Bedside Transcranial Doppler as a Dynamic Warning Tool for Rapidly Evolving Intracranial Hypertension in Pneumococcal Meningitis.

In severe bacterial meningitis, an initially non-critical or technically unreliable lumbar puncture opening pressure should not reassure when hydrocephalus and coma persist. Bedside transcranial Doppler cannot measure intracranial pressure, but bilateral near-absent diastolic flow may provide an urgent warning signal prompting repeat assessment and cerebrospinal fluid diversion.

Pneumococcal meningitis secondary to chronic orbital inflammation with skull base erosion: a case report.

This case report describes a 32-year-old female patient who developed acute pneumococcal meningitis following six-year chronic orbital inflammatory changes after strabismus corrective surgery. The patient experienced gradual deterioration of right-eye vision and recurrent retro-orbital headache; serial magnetic resonance imaging (MRI) from 2021 to 2023 demonstrated diffuse intraconal retrobulbar abnormalities and extraocular-muscle thickening. She subsequently presented with high fever, generalized seizures and impaired consciousness. Cerebrospinal fluid (CSF) metagenomic next-generation seque

Congenital Nasal Encephalocele Presenting With Pneumococcal Meningitis and Subsequent Bilateral Sensorineural Hearing Loss: A Case Report of a Pediatric Patient.

Congenital nasal encephaloceles are rare anterior skull base defects that may predispose patients to cerebrospinal fluid (CSF) leakage and subsequent meningitis. While sensorineural hearing loss (SNHL) is a known complication of pediatric meningitis, progression from an unrecognized nasal encephalocele to meningitis and bilateral profound hearing loss is infrequently reported. We describe a seven-month-old male who presented with pneumococcal meningitis and status epilepticus requiring intensive care management. Following recovery, he developed severe-to-profound bilateral SNHL. Further evalua

Elucidating the genetic architecture of yield components in pyramidale wheat (Triticum turgidum L.): a comparative approach using QTL analysis and genomic selection.

BACKGROUND: Diabetic retinopathy (DR) is a major microvascular complication of diabetes mellitus and a leading cause of visual impairment. While dyslipidemia has been implicated in DR pathogenesis, the relationship between high-density lipoprotein cholesterol (HDL-C) and DR remains controversial. This study aimed to investigate the association between HDL-C levels and DR prevalence in diabetic patients type 2 diabetes mellitus (T2DM). METHODS: This study is the second analysis based on a cross-sectional studv. A total of 2001 (858 men and 1143 women) diabetic patients who visited the diabetic

PGK1 contributes to blood-brain barrier integrity via PI3K-AKT-mTOR pathway in early pneumococcal meningitis.

Pneumococcal meningitis continues to be a devastating disease associated with high mortality and neurological sequelae, largely due to disruption of the blood-brain barrier (BBB). Emerging evidence implicates that metabolic reprogramming within the neurovascular unit constitutes a critical pathophysiological mechanism. Phosphoglycerate kinase 1 (PGK1), a glycolytic enzyme, may play a role beyond energy metabolism, but its involvement in BBB protection during meningitis is undefined. Here, we demonstrate that PGK1 preserves BBB integrity via the PI3K-Akt-mTOR pathway in early pneumococcal menin

Penetration of daptomycin in cerebrospinal fluid during pneumococcal meningitis.

OBJECTIVES: There are few data on the penetration of daptomycin into the cerebrospinal fluid (CSF) of patients with bacterial meningitis. This ancillary study of the AddaMap trial aimed to assess the CSF penetration of intravenous daptomycin in 13 patients with pneumococcal meningitis. PATIENTS AND METHODS: Daptomycin was administered at 10 mg/kg/day to 13 patients with pneumococcal meningitis admitted to the Dijon University Hospital. Blood and CSF samples were collected on days 3 and 8. The population pharmacokinetics of daptomycin in plasma and CSF were studied using a two-compartment mode

Purulent Streptococcus Pneumoniae meningitis with suppurative focus and infectious vasculitis leading to ischemic and hemorrhagic strokes: A rare case report.

