Disseminated Coccidioides posadasii Infection in Pregnancy Complicated by Central Nervous System Disease: A Case Report and Review.

Coccidioidomycosis is typically a self-limited pulmonary infection but may cause severe disseminated disease in pregnancy. Central nervous system involvement is the most serious manifestation and can be diagnostically challenging due to nonspecific symptoms. We report a case of disseminated Coccidioides posadasii infection in a pregnant patient presenting with persistent headache subsequently complicated with coccidioidal meningitis and hydrocephalus requiring neurosurgical intervention. This case highlights the importance of maintaining suspicion for endemic fungal infections in pregnant pati

Virulence factors of Cryptococcus neoformans and Cryptococcus gattii: two sides of the same coin.

Cryptococcus neoformans and Cryptococcus gattii are a globally distributed fungal pathogen that causes a significant number of cryptococcosis worldwide, especially among immunocompromised individuals, although immunocompetent may also be affected. This infection begins when Cryptococcus is inhaled from the environment into the lungs resulting in pneumonia. Critically, the fungus can disseminate systemically and cross the blood-brain barrier causing cryptococcal meningitis (CM), the severe clinical form of the disease. CM, when untreated, is fatal in a short period of time causing high mortalit

Discovery of Novel Ligands for the Treatment of Cryptococcus neoformans Infection.

Fungal pathogens are an escalating global public health concern, particularly in the context of invasive and opportunistic infections. Cryptococcosis, primarily caused by Cryptococcus neoformans var. grubii, can affect multiple organs and often leads to life-threatening meningitis in immunocompromised individuals. Given limited antifungal therapies and emergence of resistance and toxicity-related constraints, the development of novel anti-cryptococcal agents remains an urgent priority. A library of innovative 3-hydroxypyridin-4(1H)-one-based hybrids (5a-f) was synthesized and evaluated for ant

Fusariosis in Mexico: cases and gaps to be addressed.

This mini-review synthesizes four decades of Mexican fusariosis literature, organizing cases into four clinical categories: ocular, superficial/cutaneous, invasive/disseminated, and central nervous system (CNS) disease. It identified significant gaps in national surveillance, molecular diagnostics, antifungal susceptibility testing, access to effective therapies, specialized mycology infrastructure, and highlights the urgent need for a coordinated clinical mycology network in Mexico. Fusariosis, caused by Fusarium species, presents a broad clinical spectrum, from keratitis and onychomycosis in

Cryptococcal Infection Across Disease Stages: Host-Pathogen Interactions, Tissue Niches, and Translational Priorities.

Cryptococcosis often begins in the lungs and may later reach the central nervous system, where it can lead to severe or fatal disease. The course of infection is shaped by changing interactions between host cells, fungal adaptive states, and local tissue environments. Many virulence factors and immune responses have been studied in detail, but the evidence remains divided across organs, cell types, and experimental systems. As a result, pulmonary infection is better understood than later stages, including dissemination, crossing of the blood-brain barrier, CNS involvement, and long-term persis

Epidemiology and Outcomes of Extrapulmonary Invasive Fungal Infections After Kidney Transplantation in Northern India.

BACKGROUND: Extrapulmonary invasive fungal infections (IFIs) in kidney transplant recipients (KTRs) are understudied, particularly in endemic, resource-limited settings. This study evaluated the epidemiology, risk factors, and outcomes of extrapulmonary IFIs in KTRs in northern India. METHODS: In this nested case-control study of 1649 KTRs (2007-2022), microbiologically proven extrapulmonary IFI cases were identified from microbiology and histopathology databases. RESULTS: Extrapulmonary IFI occurred in 67 KTRs (4.1%). Cryptococcosis (46.3%) and candidiasis (20.9%) were the predominant infec

Macrophage phagocytosis of Coccidioides promotes its differentiation into the parasitic form.

UNLABELLED: Coccidioides is an endemic fungus that is increasing in prevalence and causes life-threatening diseases in immunocompetent people. In the environment, the spores (arthroconidia) develop into hyphae; however, when they are inhaled by a mammalian host, they develop into a unique form called the spherule. The transition to spherule can be triggered in vitro with elevated temperatures and high CO2 levels, but how the host triggers Coccidioides spherulation is not known. We used live imaging to investigate how macrophages affect the fate of Coccidioides arthroconidia. Under tissue cultu

An analysis of national guidelines for the prevention, diagnosis and treatment of cryptococcal meningitis in sub-Saharan African countries: gaps and opportunities.

INTRODUCTION: Cryptococcal meningitis (CM) is an infection of the central nervous system that accounts for approximately 19% of AIDS-related mortality worldwide, with the highest burden being in sub-Saharan Africa (SSA). In 2022, the WHO released clinical guidelines on the prevention, diagnosis and treatment of CM, based on strong evidence-based studies. It is not known how these WHO guidelines are being integrated at the national level. This study aims to highlight national clinical guidelines for the prevention, diagnosis and management of CM across SSA and how these guidelines compare to WH

Pathophysiology of intracranial hypertension in cryptococcal meningoencephalitis.

Cryptococcal meningoencephalitis (CME) is a major cause of death and disability, and intracranial hypertension is a leading, treatable contributor to mortality and neurologic sequelae. Across CME cohorts, markedly elevated cerebrospinal fluid (CSF) opening pressure is common and often occurs despite minimal ventriculomegaly or diffuse edema on neuroimaging. This review synthesizes clinical, microbiological, imaging, pathological, and experimental evidence to define priorities for mechanistic research. Intracranial pressure (ICP) physiology predicts that once intracranial compliance is exhauste

Fulminant cryptococcal meningoencephalitis with adrenal involvement in an immunocompetent patient: a clinicopathologic correlation.

Cryptococcus meningoencephalitis (CM) is the most frequently encountered manifestation of cryptococcosis. We report the case of a previously healthy, human immunodeficiency virus (HIV)-negative 65-year-old man who presented with 3 days of severe holocranial headache followed by sudden psychomotor agitation, confusion, and disorganized speech, without improvement after midazolam, requiring endotracheal intubation. Initial neuroimaging suggested inactive neurocysticercosis. Early cerebrospinal fluid (CSF) analysis showed lymphocytic pleocytosis with negative India ink but positive cryptococcal a

Update on the diagnosis, pathophysiology and treatment strategies in non-HIV-associated cryptococcal meningoencephalitis.

INTRODUCTION: Cryptococcal meningoencephalitis (CM) is the leading cause of non-viral meningitis in the United States, reflecting both the rise in immunocompromised populations and the prevention of bacterial meningitis through vaccination. Formerly linked primarily to HIV-associated immunodeficiency, effective antiretroviral therapy has reshaped CM epidemiology: solid-organ transplant recipients and apparently healthy individuals now constitute the main risk groups, with mortality remaining 30-40%. AREAS COVERED: This review synthesizes emerging evidence on post-infectious inflammatory syndr

Mortality in first-episode HIV-associated cryptococcal meningitis in Peru.

Cryptococcal meningitis (CM) remains one of the leading causes of morbidity and mortality among people living with HIV, particularly in low and middle-income countries where access to standard antifungal therapy is limited. Despite global advances in HIV care, early diagnosis and effective treatment of cryptococcal disease continue to represent major challenges in resource-limited settings. We conducted a retrospective registry-based cohort study of HIV patients and a first episode of CM admitted to a Peruvian national referral hospital between 2005 and 2015. Cox proportional hazards models we
