LacR and TrxB are key virulence factors involved in pneumococcal meningitis as identified by a genome-wide association study.

Pneumococcal meningitis is the most common form of bacterial meningitis, with high case fatality rates. Bacterial genetics influence disease severity, but the underlying mechanisms remain poorly understood. Here, we evaluated top variants from a genome-wide association study (GWAS) linking Streptococcus pneumoniae genetic variation to disease outcome. Fifteen S. pneumoniae knockout mutants were tested for virulence in a zebrafish embryo meningitis model; six showed reduced virulence compared to wild-type, but four were excluded following competitive index and growth assays. Community-Acquired Pneumococcal Ventriculitis as a Complication of Otogenic Pneumococcal Meningitis: A Case Report.

Pyogenic ventriculitis is a rare and potentially fatal infection of the ventricular system. While most reported cases are nosocomial and catheter-associated, ventriculitis as a complication of community-acquired bacterial meningitis is uncommon, particularly in adults, and carries a poor prognosis. We report the case of an 82-year-old previously healthy and unvaccinated woman who presented with a one-day history of fever, confusion, and purulent left-sided otorrhea on a 2-week background of flu-like symptoms. Cranial computed tomography was consistent with otogenic meningitis complicated by pn

Severe Pneumococcal Meningitis With Intracranial Hypertension and Cerebral Venous Sinus Thrombosis in a Patient With HIV and Prior Splenectomy.

We report the case of a 45-year-old man with a history of HIV infection (on antiretroviral therapy with a preserved CD4 count) and prior splenectomy, who presented with vomiting, diarrhea, confusion, and worsening headache. He was found to have Streptococcus pneumoniae meningitis complicated by intracranial hypertension requiring external ventricular drain placement and extensive cerebral venous sinus thrombosis. This case highlights the risk of severe pneumococcal infection in asplenic and immunocompromised patients and underscores the importance of prompt recognition and management.

Cerebral Venous Sinus Thrombosis Following Pneumococcal Meningitis: A Rare but Serious Complication.

Cerebral venous sinus thrombosis (CVST) is a rare but important complication of bacterial meningitis and is most commonly associated with Streptococcus pneumoniae. We report the case of a 60-year-old man who developed extensive CVST following the completion of a 14-day course of intravenous ceftriaxone for pneumococcal meningitis. Despite biochemical resolution of infection and absence of focal neurological deficits, persistent headache and meningism prompted further neuroimaging, which confirmed the diagnosis. He was treated with anticoagulation, resulting in subsequent radiological resolutio

Differential effects of cannabidiol on inhibitory avoidance memory reconsolidation and consolidation are associated with distinct hippocampal proBDNF and BDNF signaling profiles.

INTRODUCTION: Cannabidiol (CBD), a major non-psychoactive constituent of Cannabis sativa, exerts pleiotropic effects, including antipsychotic, anxiolytic, and neuroprotective properties, with low toxicity and minimal euphoric effects. Previous studies have demonstrated that CBD alleviates memory impairments associated with pathological conditions such as seizures, reserpine exposure, pneumococcal meningitis, and sepsis. However, its effects on memory processes, particularly inhibitory avoidance (IA) memory reconsolidation and consolidation, remain incompletely understood and inconsistently rep

Five decades of pneumococcal meningitis in Spain: a single-centre, clinical and genomic, retrospective, observational study.

BACKGROUND: Pneumococcal meningitis remains a major threat, with high fatality rates and long-term sequelae, despite advances in vaccination and treatment. We aimed to examine the associations between pneumococcal serotypes, Global Pneumococcal Sequence Cluster (GPSC), antimicrobial resistance, source of infection, and clinical outcomes in adults with pneumococcal meningitis. METHODS: In this single-centre, clinical and genomic, retrospective, observational study, we analysed all laboratory-confirmed cases of adult pneumococcal meningitis recorded at Hospital Universitari de Bellvitge, Spain.

Concurrent Detection of Streptococcus pneumoniae and Herpes Simplex Virus Type 1 (HSV-1) in Cerebrospinal Fluid: Clinical Interpretation Beyond Molecular Positivity.

