Streptococcus pneumoniae meningitis poses a significant public health challenge, imposing substantial burdens on healthcare systems. This study analyzed the costs and outcomes of pneumococcal meningitis in a Colombian cohort of children from 2017 to 2022, using correspondence analysis and cost assessment to explore the impact of vaccination. A retrospective cohort study was conducted, analyzing children under 18 years diagnosed with pneumococcal meningitis. A total of 55 cases were analyzed in this cohort study, which, while representative of confirmed cases in the country, reflects the rarity

PURPOSE: Despite advances in medical care, bacterial meningitis still poses a considerable health issue from a global perspective. An ageing population and increasing development and use of vaccines are likely to affect the incidence and aetiology. The aim of this study was to describe the incidence and aetiology of community-acquired bacterial meningitis (CABM) in our setting, as well as the serotypes of Streptococcus pneumoniae and Neisseria meningitidis causing CABM, in relation to available vaccines.
METHOD: Patients of all ages with CABM in southern Sweden 2013-2023 were included. Patien

Streptococcus pneumoniae (pneumococcus) is an opportunistic pathogen that remains the leading cause of bacterial meningitis worldwide. For meningitis to occur, pneumococcus must breach the blood-brain barrier (BBB), a highly specialized network of brain endothelial cells that comprise the microvasculature of the brain. Here, we report the use of human induced pluripotent stem cell-derived brain-like endothelial cells (iBECs) to model the BBB during pneumococcal infection. iBECs were infected with the S. pneumoniae strain TIGR4. Adherence assays showed that pneumococcal adherence to iBECs was a

The nasopharyngeal swab is the reference test for diagnosing coronavirus disease 2019 (COVID-19) and other respiratory pathogens. Although this procedure appears to be risk free and was performed thousands of times daily during peaks of the COVID-19 pandemic, as well as during influenza and bronchiolitis outbreaks, it can be associated with minor to serious risks. These range from simple epistaxis to breaches of the meninges and skull stalks. Some breaches occur in the presence of anomalies such as congenital, traumatic or surgical encephaloceles. Neuro-meningeal breaches can be complicated by

Pneumococcal meningoencephalitis is a severe infection associated with high morbidity and mortality. Although typically community-acquired, postoperative cases following elective ENT surgery are exceedingly rare. Antimicrobial resistance (AMR) among Streptococcus pneumoniae further complicates management, and missed opportunities for vaccination represent preventable risks. We report a case of a 41-year-old man with multiple comorbidities who developed fulminant S. pneumoniae meningitis 48 h after septoturbinoplasty. The clinical course was atypical, with altered consciousness but no classical

BACKGROUND: Streptococcus pneumoniae remains a leading cause of bacterial meningitis in children, despite widespread use of pneumococcal conjugate vaccines (PCVs). This study evaluates changes in serotype distribution and antimicrobial resistance (AMR) in pediatric pneumococcal meningitis in Mexico, before and after PCV introduction.
METHODS: We conducted a prospective analysis of pneumococcal isolates collected from children aged 0-17 years diagnosed with meningitis between 1993 and 2024 through the GIVEBPVac surveillance network. Serotyping was performed using the Neufeld-Quellung reaction,

INTRODUCTION: Pneumococcal meningitis is a severe form of invasive pneumococcal disease (IPD). Despite the changes introduced by conjugate vaccines, pneumococcal meningitis remains a major clinical and public health challenge. Analyzing its current patterns allows the identification of serotypes that retain a higher invasive potential, the assessment of their relationship with clinical outcomes, and the guidance of future preventive strategies.
METHODS: Retrospective descriptive study of patients with pneumococcal meningitis admitted between 2018 and 2024. Clinical, demographic, and microbiol

BACKGROUND: Mozambique introduced the 10-valent pneumococcal conjugate vaccine (PCV10) in 2013 using a three-dose primary series with no booster dose (3p+0) and later switched to the PCV13 using a schedule of two primary doses with one booster (2p+1). We aimed to describe the burden and serotype distribution of pneumococcal meningitis in children under 5 years of age in Mozambique over an eleven-year period starting with the year of PCV10 introduction, and assess the impact of the PCV vaccine and schedule changes.
METHODS: We analysed meningitis surveillance data in Mozambique from March 2013

OBJECTIVE: Pneumococcal meningitis (PM) causes significant illness and death in children worldwide, especially in high-burden countries. This multicenter study aimed to examine the epidemiology, clinical symptoms, prognosis, strain serotyping, antibiotic resistance, and outcome-related factors in children with PM. METHODS: A prospective surveillance study on children under 18 years old with laboratory-confirmed invasive pneumococcal disease (IPD) was conducted at 19 hospitals, mainly in tertiary pediatric hospitals, from 2019 to 2021. This period was early in the post-PCV13 era in China, when