Acute bacterial meningitis (ABM) is a neurological emergency that continues to cause high levels of morbidity and mortality worldwide. Despite advances in antimicrobial therapy and vaccination, it continues to cause severe neurological sequelae. In both adults and children older than 2 years, Streptococcus pneumoniae is the leading cause of acute bacterial meningitis, often leading to life-threatening complications. Vascular complications are seen in 15%-20% of all cases of acute bacterial meningitis, and intracerebral hemorrhages occur in 2% of patients. Coexistence of ischemic and hemorrhagi

Synthetic peptide V11A reduces bacterial load and inflammation in pneumococcal meningitis.

INTRODUCTION: Pneumococcal meningitis, caused by Streptococcus pneumoniae, is exacerbated by factors released during bacterial lysis, triggering inflammation and damage to host cells. Bacteriolytic antibiotics increase inflammation meaning that there is a high unmet medical need for non-bacteriolytic antimicrobials as alternative therapeutic options. Previously, an 11-amino acid peptide was discovered in the secretome of Klebsiella pneumoniae, named V11A, and was found to inhibit growth of S. pneumoniae in a bacteriostatic and species-specific manner in vitro. METHODS AND RESULTS: Here it is

Implementation of India's largest sentinel surveillance for meningitis: an exploratory qualitative analysis of the facilitators and challenges in this sentinel site network.

BACKGROUND: Meningitis remains a leading cause of childhood morbidity and mortality in India, with Streptococcus pneumoniae as the most common bacterial pathogen. The Pneumococcal Conjugate Vaccine (PCV) was introduced into India's Universal Immunization Program in 2017 with PCV-13 across five states and was later replaced by the indigenously manufactured PCV-10 in 2019. However, the absence of a nationwide surveillance platform for pneumococcal and other meningitis-causing pathogens limited data on disease burden and vaccine impact. To address this, the ICMR launched a project in 2018 to stre

Mastoiditis With Pneumocephalus on CT: An Uncommon Clue to Underlying Bacterial Meningitis.

The combination of pneumocephalus and meningitis as a complication of mastoiditis is rare, with very few cases published in the literature. We report the case of a 33-year-old previously healthy male who presented with acute confusion, fever, agitation, and neck stiffness following a syncopal episode at work. CT imaging revealed opacification of the left mastoid air cells, suggestive of mastoiditis and pneumocephalus in the left frontoparietal region. Cerebrospinal fluid (CSF) analysis confirmed the diagnosis of pneumococcal meningitis. Empirical antibiotic treatment and steroids were initiate

Neuraminidase A controls pneumococcal recognition and fate: deficiency enhances immune sensing and intracellular survival, while treatment promotes phagocytic clearance.

Streptococcus pneumoniae (pneumococcus) is a leading cause of bacterial meningitis worldwide and is associated with cerebrovascular complications and long-term neurological sequelae. These outcomes are largely driven by an excessive yet ineffective neuroinflammatory response. Although pneumococci express multiple virulence factors that enable immune evasion, how these factors modulate phagocytic responses in the central nervous system remains unclear. Here, we identify neuraminidase A (NanA) as a critical regulator of phagocytic activation in microglia and macrophages. Using murine and human m

Age-Specific Antibiograms for Bacterial Meningitis Pathogens Based on Isolates Collected in a Community Laboratory.

BACKGROUND: Creating antibiograms solely for adults may overestimate resistance of antimicrobials for certain pathogens in children. The Canadian Paediatric Society comments that areas with no cephalosporin-resistant Streptococcus pneumoniae cases should consider ceftriaxone or cefotaxime monotherapy for meningitis, despite most experts recommending adding vancomycin. The present study created age-specific antibiograms using LifeLabs data to report incidences of resistant bacterial meningitis pathogens at the regional level to determine the need for duo-coverage. METHODS: Data of common bacte
