Virulence arsenal of Acinetobacter baumannii: mechanisms driving persistence and resistance.

Acinetobacter baumannii is a Gram-negative opportunistic pathogen and a major cause of healthcare-associated infections, including ventilator-associated pneumonia, bacteraemia, meningitis, and urinary tract infections. Its persistence in hospital environments is due to its ability to survive desiccation, resist disinfectants, and colonize both biotic and abiotic surfaces. Virulence in A. baumannii is largely associated with structures such as Csu pili, biofilm-associated protein (Bap), and outer membrane protein A (OmpA), which enable surface attachment, biofilm formation, and host cell damage

Achromobacter xylosoxidans isolates exhibit genome diversity, variable virulence, high levels of antibiotic resistance, and potential intrahost evolution.

Achromobacter xylosoxidans is an emerging pathogen characterized by high levels of antibiotic resistance (AR) and increasing infection rates worldwide. This motile, opportunistic pathogen is widely distributed in the environment and can cause various infections, including pneumonia, bacteremia, endocarditis, meningitis, and others. In this study, we analyzed the population structure, AR profiles, and virulence factors of over 200 publicly available genomes. Core genome analysis revealed that A. xylosoxidans is highly adaptable, possessing a relatively small core genome. Antibiotic susceptibili

Antibiotics modulate Escherichia coli-derived bacterial extracellular vesicle production and their upregulation of ICAM-1 in human endothelial cells.

Antibiotic treatment is often necessary to eliminate life-threatening bacterial infections. However, these treatments can alter production of bacterial extracellular vesicles (BEVs), which often contain pro-inflammatory biomolecules. In this study, we examined how the clinically relevant antibiotics meropenem, tobramycin, and ciprofloxacin impacted BEV production from a urinary tract infection-associated Escherichia coli strain (CFT073 [WAM2267]) and a meningitis-associated strain (K1 RS218). BEVs from both strains caused a dose-dependent increase in expression of intercellular adhesion molecu

Listeria monocytogenes Meningitis in an Intensive Care Unit.

Community-acquired bacterial meningitis remains one of the most serious and life-threatening infectious diseases, with Listeria monocytogenes representing the third most common causative agent. Listeria meningitis occurs more frequently in immunocompromised and older adults and often requires intensive care unit (ICU) admission, depending on disease severity. We conducted a descriptive, retrospective study in an ICU that included adult patients admitted with community-acquired L. monocytogenes meningitis. Clinical features, risk factors, and management of these patients were analyzed. Over nin

Whole blood transcriptional responses associated with bacterial burden in pulmonary tuberculosis.

BACKGROUND: Pulmonary TB (PTB) patients present with a wide range of pre-treatment Mycobacterium tuberculosis ( Mtb ) burdens, which predict poor treatment outcomes. We sought to identify immune pathways and biomarkers associated with pre-treatment Mtb burden. METHODS: We conducted whole-blood RNA sequencing in 295 Vietnamese adults with PTB, quantifying bacterial load using GeneXpert Ct values. Weighted gene co-expression network analysis (WGCNA) identified gene modules, pathways, and hub genes associated with Mtb burden. Deconvolution analysis assessed contributions of immune cell types. NeuO-mediated O-acetylation of uropathogenic Escherichia coli K1 capsule enhances resistance to phage and neutrophil killing.

Uropathogenic Escherichia coli (UPEC) strains that express the K1 capsule are associated with severe invasive disease, including pyelonephritis, urosepsis, and neonatal meningitis. The K1 capsule can be modified by NeuO, a phage-encoded phase-variable O-acetyltransferase. The role of O-acetylation in pathogenesis of K1 UPEC remains to be fully elucidated. Here, we assessed the prevalence of the neuO gene in a K1 E. coli data set comprising 8,659 genomes and observed that 43.5% of genomes harbor neuO, with a high prevalence (88.1%) in the pandemic UPEC clone sequence type (ST) 95. We generated

An Unusual Presentation of Spinal Tuberculosis Masquerading as Pyogenic Meningitis: A Case Report.

