Neurosyphilis, a severe and often neglected complication of Treponema pallidum infection, poses a significant global public health threat, particularly impacting marginalized populations such as men who have sex with men and people with human immunodeficiency virus. Despite global efforts to control syphilis, neurosyphilis is increasingly prevalent, with documented surges of >700% in some regions. Its role in driving cognitive decline often misdiagnosed as psychiatric or neurodegenerative disorders exemplifies "neglected" dimension of diseases. This narrative review synthesizes recent advances

BACKGROUND: Maintaining viral suppression in the postpartum period is critical for preventing HIV transmission to sexual partners and breastfed infants. Despite systemic viral suppression, genital HIV shedding may still occur in some women. We examined the association between bacterial vaginosis (BV) and sexually transmitted infections (STIs) and genital HIV shedding in postpartum women. METHODS: In this cross-sectional postpartum study, we hypothesised that BV and other genital tract infections would modulate HIV viral load (VL) in the genital tract. Postpartum women with HIV, aged < 25 years

BACKGROUND: Oral findings may serve as early indicators of sexually transmitted infections (STIs), yet evidence of their diagnostic relevance remains limited. In this systematic review, the authors aimed to identify and synthesize the most frequent oral manifestations and associated conditions observed in patients with STIs, emphasizing their clinical diagnostic value.
TYPES OF STUDIES REVIEWED: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, a systematic search was conducted in MEDLINE, Web of Science, Scopus, Cochrane Library, and Scientific Elec

Syphilis is a systemic infectious disease that can involve both ocular tissues and the central nervous system, with syphilitic uveitis representing one of its ocular manifestations. We report a rare case of syphilitic uveitis occurring in conjunction with chiasmal optic neuritis. A 48-year-old man presented with blurred vision in the right eye and was found to have bilateral acute syphilitic posterior placoid chorioretinitis with vitreous opacity in the left eye. Visual field testing demonstrated bitemporal hemianopia-like defects, and magnetic resonance imaging revealed enhancement of the opt

In 1925, Dr Hother McCormack Hanschell, then director of the venereal diseases clinic at the Seamen's Hospital in London, published a paper entitled 'On the influence of race in venereal disease' in Transactions of the Royal Society of Tropical Medicine and Hygiene. The aim of the study, in Hanschell's words, was to evaluate whether it was possible to 'detect any racial influence on the course of three venereal diseases (ulcus molle, gonorrhoea and syphilis) in the individual patient'. The paper presents a window into epidemiological research, discourses on colonial medicine, and biological th

Neurosyphilis remains a diagnostic challenge, particularly when it presents with cranial neuropathies that mimic more common conditions such as Bell's palsy, underscoring the need for heightened clinical awareness amid the global resurgence of syphilis, especially among men who have sex with men (MSM). We report the case of a 40-year-old, previously healthy man who presented with acute-onset left peripheral facial nerve palsy, initially treated as Bell's palsy with corticosteroids; however, subtle neurological findings and limited improvement prompted further evaluation. Serologic testing reve

INTRODUCTION: Syphilis, caused by Treponema pallidum, the "great imitator," has re-emerged as a public health concern. It presents with diverse manifestations, ranging from asymptomatic meningitis and psychiatric changes to meningovascular syphilis presenting as ischemic stroke. We aim to report a case of an elderly male with cerebral neurosyphilis mimicking an acute frontal lobe infarction, emphasizing the importance of considering syphilis in patients with atypical risk factors or clinical presentations.
CASE PRESENTATION: A 77-year-old male presented with subacute personality changes and e

PURPOSE: To investigate clinical features and identify risk factors associated with chronic syphilitic uveitis.
METHODS: A retrospective chart review was performed of patients diagnosed with syphilitic uveitis at the University of Illinois Hospital and the University of Nebraska Medical Center between January 2015 and July 2024. Patients aged 18+ with syphilitic uveitis and at least 3 post-penicillin treatment follow-ups were included. Chronic uveitis was defined as inflammation persisting longer than 3 months or recurring within 3 months post-treatment.
RESULTS: Of 74 patients with syphilit

Acquired syphilis, which is historically considered a condition with almost exclusive prevalence in adults, has shown, in recent evidence, an alarming increase amongst the pediatric population. The objective of this study is to analyze and compare the time trends and epidemiological profile of acquired syphilis in the Brazilian population from ages 0 to 19 from 2015 to 2023. It consists of a descriptive, ecological study that utilizes secondary data from the Notifiable Health Conditions Information System (SINAN). The trend analysis used Joinpoint regression to calculate the Annual Percent Cha