Herpes simplex virus type 1 (HSV-1) encephalitis and acute bacterial meningitis are severe central nervous system infections associated with significant morbidity. While typically distinct, the increasing use of multiplex polymerase chain reaction (PCR) panels has led to occasional co-detection, creating diagnostic uncertainty. We describe a 44-year-old previously healthy male diagnosed with culture-confirmed pneumococcal meningitis in whom multiplex cerebrospinal fluid PCR also detected HSV-1. The clinical course was complicated by a generalized seizure on hospital day 3 and the development o

Prevention of neuroinfectious-diseases: high impact of vaccination programs - but the potential is not maxed out yet.

BACKGROUND: The greatest impact on the burden of CNS infections resulted from preventive measures, mainly vaccination programs, that have decreased the incidence of many CNS infections significantly. Here, we highlight the main cornerstones of vaccination programs on bacterial and viral CNS infections and point at future chances of upcoming vaccines. MAIN BODY: Vaccination programs have significantly decreased the number of cases due to Haemophilus influenzae type B and Neisseria meningitidis. For pneumococcal meningitis, new vaccines that prevent neuroinvasive serotypes have the potential to

Single-cell analysis identifies a type I interferon-associated neutrophil signature in bacterial meningitis.

Bacterial meningitis (BM) is a central nervous system infection characterized by high mortality and disability rates. Neutrophils, as the first-line innate immune cells in host defense against bacterial invasion, play a crucial role in the pathogenesis of this disease. However, the transcriptomic characteristics of neutrophils in BM remain poorly defined, limiting our understanding of the underlying mechanisms. In this study, we analyzed single-cell RNA sequencing (scRNA-seq) data from the public database GSE221681 to map the transcriptomic landscape of meningeal immune cells in a mouse model

Streptococcus pneumoniae: Pathogenesis, virulence factors, antimicrobial resistance mechanisms, and therapeutic strategies.

BACKGROUND: Streptococcus pneumoniae is a Gram-positive, facultative anaerobic bacterium and a leading cause of community-acquired pneumonia, meningitis, bacteremia, and otitis media, responsible for an estimated 1.6 million deaths annually. Despite widespread polysaccharide and conjugate vaccine programs and decades of antibiotic therapy, pneumococcal disease continues to pose major public health challenges. These challenges are increasingly understood to reflect the organism's sophisticated capacity for adaptive recombination, allowing it to exploit every selective pressure applied through v

Pneumococcal meningitis in children and adults before and after implementation of PCV13 into national pediatric immunization program: A cohort study in Taiwan.

BACKGROUND: Streptococcus pneumoniae remains a leading cause of bacterial meningitis worldwide. Although pneumococcal conjugate vaccines (PCVs) have markedly reduced vaccine serotypes (VTs) invasive pneumococcal disease, the emergence of non-vaccine serotypes (NVTs) has become an increasing concern. Data describing the clinical and microbiological characteristics of pneumococcal meningitis after the introduction of PCV13 remain limited. METHODS: We conducted a retrospective study of culture-confirmed pneumococcal meningitis at Chang Gung Memorial Hospital, Taiwan, from 2009 to 2025. Clinical

Macrophage IL-1β turns meningeal fibroblasts into inflammatory amplifiers in pneumococcal infection.

INTRODUCTION: In pneumococcal meningitis, a massive inflammatory reaction is triggered by the host immune system, leading to neurological damage. However, the mechanisms underlying the initiation and regulation of this response, particularly by resident cells, remain incompletely understood. Despite their strategic localization at the host-pathogen interface, the role of meningeal fibroblasts in pneumococcal meningitis remains poorly defined. This study therefore aimed to investigate their contribution to the immune response against Streptococcus pneumoniae. METHODS: Primary meningeal fibrobl

Pneumococcal meningitis complicated by extensive cerebral venous sinus thrombosis in an immunocompromised patient.

A man in his 40s who takes adalimumab for Crohn's disease presented with a 1-day history of fever, headache and rapid onset of agitation with reduced consciousness. He was intubated, and a subsequent lumbar puncture revealed severe Streptococcus pneumoniae meningitis. He was started on antibiotics and steroids; however, on sedation hold, he demonstrated abnormal posturing. Neurological imaging revealed extensive cerebral venous sinus thromboses (CVSTs). He was started on a heparin infusion and transferred to a tertiary neurological centre, where he was managed with a split treatment dose of en

Mechanistic Landscape and Farm to Fork Control Strategies for Streptococcus suis: An Emerging Foodborne Threat.

