Successful treatment of a patient with ventriculo-peritoneal shunt-associated meningitis caused by Streptococcus pyogenes.

Ventriculoperitoneal shunt (VPS)-associated bacterial meningitis is a significant neurosurgical complication that can occasionally present atypical symptoms. We report a rare case of Streptococcus pyogenes meningitis in a 40-day-old infant with a history of hydrocephalus managed by VPS placement. The patient presented with vomiting, diarrhea, fever, and -abdominal distension. Microbial culture of the removed VPS valve confirmed the diagnosis, revealing Streptococcus pyogenes. Despite the strain's sensitivity to penicillin G, the patient was treated with cefotaxime and vancomycin due to negativ

Meningeal inflammation and arachnoid barrier breakdown in a mouse model of neonatal bacterial meningitis.

Newborns are especially susceptible to bacterial meningitis, primarily caused by group B Streptococcus (GBS), due to incomplete maturation of immune and barrier systems. While meningitis is well known to break down the blood-brain barrier, its effect on the meningeal arachnoid barrier remains poorly understood. Using a neonatal bacterial meningitis model, we demonstrate that GBS infection significantly increases arachnoid barrier permeability, disrupts tight junctions, and elevates meningeal production of proinflammatory cytokines. CD206+/LYVE-1+ border-associated macrophages undergo morpholog

Innate immune response to Streptococcus suis infection: advances and future challenges.

Streptococcus suis is a pathogenic bacterium primarily found in pigs, where it causes sudden death and meningitis, but it is also capable of causing septic shock and meningitis in humans, being the most important cause of adult bacterial meningitis in Southeast Asia. As the host's innate immune system serves as the first line of defense against microbial invaders, it is crucial to understand the intricate relationship between the bacterium and this system. Despite an increasing number of studies performed in the last 20 years, the pathogenesis of S. suis infection is not fully understood, and

Severe strongyloidiasis and Gram-negative bacterial superinfections: epidemiology, clinical implications, and therapeutic strategies.

PURPOSE OF REVIEW: Strongyloidiasis is a neglected tropical disease affecting over 600 million people worldwide. Its severe forms carry a fatality rate exceeding 60%, yet death is rarely attributable to the parasite alone: the terminal event is often bacterial sepsis or meningitis of enteric origin. We review recent evidence on Gram-negative and other bacterial superinfections in severe strongyloidiasis, and their diagnostic and therapeutic implications. RECENT FINDINGS: Cohort studies show that bacteraemia is the commonest complication of severe disease, affecting 38.3% of cases against 4.1%

Invasive Bacterial Infections Other Than Escherichia coli and Streptococcus agalactiae in Febrile Infants Younger Than 90 Days: A Multicenter Emergency Department Study.

BACKGROUND: Invasive bacterial infections (IBIs) in febrile infants ≤90 days are mostly caused by Escherichia coli and Streptococcus agalactiae. However, data on IBIs caused by other pathogens remain limited. METHODS: We conducted a multicenter retrospective study of febrile infants aged ≤90 days diagnosed with IBI between 2008 and 2022 at 12 Spanish pediatric emergency departments. Epidemiology, clinical features, microbiology and outcomes of cases caused by pathogens other than E. coli/S. agalactiae were analyzed. Adverse outcomes included death, inotropic or respiratory support, acute comp

Case Report: Streptococcus suis-associated meningoencephalitis with profound bilateral labyrinthine deafness and intra-operative cochlear fibrosis treated by cochlear implantation.

Streptococcus suis is a common zoonotic pathogen that usually causes monomicrobial meningitis. Occult bloodstream detection of Klebsiella pneumoniae, together with proven central-nervous-system S. suis infection, followed by irreversible cochlear fibrotic changes requiring cochlear implantation, appears exceptionally rare. We report a 52-year-old male with chronic heavy alcohol intake and occupational swine-exposure history who developed severe sepsis, recurrent suppurative meningoencephalitis caused by S. suis, and permanent bilateral labyrinthine-related deafness. Combined blood and cerebros

Atypical Lyme Neuroborreliosis Presenting With Ventriculitis and Cerebritis: A Case Report.

Lyme neuroborreliosis is a neurological manifestation of infection by Borrelia burgdorferi sensu lato and most commonly presents with lymphocytic meningitis, cranial neuropathy, or painful radiculitis. Central nervous system parenchymal involvement is uncommon, and ventriculitis is rarely reported. We describe a man in his late 70s who presented with acute confusion, dysarthria, and left-sided weakness following several days of severe lower-limb pain and progressive mobility impairment. His neurological status rapidly deteriorated, with secondary seizures requiring invasive mechanical ventilat

Case Report: Fulminant meningococcus-meningitis under treatment with ravulizumab despite complete vaccination-a rare but severe complication.

