Lipid-based amphotericin B formulations (ABCD/L-AmB) were associated with better outcomes than deoxycholate (AmB-d) in non-HIV-associated cryptococcal meningitis: A retrospective cohort study.

This retrospective cohort study compared the efficacy and safety of lipid-based amphotericin B formulations (amphotericin B colloidal dispersion [ABCD] and liposomal amphotericin B [L-AmB]) versus conventional amphotericin B deoxycholate (AmB-d) for the treatment of non-human immunodeficiency virus (HIV)-associated cryptococcal meningitis (CM). Data from 199 patients were analyzed, divided into three groups based on induction therapy: AmB-d (n = 110), ABCD (n = 44), and L-AmB (n = 45). At baseline, a higher proportion of patients in the lipid-based groups presented with altered mental status.

Hyponatremia as a "red flag" for concurrent meningitis in pulmonary cryptococcosis: a risk stratification model.

BACKGROUND: Pulmonary cryptococcosis (PC) is increasingly diagnosed in human immunodeficiency virus (HIV)-negative and apparently immunocompetent individuals. A critical complication of PC is dissemination to the central nervous system (CNS), leading to concurrent cryptococcal meningitis (CM). Identifying CNS involvement remains a clinical challenge due to insidious presentations and non-specific radiological findings. This study aimed to identify independent clinical and laboratory predictors of concurrent CM in patients presenting with PC. METHODS: We conducted a retrospective study of patie

Esculetin exhibits antifungal effects against Cryptococcus neoformans by inhibiting titan cell formation.

Cryptococcus neoformans is responsible for pulmonary cryptococcosis and life-threatening cryptococcal meningitis, particularly in immunocompromised individuals. Despite its high mortality rate and the diagnostic challenges, current antifungal therapies are hampered by limitations such as side effects and the emergence of drug resistance. A critical virulence factor of C. neoformans is its ability to form titan cells, enlarged polyploid cells that promote immune evasion and dissemination. However, therapeutic interventions specifically targeting titan cell formation remain unexplored. In this s

Pulmonary cryptococcosis in routine care: the clinical-radiologic spectrum and transparent evidence-tier reporting (confirmed versus presumed) among patients with no recorded immunocompromising conditions.

Introduction. Pulmonary cryptococcosis is increasingly identified in routine care, including among patients with no recorded immunocompromising conditions.Hypothesis/Gap Statement. Real-world descriptions of pulmonary cryptococcosis are difficult to interpret because diagnostic intensity and case definitions vary across studies, and many cohorts reflect pathway-based rather than population-based case capture.Aim. This study aimed to describe the clinical presentation, including asymptomatic/incidental cases, and chest computed tomography (CT) patterns of pulmonary cryptococcosis by two-level i

Cryptococcal meningitis in an immunocompetent host: acute bilateral sensorineural hearing loss with normal intracranial pressure.

BACKGROUND AND PURPOSE: Cryptococcal meningitis typically affects immunocompromised hosts and presents with headache and fever. Sensorineural hearing loss (SNHL) occurs in up to 30% of cases but is traditionally associated with elevated intracranial pressure. CASE PRESENTATION: A 59-year-old immunocompetent man presented with rapidly progressive bilateral SNHL, dizziness, and gait instability over three weeks. Initial brain MRI was unremarkable, leading to a preliminary diagnosis of idiopathic sudden sensorineural hearing loss. Despite steroid treatment, he deteriorated. Repeat MRI revealed sy

The application of treatment response biomarkers in clinical trials for cryptococcosis, invasive aspergillosis and candidiasis.

Just three classes of antifungals are available for the treatment of WHO critical priority fungal pathogens Cryptococcus, Candida and Aspergillus spp. Drug development and optimization via phase II and III clinical trials is reliant on binary composite endpoints (clinical/radiological/mycological), requiring large patient cohorts with challenging logistics at great expense. The validation and implementation of continuous mycological treatment response biomarkers have the potential to facilitate smaller investigator-led trials to optimize existing and novel drugs and combinations, as demonstrat

Discovery of New Pyrazolone Derivatives for Treating Invasive Fungal Infections by Iron Homeostasis Disruption.

Invasive fungal infections caused by Cryptococcus neoformans and Candida glabrata pose a severe threat to global public health due to high mortality, urgently requiring the development of antifungal agents with novel mechanisms. Herein, a series of new pyrazolone derivatives were designed and evaluated. In particular, compound D2 exhibited subnanomolar antifungal activity against Cryptococcus neoformans and Candida glabrata strains (MIC range: 0.00012-0.0078 μg/mL). Mechanistically, compound D2 disrupted fungal iron homeostasis, impaired mitochondrial structure and function, and triggered oxid

[Immune reconstitution inflammatory syndrome associated with invasive pulmonary fungal disease].

Immune reconstitution inflammatory syndrome (IRIS) refers to an exaggerated inflammatory response to existing infections or latent pathogens that occurs during rapid immune recovery in immunocompromised hosts, paradoxically leading to clinical deterioration despite appropriate treatment. IRIS is commonly observed in patients with human immunodeficiency virus (HIV)-associated tuberculosis and cryptococcal meningitis after starting antiretroviral therapy. However, it can also occur in immunocompromised individuals without HIV, which remains relatively under-recognized, particularly following neu

[Cryptococcosis].