Neurosyphilis (NS) remains a diagnostic challenge due to its diverse clinical manifestations and its ability to mimic other neurological disorders. Despite the global decline in syphilis incidence during the antibiotic era, a recent resurgence has led to an increase in atypical and meningovascular presentations. Early diagnosis is critical, as timely treatment can prevent irreversible neurological damage. We report the case of a 38-year-old immunocompetent male who presented with acute vertigo, unsteady gait, and subsequent confusion. Initial imaging and laboratory investigations were unremark

Syphilis is an infection caused by Treponema pallidum. Without treatment, neurosyphilis may develop and lead to complications such as Charcot arthropathy (CA). First described by Jean-Martin Charcot, this condition was initially recognized as a manifestation of syphilitic infection. With the advent of antibiotics, the incidence of syphilis declined; however, it has since re-emerged in regions including Europe, North America, and China. In recent years, there has been a relative paucity of literature on syphilitic CA, with most reports involving the hip or knee. This article describes a rare pr

Simultaneous diagnosis of myasthenia gravis (MG) and early asymptomatic neurosyphilis is extremely rare. There have only been two cases described in the literature. To the best of our knowledge, this was the first case of MG resolving with concurrent treatment of neurosyphilis. Our patient was a 67-year-old male who presented to the hospital with a diffuse rash, bilateral ptosis, bilateral facial weakness, and bulbar symptoms (dysarthria, difficulty with mastication, and difficulty drinking fluids). After testing positive for syphilis in serum and cerebrospinal fluid (CSF), the patient was di

BACKGROUND: Treponema pallidum subsp. pallidum (T. pallidum), the etiological agent of syphilis, was first isolated from infected humans in the early 20th century and, for decades, could only be propagated in laboratory rabbits. In 2018, a reproducible in vitro cultivation system was established using rabbit epithelial Sf1Ep cells and a complex culture medium, allowing stable growth of T. pallidum. While in vitro cultivation has revolutionized T. pallidum research, further humanization of this system is necessary to study host–pathogen interactions. RESULTS: We evaluated three human foreskin f

The rising incidence of syphilis in recent decades underscores the need to improve diagnostic and control strategies. The infection caused by Treponema pallidum subsp. pallidum is commonly diagnosed using serological tests. However, these methods exhibit limitations in the early or late stages of disease, when antibody responses and/or bacterial loads are low. Molecular biology detection using serum samples is also hampered by low circulating bacterial loads during asymptomatic periods. Ancient DNA (aDNA) studies apply methods adapted to recovering low concentrations and degraded DNA. In this

Secondary syphilis is renowned for its ability to mimic a wide range of dermatologic and systemic diseases, including vasculitides. Misinterpretation of its cutaneous manifestations can delay appropriate therapy and expose patients to unnecessary immunosuppression. We report the case of a young female who presented with an acute, maculopapular, and annular rash initially affecting the upper body, including the back, chest, and upper limbs, followed by involvement of the lower body. It was preceded by redness and inflammation of her eyes. Initial clinical evaluation supported a diagnosis of cut

BACKGROUND: Eyebrow madarosis, or eyebrow alopecia, has significant cosmetic and psychosocial implications. Nonscarring causes are diverse, including autoimmune, infectious, endocrine, genetic, nutritional, traumatic, and iatrogenic etiologies. Despite its frequency in dermatologic practice, guidance for evaluation and management remains limited.
METHODS: A comprehensive PubMed search (1960–2025) was performed using terms related to "eyebrow alopecia," "eyebrow madarosis," and specific disease processes. English-language studies were included if they evaluated eyebrow involvement and pr

BACKGROUND: Neurosyphilis is an infectious disease of the central nervous system (CNS) caused by Treponema pallidum, which presents with a wide variety of symptoms depending on the affected CNS structures. We describe two cases of neurosyphilis with ocular motor nerve palsy (OMNP) defined as palsy involving cranial nerves (CNs) III, IV, and/or VI, and provide a systemic review of the literature on OMNP associated with neurosyphilis. METHODS: We describe two patients who presented with unilateral palsy of CN III caused by neurosyphilis. We performed a systematic literature search according to P

This essay focuses on the Vespasiano Gonzaga Colonna (1531-1591), a remarkable example of long-term life following cranial trepanation during the Renaissance. Gonzaga suffered from syphilis and frequent migraines; therefore, in 1578, his court surgeon used a scraping procedure to perform cranial trepanation. The right parietal bone had obvious indications of healing, and osteological data from his tomb validated the surgery. One of the longest reported survivors following cranial trepanation in the Italian archaeological record, Gonzaga, lived for 13 years after the treatment. This instance ad
