Cerebrospinal Pharmacokinetic and Pharmacodynamic Evaluation of Sulbactam: Dosing Considerations for Acinetobacter baumannii Meningitis in Pediatric Patients.

BACKGROUND: Effective sulbactam dosing regimens for treating Acinetobacter baumannii meningitis in pediatric patients, considering the degree of cerebrospinal fluid (CSF) inflammation and its consequent effect on drug penetration, have not been thoroughly studied. This study aimed to describe a cerebrospinal pharmacokinetic (PK) model of sulbactam and propose a dosing strategy in pediatric patients based on pharmacodynamic (PD) evaluation with stochastic simulation. METHODS: Publications were systematically extracted from MEDLINE for sulbactam CSF PK data collection. A cerebrospinal PK model

The Citrobacter freundii Complex as an Emerging Pathogen: Genomic Plasticity, Virulence, and Antimicrobial Resistance.

The Citrobacter freundii (C. freundii) complex represents an increasingly significant group of opportunistic pathogens within healthcare settings. This bacterial complex demonstrates remarkable genomic plasticity, characterized by extensive horizontal gene transfer capabilities that facilitate rapid acquisition of resistance determinants and virulence factors. Although originally considered environmental organisms with limited pathogenic potential, members of the C. freundii complex have emerged as important nosocomial pathogens responsible for urinary tract infections, bacteremia, wound infec

Determinants of susceptibility and tolerance of Streptococcus pneumoniae to hypothiocyanous acid.

Infections caused by Streptococcus pneumoniae (Spn, pneumococcus), manifesting as pneumonia, meningitis, and otitis media, represent a leading cause of severe morbidity and mortality globally. Escalating antibiotic resistance and serotype replacement by conjugate vaccines underscore the need for serotype-independent therapies. Hypothiocyanous acid (HOSCN), a reactive oxidant naturally produced by the host innate immune system, is formed through the interaction of hydrogen peroxide, thiocyanate anions (SCN-), and peroxidase enzymes. HOSCN is produced at mucosal surfaces, including the respirato

A digital twin for Escherichia coli K1 neonatal meningitis.

Escherichia coli K1 is a major Gram-negative pathogen responsible for neonatal meningitis. Despite significant progress in antimicrobial therapy and neonatal intensive care, clinical outcomes remain problematic due to delayed diagnosis, rapid disease progression and a lack of precision tools for personalized management. Here, we propose a technical and translational digital twin framework for E. coli K1 infection in neonates that integrates clinical, microbiological, physiological and molecular data within a continuously adaptive computational model. This twin would simulate bloodstream invasi

Aseptic and Bacterial Meningitis: Diagnosis, Treatment, and Prevention.

Meningitis refers to inflammation of the meninges surrounding the brain. Meningitis may vary from a benign, self-limited condition to one that may cause disability or death. The most common bacterial pathogens are Streptococcus pneumoniae (pneumococcus), Haemophilus influenzae type B, Neisseria meningitidis (meningococcus), and group B beta-hemolytic streptococci (Streptococcus agalactiae). Adults and older children with meningitis may present with neck stiffness, fever, headache, and altered mentation. Lumbar puncture with cerebrospinal fluid analysis is the mainstay for diagnosis and should

Meningeal inflammation and arachnoid barrier breakdown in a mouse model of neonatal bacterial meningitis.

Newborns are especially susceptible to bacterial meningitis, primarily caused by Group B Streptococcus (GBS), due to incomplete maturation of immune and barrier systems. While meningitis is well known to break down the blood-brain barrier (BBB), how the meningeal arachnoid barrier, a critical component of the blood-cerebrospinal fluid barrier (B-CSFB), responds to infection is poorly understood. Using a neonatal mouse model of bacterial meningitis, we demonstrate that GBS infection significantly increases arachnoid barrier permeability, coinciding with alterations in Claudin-11 tight junction

Friends and foes: impact of bacteria on genome stability.

Bacteria, present in normal conditions in the microbiome or during infections, exert profound effects on genome stability, with both genotoxic and genoprotective consequences. Certain pathogenic bacteria, such as Escherichia coli (colibactin-producing strains), Helicobacter pylori, Fusobacterium nucleatum, and Campylobacter jejuni, induce DNA damage and are implicated in cancer development through direct toxin production, chronic inflammation, immune modulation, and disruption of host cell signaling. Genotoxins such as colibactin, the cytolethal distending toxin, and the typhoid toxin induce D

Lineage dynamics of invasive Escherichia coli isolates in the Netherlands from 1975 to 2021: a retrospective longitudinal genomic analysis.