BACKGROUND: Spinal tuberculosis (TB), though accounting for up to 50% of skeletal TB cases, typically presents insidiously with back pain, kyphotic deformity, or neurological deficits. Atypical presentations, including initial neutrophilic pleocytosis in cerebrospinal fluid (CSF), can mimic bacterial meningitis and complicate diagnosis. CASE PRESENTATION: We report the case of a 46-year-old previously healthy male who initially presented with high-grade fever, headache, neck stiffness, and neutrophilic CSF pleocytosis suggestive of bacterial meningitis. Despite empirical antibiotic therapy, t

Management of Young Infants With Incomplete Evaluation for Meningitis.

Evaluation of neonates and young infants with fever is a common problem in pediatrics. Lumbar puncture is frequently indicated, but results are often difficult to interpret due to dry tap, blood contamination, and/or antibiotic pretreatment. Pediatric infectious diseases clinicians are often called upon to determine the risk of meningitis requiring definitive treatment in the absence of optimal data. Pediatric clinicians can use clinical data, such as age, physical exam, and lab values to estimate the pre-test probability of meningitis and may be able to use ancillary testing, such as imaging

Duration of Antibiotic Therapy for Escherichia coli Bacteremia in Infants Without Cerebrospinal Fluid Evaluation.

OBJECTIVE: Escherichia coli is a leading cause of Gram-negative rod bacteremia in infants. Cerebrospinal fluid (CSF) evaluation is often deferred, and guidance on antibiotic duration is lacking in these circumstances. We evaluated whether treatment duration is associated with infection recurrence or mortality. METHODS: This cohort study included infants discharged from Pediatrix Medical Group neonatal intensive care units (NICUs) from 1997 to 2022 with first instance of E coli bacteremia. Infants with CSF evaluation, delayed antimicrobial initiation (>2 days), or death/discharge during therap

Temporal patterns and risk factors of diarrheal comorbidity among children aged < 5 years in rural western Kenya: Evidence from three consecutive enteric studies, 2008-2024.

Sub-Saharan Africa bears the highest burden of diarrhea, often complicated by comorbidities that delay diagnosis, hinder treatment, and worsen outcomes. As the epidemiology of diarrheal disease evolves, understanding comorbidity patterns is critical for effective public health responses. We examined the temporal patterns and risk factors of diarrheal comorbidity in Kenyan children aged < 5. We conducted secondary pooled analysis with a retrospective cohort design leveraging data from the Global Enteric Multicenter Study (GEMS, 2008-2012), the Vaccine Impact on Diarrhea in Africa (VIDA, 2015-20

Pathogenic Role of β-Endo-N-Acetylglucosaminidase (LytB) in Streptococcus pneumoniae: Functional Insights Into a Choline-Binding Cell Wall Hydrolase.

Streptococcus pneumoniae, which imposes the highest burden on children under 2 years of age, immunocompromised individuals and the elderly in developing countries, causes a range of diseases including pneumonia, meningitis, bacteraemia and septicaemia. Its virulence is driven by an array of surface proteins categorised into four families: non-classical surface proteins, LPXTG-sequence proteins, lipoproteins and choline-binding proteins. The different choline-binding proteins include LytA, LytB, LytC, PspA, CbpA, CbpD, CbpL and CbpF, each with varying numbers of choline-binding repeats in their

DNA-Templated Silver Nanoclusters Demonstrate Potent Antimicrobial Activity Against the Clinically Relevant Pathogens, Neisseria meningitidis and Streptococcus pneumoniae.

Antimicrobial resistance is growing among the causative agents of bacterial meningitis, Neisseria meningitidis and Streptococcus pneumoniae, which trigger detrimental neuroinflammatory responses within the central nervous system. Here, we evaluated the antimicrobial potential of DNA-templated and stabilized silver nanoclusters (AgNCs). AgNCs were templated on a single hairpin (HP) or fibrous hairpin structures (HP-F). HP-F provided higher local concentrations of AgNCs, when compared to HP, and exhibited stable physicochemical properties and potent antimicrobial activity. Furthermore, at low si
