Survival after cardiac arrest secondary to Capnocytophaga canimorsus meningitis: A case report.

BACKGROUND: Capnocytophaga canimorsus is a fastidious zoonotic pathogen, commonly transmitted from dogs or cats, that may cause fulminant sepsis and meningitis. Diagnosis is frequently delayed due to its slow growth, underscoring the importance of early clinical recognition and empiric therapy. CASE PRESENTATION: We present a case of Capnocytophaga sepsis with meningitis that resulted in seizure, multi-organ failure and cardiac arrest in a previously immunocompetent individual. With prompt investigation and diagnostics, Capnocytophaga was identified and with supportive therapies, the patient

Empirical oral AntibioticS for possible UTI in well appearing Young febrile infants (EASY).

BACKGROUND: Urinary tract infections (UTIs) are the most common serious bacterial infections in febrile infants. Current UK guidelines recommend parenteral antibiotics for infants under three months with suspected UTI, despite evidence supporting oral therapy in low-risk infants. OBJECTIVES: To assess whether oral antibiotics are non-inferior to parenteral antibiotics for treating suspected UTIs based on treatment failure, need for additional therapy, and secondary outcomes. DESIGN: Multicentre, randomised controlled, open-label, non-inferiority trial with embedded internal pilot. SETTING:

Bacterial meningitis in adults: therapeutic challenges in the era of antibiotic resistance and the potential of bacteriophages and associated by products.

Bacterial resistance to antibiotics is one of the leading factors encouraging the development of new therapeutic strategies. The increased resistance to antibiotics can be attributed to several factors, such as early and unnecessary administration, incorrect dosing, or incomplete antibiotic treatment. One of the diseases that calls for improved understanding of this problem is meningitis, which - if ineffectively treated - may result in severe neurological complications and death. This study provides an overview of the current antibiotic strategies for bacterial meningitis along with the thera

Fucose utilization-associated genes fcsR and fcsK contribute to the colonization of Streptococcus pasteurianus in mice.

Fucose, a sugar abundant in the host environment, is exploited by many bacteria through fucose utilization gene cluster (FUC) to enhance growth and virulence. Streptococcus pasteurianus is an emerging zoonotic pathogen increasingly recognized as a cause of meningitis and bacteremia in both animals and humans worldwide, yet its pathogenic mechanisms remain poorly understood. In this study, we characterized a putative FUC (E8M05_RS09690-E8M05_RS09645) in S. pasteurianus strain WUSP067, which was isolated from the brain of a piglet with meningitis. The predicted proteins encoded by this cluster s

Fatal pneumocephalus caused by Clostridium perfringens bacteremia originating from emphysematous cystitis after stroke-related blood-brain barrier disruption.

Infectious pneumocephalus is rare and usually associated with meningitis or neurosurgical procedures. We report a fatal case of diffuse pneumocephalus caused by Clostridium perfringens bacteremia originating from emphysematous cystitis in a patient recently admitted for a large right middle cerebral artery infarction. Imaging demonstrated subarachnoid gas strictly confined to the infarcted hemisphere, followed by subdural air accumulation and fatal mass effect. We hypothesize that stroke-related blood-brain barrier disruption permitted hematogenous bacterial and gas diffusion into the subarach

Risk Factors and Outcomes of Extensively Drug-Resistant Gram-Negative Bacilli in Neonates with Late-Onset Sepsis.

Background/Objective: To identify the risks and outcomes of extensively drug-resistant Gram-negative bacilli (XDR-GNB) in neonates. Methods: This retrospective case-control study (1995-2024) included neonates with late-onset sepsis (n = 132) and XDR-GNB bacteremia (n = 26) compared with those without XDR-GNB (n = 106). Results: Median gestational age was 31 weeks and birth weight 1540 g. The prevalence of XDR-GNB was 19.7%. The most common XDR-GNB and non-XDR-GNB pathogens were Acinetobacter baumannii and Klebsiella pneumoniae. Sepsis onset occurred earlier in the XDR-GNB group than in the non

Genomic analysis of a clinical Streptococcus suis ST1 isolate from CSF reveals antimicrobial resistance, virulence, and an evolutionary link to ST7.

Streptococcus suis (S. suis) has emerged as an important zoonotic pathogen that can cause serious human infections. S. suis sequence type 1 (ST1) is predominant in sporadic human infections. However, few studies have characterized S. suis ST1 isolated directly from human clinical specimens.Streptococcus suis 366 was isolated from the cerebrospinal fluid (CSF) of a patient with purulent meningitis. Antimicrobial susceptibility testing and whole-genome sequencing were performed. Detailed functional annotations (CARD, VFDB, GO, KEGG, and COG), along with pangenome, phylogenetic, and comparative g

Subtractive Proteomics-Based Prioritization of Drug Targets in Enterococcus faecalis V583 and Structure-Based Identification of Plant-Derived Inhibitors Targeting Alanine Racemase: A High Priority Target.

Enterococcus faecalis, a Gram-positive facultative anaerobe, is a typical inhabitant of the human gut microbiota but becomes an opportunistic pathogen when translocated, leading to serious infections such as meningitis and bloodstream infections (BSIs). According to the World Health Organization (WHO) and recent studies, E. faecalis is responsible for a significant number of BSIs and is listed as a high-priority pathogen for antimicrobial research due to its high antibiotic resistance rates. Despite the availability of antibiotics like ampicillin, vancomycin, and amoxicillin, the emergence of

Construction and evaluation of a diagnostic prediction model for bacterial meningitis based on clinical and laboratory data.

Bacterial meningitis refers to the rapid inflammation of the meninges caused by bacteria or their byproducts, impacting the pia mater, arachnoid mater, and the subarachnoid space. This condition is a serious infectious illness affecting the central nervous system, if not diagnosed and treated promptly, it may result in severe neurological complications or even fatalities, making prompt and precise diagnosis essential for better outcomes. The objective of this research was to develop and assess a diagnostic prediction model for bacterial meningitis utilizing clinical and laboratory information.

Delayed brain abscess formation after mechanical thrombectomy for probable septic cerebral embolism: A case report and literature review.

BACKGROUND: Septic cerebral embolisms (SCE) are serious neurological complications of sepsis. An embolic occlusion due to infected thrombi can precipitate ischemic stroke and a spectrum of secondary pathologies, including intracerebral hemorrhages, mycotic aneurysms, meningitis, and brain abscesses, which are a major complication of SCE. However, delayed abscess formation following cerebral infarction is rarely encountered, and the underlying mechanisms remain poorly understood. Here, we present a rare case of SCE in a patient who underwent a mechanical thrombectomy (MT) for large-vessel occlu

Immunogenic insights and experimental validation of a multiepitope vaccine construct against Streptococcus pneumoniae.

Streptococcus pneumoniae is a predominant cause of pneumonia, sinusitis, bacteremia, meningitis and otitis media especially in children and the elderly. The emergence of antibiotic resistance and shift towards non-vaccine serotypes has compromised the efficacy of the treatment regime and available vaccines. The available polysaccharide-based vaccines confer protection against limited serotypes. In this study, we have conducted the experimental validation of a promising multi epitope vaccine candidate. The vaccine construct was cloned into pET-28a vector and expressed in Escherichia coli. The e