OBJECTIVES: Complement C5 inhibitors such as ravulizumab are increasingly used for acetylcholine receptor-positive generalized myasthenia gravis (MG) but confer a relevant risk of invasive meningococcal infection. We report a rare case of fulminant meningococcal meningitis despite complete vaccination, highlighting the limits of current preventive strategies. METHODS: Case report. RESULTS: A 41-year-old woman with acetylcholine receptor-positive MG was treated with ravulizumab after two-dose series meningococcal vaccination (MenACWY and MenB). One month after therapy initiation, she presente

Climate change and vertically transmissible infections in maternal and neonatal populations: a systematic review.

BACKGROUND: Climate change influences infectious disease dynamics and disrupts maternal and neonatal health services, both of which are pathways through which the risk of vertically transmissible infections may increase. METHODS: We systematically reviewed evidence on the association between climate-related environmental exposures and infection-related outcomes relevant to vertical transmission in pregnant or intrapartum women, neonates, and infants. Screening, extraction, and risk-of-bias assessment were performed independently in duplicate. Owing to substantial heterogeneity, findings were

Escherichia coli ST131 isolates are rare in human meningitis but belong to a distinct O25b-fimH22 clade B lineage shared with avian pathogenic Escherichia coli.

Escherichia coli ST131 is a globally disseminated extraintestinal lineage best known for its resistance-driven expansion, yet it is rarely reported as a cause of neonatal meningitis. Whether specific ST131 sublineages retain neuroinvasive potential remains poorly understood. We analysed a 25-year nationwide French collection of 355 neonatal meningitis E. coli isolates recovered between 2000 and 2024. ST131 isolates were characterized by whole-genome sequencing, and ST131-O25b-fimH22 genomes were contextualized in a phylogeny including human-associated genomes and selected animal- and food-asso

Application of procalcitonin in pediatric emergency and critical care settings.

This expert consensus on the clinical application of procalcitonin (PCT) in pediatric emergency and critical care was convened by the Taiwan Society of Pediatric Emergency Medicine (TSPEM) and formulated by a committee of 14 leading experts. In pediatric emergency and critical care medicine, distinguishing between bacterial and viral etiologies remains a significant challenge, as clinical symptoms often lack specificity. While traditional biomarkers such as white blood cell count (WBC) and Creactive protein (CRP) provide reference values, they possess limitations regarding diagnostic timelines

Acinetobacter baumannii Nosocomial Meningitis Secondary to Neurosurgical Intervention: A Case Report.

Nosocomial meningitis is a serious complication of neurosurgical procedures, associated with high morbidity and mortality, as well as significant diagnostic and therapeutic challenges. Among the causative pathogens, Acinetobacter baumannii (A. baumannii) is particularly concerning due to its ability to survive for prolonged periods in the hospital environment, its opportunistic virulence, and, especially, its tendency to develop multidrug resistance to antimicrobial agents. Diagnosis is often difficult in the postoperative setting, particularly when the cerebrospinal fluid (CSF) is altered as

Meningococcal meningitis in patients under prolonged eculizumab therapy: Clinical and immunological observations from a case series.

Treatment with eculizumab has revolutionized the management of complement-mediated disorders such as atypical hemolytic uremic syndrome (aHUS) and paroxysmal nocturnal hemoglobinuria (PNH). However, this therapeutic benefit comes at the cost of compromising a key arm of the innate immune response, which plays a crucial role in defending against encapsulated bacteria, including Neisseria meningitidis. As a result, patients receiving eculizumab are at a markedly increased risk of developing life-threatening meningococcal infections, even in the presence of prior immunization. This study analyzes

Rare Fatal Event of Multidrug-resistant Shigella Septicemia and Cryptococcal Meningitis in an Immunocompromised Host.

Bacteriemia due to Shigella spp. is a rare event. However, an individual with impaired immunity remains at risk of such invasion where the infective dose is as low as 10 viable bacilli. This case report describes one such fatal event where a 40-year-old immunocompromised male presented to a tertiary-level healthcare facility with a history of diarrhea for 10 days and seizures. Prompt microbiological workup established multidrug-resistant Shigella flexneri septicemia and cryptococcal meningitis.

Methicillin-resistant Staphylococcus aureus meningitis following revision microdiscectomy: a case report.

Healthcare-associated bacterial meningitis is a rare but life-threatening complication of spinal surgery, most commonly associated with incidental durotomy. A 59-year-old man developed fever, altered mental status, and generalized seizures 1week after uncomplicated revision L4-L5 microdiscectomy. Magnetic resonance imaging demonstrated diffuse intracranial and spinal leptomeningeal enhancement with a postoperative lumbar fluid collection. Cerebrospinal fluid (CSF) analysis was consistent with bacterial meningitis. Emergent irrigation and debridement revealed copious purulent fluid without evid
