Keratoscleritis and Choroidal Exudative Detachment in the Setting of Ocular Pyoderma Gangrenosum.

PURPOSE: The purpose of this study is to report a rare case of ocular pyoderma gangrenosum (PG) presenting as keratoscleritis with choroidal effusion following cataract surgery and to highlight its clinical recognition and management. METHODS: A 79-year-old man developed a corneal ulcer 2 days after uncomplicated phacoemulsification with intraocular lens implantation. Initial management with topical vancomycin and ceftazidime failed. Comprehensive systemic workup including TPMT, HLA-B51, ANCA, ANA, rheumatoid factor, viral serologies, and syphilis testing was performed. Cultures for bacteria,

An Unusual Presentation of Nicolau Syndrome in the Upper Limb: A Case Report from Northern Ecuadorian Amazonia.

Nicolau syndrome, also known as embolia cutis medicamentosa, is a rare iatrogenic reaction that may occur following parenteral drug administration, including inadvertent intra-arterial or periarterial injection. Its pathophysiology remains poorly understood; however, several mechanisms have been proposed, including vasospasm, embolization, cytotoxic inflammation, and secondary tissue necrosis. We report the case of a 22-year-old transgender woman who received intravenous benzathine penicillin in the left arm without a medical prescription following a reactive syphilis screening performed outsi

Terbinafine-resistant Trichophyton mentagrophytes type VII: An emerging sexually transmitted dermatophytosis.

Sexually transmitted dermatophytosis caused by Trichophyton mentagrophytes genotype VII (TMVII) has recently emerged as a novel clinical entity, particularly within sexual networks. This pathogen is characterized by inflammatory genital lesions, diagnostic challenges, and increasing antifungal resistance. We report a case of genital dermatophytosis caused by terbinafine-resistant TMVII in a heterosexual woman following sexual exposure abroad. A 35-year-old otherwise healthy woman presented with a 3-week history of rapidly progressive, intensely pruritic genital lesions unresponsive to topical

Secondary Syphilis Presenting with Complex Ulcers in the Oral Mucosa and Pubic Area: A Case Report.

Syphilis is an infectious disease caused by Treponema pallidum, which represented as a global public health challenge. We reported a case of atypical secondary syphilis presenting with ulcers confined to the oral commissures and pubic region. Notably, the patient was initially misdiagnosed as skin infection. Given the patient's persistent ulcers on the oral commissure and mons pubis for over a month, characterized by the absence of significant pruritus or pain and unresponsive to oral antibiotics and topical medications, we recommend proceeding with the serological testing for syphilis. A rare case of early-onset cerebral syphilitic gummata with severe neurological decline in early-stage syphilis: A case report.

Cerebral syphilitic gummata are rare granulomatous lesions associated with tertiary syphilis, typically developing several years after the initial infection. Early-stage syphilis with cerebral gummata and accompanying severe neurological symptoms is rare. Failure to recognize the lesions or delays in diagnosis may lead to marked and potentially irreversible central nervous system injury. In the present study, the case of a 60-year-old man with a 6-month history of syphilis who presented with progressive right-sided limb weakness and dysarthria is reported. Brain magnetic resonance imaging reve

Overcoming medical scholasticism in New Spain: experience and indigenous knowledge in Arias de Benavides' treatment of syphilis.

This article examines the impact of non-Western medical knowledge on European medicine during the Early Modern period, with a specific focus on the medical work of Spanish surgeon Pedro Arias de Benavides in New Spain. The paper discusses Benavides' innovative approach to treating syphilis in the New World, which challenged the inefficiencies of European scholastic medicine when dealing with diseases prevalent in the Americas. It shows that Benavides´scientific contribution stems from the assimilation of indigenous healing practices and American materia medica into his medical background, ofte

Characteristics and geographical distribution of syphilis among people with human immunodeficiency virus and the National Population in Republic of Korea.

INTRODUCTION: The global incidence of syphilis has been increasing in recent years, with multiple factors contributing to this trend. Syphilis disproportionately affects specific populations. We aimed to evaluate trends in the incidence and geographical distribution of syphilis at the provincial level among people living with human immunodeficiency virus (PLWH) and the total population in Korea. METHODS: PLWH were enrolled from the Korea National Health Insurance Service Database. Early syphilis was defined as having an International Classification of Diseases, 10th revision diagnostic code a

Clinical characteristics and diagnostic challenges of otosyphilis in patients with HIV coinfection: a retrospective analysis.

BACKGROUND: The global incidence of syphilis has been rising, and atypical clinical presentations are increasingly recognized. Human immunodeficiency virus (HIV)-induced immune changes can alter syphilis progression and complicate diagnosis. Otosyphilis is a rare but potentially reversible cause of sensorineural hearing loss (SNHL). This study investigates otologic involvement in patients co-infected with syphilis and HIV. METHODS: Medical records of patients diagnosed with both syphilis and HIV infection at Etlik City Hospital between January 2023 and June 2025 were retrospectively reviewed.

