, bleeding, arrhythmia and venous thrombosis. One of the rare complications of chemoport is catheter fracture/dislodgement and subsequent migration, with an incidence of 0.1-2.1%. Other rare complications are vascular erosion and embolization, vocal cord pal

A 15-year Single-center Analysis of Late-onset Group B Streptococcus Infection Correlating Clinical Severity With Pathogen Virulence Determinants.

BACKGROUND: Group B Streptococcus (GBS) is a major cause of infant meningitis, often requiring intensive care. The molecular determinants of severe disease (meningitis/Intensive Care Unit (ICU) admission) remain unclear. Although late-onset disease (LOD) and very late-onset disease (VLOD) appear clinically similar, their genomic distinctions are not well described. METHODS: We characterized invasive GBS isolates from 87 patients at Boston Children's Hospital over 15 years using whole-genome sequencing. Isolate diversity was compared with global samples. We assessed vaccine coverage, antimicro

Prediction of Bacteremia and Bacterial Meningitis Among Febrile Infants Aged 28 Days or Younger.

IMPORTANCE: Fever in the first month of life is often the only sign of life-threatening invasive bacterial infection, specifically bacteremia or bacterial meningitis. Most international guidelines recommend routine lumbar punctures for all febrile infants 28 days or younger to rule out bacterial meningitis. Clinical prediction rules may allow for select testing, but limited information exists on their performance to identify infants at low risk for invasive bacterial infections. OBJECTIVE: To evaluate the diagnostic accuracy of the updated Pediatric Emergency Care Applied Research Network (PE

A Quality Improvement Initiative to Decrease Intravenous Antibiotic Use in Febrile Young Infants.

BACKGROUND: The American Academy of Pediatrics (AAP) published guidelines in 2021 recommending the use of inflammatory markers (IMs), including procalcitonin (PCT), to evaluate febrile infants and identify those at low risk for bacterial infection for whom clinicians may forego antibiotics. This quality improvement (QI) project aimed to safely decrease antibiotic administration in febrile infants 22 to 56 days old presenting to the emergency department from a baseline of 46% to 36% within 1 year, inclusive of race and ethnicity and preferred language. METHODS: A multidisciplinary team identif

The ZNF148-ZEB1-AS1-IGF2BP2-NOD2 Axis Drives Microglial Antipneumococcal Immunity in Bacterial Meningitis.

Streptococcus pneumoniae (Spn) meningitis remains a lethal central nervous system (CNS) infection with limited therapies. This study identifies the lncRNA ZEB1-AS1 as a central coordinator of microglial immunity against Spn through a multi-tiered regulatory cascade. Transcriptomic analysis revealed Spn-induced ZEB1-AS1 upregulation in human microglia, driven by ZNF148, which directly binds its promoter. Functional interrogation demonstrated that ZEB1-AS1 knockdown impairs bacterial clearance and pro-inflammatory cytokine production (IL-1β, IL-6, TNF-α, p < 0.01), while its overexpression ampli

Continuous Infusion of Aztreonam-Avibactam After High Loading Dose for an Infection Caused by an OXA-48- and NDM-1-Co-producing ST147 Klebsiella pneumoniae.

INTRODUCTION: Infections caused by metallo-β-lactamase-producing Enterobacterales offer limited therapeutic options. Aztreonam-avibactam (ATM-AVI) provides a promising alternative, but its approved intermittent regimen is complex and can lead to substantial drug waste. METHODS: We describe a case of mastoiditis with a retrotympanic abscess due to OXA-48- and NDM-1-producing Klebsiella pneumoniae, managed with continuous infusion (CI) of ATM-AVI after a full-vial loading dose, supported by therapeutic drug monitoring (TDM) and whole-genome sequencing (EPISEQ CS V2.0, bioMérieux). Meningitis due to pandrug resistant Chryseobacterium gleum: microbiological cure and clinical response with linezolid-based combination therapy.

We report a rare case of hospital-acquired meningitis caused by pandrug-resistant Chryseobacterium gleum, which presumed susceptibility to linezolid (inhibition zone, 22 mm). Treatment with ciprofloxacin and piperacillin-tazobactam failed, but adding linezolid led to recovery. This case suggests that linezolid may represent a potential adjunctive option for pandrug-resistant C. gleum meningitis.
