BACKGROUND: The clinical features of India ink-negative and culture-negative cryptococcal meningitis (CM) in HIV-negative patients remain unclear. METHODS: A total of 395 antifungal-naïve HIV-negative CM patients were enrolled in this retrospective study. The 90-day poor outcome and associated risk factors were compared between patients with positive and negative India ink, and those with positive and negative culture in cerebrospinal fluid (CSF). RESULTS: India ink-negative patients had a high level of glucose [mmol/L; 2.6 ± 1.5 vs. 2.1 ± 1.4, P = 0.005], total protein level [g/L; 1.1 (0.7–1.

Antifungal resistance is considered a global health threat. However, enzymatic inactivation of antifungals, a common mechanism seen in antibacterial resistance, has not yet been described in fungi. From a One Health perspective, this study demonstrates that Cryptococcus deuterogattii and C. neoformans, the leading agents of cryptococcosis, enzymatically inactivate fluconazole. Agrochemicals induce the overexpression of genes that code for acetyltransferases, specifically GCN5 and NAT10. These enzymes catalyze the acetylation of fluconazole into O-acetyl-fluconazole. This metabolite is unable t

We evaluated the in vivo activity of EL219 against central nervous system coccidioidomycosis. Mice were inoculated intracranially with arthroconidia of Coccidioides immitis, and treatment with EL219 (5, 10, or 20 mg/kg QD by intraperitoneal injection) or fluconazole (25 mg/kg orally BID) began 2 days later. Each dose of EL219 and fluconazole significantly improved survival. Brain fungal burden was also reduced compared to vehicle control. Further studies of EL219 against coccidioidomycosis are warranted.

BACKGROUND: Arthrographis spp. are environmental fungi commonly found in soil and compost. Infections caused by Arthrographis species remain an uncommon clinical occurrence. Although these infections are infrequent in the general population, their incidence appears to be elevated among immunocompromised patients or those with significant comorbidities.
OBJECTIVES: This review seeks to examine all documented human cases of Arthrographis spp. infections, with particular focus on aspects such as epidemiology, microbiological features, resistance patterns, therapeutic approaches and associated mo

Coccidioidomycosis, a disease induced by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to the southwestern United States and occurs with a myriad of clinical and imaging manifestations, which range from limited pulmonary disease to disseminated multiorgan infections. Humans are infected when arthroconidia in the soil are disturbed and dispersed into the air. Once inhaled, arthroconidia form spherules, which trigger a host cell-mediated immune response. If this response is adequate, it results in granuloma formation, halting the infection. However, an insuffici

UNLABELLED: Cryptococcus neoformans, the etiological agent of cryptococcal meningitis (CM) is a globally distributed environmental yeast that mainly causes infections in immunocompromised individuals. Particularly in low-resource countries, the mortality rate of CM can reach 81% and accounts for ~19% of HIV/AIDS-related deaths each year. In immunocompromised individuals, once inhaled, C. neoformans escapes from the lungs and disseminates with special predilection for the central nervous system (CNS). Once in the brain, C. neoformans interacts with microglia, the tissue-resident macrophages of

Cryptococcal meningitis remains the top cause of death from an invasive fungal infection in humans, responsible for ~100,000 deaths annually of vulnerable patients with underlying immune deficiencies. Animal models of cryptococcal meningitis are important for understanding the immune parameters that correlate with protection. However, modeling this infection in mice is challenging. There is wide variability in infection routes, doses, and mouse background used in the field, which makes understanding phenotypes of mutants and immune interventions difficult to broadly apply. Our intention was to

A 43-year-old man with myelodysplastic syndrome underwent umbilical cord blood transplantation (CBT). On day 4 post-transplantation, he developed right periorbital pain and oculomotor nerve palsy. Brain MRI revealed fluid accumulation in the sphenoid sinus and mucosal thickening in the right maxillary sinus. Given the extraordinarily early onset and neurological involvement, fungal meningitis, including mucormycosis, was suspected, and empirical high-dose liposomal amphotericin B (10 mg/kg/day) was initiated. Nasal discharge culture yielded Rhizomucor species, later identified as Rhizomucor mi

Cryptococcus neoformans (Cn) is the main cause of fungal meningitis in people living with HIV. Perturbations in normal immunoglobulin (Ig) levels are observed in these individuals, but their association with Cn pathogenesis is unclear. Here, we investigated the physical and biological effects of normal (not elicited by known cryptococcal infection) human immunoglobulins (Igs), IgM, IgG, and IgA on Cn (strain H99). Each isotype affected the growth, surface morphology, and proteome of Cn. However, IgA had the most prominent effect. It induced growth inhibition after 24 h of co-culture with Cn, a

As a globally distributed opportunistic fungus, Cryptococcus neoformans poses a significant threat to human health by causing fatal meningitis, which is associated with high mortality and morbidity rates. Current antifungals-polyenes, flucytosine, and azoles-are limited by toxicity and resistance, underscoring the need for new agents. This study evaluated dimercaptosuccinic acid (DMSA), a chelating agent used in the treatment of heavy metal poisoning, beginning with its effect on the virulence factor laccase. DMSA showed broad-spectrum activity against standard (H99, B3501) and clinical isolat