We report a 45-year-old woman with sickle cell anemia (SCA) who presented with fever, severe headache, and decreased level of consciousness. Initial brain computed tomography (CT) was unremarkable. Diagnostic lumbar puncture (LP) revealed the uncommon, frankly purulent cerebrospinal fluid (CSF) with profound neutrophilic pleocytosis, markedly elevated protein, undetectable glucose, and polymerase chain reaction (PCR)/culture confirmation of Streptococcus pneumoniae. Empiric therapy with ceftriaxone, vancomycin, dexamethasone, and acyclovir was initiated. Within 72 hours, brain magnetic resonan

Sirolimus, an inhibitor of mammalian target of rapamycin complex (mTORC) 1, a regulatory protein involved in seizures and a modulator of inflammation, represents a potential new treatment strategy for pneumococcal meningitis. This study investigates the effects of mTORC1 inhibition using sirolimus in a mouse model of experimental pneumococcal meningitis. In a prospective, investigator-blinded and randomized trial, 96 mice were infected intracisternally with Streptococcus pneumoniae serotype 2. Mice were randomized for treatment with sirolimus and all mice received antibiotic treatment. In a cl

Austrian syndrome is a rare and severe manifestation of invasive Streptococcus pneumoniae infection, defined by the triad of pneumonia, meningitis, and infective endocarditis. Although it is uncommon in the modern antibiotic and vaccination era, Austrian syndrome is associated with significant morbidity and mortality. Here we present a case of a 62-year-old male with a history of human immunodeficiency virus infection and intravenous drug use who presented with cough, fatigue, and left upper extremity weakness. He developed acute hypoxic respiratory failure and status epilepticus requiring int

Recurrent bacterial meningitis (≥2 episodes with complete recovery between episodes) warrants evaluation for skull-base defects with cerebrospinal fluid (CSF) leak and for immune deficiencies. In adults, Streptococcus pneumoniae often indicates an anatomic breach (e.g., post-traumatic encephalocele/CSF rhinorrhea), whereas Neisseria suggests complement deficiency. We report a case of a 22-year-old man with his second attack of pneumococcal meningitis within a one-year timeframe after sustaining a road traffic accident about four years ago. The patient sustained a left frontal bone fracture com

Meningitis is an inflammation of the protective membranes surrounding the brain and spinal cord, typically caused by viral or bacterial infections. Bacterial meningitis is a medical emergency requiring prompt diagnosis and treatment, which is associated with a significant mortality rate, even with optimal care. We report the case of a previously healthy 57-year-old man who presented to the emergency department with a three-week history of fever, dry cough, myalgia, fatigue, weakness, anorexia, and excessive sweating. Physical examination revealed a tympanic temperature of 38.9ºC, leukocytosis

BACKGROUND: Toxic heavy metals such as chromium (Cr), arsenic (As), cadmium (Cd), mercury (Hg), and lead (Pb) are known to be priority pollutants due to their high degrees of toxicity and widespread presence in the environment. This review aimed to explore the association between heavy metals and noncancerous thyroid diseases by synthesizing findings from observational and experimental studies. This review addressed a critical intersection of environmental health, endocrinology, and public health. The findings would be of interest to a wide range of disciplines given the ubiquitous presence of

Community-acquired bacterial meningitis has a high fatality rate, and survivors may have significant long-term neurologic sequelae, despite appropriate antibiotics. Although cerebral spinal fluid cultures and/or PCR testing are necessary to establish a definitive case of bacterial meningitis, antibiotic administration should never be delayed while waiting to obtain a lumbar puncture. Patients with suspected bacterial meningitis should be started on empiric antibiotics and adjunctive dexamethasone within an hour of presentation, with subsequent regimens targeted toward the identified pathogen.

INTRODUCTION: Lipopolysaccharide (LPS) is a major virulence factor during both meningococcal and Haemophilus influenzae meningitis. Pneumococcus does not produce LPS but could be responsible for bacterial digestive translocation as a consequence of sepsis. We addressed this question in the context of pneumococcal meningitis.
METHODS: A cross-sectional study on 24 patients with pneumococcal meningitis (20 (83%) admitted in intensive care unit, 4 (17%) with septic shock) and 34 prospectively-enrolled healthy volunteers. Interleukin 6 and C-reactive proteins plasma concentrations were measured a

BACKGROUND: Streptococcus pneumoniae is a nasopharyngeal coloniser but can also cause life-threatening invasive disease such as meningitis. The role of anti-cytokine autoantibodies as mechanism of acquired immunomodulation and immunodeficiency has recently been recognised.
METHODS: This study investigates the presence and potential role of anti-cytokine autoantibodies in central nervous system infections by analysing 14 anti-cytokine autoantibodies in the cerebrospinal fluid of 623 patients with pneumococcal meningitis, 86 with meningococcal meningitis, 56 with viral meningoencephalitis, 206
