Elizabethkingia Species as an Emerging Pathogen: A Comprehensive Review of Clinical and Microbiological Evidence.

Elizabethkingia species are rare but increasingly recognised Gram-negative pathogens linked to healthcare-associated transmission, intrinsic multidrug resistance, and severe infection in vulnerable hosts. We performed a comprehensive review of human Elizabethkingia infections by systematically searching PubMed on 18 October 2025 and included English-language case reports, case series, and outbreak investigations; species were analysed as reported (legacy nomenclature retained), and adults were defined as ≥18 years. In total, 374 studies were included (300 case reports, 41 case series, 33 outbr

Pyogenic Spondylitis with Epidural Abscess Caused by Streptococcus suis Serotype 2 ST7: Tissue mNGS Confirmation and Whole-Genome Characterization of a Human Isolate.

Streptococcus suis is an emerging zoonotic pathogen that typically causes bacteremia or meningitis in humans, whereas vertebral osteomyelitis with epidural abscess is exceedingly rare and may be missed. We describe a 65-year-old farmer with fever and severe low back pain after long-term bare-handed handling of raw pig lungs. Pre-treatment blood cultures yielded S. suis identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). After transient improvement on empirical therapy, fever recurred with worsening lumbar pain. Contrast-enhanced magnetic r

Early Transition From Parenteral to Oral Antibiotics for Neonates With Bacteremic Urinary Tract Infections.

We conducted a retrospective cohort study of 49 neonates with bacteremic urinary tract infections (UTIs) to assess the efficacy of transition to oral antibiotics. Forty-one received oral step-down therapy after parenteral antibiotics; 2 had relapsed UTIs. No cases of bacteremia or meningitis occurred within 30 days post-treatment, suggesting transition to oral antibiotics as effective.

Distribution of piperacillin-tazobactam into cerebrospinal, peritoneal, and pleural fluid in children aged 0-18 years.

BACKGROUND: Piperacillin-tazobactam is increasingly used to treat serious infections in children; however, data on its penetration into body fluids remain limited. We aimed to evaluate drug penetration into these sites and identify effective dosing regimens. METHODS: Free piperacillin-tazobactam concentrations were measured in serum, cerebrospinal fluid (CSF), peritoneal, and pleural fluid. A population pharmacokinetic model was developed to determine the penetration (AUC0 - ∞,  target site/AUC0 - ∞,  serum) into the relevant sites. Simulations evaluated intermittent (q8h, q6h, q4h) and conti

Global survey of extraintestinal pathogenic Escherichia coli identifies stable serogroup-virulence-resistance linkages.

BACKGROUND: Extraintestinal pathogenic Escherichia coli (ExPEC) is a leading cause of urinary tract infections, sepsis, and neonatal meningitis globally. However, global patterns linking O-serogroups, sequence types (STs), virulence, and antimicrobial resistance (AMR) remain poorly resolved. METHODS: We analyzed 12,220 ExPEC genomes, including 12,160 publicly available genomes retrieved from the NCBI database and 60 representative isolates collected and sequenced in this study. Serogroups were assigned by in silico wzm/wzt/wzx/wzy and fliC gene analysis, and phylogenies constructed using maxim

Editorial: New Target Product Profiles From the WHO as Climate Change, Global Population Migration, and Conflict Drive Antimicrobial Resistance.

Climate change, population migration, and conflict have driven antimicrobial resistance (AMR) globally, and at a time when there remains a need for a reliable pipeline of affordable, accessible, and innovative antimicrobial agents that can be made available to all. In 2023, the World Health Organization (WHO) estimated that in 2019, bacterial AMR directly caused 1.27 million deaths and contributed to 4.95 million deaths worldwide. In 2024, the WHO published its list of bacterial priority pathogens and their associated antimicrobial resistance, grouped into medium, high, and critical categories

Hyrtl's fissure - a rare congenital temporal bone anomaly.

Hyrtl's fissure is a rare congenital temporal bone anomaly that may cause cerebrospinal fluid (CSF) otorrhea and recurrent bacterial meningitis in children. We report a case of a 9-year-old girl who presented with headache, fever, neck stiffness, photophobia, and right ear pain. Cerebrospinal fluid analysis confirmed bacterial meningitis caused by Streptococcus pneumoniae. During planned tympanostomy, continuous drainage of clear fluid was observed, and bedside glucose testing suggested CSF leakage. Imaging, including high-resolution computed tomography (CT) and contrast-enhanced magnetic reso

Meningoencephalitis and secondary pituitary abscess in a 16-year-old patient with gigantism.