BACKGROUND: Escherichia coli is a common cause of invasive infections such as bloodstream and cerebrospinal fluid infections in neonates. Strains positive for the K1 capsule are considered the most common cause of such neonatal invasive infections. This assumption of K1 dominance, and indeed the population genomics of E coli causing invasive infections in general is largely unstudied. We aimed to provide a comprehensive characterisation of this pathogen population using a longitudinal isolate collection. METHODS: In this analysis we report the findings of the SENTINEL study, a longitudinal ge

Cases of Septic Arthritis During Meningitis Infection at Zinder National Hospital, Niger Republic.

septic arthritis is the consequence of bacterial aggression of the articular synovium, which can occur during meningitis. We report two cases of arthritis in patients hospitalized for meningitis in the infectious and tropical diseases department of Zinder National Hospital. Two cases admitted initially for meningitis at the Zinder National Hospital, developed secondary localized arthritis. Laboratory confirmation was carried out using QIAstat-Dx Meningitis/Encephalitis Panel kit (QIAGEN) on cerebrospinal fluid and joint fluid for the respective diagnosis of meningitis and arthritis. While Haem

Anthocyanin-Incorporated Chromogenic Agar for Rapid, Selective Detection of Streptococcus pneumoniae via Hydrogen Peroxide-Mediated Oxidation.

The rapid and cost-effective identification of Streptococcus pneumoniae (S. pneumoniae) is crucial for the prompt treatment of pneumonia and meningitis. However, conventional culture methods are time-consuming and molecular techniques are prohibitively expensive in resource-limited settings. To address this challenge, we developed a novel anthocyanin chromogenic agar as a diagnostic tool for the selective and rapid detection of S. pneumoniae. Unlike traditional pH-based indicators, this method exploits the unique oxidative capability of S. pneumoniae, which releases excessive amounts of hydrog

LY294002 and LiCl Mitigate Neonatal ExPEC Meningitis Through Akt/GSK3β Signaling Modulation.

BACKGROUND: Neonatal purulent meningitis (NPM) is a life-threatening condition associated with high mortality rates and a substantial risk of severe long-term neurological sequelae among survivors. Extraintestinal pathogenic Escherichia coli (ExPEC) is the predominant causative agent of NPM and exhibits multidrug resistance. Targeting host signaling pathways is a promising therapeutic approach. In this study, we aimed to evaluate the therapeutic potential of LY294002 (a PI3K/Akt inhibitor) and lithium chloride (LiCl) (a GSK3β inhibitor) in a neonatal mouse model of ExPEC meningitis. METHODS:

Outcomes of acute meningitis according to immunosuppression status: 15-year retrospective cohort.

Acute meningitis remains a primary global health concern. Immunosuppressed patients have risks due to atypical clinical presentations and a broader range of causative pathogens. We aimed to describe the outcomes and clinical features of acute meningitis according to immunosuppression status. We performed a retrospective cohort study of adults with acute meningitis from January 2009 to December 2023. Patients with postsurgical meningitis were excluded. Outcomes and demographic, clinical, and laboratory features were compared using non-parametric statistical tests, and mortality was analyzed usi

Phage-steering permits antibody-mediated clearance of E. coli K1 from the gut.

Escherichia coli (E. coli) strains expressing the capsule serotype K1 (E. coli K1) are a prevalent cause of neonatal sepsis and meningitis. The gut microbiota of healthy adults is a natural reservoir of E. coli K1, from which it can spread to extra-intestinal sites or be transmitted from mother to infant during birth. Accordingly, shifting gut colonization from potentially pathogenic E. coli strains to more benign strains could reduce the risk of disease. Here, we leverage selective pressures exerted by bacteriophage and mucosal antibodies to limit gut colonization by E. coli K1 and prevent it

Streptococcus suis disrupts the blood-brain barrier through inducing ubiquitin-proteasome-mediated degradation of KAT2A.

Streptococcus suis (S. suis) induces host cell death and has the ability to invade the blood-brain barrier (BBB). However, S. suis-induced BBB disruption has not been completely elucidated. In this study, we focused on the regulatory role of lysine acetyltransferase 2A (KAT2A) on BBB, which is a key regulator of cell death. We found that S. suis strain SC19 induces cell death in human cerebral microvascular endothelial cell line D3 (hCMEC/D3) by membrane disruption and LDH release. During SC19 infection, KAT2A protein expression was markedly reduced but its mRNA expression was not affected. Fu

Pediatric Invasive Group A Streptococcus Infections with Central Nervous System Involvement: A Single Institution Case Series in Southern New England.

Pediatric invasive Group A Streptococcus (iGAS) infections are rare, affecting sterile sites including the central nervous system (CNS), and cause significant morbidity and mortality. An increased incidence of pediatric iGAS infections, including cases with CNS involvement, has been noted following the COVID-19 pandemic both domestically and internationally. Regional New England iGAS data specific to pediatric populations have been limited. This case series describes three pediatric cases of iGAS with CNS involvement which presented within the same two-month period in early 2025, all from with