Cryptococcosis is an infectious disease caused by encapsulated fungi of the genus Cryptococcus. It predominantly occurs in individuals with impaired immune function, such as those with human immunodeficiency virus (HIV) infection, malignancies, or those having undergone organ transplantation or immunosuppressive therapy. However, it can also occur in healthy individuals. Cryptococcus primarily infects the lungs and central nervous system but can also affect the skin, prostate, peritoneum, and eyes. In this review, we provide an overview of cryptococcosis, with a particular focus on cryptococca

[Candidiasis].

CNS candidiasis typically occurs in patients with candidemia, especially immunocompromised hosts or patients with neurosurgical CNS devices. The gold standard for diagnosis is identification of Candida growth in CSF culture. For initial treatment, liposomal amphotericin B with or without flucytosine is recommended. If the isolate is susceptible to initial treatment, fluconazole is added. When possible, CNS devices should be removed. Autosomal recessive CARD9 deficiency is a primary immunodeficiency disorder associated with chronic mucocutaneous or invasive candidiasis, especially drug-resistan

Pyogranulomatous dermatitis associated with Sporobolomyces roseus in a dog.

BACKGROUND: Sporobolomyces spp. is a yeast-like fungal organism commonly found in the environment. It has been sporadically reported in human medicine as a potential cause of dermatitis, lymphadenitis, meningitis, and endophthalmitis. In veterinary medicine, this fungus has only been described once in a dog affected with granulomatous meningoencephalitis. CASE PRESENTATION: A 3-year-old Jagd Terrier was presented with crusted, erosive and ulcerative skin lesions primarily affecting the nose, but also the limbs and tail. A pyogranulomatous dermatitis was identified on skin histopathological se

Venous sinus stenting worsening in fulminant intracranial hypertension secondary to Cryptococcus gattii: lessons learnt.

Idiopathic intracranial hypertension (IIH) is a diagnosis of exclusion, and venous sinus stenosis may coexist with IIH, making careful evaluation for secondary causes essential. We report an immunocompetent man in late 30s who presented with headache and visual blurring. Brain MRI demonstrated features of raised intracranial pressure with transverse sinus stenosis. Lumbar puncture revealed high opening pressure and IIH was presumed. The patient underwent left transverse-sigmoid sinus stenting with transient improvement, followed by recurrent headache and rapid visual deterioration. Repeat angi

Hidden battles: dissecting host-fungal interactions and immune heterogeneity in infection outcomes.

Fungal infections remain a major global human health problem, responsible for billions of infections and millions of deaths each year. Dissecting the immune mechanisms that protect us against these infections is an important step toward developing alternative therapies for patients at risk for fungal diseases, who are typically immunocompromised. Understanding antifungal immune responses is complicated by heterogeneity within host cell populations, as well as in-host adaptation strategies employed by fungi, which may also drive multiple distinct populations during infection. In this article, I

Advances in cryptococcal infections-associated immunopathology and potential therapeutic strategies.

Globally, the Cryptococcus species, particularly Cryptococcus neoformans and Cryptococcus gattii, have a significant impact on human health. Concurrent with the evolving epidemiology of cryptococcosis, it has now become evident how host immunity contributes to the pathophysiology of severe disease. The only pharmacological antifungal therapeutic agents available are 5-flucytosine, fluconazole, and amphotericin B. Echinocandins, the newest fungicidal class for invasive mycoses, are ineffective against cryptococcosis caused by C. neoformans. Antifungal treatment is hampered by the high toxicity,

Patient and caregiver experiences of barriers to longitudinal care in coccidioidal meningitis in California's central valley: a qualitative study.

BACKGROUND: Coccidioidomycosis (Valley fever) is an endemic fungal infection of the southwestern United States, with an estimated 273,000 symptomatic infections, 23,000 hospitalizations, and 900 deaths annually. Coccidioidal meningitis, the most severe manifestation, requires lifelong antifungal therapy and carries high rates of rehospitalization, nonadherence, and mortality. Quantitative studies have documented poor outcomes, but the barriers driving these outcomes have not been examined from the patient's perspective. This study aimed to characterize structural and systemic barriers to longi

New Perspectives on Antimicrobial Agents: Fosmanogepix.

Invasive fungal infections (IFIs) are associated with high mortality rates and pose important therapeutic challenges because of the limited number of antifungal drug classes, their toxicity, and the emergence of resistance. Novel first-in-class antifungal agents have been recently approved (ibrexafungerp) or are currently being tested in clinical trials and are available via expanded access programs (olorofim, fosmanogepix). Among them, fosmanogepix (a prodrug of manogepix) represents a promising future therapeutic option for many IFIs because of its broad-spectrum antifungal activity and its

Low-intensity pulsed ultrasound combined with microbubbles enhances amphotericin B delivery across the blood-brain barrier for improved therapy of cryptococcal meningitis.

Cryptococcal meningitis (CM), a life-threatening fungal infection of the central nervous system, faces therapeutic challenges due to poor blood‒brain barrier (BBB) penetration of amphotericin B. This study investigates the efficacy of low-intensity pulsed ultrasound (LIPUS) combined with microbubbles in enhancing BBB permeability and improving prognosis in a murine CM model. Our findings suggest that LIPUS combined with microbubbles treatment significantly increased BBB permeability, leading to elevated cerebral concentrations of amphotericin B and improved fungicidal activity against Cryptoco