Case Report: When syphilis imitates cancer: a case of misdiagnosed cutaneous T-cell lymphoma.

Syphilis is an infectious disease caused by Treponema pallidum, which is regarded as one of the most significant imitators in daily clinical practice. Its manifestations are various, comprising numerous autoimmune, inflammatory, and neoplastic disorders. Here we present a case of a 27-year-old male initially suspected of cutaneous T-cell lymphoma (CTCL) based on lymphadenopathy, systemic weight loss, ulcerative cutaneous lesions, and fluorodeoxyglucose positron emission tomography-computed tomography (FDG-PET-CT) findings of hypermetabolic nodes above and below the diaphragm. Changing epidemiology and clinical presentation of rectal lymphogranuloma venereum in the HIV-PrEP era: A comparative study from Bologna, Italy.

IntroductionLymphogranuloma venereum (LGV), caused by Chlamydia trachomatis (CT) serovars L1-L3, has re-emerged in Europe since the early 2000s, predominantly affecting men who have sex with men (MSM), particularly people living with HIV (PLWH). The widespread implementation of HIV pre-exposure prophylaxis (HIV-PrEP) and the associated expansion of sexually transmitted infection (STI) screening programmes may have influenced the epidemiology and clinical presentation of LGV. MethodsWe analysed the clinical and epidemiological characteristics of rectal LGV cases diagnosed in the Bologna metropo

Neurosyphilis and Parkinsonism: Overlapping Pathophysiology and Emerging Therapeutic Insights.

INTRODUCTION: Neurosyphilis, caused by Treponema pallidum, is a complex neurological condition arising from untreated syphilis and can present with diverse manifestations, including Parkinsonism-like motor dysfunction. Its pathophysiology involves Blood-Brain Barrier (BBB) disruption, dopaminergic neuronal loss, and neuroinflammatory cascades that resemble mechanisms seen in idiopathic Parkinson's Disease (PD). METHODS: A literature review from PubMed, ScienceDirect, Scopus, and Google Scholar (2014- 2025) explored studies on neurosyphilis, dopaminergic degeneration, BBB dysfunction, neuroinf

A Diagnostic Pitfall: Secondary Syphilis Mimicking Psoriasis in a Patient With Alcoholic Liver Disease.

Syphilis is a sexually and vertically transmitted infection caused by Treponema pallidum that remains a significant public health concern due to its rising incidence and protean clinical manifestations. The disease is characterized by distinct stages and has the potential to involve multiple organ systems, leading to serious complications if diagnosis and treatment are delayed. Secondary syphilis, in particular, is notorious for its wide spectrum of clinical presentations and its ability to closely mimic numerous dermatologic and systemic conditions, often resulting in diagnostic uncertainty.

Tabetic Arthropathy as an Uncommon Cause of Destructive Joint Disease: A Case Report.

Tabetic arthropathy is a destructive neurogenic joint disease that has become rare due to the early treatment of syphilis with penicillin G. It results from neurosyphilis caused by Treponema pallidum. The diagnosis is suspected in patients presenting with severe joint destruction disproportionate to the absence or minimal intensity of pain and is confirmed by positive syphilitic serological tests. We report the case of a 62-year-old patient with a history of high-risk sexual behavior and a remote episode of cervical lymphadenopathy treated 40 years ago. He presented with chronic painless monoa

Factors influencing implementation of point-of-care tests for maternal and newborn screening and diagnosis in low-resource settings: a systematic review.

BACKGROUND: Despite the potential of point-of-care (POC) tests to improve maternal and newborn health, implementation in low- and middle-income countries remains inconsistent. POC testing enables rapid, decentralized screening, but barriers across health system, facility, and individual levels may undermine uptake. This review synthesizes evidence on factors influencing the adoption, implementation, and sustainability of POC tests within maternal-child health programmes in low-resourced settings. METHODS: A comprehensive search was performed across major bibliographic databases and grey liter

Cytotoxic T-cell deficiency and sustained B-cell activation in serofast syphilis: a prospective observational study.

INTRODUCTION: The serofast state, defined by persistent non-treponemal reactivity after adequate therapy for syphilis, remains a clinically relevant yet poorly understood condition. Whether serofast reflects residual infection or persistent immune activation remains uncertain. We aimed to characterize peripheral blood lymphocyte subpopulations and anticardiolipin antibodies in patients with serofast syphilis and compare them with individuals achieving a serological cure. METHODS: We conducted a prospective observational study including adults with early syphilis treated with benzathine penici

Adverse Cardiovascular Outcomes in Patients With Syphilis.

IMPORTANCE: Syphilis, which is caused by Treponema pallidum, has historically been associated with cardiovascular complications. Despite a resurgence of syphilis incidence in the US, large-scale studies evaluating its independent cardiovascular impact are lacking. OBJECTIVE: To determine whether syphilis infection is independently associated with increased risk of adverse cardiovascular outcomes in a US health system population. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study with a 15-year follow-up (January 1, 2011, to July 1, 2025) was performed within a tertiary health