Pituitary adenomas occur in children quite rarely. Furthermore, pituitary abscesses are recognized as particularly rare conditions, posing considerable challenges in both diagnosis and management. We report the case of a 16-year-old patient presenting with bacterial meningoencephalitis of unknown origin, complicated by a superinfected pituitary macroadenoma. The patient initially exhibited clinical signs suggestive of meningitis, and lumbar puncture revealed a macroscopically turbid cerebrospinal fluid. Magnetic resonance imaging identified a frontobasal meningoencephalitis complicated by a su

Association of the blood urea nitrogen to serum albumin ratio on prognosis in patients with bacterial meningitis: a retrospective cohort study.

INTRODUCTION: Early identification of high-risk patients is crucial for improving outcomes of bacterial meningitis (BM) patients. The blood urea nitrogen to albumin (BUN/ALB) ratio has demonstrated prognostic value in various critical illnesses, but its role in BM remains unestablished. METHODS: This retrospective cohort study included 146 adult patients with microbiologically confirmed BM hospitalized between 2010 and 2025. BUN/ALB ratio within 6 hours of admission was calculated. Based on 3-month post-discharge modified Rankin Scale (mRS) scores, patients were dichotomized into favorable (m

Meningitis and Bacteremia Caused by Streptococcus suis: First Case Report in Colombia.

The first documented instance of acute bacterial meningitis caused by Streptococcus suis in Colombia has been reported. A 69-year-old male resident of a rural area with a history of direct contact with pigs presented to the clinic with a persistent fever, severe headaches, and altered consciousness. Cerebrospinal fluid analysis showed neutrophilic pleocytosis, increased cerebrospinal fluid protein levels, and decreased glucose due to consumption. Initial culture and confirmation by mass spectrometry MALDI-TOF identified S. suis, with an MLSB resistance profile and resistance to tetracyclines,

Development and validation of a nomogram for suspected post-neurosurgical bacterial ventriculitis/meningitis.

OBJECTIVE: The diagnosis of post-neurosurgical bacterial ventriculitis/meningitis (BV/M) remains challenging, particularly in the patients with leukocytic pleocytosis and hypoglycorrhachia. This study aimed to establish a nomogram identifying the high-risk and low-risk post-neurosurgical BV/M and evaluate the utility of this risk scoring system. METHODS: Adult patients with CSF leukocytes ≥ 100/mm3 and glucose ≤ 2.2 mmol/L experienced neurosurgical or invasive procedures in three hospitals were divided into training and validation cohort, patients from Medical Information Mart for Intensive C

Rapid identification and methicillin resistance test to Staphylococcus aureus in cerebrospinal fluid by MALDI-TOF MS.

Bacterial meningitis is a disease with high mortality and morbidity, and it primarily manifests as symptoms involving the central nervous system (CNS). Hence, it would be of great importance to make an early diagnosis and initiate empirical antimicrobial treatment in time for this disease. In this study, we investigated the feasibility of rapid Staphylococcus aureus (S. aureus) identification and drug resistance analysis through the combination of centrifugation-based enrichment of bacteria and matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS). Delayed Hydrocephalus Following Listeria monocytogenes Meningitis in an Immunocompetent Child.

Listeria monocytogenes meningitis in immunocompetent pediatric patients may present as a severe central nervous system infection and requires prompt recognition and appropriate antimicrobial therapy. We report the case of a previously healthy twelve-year-old boy who presented with acute-onset fever, severe holocranial headache, nausea, and recurrent vomiting consistent with bacterial meningitis. Given his stable neurological examination without focal deficits or signs of increased intracranial pressure, brain magnetic resonance imaging was performed as the initial neuroimaging study and demons

Meta-Analytic Modeling to Define Decision Thresholds for Cerebrospinal Fluid Heparin-Binding Protein in Healthcare-Associated Ventriculitis and Meningitis.

Background/Objectives: Healthcare-associated ventriculitis and meningitis (HAVM) is a life-threatening complication of neurosurgical procedures. Conventional cerebrospinal fluid (CSF) indices cannot reliably distinguish bacterial infection from sterile postoperative inflammation, and cultures are frequently delayed or negative. We conducted the first systematic review and meta-analysis to determine the pooled diagnostic accuracy of CSF heparin-binding protein (HBP) for HAVM and to establish clinically actionable decision thresholds. Methods: PubMed, Embase, the Cochrane Library, and China Nati