Cryptococcus neoformans meningitis (CM), a severe AIDS-defining illness with high mortality, remains refractory to effective therapies. This study investigated the mechanisms underlying Cryptococcus neoformans (Cn)-induced disruption of the blood-brain barrier (BBB). Using in vitro models of cerebral microvascular endothelial cells, we demonstrated that Cn disrupted the BBB via adhesion and invasion, leading to pyroptosis in these cells. Furthermore, our findings revealed that Cn induced upregulation of the α7 nicotinic acetylcholine receptor (α7nAChR). Importantly, inhibition of α7nAChR allev

We report a rare and severe case of drug reaction with eosinophilia and systemic symptoms (DRESS) in a 72-year-old woman, complicated by hemophagocytic lymphohistiocytosis (HLH) and cryptococcal meningitis. The patient initially presented with fever, rash, eosinophilia, and liver dysfunction following antibiotic exposure. Despite corticosteroid therapy, she developed progressive cytopenias and neurological symptoms. Further evaluation confirmed HLH and cryptococcal infection. Treatment included intravenous immunoglobulin, antifungal agents, and multidisciplinary supportive care, resulting in g

BACKGROUND: In humans, Cryptococcus neoformans and Cryptococcus gattii species complexes are the leading cause of fungal meningitis globally. To establish CNS infection, Cryptococcus must breach the BBB, primarily comprised of specialized brain microvascular endothelial cells (BMECs), which is the prerequisite for cryptococci to invade the brain. Despite its clinical impact, the mechanisms underlying host-pathogen interaction at the BBB particularly involving environmental isolates remain under-characterized.
OBJECTIVE: This study aimed to investigate the cyto-morphological and transcriptomic

Cryptococcus is a deadly opportunistic pathogenic fungus that causes severe infections such as meningitis in immunocompromised patients. Current antifungal therapeutics face the problem of drug resistance and the limitation of high side effects, and there is an urgent need to develop new therapeutic strategies. The growth and pathogenicity of Cryptococcus depend on its complex metabolic network, and by interfering with the metabolic pathways of Cryptococcus, its virulence can be effectively reduced and the immune response of the host enhanced. This paper systematically summarizes the potential

Amphotericin B (AmB) is a broad-spectrum antifungal drug, but its use in the treatment of fungal infections in the central nervous system (CNS) has been limited by the constraints of the blood-brain barrier (BBB) and the severe toxicity of systemic administration. Nose-to-brain administration, as a non-invasive strategy, may bypass the BBB and deliver drugs to the brain via the olfactory or trigeminal pathways, increasing intracerebral drug concentrations while reducing systemic side effects. Therefore, this study constructed a nanocomplex carrier based on HS15, lecithin and cholesterol (HS15-

BACKGROUND: Cryptococcus neoformans, the etiologic agent of cryptococcosis, can survive and replicate within host immune cells, contributing to development of cryptococcal meningitis. Within the lung, C neoformans interacts with innate immune subsets, including macrophages and dendritic cells (DCs). Our previous work demonstrated that Ly6c- monocyte-like macrophages restrict fungal proliferation, whereas CD11b+ DCs promote fungal growth. Transcriptomic profiling revealed differential expression of MHC-I gene H2-K1, calreticulin, and metabolic genes. Therefore, we hypothesized that H2-K1 and me

Cryptococcal meningitis is a fatal complication. Macrophages have been proposed to function as candidate "Trojan horse" cells, transferring Cryptococcus neoformans (C. neoformans) into the brain. The mechanisms of Trojan horses in cryptococcal meningitis are largely elusive. In this study, we performed scRNA-Seq on immune cells infiltrating the brain in a murine model of cryptococcal meningitis. Bioinformatics analysis revealed that phosphodiesterase 4B (PDE4B) is a candidate regulator associated with C. neoformans infected-macrophage. C. neoformans increases the total level of PDE4B in macrop

PURPOSE: Candida meningitis is a rare but potentially deadly complication in neurosurgical patients. Most of the published case series include patients who underwent a craniotomy, while just three cases have been reported in patients operated on by transsphenoidal surgery.
METHODS: We performed an institutional review of patients who underwent transsphenoidal surgery over the last 5 years (2020-2024) at Karolinska University Hospital, Stockholm, Sweden, and searched for patients who were also diagnosed with Candida meningitis as a postoperative complication.
RESULTS: Out of over 400 operated

Cryptococcal meningitis (CM) is often approached under the assumption that Cryptococcus neoformans strains from any environment have equal potential for rapid central nervous system (CNS) invasion. This one-size-fits-all view shapes both treatment strategies and how virulence is studied. In this short communicationcont, we highlight increasing evidence showing that environmental nutrient profiles can "pre-condition" C. neoformans for distinct pathogenic trajectories. For example, phosphate-rich pigeon guano often yields small-capsule morphotypes suited for systemic dissemination, but their pro

Cryptococcus neoformans causes serious infections of the central nervous system, including meningitis, encephalitis, or cryptococcomas, and is increasingly reported in immunocompetent individuals. Cryptococcal choroid plexitis is a rare entity that often presents with ependymitis and synechiae formation, resulting in "trapped ventricles." Cryptococcomas forming gelatinous pseudocysts manifest as prominent Virchow-Robin spaces and lead to false-negative smear and culture reports, leading to a delay in diagnosis. We report a 37-year-old woman from Assam with chronic headache for 3 months, recurr