Streptococcus suis (S. suis) is an emerging zoonotic agent that now rivals classical food-borne pathogens in its global clinical burden of meningitis and septic shock. Recent epidemiological syntheses covering more than 30 countries now list over 1600 laboratory-confirmed human cases with a pooled case-fatality of about 12%, climbing beyond 18% in East Asia. The widely cited pooled case-fatality estimate derives from a PRISMA-guided systematic review and meta-analysis that searched PubMed, Scopus, Web of Science, ScienceDirect, and Google Scholar through December 2012 and pooled study-level ev

Pneumococcal meningitis among hospitalised children after introduction of pneumococcal conjugate vaccine in India: a sentinel hospital surveillance (2019-2022).

BACKGROUND: In 2017 India introduced the 13-valent pneumococcal conjugate vaccine (PCV13; Prevenar 13®) in select high-burden sites and in 2021, transitioned to nationwide introduction of 10-valent PCV (PCV10-SII; PNEUMOSIL®). We examined data from a large sentinel paediatric meningitis surveillance network to assess early changes in pneumococcal meningitis among hospitalised children after introduction of pneumococcal vaccine. METHODS: Children (1-59 months) who were hospitalised with suspected meningitis at 32 hospitals across 19 states of India were enrolled in this hospital-based surveill

Clinical and population genetic insights into primary complement component 2 (C2) deficiency: A founder variant in Ashkenazi Jews.

Complement component 2 deficiency (C2D) is the most common inherited complement deficiency and is associated with severe bacterial infections and autoimmunity. We describe a 14-year-old Ashkenazi Jewish boy presenting with bullous cutaneous lupus erythematosus and lupus nephritis, whose diagnosis of C2D was prompted by a family history of fatal pneumococcal meningitis in a sibling. Genetic analysis identified homozygosity for the recurrent C2 c.841_849 + 19del variant, and complement studies confirmed absent classical pathway activity and profoundly reduced C2 levels. [Austrian Syndrome : when Pneumococcus strikes three times].

Austrian syndrome is characterized by the triad of pneumonia, meningitis, and endocarditis due to Streptococcus pneumoniae («pneumococcus»). Endocarditis, often occurring in the background, is frequently diagnosed late and requires prompt medical and sometimes surgical management. We report the case of a 51-year-old man admitted for confusion. The initial evaluation revealed bacterial meningitis documented by purulent cerebrospinal fluid, associated with right basal pneumonia. Blood cultures and cerebrospinal fluid cultures identified Streptococcus pneumoniae. The clinical course was marked by

Bioengineered Extracellular Vesicles Mitigate Neuroinflammation by Neutralizing Pneumolysin and Delaying Disease Onset in Experimental Pneumococcal Meningitis.

Bacterial meningitis is a life-threatening neurological disorder frequently caused by a Streptococcus pneumoniae (the pneumococcus) infection of the brain. Standard treatment consists of antibiotics to eliminate bacteria and dexamethasone to reduce inflammation. Despite this, mortality reaches 20% in treated individuals, and half of the survivors suffer long-term neurological sequelae. This is largely due to the poor capacity of antibiotics to reach the brain and the lack of antimicrobial treatment capable of neutralizing the pneumococcal toxin pneumolysin (Ply). To address these limitations,

A "three-in-one" nose-to-brain delivery strategy: intranasal vancomycin spray achieves simultaneous clearance of pneumococcal colonization, bacteremia, and meningitis.

Bacterial meningitis caused by Streptococcus pneumoniae is a lethal central nervous system infection, yet conventional intravenous vancomycin struggles to cross the blood-brain barrier effectively. Interestingly, the natural pathology of this pathogen originates from nasopharyngeal colonization, disseminates into systemic bacteremia, and ultimately breaches the meninges. Inspired by this sequential invasion, we hypothesized that administering vancomycin directly at the exact starting point via a nasal spray could achieve a simultaneous "three-in-one" eradication of all infection stages. Gene Polymorphisms of TLR2 Subfamily and Bacterial Meningitis in Angolan Children.

BACKGROUND/OBJECTIVES: Bacterial meningitis is a severe disease with a fatality rate of 5-50%. It is mainly caused by Streptococcus pneumoniae or Neisseria meningitidis, which can also cause simultaneous infections outside the central nervous system. Toll-like receptors (TLRs) have an important role in the innate immune system. The TLR2 subfamily comprises the four highly homologous members TLR1, TLR2, TLR6, and TLR10, which also have an important immunomodulatory role in infectious diseases. METHODS: The study cohort consists of 190 bacterial meningitis patients aged 1 to 147 months from ran
